관절고정에 의한 조직변화와 구축의 분류에 대한 고찰 (A Review of tissue changes caused by joint immobilization and classification of contracture)

  • Contracture is defined as the lack of full passive range of motion resulting from pint, muscle or soft tissue limitationprolonged Pint immobilization will result in stress and stretch deprivation and gradual development of contracture. the tissue changes caused by immobilization may be categorized as cellular modeling, ground substance and collagen response, and tissue response. contracture can be divided into three categories according to the anatomical location of pathological changes :arthrogenic, myogenic, soft tissue contractures Therapeutic approach of contracture is thermal or cold agents application, stretch or restoration of length, traction, manipulation, mobilization positioning and restoration of function. The purpose of this article is to review current concepts of mechanical properties and synthesis of collagen tissue and the underlying pathomechanics as it relates to evaluation and treatment of contracture.

비복 동맥 피판을 이용한 슬관절 주위의 연부 조직 결손의 치료 (Sural Artery Flap for the Treatment of Soft Tissue Defects around Knee Joint)

  • Purpose: This report presents the authors' experience of twelve patients with sural artery flap for soft tissue defects around the knee joint. Materials and Methods: The patients' age ranged from 25 to 80 years; seven of the patients were male and five were female. The cause of soft-tissue defects involved wide excision for malignant soft tissue tumor, tumor prosthesis related infection, infection after total knee arthroplasty and chronic osteomyelitis. Postoperative range of motion was checked. The sensibility of flap was evaluated by Semmes-Weinstein monofilaments and two-point discrimination. Results: All flaps survived and provided satisfactory coverage of the defect. There was no complication except one delayed skin graft incorporation at donor site. Seven knee joints which had been stiff previously gained average 58 degrees of ROM postoperatively. All flaps retained sensibility and showed no significant increase in sensory thresholds comparing with contralateral side. Conclusion: Sural artery flap not only shows high survival rate and broad coverage ability, but also offers improvement in range of motion and preservation of sensation. We speculate that sural artery flap is valuable for the reconstruction of the soft tissue defects around knee joint.

  • knee injuries are among the most common problems confronting patients, physical therapist and physicians. since the bony structure provides little stability, the soft tissues are required to withstand high forces, often resulting in tissue overload and injury immobilization of the knee in the presence of a hemarthrosis usually leads to proliferation of intraarticular connective tissue adhesions and pint fibrosis. the physical basis of treating scar tissue is early controlled motion. the management of the knee with increased instability is to strengthening the muscles to properly treat an soft tissue injuries, physical therapist should know about the anatomic structures and biomechanical function of the knee joint, in this review article, we discuss the physiologic properties of soft tissue, biomechanical observation in fibrous connective tissue, managing soft tissue contractures, principles in treating scar tissue and treatment of the knee with increased instability.

Development of Respiratory Motion Reduction Device System (RMRDs) for Radiotherapy in Moving Tumor: Construction of RMRDs and Patient Setup Verification Program

  • The purpose is to develop a system to reduce the organ movement from the respiration during the 3DCRT or IMRT. This research reports the experience of utilizing personally developed system for mobile tumors. The patients clinical database was structured for 10 mobile tumors and patient setup error measurement and immobilization device effects were investigated. The RMRD system is composed of the respiratory motion reduction device utilized in prone position and abdominal strip device(ASD) utilized in the supine position, and the analysis program, which enables the analysis on patients setup reproducibility. Dose to normal tissue between patients with RMRDs and without RMRDs was analyzed by comparing the normal tissue volume, field margins and dose volume histogram(DVH) using fluoroscopy and CT images. And, reproducibility of patients setup verify by utilization of digital images. When patients breathed freely, average movement of diaphragm was 1.2 cm in prone position in contrast to 1.6 cm in supine position. In prone position, difference in diaphragm movement with and without RMRDs was 0.5 cm and 1.2 cm, respectively, showing that PTV margins could be reduced to as much as 0.7 cm. With RMRDs, volume of the irradiated normal tissue (lung, liver) reduced up to 20 % in DVH analysis. Also by obtaining the digital image, reproducibility of patients setup verify by visualization using the real-time image acquisition, leading to practical utilization of our software. Internal organ motion due to breathing can be reduced using RMRDs, which is simple and easy to use in clinical setting. It can reduce the organ motion-related PTV margin, thereby decrease volume of the irradiated normal tissue.

Toe Tissue Transfer for Reconstruction of Damaged Digits due to Electrical Burns

  • Background : Electrical burns are one of the most devastating types of injuries, and can be characterized by the conduction of electric current through the deeper soft tissue such as vessels, nerves, muscles, and bones. For that reason, the extent of an electric burn is very frequently underestimated on initial impression. Methods : From July 1999 to June 2006, we performed 15 cases of toe tissue transfer for the reconstruction of finger defects caused by electrical burns. We performed preoperative range of motion exercise, early excision, and coverage of the digital defect with toe tissue transfer. Results : We obtained satisfactory results in both functional and aesthetic aspects in all 15 cases without specific complications. Static two-point discrimination results in the transferred toe cases ranged from 8 to 11 mm, with an average of 9.5 mm. The mean range of motion of the transferred toe was $20^{\circ}$ to $36^{\circ}$ in the distal interphalangeal joint, $16^{\circ}$ to $45^{\circ}$ in the proximal interphalangeal joint, and $15^{\circ}$ to $35^{\circ}$ in the metacarpophalangeal joint. All of the patients were relatively satisfied with the function and appearance of their new digits. Conclusions : The strategic management of electrical injury to the hands can be both challenging and complex. Because the optimal surgical method is free tissue transfer, maintenance of vascular integrity among various physiological changes works as a determining factor for the postoperative outcome following the reconstruction.

심장 자기공명영상 (Cardiac MRI)

  • 심장의 영상화에 장애가 되는 요인은 심장 운동, 호흡, 심장 내 혈류 등에 의한 인공물(artifact) 과 심장 조직의 용적이 작음으로 인한 낮은 신호 대 잡음비 등이 있다. 심장 운동에 의한 화질 저하를 막기 위해 신속영상기법(fast imaging technique) 을 이용하여 심장 운동의 특정 위상(phase) 에서만 영상을 얻는 심장동기(cardiac gating) 방법을 이용하고 있다. MRI를 이용한 심장의 검사는 심장의 형태, 심실 기능, 심근 관류, 심근 대사, 관상동맥 영상 등을 대상으로 한다. 심장의 형태적 진단에 있어서 심근내 수분의 정도와 지방조직을 보기 위해 이중(double) 혹은 삼중역전회복기법(triple inversion recovery technique) 을 사용한다. 심근관류검사를 위해서는 조영증강신속경사에코법(contrast-enhanced fast gradient echo technique)을 사용하여 일차통과조영증강(first-pass enhancement) 을 검사한다. 또한 10-15분 지연영상을 얻어 심근내 조영제의 재분포를 검사하여 만성심근경색 등의 심근파괴부위를 확인한다. 심실기능 평가를 위해서는 신속경사에 코법을 이용한 영화영상(cine image) 이 사용되며 심실의 국소적 운동이상 및 심실기능의 정량적 검사가 가능하다. MRI는 관상동맥영상을 제외한 포괄적 심장검사에 실용성이 있다. 특히 지연영상은 다른 검사장비에선 얻을수 없는 유용한 정보이다.

간암의 방사선치료 시 호흡운동 감소장치(respiratory motion reduction device, RRD)의 유용성에 관한 연구 (Use of Respiratory Motion Reduction Device (RRD) in Treatment of Hepatoma)

  • 목적 : 폐, 간 등의 상 복부에 위치한 종양의 방사선 조사 체적은 호흡에 의한 종양의 이동을 포함하는 영역으로 조사 체적이 증가된다. 이로 인하여 방사선 독성 및 정상조직 선량이 증가되며, 호흡으로 인한 환자자세의 변화로 인해 종양의 정확한 위치파악이 어렵게 된다. 본 연구에서는 호흡에 따른 장기 움직임 유형을 분석하여 호흡에 의한 장기의 움직임을 최소화 할 수 있는 호흡운동 감소장치를 고안하고 방사선치료 시 호흡운동 감소장치의 유용성을 평가해 보고자 하였다. 대상 및 방법 : 간암환자 10명을 대상으로 하여 MeV-Green과 벨트, 스티로폼판 등을 사용하여 호흡운동감소장치(respiratory motion reduction device, RRD)를 제작하였다. 내부장기의 이동정도는 모의치료 시에 관찰된 횡경막의 이동 정도로 평가하였으며 앙와위와 복와위 및 RRD의 사용 시 이동정도를 쏠아보았고, 각각의 경우에서 이동정도를 고려하여 방사선치료계획을 수립하였다. 선량체적 히스토그램(dose-volume histogram, DVH)을 통해서 전체 간 용적 중 처방선량의 $50\%$가 조사되는 정상간 용적을 구하였다. 결과 : 호흡에 따른 횡경막의 평균이동거리는 앙와위 자세에서 $16{\pm}1.9\;mm$ 이었고, 복와위 자세에서는 $12{\pm}1.9mm$임을 알 수 있었다. 복와위 자세에서 본원에서 자체 제작한 RRD를 사용한 경우에는 $5{\pm}1.4\;mm$으로 감소되었고, 벨트 고정장치의 추가 사용 시에는 $3{\pm}0.9\;mm$으로 감소하여 총 9 mm 감소함을 알 수 있었다. 방사선치료계획에 따른 DVH에서 처방선량의 $50\%$가 조사되는 정상간의 용적은 호흡운동감소장치를 사용하지 않은 경우에 앙와위 자세에서 $43.7\%$, 복와위 자세에서 $40\%$ 이었고, 호흡운동 감소장치를 사용한 경우에 복와위 자세에서 $30.7\%$, 여기에 벨트 고정장치를 추가 사용하였을 경우에는 $21\%$로서 전체 간 용적 중 방사선에 조사되는 정상 간 용적은 최대 $22.7\%$ 감소됨을 알 수 있었다. 결론 : 호흡에 따른 내부장기의 움직임을 최소화 할 수 있는 RRD를 사용하여 정상조직에 불필요하게 조사되는 방사선을 감소시킬 수 있었다.

Common-path Optical Interferometry for Stabilized Dynamic Contrast Imaging: A Feasibility Study

  • The motion of organelles inside a cell is an important intrinsic indicator for assessing cell physiology and tissue viability. Dynamic contrast full-field optical coherence tomography (D-FFOCT) is a promising imaging technology that can visualize intracellular movements using the variance of temporal interference signals caused by biological motions. However, double-path interferometry in D-FFOCT can be highly vulnerable to surrounding noise, which may cause turbulence in the interference signals, contaminating the sample dynamics. Therefore, we propose a method for stabilized D-FFOCT imaging in noisy environments by using common-path interferometry in D-FFOCT. A comparative study shows that D-FFOCT with the proposed method achieves stable dynamic contrast imaging of a scattering phantom in motion that is over tenfold more noise-insensitive compared to the conventional one, and thus this imaging capability can provide cleaner motion contrast images. With the proposed approach, the intracellular dynamics of biological samples are imaged and monitored.

Changes in Matrix Metalloproteinase and Tissue Inhibitors of Metalloproteinase in Patients with Rotator Cuff Tears

  • Background: The purpose of this study was to determine whether in patients with rotator cuff tears a correlation exists between molecular changes and clinical parameters such as age, duration of symptom, range of motion, and tear size. Molecular changes of matrix metalloproteinase (MMP) and tissue inhibitor of metalloproteinase (TIMP) were assessed by measuring messenger RNA (mRNA) levels of the two proteins. Methods: The rotator cuff tissue from was obtained from the edge of a torn tendon revealed after debridement by a motorized shaver. Using the sample of rotator cuff tissue, the reverse transcription polymerase chain reaction was performed to quantify MMP-2 and TIMP-2 mRNA expression. To determine whether mRNA levels and the clinical variables, such as age, defect size, range of motion (ROM) of shoulder, and duration of symptoms, show any correlation, Spearman's correlation coefficients were used to test for significant differences. Results: There was an inverse correlation between the mRNA levels of MMP-2 and TIMP-2 from the torn rotator cuff tendons regardless of the clinical variables. However, comparison of mRNA levels versus clinical parameters such as age, defect size, range of motion and duration of symptoms revealed a number of findings. We found a significant correlation between age and mRNA levels of MMP-2 from torn cuffs (r = 0.513, p = 0.021). Further, we found a significant correlation between defect size in the full thickness tears and mRNA levels of MMP-2 (r = 0.454, p = 0.045). Conversely, no significant association between mRNA levels of MMP-2 and ROM or duration of symptom was found. Conclusions: Our results suggest that both MMP-2 and TIMP-2 may be involved in the disease process of rotator cuff tears. Although the level of mRNA expression of MMP-2 and TMP-2 remain constant in torn rotator cuffs irrespective of the clinical variables, their levels may be influenced by age and defect size, which could account to change in tendon degradation and the healing process.
