진동을 이용한 IASTM이 만성 어깨통증 환자의 어깨 근활성도, 유연성, 통증에 미치는 영향 (The Effects of IASTM Using Vibration Stimulation on Shoulder Muscle Activity, Flexibility and Pain of Chronic Shoulder Pain Patients)

  • Purpose : Instrument-assisted soft tissue mobilization (IASTM) has been widely used to improve pain and range of motion. However, research on IASTM using vibration stimulation is lacking. Therefore, this study investigates how IASTM using vibration stimulation affects chronic pain on muscle activity and flexibility of the shoulders. Methods : A total of 20 patients with chronic shoulder pain were divided into the experimental group and the control group. The experimental group (n=10) used vibration stimulus for IASTM, while the control group (n=10) used general IASTM. For six weeks, this training lasted for 30 min a day, twice a week. Muscle activity was evaluated using the surface electromyograph. The back and reach test was used to assess flexibility, the visual analogue scale to measure pain, and the paired t-test to compare the groups before and after the experiment. An independent ANCOVA was conducted to assess differences in the degree of transition between the two groups before and after the experiment. Results : Significant differences in muscle activity, flexibility, and pain in both groups before and after the experiment (p<0.05) were observed, as well as in the muscle activity of the pectoralis major and associated pain (p<0.05). however, muscle activity and flexibility of the upper trapezius and infraspinatus were not significantly different between groups (p>0.05). Conclusion : This study confirms the importance of IASTM using vibration. Design and manufacture of IASTM using vibration based on our results would be useful in the management of shoulder pain, Further clinical evidence are needed.

발등 굽힘 제한이 있는 사람에게 장딴지근 신장운동과 발바닥 자가근막이완이 발등 굽힘 각도와 장딴지근 긴장도에 미치는 즉각적인 효과 비교 (A Comparative Study on the Immediate Effect of Performing Gastrocnemius Stretching with and without Myofascial Release of the Sole on Ankle Dorsiflexion Angles and Gastrocnemius Muscle Tone in Subjects with Limited Ankle Dorsiflexion)

  • PURPOSE: This study compared the effect of performing gastrocnemius stretching with and without the self-myofascial release of the sole on the active and passive ankle dorsiflexion angles and muscle tone of the gastrocnemius muscle in subjects with short gastrocnemius muscle. METHOD: A total of 23 subjects with short gastrocnemius muscles were included in this study. The study participants were divided into two experimental groups. Group A performed gastrocnemius muscle self-stretching exercises only, while group B performed self-myofascial release of the sole using a massage ball after the gastrocnemius muscle self-stretching exercises. For both groups, the active and passive ankle dorsiflexion angles were measured using a goniometer, and the tone of the gastrocnemius muscle was assessed using the MyotonPRO®. RESULTS: Within-group comparison showed that the participants in both groups A and B had significantly increased active and passive ankle dorsiflexion angles and decreased gastrocnemius muscle tone (p < .05) after performing their respective exercises. However, no significant differences in the said criteria were observed between groups A and B (p>.05). CONCLUSION: The results of this study showed that both methods were effective in increasing active and passive dorsiflexion angles and decreasing muscle tone. Thus, it is recommended to tailor gastrocnemius stretching exercises according to the patient's condition. If the patient does not experience discomfort in the plantar fasciae, it is recommended to perform the gastrocnemius stretching exercise only without myofascial release and use a massage ball afterward.

One Year Experience of the Hand Allotransplantation First Performed after Korea Organ Transplantation Act (KOTA) Amendment

  • The revision of the Korea Organ Transplantation Act (KOTA) in 2018 included hand/arm among the organs that can be transplanted. The first hand transplantation since the revision of KOTA took place in January 2021. A 62-year-old male patient experienced hand amputation on July 13, 2018, by a catapult injury. The patient first visited our institute 3 months after the injury. After serial interviews and an overall evaluation, the patient was registered on the hand transplantation waiting list in January 2020. On January 9, 2021, the patient underwent hand transplantation at the right distal forearm level. The total operation time was 17 hours 15 minutes, and the cold ischemic time was 4 hours 9 minutes. Postoperative immunosuppression was administered based on the protocol used for kidney transplantation. Two acute rejection episodes occurred, on postoperative days 33 and 41. Both rejection episodes were reversible with rescue therapy of a higher tacrolimus trough level, steroid pulse therapy, and topical immunosuppressants. Controlled passive range of motion exercise was started on postoperative day 10. Dynamic splint was applied on postoperative day 18. At 1 year, graft maintenance and functional improvement were satisfactory, and the patient showed a Disabilities of Arm, Shoulder and Hand score of 25.8. We successfully performed the first hand transplantation surgery under the KOTA amendment. It came from the organic and effective cooperation of plastic, orthopaedic, and transplantation departments and we believe it will guarantee the future ongoing success.

Evaluation of Treatment Plan Quality between Magnetic Resonance-Guided Radiotherapy and Volumetric Modulated Arc Therapy for Prostate Cancer

  • Purpose: This study evaluated the quality of plans based on magnetic resonance-guided radiotherapy (MRgRT) tri-Co-60, linac, and conventional linac-based volumetric modulated arc therapy (linac-VMAT) for prostate cancer. Methods: Twenty patients suffering from prostate cancer with intermediate risk who were treated by MAT were selected. Additional treatment plans (primary and boost plans) were generated based on MRgRT-tri-Co-60 and MRgRT-linac. The planning target volume (PTV) of MRgRT-based plans was created by adding a 3 mm margin from the clinical target volume (CTV) due to high soft-tissue contrast and real-time motion imaging. On the other hand, the PTV of conventional linac was generated based on a 1 cm margin from CTV. The targets of primary and boost plans were prostate plus seminal vesicle and prostate only, respectively. All plans were normalized to cover 95% of the target volume by 100% of the prescribed dose. Dosimetric characteristics were evaluated for each of the primary, boost, and sum plans. Results: For target coverage and conformity, the three plans showed similar results. In the sum plans, the average value of V65Gy of the rectum of MRgRT-linac (2.62%±2.21%) was smaller than those of MRgRT tri-Co-60 (9.04%±3.01%) and linac-VMAT (9.73%±7.14%) (P<0.001). In the case of bladder, the average value of V65Gy of MRgRT-linac was also smaller. Conclusions: In terms of organs at risk sparing, MRgRT-linac shows the best value while maintaining comparable target coverage among the three plans.

  • Engineers have developed new instruments that aid in diagnosis and therapy Ultrasonic imaging has provided a nondamaging method of imaging internal organs. A complex transducer emits ultrasonic waves at many angles and reconstructs a map of internal anatomy and also velocities of blood in vessels. Fast computed tomography permits reconstruction of the 3-dimensional anatomy and perfusion of the heart at 20-Hz rates. Positron emission tomography uses certain isotopes that produce positrons that react with electrons to simultaneously emit two gamma rays in opposite directions. It locates the region of origin by using a ring of discrete scintillation detectors, each in electronic coincidence with an opposing detector. In magnetic resonance imaging, the patient is placed in a very strong magnetic field. The precessing of the hydrogen atoms is perturbed by an interrogating field to yield two-dimensional images of soft tissue having exceptional clarity. As an alternative to radiology image processing, film archiving, and retrieval, picture archiving and communication systems (PACS) are being implemented. Images from computed radiography, magnetic resonance imaging (MRI), nuclear medicine, and ultrasound are digitized, transmitted, and stored in computers for retrieval at distributed work stations. In electrical impedance tomography, electrodes are placed around the thorax. 50-kHz current is injected between two electrodes and voltages are measured on all other electrodes. A computer processes the data to yield an image of the resistivity of a 2-dimensional slice of the thorax. During fetal monitoring, a corkscrew electrode is screwed into the fetal scalp to measure the fetal electrocardiogram. Correlations with uterine contractions yield information on the status of the fetus during delivery To measure cardiac output by thermodilution, cold saline is injected into the right atrium. A thermistor in the right pulmonary artery yields temperature measurements, from which we can calculate cardiac output. In impedance cardiography, we measure the changes in electrical impedance as the heart ejects blood into the arteries. Motion artifacts are large, so signal averaging is useful during monitoring. An intraarterial blood gas monitoring system permits monitoring in real time. Light is sent down optical fibers inserted into the radial artery, where it is absorbed by dyes, which reemit the light at a different wavelength. The emitted light travels up optical fibers where an external instrument determines O2, CO2, and pH. Therapeutic devices include the electrosurgical unit. A high-frequency electric arc is drawn between the knife and the tissue. The arc cuts and the heat coagulates, thus preventing blood loss. Hyperthermia has demonstrated antitumor effects in patients in whom all conventional modes of therapy have failed. Methods of raising tumor temperature include focused ultrasound, radio-frequency power through needles, or microwaves. When the heart stops pumping, we use the defibrillator to restore normal pumping. A brief, high-current pulse through the heart synchronizes all cardiac fibers to restore normal rhythm. When the cardiac rhythm is too slow, we implant the cardiac pacemaker. An electrode within the heart stimulates the cardiac muscle to contract at the normal rate. When the cardiac valves are narrowed or leak, we implant an artificial valve. Silicone rubber and Teflon are used for biocompatibility. Artificial hearts powered by pneumatic hoses have been implanted in humans. However, the quality of life gradually degrades, and death ensues. When kidney stones develop, lithotripsy is used. A spark creates a pressure wave, which is focused on the stone and fragments it. The pieces pass out normally. When kidneys fail, the blood is cleansed during hemodialysis. Urea passes through a porous membrane to a dialysate bath to lower its concentration in the blood. The blind are able to read by scanning the Optacon with their fingertips. A camera scans letters and converts them to an array of vibrating pins. The deaf are able to hear using a cochlear implant. A microphone detects sound and divides it into frequency bands. 22 electrodes within the cochlea stimulate the acoustic the acoustic nerve to provide sound patterns. For those who have lost muscle function in the limbs, researchers are implanting electrodes to stimulate the muscle. Sensors in the legs and arms feed back signals to a computer that coordinates the stimulators to provide limb motion. For those with high spinal cord injury, a puff and sip switch can control a computer and permit the disabled person operate the computer and communicate with the outside world.

두경부암 환자에 대한 선량체적 히스토그램에 따른 토모치료외 선형가속기기반 세기변조방사선치료의 정량적 비교 (Comparison of Helical TomoTherapy with Linear Accelerator Base Intensity-modulated Radiotherapy for Head & Neck Cases)

  • 토모치료기는 선형가속기에서 사용되는 세기 변조방사선치료와 Mega-voltage Computed Tomography (MVCT) 3차원 영상정보에 의한 영상유도방사선치료(Image Guide Radiation Therapy, IGRT)가 가능하고 나선식 빔 조사를 통해 표적과 정상조직에 효율적으로 선량(dose)을 집중하고 분산할 수 있다는 장점을 가진다. 이에 본 논문에서는 2006년 9월부터 운용을 시작한 국립암센터의 토모치료기를 이용하여 10명의 두경부암 환자를 대상으로 토모치료계획과 선형가속기기반 세기변조방사선치료계획의 선량분포를 비교하였으며 또한, 영상유도 방사선치료장치가 구비되지 못하여 환자의 위치보정이 부정확한 구형 세기변조방사선치료기에 대해 표적의 움직임에 따라 균일성(Homogeneity)과 정상조직부작용율(Normal Tissue Complication Probability, NTCP)이 상대적으로 어떻게 변화하는지 연구하였다. 이 연구를 위하여 토모치료기의 치료계획용 시스템인 Hi-Art (Hi-Art2_2_4 2.2.4.15, TomoTherapy Incorporated, 1240 Doming Way, Madison, WI S3717-1954, USA)와 선형가속기기반 세기변조방사선치료계획용 시스템인 CadPlan (CadPlan R.6.4.7, Varian Medical System Inc. 3100 Hansen Way, Palo Alto, CA 94304-1129, USA)을 이용한 역치료계획(inverse Planning)이 수행되었으며 치료계획은 각각의 환자에 대해 같은 표적과 최적화조건을 가지도록 고려되었다. 총 10명의 환자를 대상으로 한 연구결과, 환자의 신체내 외부가 완전히 고정되어 있는 이상적인 경우에 대해서 토모치료기를 이용한 치료계획이 선형가속기에 의한 세기 변조 방식보다 표적에 대해 보다 높고 일정한 선량분포를 주면서도 주변 장기의 선량이 선량체적 히스토그램(Dose Volume Histogram, DVH)에서 오히려 감소함을 보여 주었다 또한 토모치료기의 MVCT를 이용하여 한 달간 토모 환자의 표적위치변화를 측정하였고, 이를 토대로 평균 표적위치변화를 고려하였을 경우 선형가속기기반 세기변조방사선치료계획인 CadPlan에서 표적의 등가균질선량(Equivalant Uniform Dose, EUD)이 감소하고 정상조직의 부작용율의 발생가능성이 상대적으로 증가함을 발견하였다.

간암환자를 대상으로 한 래피드아크 치료계획에서 아크수 및 회전범위가 선량분포에 미치는 영향 (Effects of Arc Number or Rotation Range upon Dose Distribution at RapidArc Planning for Liver Cancer)

  • 본 논문에서는 간암 환자를 대상으로 3차원입체조형치료와 세기조절방사선치료와 3종류의 래피드아크 치료를 위한 치료계획을 수행하여 각각의 선량분포와 선량 체적 히스토그램(Dose Volume Histogram, DVH)의 특성을 비교, 평가하고 이를 통해 래피드아크 치료 환자에 대한 적절한 갠트리 회전수의 범위를 제시하고자 하였다. 치료계획은 작은 종양의 용적을 가지며 내부 장기 및 종양의 움직임이 비교적 작은 간암환자를 대상으로 3차원입체조형치료와 세기조절방사선치료와 더블아크(double arcs)와 제한적인 트리플아크(limited triple arcs)와 멀티플아크(multiple arcs)치료에 대해 Eclipse 8.6 버전에서 시행하였다. 또한 치료계획 시 임상표적용적(Clinical Target Volume, CTV)과 치료표적용적(Planning Target Volume, PTV)에 동일한 최적화 조건을 적용하였고, 각각의 정상조직에는 개별적인 선량 제한치 적용대신 종양 주변 정상조직에서의 선량 감소율을 일괄적으로 적용하였다. 임상표적용적과 치료표적용적의 전체 몸에 적용한 치료계획 선량 제한치에 대한 실제 계산결과의 만족도는 래피드아크가 세기조절방사선치료보다 높았으며 래피드아크에서 더블아크와 제한적인 트리플아크와 멀티플아크는 만족도가 거의 동일하였다. 또한 각 치료계획에서 치료표적용적에 계획한 선량 제한치에 따라 SALT group이 제안한 Conformity Index (CI)가 0.98인 선량 분포범위에 처방선량을 결정하였다. 이때 RTOG에서 제안한 CI, Homogeneity Index (HI), Quality of Coverage (QOC)와 Lomax and Scheib에서 제안한 Healthy tissue conformity index (HTCI) 등을 평가하였다. 항목별로 차이는 있지만 총체적인 평가 결과는 멀티플아크가 전체적으로 좋았으며 모니터 단위 값의 비교에서도 멀티플아크가 가장 작았다. 본 연구를 통하여 래피드아크 치료는 기존에 시행하고 있는 3차원입체조형치료 또는 세기조절방사선치료 기법보다 더욱 최적의 선량 분포를 구현한다는 것을 알 수 있었다. 또한 래피드아크 치료계획에서 멀티플아크 치료계획은 더블 또는 제한된 트리플아크 치료계획보다 모니터 단위 값이 작고, 선량의 균질성 및 종양내의 최소선량은 비슷하거나 좋아지는 동시에 주변 정상조직에는 선량이 감소되는 것을 알 수 있었다.

전산화단층촬영 주사시간(Scan Time)이 폐종양운동의 재현성에 미치는 영향 분석 (Impact of the Planning CT Scan Time on the Reflection of the Lung Tumor Motion)

  • 목적 : 방사선치료계획용 전산화단층활영(computerized tomography, 이하 CT) 시 주사시간이 종양부피의 재현성에 미치는 영향을 살펴보고자 하였다. 대상 및 방법 : 인공호흡기의 환기작용에 의해 세로방향의 왕복 운동을 하는 N자형 모형을 제작하였고, 호흡주기를 T로 하였을 때 주사시간을 상대적으로 각각 0.33, 0.50, 0.67, 0.75, 1.00, 1.33, 그리고 1.53 T로 설정하고 CT촬영을 시행하였다. 또한 2002년 3월 19일부터 2002년 5월 21일까지 서울아산병원 방사선종양학과에서 정위방사선수술(stereotactic radiosurgery)을 시행받은 비소세포성 폐암 환자 3명을 대상으로, 주사시간이 빠른 CT (LightSpeed, GE Medical Systems, 주사시간 0.8초)와 느린 CT (IQ Premier, Picker, 주사시간 2.0초)를 각각 $1\~4$회 시행하였다. 각각의 slice에서, N자형 모형의 왕복 운동이 CT 영상에 반영된 좌우방향 선분의 길이를 측정하여 치료계획용 CT 촬영이 모형의 움직임을 반영하는 정도를 측정하였다. 환자를 대상으로 한 연구에서는 정위적 체부 고정틀을 이용하여, 빠른 CT 및 느린 CT의 종양을 하나의 CT 영상조합에서 재구성하여 종양의 체적과 장경을 측정하여 비교하였다. 결과 : 모형실험에서 주사시간에 비례하여, 세로방향 운동을 CT 촬영에서 반영하는 정도는 증가하였으며 1.00 T 이상에서는 일정한 양상을 보였다 주사시간 1.00 T 이상에서 얻어진 결과를 기준으로, 1.00 T 미만의 주사시간을 가지는 CT촬영에서 모형운동을 반영하지 못하는 비율이 각각 $0.33\;T:\;30\%,\;0.50\;T:\;27\%,\;0.67\;T:\;20\%,\;0.75\;T:\;7.0\%$로 측정되었다. 또한 투시검사로 측정한 종양의 세로 방향 움직임이 각각 3 mm, 5 mm, 10 mm이었던 각각의 환자에서 느린 CT에서 얻어진 종양의 세로방향 장경이 빠른 CT에 비해 $5.3\%\;17\%,\;23\%$ 증가하였다. 결론 : 주사시간을 환자의 호흡주기 이상으로 하는 경우 setup margin만을 고려하여 계획용표적체적(planning target volume, PTV)을 정의할 수 있으므로 정상 폐조직에 조사되는 방사선량을 줄여 치료효율을 향상시킬 수 있을 것으로 생각된다.

4D-CT와 호흡동조시스템을 이용한 간암 환자의 방사선치료 표적 움직임 분석 (Target motion analysis of the respiratory gated guided radiotherapy in liver cancer patients using 4D-CT)

  • 방사선치료의 최대 목표는 정상조직을 보호하고 종양부위에 방사선량을 충분히 조사하는 것이다. 그 중에서 환자호흡에 의한 문제점을 보안하기 위해 환자호흡에 따른 주기적 움직임을 분석하여 안정된 일정영역에서만 방사선이 조사되도록 하는 호흡동조방사선치료 방법이 임상에서 활용되고 있다. 본 논문은 특히 호흡에 의해 움직임 영향을 많이 받는 간암 환자 40명을 대상으로 실제 방사선 치료 계획에 이용되는 4D-CT와 호흡동조방사선 치료에 사용하는 RPM을 이용하여 간암의 움직임을 환자의 나이, 성별 및 움직임 방향으로 나누어 분석하였다. 움직임의 변화는 방향으로는 right-left($3.19{\pm}1.29$), anteroposterior($5.44{\pm}2.07$), craniocaudal($12.54{\pm}4.70mm$)이었고, 이는 성별과 관련이 없고 나이가 고령일수록 움직임이 크다는 것을 알 수 있다. 따라서 호흡에 의해 움직임으로 인한 치료부위의 방향이 변화가 생김으로 RPM 호흡동조방사선 치료로 움직임에 대한 보정을 실시하여 간암 치료에서 치료효과를 높일 수 있다.

우측 관상동맥 폐쇄 환자에서 관상동맥내 Thallium-201 주사를 이용한 측부 혈행의 의의 (Functional Significance of Angiographic Collaterals in Patients with Totally Occluded Right Coronary Artery: Intracoronary Thallium-201 Scintigraphy)

  • To compare the myocardial viability in patients suffering from total occlusion of the right coronry artery (RCA) with the angiographic collaterals, intracoronary injection of Thallium-201 (T1-201) was done to 14 coronary artery disease (CAD) patients (pts) with total occlusion of RCA and into four normal subjects for control. All 14 CAD pts had Grade 2 or 3 collateral circulations. There were 14 male and 4 females, and their ages ranged from 31 to 70 years. In nine pts, T1-201 was injected into left main coronary artery (LCA) ($300{\sim}350{\mu}Ci$) to evaluate the myocardial viability of RCA territory through collateral circulations. The remaining five pts received T1-201 into RCA ($200{\sim}250{\mu}Ci$) because two had intraarterial bridging collaterals and three had previous successful PTCA. Planar & SPECT myocardial perfusion images were obtained 30 minutes, and four to five hours after T1-201 injection. Intravenous T1-201 reinjection (six pts) or $^{99m}Tc-MIBI$ (two pts) were also performed in eight CAD pts. Intracoronary myocardial perfusion images were compared with intravenous T1-201 (IV T1-201) images, ECG, and ventriculography. Intracoronary T1-201 images proved to be superior to that of IV T1-201 due to better myocardial to background uptake ratio and more effective in the detection of viable tissue. We also found that perfusion defects were smaller on intracoronary T1-201 images than those on the IV T1-201. All of the 14 CAD pts had either mostly viable myocardium (seven pts) or large area of T1-201 perfusion (seven pts) in RCA territory, however ventriculographic wall motion and ECG did not correlate well with intracoronary myocardial perfusion images. In conclusion, total RCA occlusion patients with well developed collateral circulation had large area of viable myocardial in the corresponding territory.
