Surgical Treatment of Proximal Tibial Physeal Fracture and Avulsion Fracture of Tibial Tuberosity in a Immature Dog (미성숙견에서 경골 근위성장판 골절 및 경골결절 결출성 골절의 외과적 치료)

  • A 8 months old female dog with the combination of a fracture of the proximal tibia (Salter-Harris type I) with an avulsion of the tibial tuberosity was repaired with cross intramedullary pin for proximal tibial physeal fracture, and intramedullary pin combined with tension band wire for avulsion fracture of tibial tuberosity, resulted in complete healing. At 45 days after operation, on the radiological views, there was premature closure of growth plate of proximal tibia and tibial tuberosity, but at 7 months no developing growing deformities.

The Results of Primary Repair in Acute Injuries of the Posterior Cruciate Ligament (후방 십자 인대 급성 손상에서 일차 복원술의 결과)

  • Background: We analyze the functional evaluation, posterior stability after surgery of acute injuries in the femoral detachment and tibial avulsion fracture of posterior cruciate ligament. Materials and Methods: Twenty two patients who underwent primary repair were followed more than one year after operation (mean follow-up 33.7 months). The patients were evaluated with KT 2000 arthrometer and posterior stress test for posterior stability. Lysholm knee score was used for knee function. Results: Lysholm score revealed 85.3 in femoral detachment group and 91.1 in tibial avulsion fracture group (P<0.05). Posterior displacement of the posterior stress test showed 17.2 mm at initial injury and 8.6 mm at last follow up in femoral detachment group, 16.8 mm at initial injury and 7.1 mm at last follow up in tibial avulsion fracture group. There revealed the tendency of the improved posterior stability of the tibial avulsion group compared with femoral detachment group, but there showed no statistical significance in KT 2000 arthrometer and posterior stress x-ray. Conclusion: Functional results of tibial avulsion fracture group revealed the better outcome compared with femoral detachment group, but the degree of posterior stability in tibial avulsion fracture group showed no statistical significance in posterior stability.

Avulsion of the Tibial Attachment of the Posterior Cruciate Ligament in a 13-year-old child - A Case Report - (13세 소아에서 후방십자인대의 경골 부착 부위의 견열 골절 - 증례보고-)

  • The authors report one case of isolated avulsion fracture of the tibial attachment of the posterior cruciate ligament. A 13 years old boy was admitted for right knee pain and swelling after the soccer injury on his knee. Hemarthrosis and posterior instability were present. Simple roentgenographic examination showed an isolated avulsion fracture of the tibial attachment of the posterior cruciate ligament. Open approach through the popliteal fossa was used and bony fragment was fixed into its bed using pull-out sutures. Postoperatively, a long leg cast was applied for 4 weeks. The patient was followed for 6 months. He returned to his previous activity levels. There was no residual laxity. Isolated avulsion of the posterior cruciate ligament from the tibia before the epiphyseal fusion is very rare in children. We reported one case of our experience and its result.

  • A 4-month-old intact male Jindo dog was presented for left hindlimb lameness. On radiographs, a small mineralized piece was identified beneath the apophysis of tibia with no or minimal displacement of apophysis bilaterally. Subsequently, the cranioproximal bony aspect of the tibia changed radiolucent to radiopaque. The left side recovered after surgical management, but the right side was self-limited with conservative management. The diagnosis was partial avulsion fracture of the tibial tuberosity which has been called 'Osgood-Schlatter disease' in dogs.

  • Avulsion fractures of the intercondylar eminence of the tibia are uncommon. This kind of fracture is most commonly found in adolescents, with an increasing incidence for those involved in competitive athletics. Recently, an arthroscopic procedure is preferred to reduce the operation-related morbidity. Screws, K-wires, Suture anchor have been described as fixation devices. Cannulated screw fixation is a simple method, but this instrument has the risk of causing damage to the growth plate. The authors report good results using cannulated screw in arthroscopic treatment for pediatric anterior cruciate ligament avulsion fracture.

Arthroscopic Reduction and Fixation of an Anterior Cruciate Ligament Avulsion Fracture From the Tibial Eminence Using Bioabsorbable Pins - Technical Note - (생흡수성 핀을 이용한 소아 경골 과간부 견열 골절의 관절경적 정복 및 고정 방법 - 수술 술기 -)

  • Purpose: We describe a new and simple technique for arthroscopic fixation of tibial intercondylar eminence avulsion fracture using bioabsorbable pins in skeletally immature patients. Operative Technique: Diagnostic knee arthroscopy is performed using anterolateral and anteromedial portals. Fracture debris and blood clot are debrided to expose the injured site well. The fragment is reduced with the probe and fixed temporarily with a 1.1-mm diameter K-wire that is inserted percutaneously from the anterosuperior aspect of the knee joint. The drill guide is introduced into the joint and the fragment is secured by bioabsorbable, poly-p-dioxanone 1.3-mm pins inserted from different angles. The pins are 40 mm in length. The knee is placed in a long leg cast in extension for 4 weeks to assure that full extension is obtained. Conclusion: Arthroscopic fixation of an tibial intercondylar eminence avulsion fracture using bioabsorbable pins is not a technically demanding, suitable method that ensures fracture healing and restores the stability of the joint.

Tillaux Fracture in an Adolescent with a Trimalleolar Ankle Fracture (족관절 삼과 골절과 동반된 청소년기 Tillaux 골절)

  • The adolescent Tillaux fracture is an avulsion fracture of the anterolateral corner of the distal tibial epiphysis caused by external rotation of the foot. This type of fracture occurs during epiphyseal fusion in adolescence. A difficult to detect Tillaux fracture was discovered on a preoperative radiograph, such fractures have rarely been reported to accompany a predominantly adult-type ankle fracture. Especially, to the best of our knowledge, no case of a trimalleolar ankle fracture in an adolescent with a Tillaux fracture has been reported. Thus, we present a case of a trimalleolar ankle fracture in a 15-year-old adolescent with a Tillaux fracture who was successfully treated surgically.

  • Tibial eminence fracture is caused by distortion, excessive flexion and extension, varus and valgus injury of the knee joint in the form of avulsion fracture. A failure over the exact anatomical reduction of fragment can lead to instability and limitation of joint motion. Recently, a variety of arthroscopic assisted reduction and fixation technique have been used. In the tibial eminence fracture, we created an arthroscopic pull-out wire fixation technique using a cannulated screw that is easy and more convenient than in the conventional technique. So we report this technique with a review of current literatures.

Ankle Arthroscopy for Occult Injuries in the Acute Ankle Fracture (급성 족 관절 골절에서 잠재 손상에 대한 족 관절경)

  • Purpose : To identify occult intra-articular pathologies in the acute ankle fracture using arthroscopy and to investigate the factors to predict its possibility of occult injuries that could be occurred. Materials and Methods : This prospective study included fifty patients who got operation for the ankle fractures. Ankle arthroscopy was performed to document the type and anatomical location of occult lesion. We investigated the correlation between incidence of occult injuries and specific fracture type. Results : Of fifty ankles, 37 ankles had occult lesion. Twenty five loose bodies including displaced fragments were found and various cartilage lesions were found in 31 ankles. Tibial avulsion fragment by anterior inferior tibiofibular ligament was occurred in 6 cases. There was no correlation between the incidence of occult lesion and various factors including age, sex, injury mechanism and fracture type(p>0.05). Conclusion : The ankle arthroscopy had an effective role for the detection and treatment of occult injuries and it was difficult to predict the occurrence or associated accurately in all of th ankle fractures.
