선 자세 몸통 안정화 운동이 길리안 바레 환자의 균형과 보행에 미치는 효과: 증례보고 (Effects of standing position of the trunk stabilization exercise on balance and gait of Guillain-Barre patients, case report)

  • Background: In the present study to investigate the immediate effects of standing position of the trunk stabilization exercise on Guillain-Barre syndrome patient's balance and gait abilities were examined. Design: Case report and conducted as a single-blind. Methods: Standing position of trunk stabilization exercise was conducted for five Guillain-Barre syndrome patient's who met the selection criteria were recruited. The subjects were conducted conservative exercise and trunk stabilization exercise. Conservative exercise was implemented for thirty min, five times for 8 weeks, and trunk stabilization exercise was implemented for 15 min. The participants' balance was measured via force plate (COP), functional reach test (FRT), timed up and go test (TUG), gait was measured using the 10 m walk test, and 6 minute walk test. Results: After training, the change values of the balance and gait ability in the subjects were significantly greater than pre-test. participants showed significant improvements in COP, FRT, TUG, 10 m walk test and 6 minute walk test pre and post the training (p<0.05). Conclusions: standing position of the trunk stabilization exercise was effective on Guillain-Barre syndrome patient's balance (COP, FRT, TUG) and gait (10 m walk test and 6 minute walk test) abilities were examined.

  • Purpose: Myofascial release (MFR) is an effective treatment for improving muscle stiffness and balance in neurological patients. This study examined long-term effects of MFR on the balance ability of stroke patients. Methods: MFR using a tennis ball was applied to the lower extremity and sacroiliac joint of the affected side of four stroke patients. The four subjects performed the Berg balance scale (BBS) and Timed up and go (TUG) test at the beginning of an 8-week intervention, directly after the intervention, and 2 years later. Results: The BBS scores indicated a trend toward maintenance of balance (p=0.05), but there was no difference in the TUG time (p=0.47). Conclusion: MFR may be a clinically meaningful intervention to maintain balance in stroke patients over a long period. However, the sample size in this study was too small to draw general conclusions. A larger study with more participants is needed.

PNF 통합 패턴 운동이 만성 요통 노인 환자의 균형에 미치는 영향 (The effects of combination patterns exercise of proprioceptive neuromuscular facilitation on balance in chronic low back pain elderly patients)

  • 본 연구에서는 스프린터와 스케이터 패턴을 통합한 PNF 통합 패턴 운동이 만성 요통 노인 환자의 정적 균형과 동적 균형의 변화를 알아보고자 실시하였다. 본 연구는 만성 요통의 질환이 있는 노인 환자 34명을 대상으로 하였고, 34명을 무작위로 각각 PNF 통합 패턴 운동군(17명)과 스위스볼 운동군(17명)으로 나누었다. 운동전후에 정적 균형 능력 측정을 위해 GOOD BALANCE system(Metitur Oy, Palokka, Finland, 2003)을 사용하였다. 동적 균형 능력 측정을 위해 기능적 팔 뻗기 검사(functional reach test; FRT)와 일어나 걸어가기 검사(timed up and go test; TUG)를 시행하였다. 정적 균형 능력과 동적 균형 능력의 거의 대부분의 항목에서 유의한 차이를 나타내었다(p<.05). 군간 비교에서도 다수의 항목에서 스위스볼 운동군과 PNF 통합 패턴 운동군 사이에 유의한 차이를 나타내었다(p<.05). 이와 같은 연구 결과로 볼 때 PNF 통합 패턴 운동은 만성 요통 노인 환자의 균형 능력을 개선시키는데 효과적임을 알 수 있었으며, 임상에서 PNF 통합 패턴 운동을 이용할 수 있는 기초 자료를 충분히 제공하였다고 할 수 있을 것이다.

Correlation among Motor Function and Gait Velocity, and Explanatory Variable of Gait Velocity in Chronic Stroke Survivors

  • Objective: The purpose of this study to investigate the correlations among the motor function, balance, and gait velocity and the strength that could explain the variation of gait velocity of chronic stroke survivors. Design: This was a cross-sectional cohort study. Methods: Thirty hemiplegic stroke survivors hospitalized in an inpatient rehabilitation center were participated. The muscle tone of ankle plantarflexor and muscle strength of ankle dorsiflexor were measured respectively with modified Ashworth scale (MAS) and hand-held dynamometer. And the motor recovery and function with Fugl-Meyer assessment (FMA), balance with Berg balance scale (BBS) and timed up and go (TUG) test were measured. Gait velocity was measured with GAITRite. The correlation among motor function, muscle tone, muscle strength, balance, and gait were analyzed. In addition, the strength of the relationship between the response (gait velocity) and the explanatory variables was analyzed. Results: The gait velocity had positive correlations with FMA, muscle strength, and BBS, and negative correlation with MAS and TUG. Regression analysis showed that TUG (𝛽=-0.829) was a major explanatory variable for gait velocity. Conclusions: Our results suggest that gait velocity had correlations with muscle strength, MAS, FMA, BBS, and TUG. The tests and measurements affecting the variation of gait velocity the greatest were TUG, followed by FMA, BBS, muscle strength, and MAS. This study shows that TUG would be a possible assessment tool to determine the variation of gait velocity in stroke rehabilitation.

Comparison of the Effects of Talus Stabilization Taping and Kinesio Taping on Balance and Walking Speed in Persons with Chronic Stroke

  • Objective: The purpose of this study was to compare walking speed and balance abilities according to various taping methodsin patients with stroke. Design: Cross-sectional study Methods: Twenty patients with stroke who were hospitalized at a rehabilitation hospital were allotted to the either the talus stabilization, Kinesio or sham taping, or barefoot conditions by drawing lots. Assessment tools used were the Functional Reach Test(FRT), Timed Up and Go test(TUG), One-Legged Stance Test (OLST), and the 10-Meter Walk Test(10MWT).After each test was measured three times, the mean values of each test was used for analysis. Results: Significant results were observed with thetalus stabilization and Kinesio taping groupcompared to the barefoot and sham taping method for theFRT,TUG, the OLST, and the 10MWT(p<0.05). Also, significant differences in the resultswere seen in the OLST, TUG, and the 10MWTwith the talus taping compared to the Kinesio taping method(p<0.05). Conclusions: The use oftalus stabilization taping applied to the ankle of patients with stroke was more effective for balance and walking ability improvement than Kinesio taping through the correction of an abnormal position of the talus.It is considered that these methods of taping can be applied effectively in the clinic.

The Effects of Bridge Exercise with Abdominal Drawing-in on Balance in Patients with Stroke

  • Purpose: The aim of this study was to evaluate the effect of Bridge exercise with abdominal drawing-in on static and dynamic balance in patients with stroke. Methods: Forty patients with stroke participated in this study. Participation was randomly assigned to the Bridge exercise group (n=20) and the Bridge exercise with abdominal drawing-in group (n=20). A bio-feedback device was used when patients performed the Bridge exercise with abdominal drawing-in. This training was performed without any motion on the patient's spine and upper belly part, and the pressure was held with the biofeedback device as 40-70 mmHg. Both groups received training 30 minutes per day, three times per week, for four weeks. Weight bearing, anterior limit of stability, and posterior limit of stability for static balance ability were measured, and Berg balance scale (BBS), Timed up and go test (TUG) for dynamic balance ability were also measured. Results: Participants showed significant differences between pre- and post-mediation in terms of weight bearing, anterior limit of stability, posterior limit of stability, Berg balance scale, and Timed up and go test (p<0.05). The Bridge exercise with abdominal drawing-in group showed a more significant increase (p<0.05). Conclusion: According to the results of this study, both exercises were effective for improving the static and dynamic balance ability. However we suggest that the Bridge exercise with abdominal drawing-in is more efficient for increasing balance ability in patients with stroke.

천장관절에 적용한 움직임을 동반한 관절가동술이 뇌졸중 환자의 보행에 미치는 효과 (The Effect of Mobilization With Movements Applied Sacro-Iliac Joint on Gait of Stroke Patient)

  • Background: To determine effects of mobilization with movements (MWM) application on stroke patient with hypomobility on sacroiliac joint. Methods: The subject was 47years old male who have left hemiplegia because of right intra-cerebral hemorrhage. The subject was have hypomobility on sacroiliac joint. MWM using during 4weeks and using Berg balance scale (BBS), Timed up and go (TUG), 10meter walking (10MW) test for evaluation. Results: The results of this study were summarized below; BBS score for evaluating balance ability was increased, and TUG time, 10MW test time was decrease. Conclusion: We consider that MWM application on stroke patient with hypomobility on sacroiliac joint is not only regain mobility on sacroiliac joint but also increase in balance ability and walking speed.

Effect of Balance Exercise using a Combination of Isotonics for Proprioceptive Neuromuscular Facilitation on Balance and Walking Ability in Patients with Hemiplegia Due to Stroke

  • Objective: This study aimed to determine the effect of proprioceptive neuromuscular facilitation (PNF) balance exercise on the ability to balance and walk in patients with hemiplegia caused by stroke. Design: A randomized controlled trial Methods: Following baseline measurements, patients (n=24) with hemiplegia caused by stroke were randomized into two groups: the PNF balance group (n=12) that received PNF balance exercise and the balance group (n=12) that received general balance exercise. Each group joined the intervention for 30 minutes, 5 times per week for 6 weeks. Both groups performed the Timed Up and Go test (TUG) and Berg Balance Scale (BBS) for balance, as well as the 10-meter walking test (10MWT) and 6-minute walk test (6MWT) for walking. The data were collected both before and after the intervention. The paired t-test was used to compare the post-intervention changes compared with pre-intervention data. An independent t-test was used to analyze the differences in the dependent variables between the two groups. Results: After the 6-week intervention, both groups showed significant improvements in balance (TUG, BBS) and walking (10MWT, 6MWT) parameters (p<0.05). The patients in the PNF balance group showed greater improvements in balance (TUG, BBS) and walking (10MWT, 6MWT) than those in the balance group (p<0.05). Conclusions: PNF balance exercise shows improvements in balance and walking parameters in patients with hemiplegia caused by stroke.

푸셔 증후군이 있는 뇌졸중 환자에서 시각적 피드백기반 로봇보조 재활치료의 효과 (Effect of Robot Assisted Rehabilitation Based on Visual Feedback in Post Stroke Pusher Syndrome)

  • 본 연구의 목적은 가상현실과 로봇보조재활치료를 이용하여 뇌졸중 후 푸셔 증후군에 대한 치료적인 효과를 조사하고자 하기 위함이다. 총 10명의 뇌졸중 후 푸셔 증후군을 보이는 환자가 모집되었다. 환자는 각각 5 명씩 로봇보조재활치료와 대조군으로 배정되었다. 실험군은 로봇보조재활치료와 일반적인 재활치료를 함께 받았으며, 대조군은 하루 2회 일반적인 재활치료를 받았다. 중재 시간은 30분간 진행되었으며, 주 5회, 4주간 시행하였다. 치료 전후 변화는 Scale for contraversive pushing (SCP), Berg balance scale (BBS), falling index (FI), Timed up and go test (TUG)을 이용하여 측정하였다. 4 주간의 중재 치료 후 로봇보조재활치료 군에서 SCP (p=0.046), BBS (p=0.046), FI (p=0.038), TUG (p=0.038)은 대조군에 비하여 유의하게 향상되었다. 또한 SCP와 BBS (p=0.024), FI (p=0.039), TUG (p=0.030)는 유의한 상관관계가 관찰되었다. 결과적으로 가상현실을 이용한 로봇보조재활치료는 일반적인 재활치료에 비하여 뇌졸중 후 푸셔 증후군을 회복하는데 더 도움이 되었으며, 푸셔 증후군의 회복은 균형과 보행기능의 향상과 관련이 있었다.

Effects of Proprioceptive Neuromuscular Facilitation and Treadmill Training on the Balance and Walking Ability of Stroke Patients

  • Purpose: The purpose of this study was to investigate the effects of combined training using proprioceptive neuromuscular facilitation (PNF) patterns and treadmills on the balance and walking ability of stroke patients. Methods: Twenty-three stroke patients were randomized into a control group (n=11), receiving only treadmill training and an experimental group (n=12) receiving combined training. The use of both PNF exercise and treadmill were implemented in the combined training. Interventions were performed 5 times a week for 6 weeks. Balance ability was measured by a timed up and go (TUG) test. Walking ability was measured by a 10-meter walk test (10MWT) and a 6-minute walk test (6MWT). A paired t-test was used to compare differences between pre- and post-intervention and independent t-tests were used to compare between groups. Results: Changes in TUG, 10MWT, and 6MWT before and after interventions were significantly different for both the experimental group and the control group (p<0.05). In addition, within-group changes in the TUG, 10MWT, and 6MWT were more effective in the experimental group than in the control group (p<0.05). Conclusion: Combined training using PNF techniques and treadmills may be useful in improving the balance and walking ability of stroke patients.
