교통사고에 의한 소아 족부 압궤손상에 대한 치료결과 (The Results of Treatment for Motor Vehicle-related Crushing Injuries of Foot in Children)

  • Purpose: The purpose of this work was to describe the results of treatment for motor vehiclerelated crushing injuries among children and adolescents under sixteen years in Korea. Materials and Methods: A retrospective analysis was conducted of data from children who were under sixteen year and injured foot by motor vehicles. Cases were documented 1) age at the time of injury, 2) injured site, 3) the area of accident, 4) the kind of vehicle, 5) associated injuries, 6) methods of treatment for soft tissue reconstruction and 7) complications. The relationships between the area of accident and associated injuries, and the kind of vehicle and associated injuries were analyzed using Chi-square test and Fisher exact test. Results: There were 97 children who were 15 year and younger. The mean age was 7.4 years, and 65% were boys. The left foot was more dominant side of injury (57%). Seasonal variation was seen with the number of injuries peaking during the summer (43%, p<0.05). Among the vehicles, 78.3% were the large vehicles (bus, truck or van). The where of accident was more frequent at an alley or less than two lanes of traffic. But, the relationships between the place of accident and associated injury or the kind of vehicles and associated injury were not statistically significant. The associated injury were fracture or dislocation (23 cases, 35.9%), injury of tendon (21 cases, 32.8%). There were amputation or disarticulation of foot in 8 cases (8.2%) and post-traumatic deformities such as flatfoot, hindfoot varus or valus deformities by tendon injury in 7 cases (7.2%). Conclusion: More than 50% of crushing or degloving injuries of child's foot by traffic accidents happened in boys between 5 to 9 years old. The associated injury was unrelated with size of vehicles or accident place at the time of accident. But, even though foot injury happened in an alley or one lane by small vehicles, child who hurt feet by car need thorough investigation about associated injury. If a surgeon keep in mind and treat child to associated injury necessarily, can minimize complication. Microsurgical reconstruction for soft tissue defect was prior to other methods.

Treatment Option for High Grade Spleen Injury and Predictive Factors for Non-operative Management

  • Purpose: Purpose: The prognostic factors of non-operative management (NOM) in high-grade spleen injuries have been extensively studied, but factors that would help treatment decisions are lacking. We compared the characteristics of the patients to identify the factors affecting treatment choices. Methods: This is a review of 207 blunt spleen injury patients from January 2004 to December 2013. We compared clinical features and mortality between surgery and NOM, and used multivariate regression analysis to find the factor most strongly associated with prognosis. Results: Of the 207 patients, 107 had high-grade spleen injury patents (grade III or above). Of these, 42 patients underwent surgery and 65 patients underwent NOM. The mortality was 7% following surgery, 3% with NOM. The amount of packed red blood cells transfused in the first 24 hours and spleen injury grade were associated with management type, and mortality was highly associated with activated partial thromboplastin time (aPTT) and spleen injury grade. Conclusions: The grade of spleen injury was associated with management and mortality, so correctly assessing the spleen injury grade is important.

Intra-Spinal Bone Marrow Mononuclear Cells Transplantation Inhibits the Expression of Nuclear Factor-${\kappa}B$ in Acute Transection Spinal Cord Injury in Rats

  • Objective : To assess the effect of bone marrow mononuclear cells (BMMNCs) transplantation in the expression of nuclear factor-${\kappa}B$ (NF-${\kappa}B$) in spinal cord injury (SCI) in rats. Methods : BMMNCs were isolated from tibia and femur by a density gradient centrifugation. After establishment of acute transection SCI, rats were divided into experiment (BMMNCs), experiment control (0.1 M PBS infused) and sham surgery groups (laminectomy without any SCI). Locomotor function was assessed weekly for 5 weeks post-injury using BBB locomotor score and urinary bladder function daily for 4 weeks post-injury. Activity of NF-${\kappa}B$ in spinal cord was assessed by immunohistochemistry and reverse transcriptase polymerase chain reaction. Results : At each time point post-injury, sham surgery group had significantly higher Basso, Beattie, Bresnahan locomotor and urinary bladder function scores than experiment and experiment control group (p<0.05). At subsequent time interval there were gradual improvement in both experiment and experiment control group, but experiment group had higher score in comparison to experiment control group (p<0.05). Comparisons were also made for expression of activated NF-${\kappa}B$ positive cells and level of NF-${\kappa}B$ messenger RNA in spinal cord at various time points between the groups. Activated NF-${\kappa}B$ immunoreactivity and level of NF-${\kappa}B$ mRNA expression were significantly higher in control group in comparison to experiment and sham surgery group (p<0.05). Conclusion : BMMNCs transplantation attenuates the expression of NF-${\kappa}B$ in injured spinal cord tissue and thus helps in recovery of neurological function in rat models with SCI.

척수손상자의 일상생활적응에 관한 연구 (A Study on the Activities of Daily Living Adaptation of Spinal Cord Injured Patients)

  • The purpose of this study was to evaluate the problem on activities of daily living ; the problem which spinal cord injured patients have when they adapt in daily living ; Subjects were 113 members who used the hospital which is located in Kwangju-city from November 20, 2001 to May 20, 2002. The evaluation of the ADL was performed according to MBI and collected data were statistically analysed by SPSS PC for paired Chi-square test T-test, One way ANOVA and Duncan's post-hoc test. The result's were as follows; 1. Modified Barthel Index average mark was $63.77{\pm}33.60$ points and MBI score distribution according to characteristics of injury is as following. 1) A patient who had long duration of injury, small injury region, incomplete paralysis in paralysis degree, paraplegia in paralysis type got high MBI score as statistical and significantly(p<0.05). 2. Society adaptation state by characteristics of spinal cord injured is an following. 1) After lapse of time of disease, a patient who is injured for a long term present surrounding environmental problem, a patient who is injured for a short term shows psychological problem. In society activity, as lapse of time of disease is long, patient did many hobby activity and same private club, on the other hand as lapse of time of diseases is short, the others appeared high and significantly as statistical(p<0.01). 2) In society activity by injury region, cervical injury and thoracic injury did more hobby activity than lumbar injury and in lumbar injury same private club or religion life appeared higher than thoracic injury of cervical injury significantly as statistical(p<0.01). 3) In walk method by paralysis degree Complete paralysis had more wheelchair life than incomplete paralysis(p<0.01). 4) In serious problem by paralysis type psychological problem in quadriplegia and surrounding environmental problem in paraplegia appeared high and significantly as statistical(p<0.01). 3. In society adaptation state by MBI score difference between variables appeared but it wasn't significantly.

Forearm Replantation for a Patient Presented with Major Amputation Injury: A Case Report

  • With the development of safety measures for employees who work with dangerous machinery, the frequency of amputation injuries has been decreasing with resultant decrease in replantation procedures. However, in some patients with major amputation injury, replantation is still necessary for the preservation of limb and it's function. The replantation of the upper extremity (UE) is a complex and technically demanding surgical procedure. For the successful replantation of UE, the type of injury, reconstruction sequence, ischemic time, and other combined injury of patient should be considered. We report a case of major amputation of UE by guillotine-type injury and discuss the treatment process of this patient.

Effect of Amino Acids on Anoxia-induced Cell Injury

  • This study was undertaken to examine the effect of amino acids on anoxia-induced cell injury in rabbit renal cortical slices. In order to induce anoxic cell injury, slices were exposed to a 100% $N_2$ atmosphere and control slices were exposed to 100% $O^2$. Irreversible cell injury was estimated by measuring lactate dehydrogenase (LDH) release and alterations in renal cell function were examined by measuring p-aminohippurate (PAH) uptake. Anoxia caused the increase in LDH release in a time-dependent manner. Glycine and glutathione almost completely prevented anoxia-induced LDH release. Of amino acids tested, glycine and alanine exerted the protective effect against anoxia-induced cell injury. However, asparagine with amide side chain, leucine and valine with hydrocarbon side chain, and basic amino acids (lysine, histidine, and arginine) were not effective. Anoxia-induced inhibition of PAM uptake was prevented by glycine. ATP content was decreased by anoxia, which was not affected by glycine. Anoxia-induced depletion of glutathione was significantly prevented by glycine. These results suggest that neutral amino acids with simple structure exert the Protective effect against anoxia-induced cell injury the involvement of specific interaction of amino acids and cell structure.

척수장애 성인남성의 신체측정치 분석 (Analysis of Body Size of Disabled Men with Spinal Cord Injuries)

  • Today the number of the disabled people with a spinal cord injury is increasing by the acquired factor. The purpose of the study is to furnish basic data for the study of satisfactory clothing construction data by considering the physical variation by the time passed the since the injury. Also the study includes the actual measuring of the disabled men with spinal cord injuries which is compared with the data of the 5th national anthropometric survey of Korea and makes it possible to grasp the distinguished physical feature of the disabled men with the spinal cord injury. As a result, first, the measured data are smaller than those of the 5th national anthropometric survey of Korea in the items of height and length of the upper half of the body related to stature. Second, the measured data are bigger than those of the 5th national anthropometric survey of Korea in the items of breadth, depth and circumference of the upper half of the body related to the exercise. Third, there are differences in the items of depth of the lower half of the body, breadth and circumference in the analysis of the time passed since the injury.

Experimental analysis of whiplash injury with hybrid III 50 percentile test dummy

  • In this study, the effects of sitting position of the driver on the whiplash neck injury have been analyzed experimentally by using hybrid III series 50 percentile male crash test dummy. A testing platform consisting of vehicle ground, driver foot rest, driver seat and a 3-point seatbelt has been prepared. This testing platform and the instrumented crash test dummy are prepared for tests according to the Euro NCAP whiplash testing protocol. The prepared test set-up has been exposed to 3 different acceleration-time loading curves defined in the Euro NCAP whiplash testing protocol by performing sled tests. 9 different sled tests have been performed with the combinations of 3 different seating positions of the crash test dummy and 3 different acceleration-time loading curves. The sensor data obtained from the crash test dummy and high-speed videos taken are analyzed according to the injury assessments criteria defined in the Euro NCAP whiplash testing protocol and the criticality of the whiplash injury is defined. It is seen that the backset distance of the driver head with the headrest and the height difference of the top of the head of the driver with the headrest have a great importance on whiplash injuries.

Time to Surgery and Injury Severity Score

  • Purpose: To evaluate the association between time to surgery and injury severity score (ISS). Methods: Medical charts and records were reviewed for polytrauma patients who underwent trauma surgery from November 2014 to March 2016. The patients were divided into two groups based on the ISS. Results: Among the 217 operated patients, 22 patients underwent first and second surgery. The patients with an ISS over 17 (mean 13.0 days) had a longer interval between surgeries than patients with an ISS of 17 or less (mean 7.5 days) (p=0.031). One hundred and twenty-one patients only underwent elective surgery and there is a positive correlation between ISS and time to elective surgery (p<0.028, Pearson's correlation coefficient=0.224). Seventy-four patients underwent emergent surgery only. Among these, the patients with an ISS of 17 or less underwent general surgery (86%) but the patients with an ISS more than 17 underwent neurological surgery (47%). Conclusion: Patients with high ISS need critical care during the preoperative and postoperative period.

척수손상환자의 보행에 영향을 주는 SCIM 요인 분석 (The Influence Factor Analysis of Spinal Cord Independence Measure(SCIM) on Walking in Spinal Cord Injury)

  • This study aims to the influenced factor analysis of spinal cord independence measure(SCIM), on walking velocity, walking endurance, time up & go(TUG), and subject characteristics. The subject of this study were 12 persons with incomplete spinal cord injury(ASIA C, D). All subject ambulatory with or without an assistive device. All participants were assessed on SCIM(score), walking velocity(m/s), walking endurance(m) and TUG(s). The data were analyzed using independent t-test and stepwise multiple regression. The results revealed that no statistical difference was noted in subject characteristics among SCIM, walking velocity, walking endurance, TUG(p>0.5). The independence score, breathing-sphincter control and ambulation were important factors in TUG(31.4%). The results suggest that SCIM may be an inappropriate assessment tool to predict gait ability of patient with incomplete spinal cord injury. Further study about gait speed, gait endurance and TUG by change of SCIM is needed using to patient of incomplete spinal cord injury.
