Extremely Low Frequency Magnetic Field is an Environmental Stress Factor by Exerting Oxidative Stress

  • The previous study reported the biological effect of magnetic field exerted by acting on endocrine and anti-oxidant system. The present study aims to study whether ELF-MF (extremely low frequency magnetic field) affects the physiological endocrine systems such as thyroid and whether ELF-MF affects the defense system against oxidative stress when it alters the function of thyroid. Finally, we correlate the effects of MF on oxidative stress, and adrenal and thyroid with an environmental stress factor. We exposed sham or MF to rats for 5 or 25 days. After the exposure, we determined pain sensitivity, level of TSH, $T_3$ and free $T_4$ in plasma. We also assayed in whole brain, lipid peroxidation, the activity of enzymatic anti-oxidant defense including superoxide dismutase(SOD) and glutathione peroxidase (GPx), and non enzymatic defense such as reduced or oxidized glutathione contents. MF induced the hypersensitivity to thermal stimuli with the reduction of latency. $T_3$ and $T_4$ levels were also increased by the exposure of MF. In addition, we observed the rise of MDA level in rat brain by MF although the MF did not change superoxide dismutase and glutathione peroxidase activity. The effect of MF on both reduced and oxidized glutathione results in decrease in reduced or oxidized glutathione in whole brain. In every experiment, there was no significant difference in MF influence between short term (5 days) and long term (25 days) exposure. Taken together, MF exposure affects the thyroid hormonal control in brain. The elevated thyroid hormone acts on brain, leading to hyper-utilization of oxygen. This phenomenon may be correlated with oxidative stress resulting from MF exposure. In conclusion, we suggest that MF exposure may be an environmental stress by exerting oxidative stress.

임신과 갑상선 기능의 평가 (Evaluation of Pregnancy and Thyroid Function)

  • 갑상선의 기능적 발달 이전인 임신초기에 갑상선자극호르몬은 갑상선 장애 및 임신 동안 매우 민감한 지표이다. 임신동안 정상수준은 감소 변화를 보이는 것은 모체로부터 태아가 받는 갑상선호르몬의 영향이다. 갑상선자극호르몬과 유리 티록신의 농도는 임신동안 및 검사실간에 다양하게 보고된다. 갑상선자극호르몬의 참고치는 비임신군에 비하면 상한값과 하한값의 참고범위의 감소가 임신동안 보인다. 각 실험실은 임신 중에 다른 결과를 산출하는 호르몬 분석법과 일부 자체 기준에 맞는 혈중 농도를 산출한다. 따라서, 혈청 유리 티록신 분석에 사용되는 자동화 된 면역 분석법은 여전히 널리 사용되고 있지만 고려할 사항이 있다. 집단 기준, 삼분위 기간별 특이적 참고범위 사용은 문제해결의 최상의 방법이다. 임신 7~12주에 발생하는 호르몬의 하향은 7주 이전에는 보이지 않아 참고범위의 개별적 정량적으로 상승 및 하강을 보여준다. 본 연구에서는 집단별과 임신 특이적 갑상선 매개 변수의 참조범위에 대한 산출 중요성을 강조한다. 정밀의학 의 도래에 있어서 많은 개체의 유전적, 생물학적, 심리사회학적, 환경적 변수를 수집하고 분석이 필요하다. 이러한 논쟁을 해결하기 위해서는 다량의 전향적인 무작위 통제 연구가 필요하다.

정상한국인(正常韓國人) 및 각종갑상선질환(各種甲狀腺疾患)에서의 아킬레스 건반사시간(腱反射時間)에 관(關)한 연구(硏究) (A Study of Achilles Tendon Reflex in Normal Korean and Various Thyroid Diseases)

  • In an attempt to establish the diagnostic value of Achilles tendon reflex and to determine the normal value of Achilles tendon reflex time in normal Korean, the author measured the Achilles tendon reflex time by photomotograph. This study was carried out in 272 cases with various thyroid diseases and 340 normal Korean. 1) The Achilles tendon reflex time in normal Korean was like this, between 11 years old and 20 years old; male (62cases); $250{\pm}27$ msec, female (36 cases); $266{\pm}27$ msec, between 21 years old and 30 years old; male (38 cases); $271{\pm}27$ msec, female (21 cases); $284{\pm}27$ msec, between 31 years old and 40 years old; male (26 cases); $275{\pm}25$ msec, female (29 cases); $291{\pm}27$ msec, between 41 years old and 50 years old; male (20 cases); $286{\pm}35$ msec, female (24 cases); $307{\pm}42$ msec, between 51 years old and 60 years old, male (20 cases); $296{\pm}33$ msec, female (20 cases); $318{\pm}46$ msec, over 61 years; male (24 cases) $301{\pm}33$ msec, female (20 cases); $325{\pm}35$ msec. The Achilles tendon reflex time was delayed with increasing age and delayed in the female. 2) The Achilles tendon reflex time was markedly shortened to $221{\pm}20$msec in untreated hyperthyroidism. 3) The Achilles tendon reflex time was markedly delayed to $435{\pm}59$msec in hypothyroidism. 4) The Achilles tendon reflex time was not changed significantly in.other thyroid diseases with normal thyroid function. 5) The Achilles tendon reflex time showed good,correlationship with ETR, $T_3RU,\;^{131}I$ thyroid uptake and serum TSH. 6) Reproducibility of Achilles tendon reflex: time was good, and no significant difference between left and right was noted. 7) Diagnostic accuracy of Achilles tendon reflex time was 71% in hyperthyroidism and 90% in hypothyroidism. 8) The Achilles tendon reflex time showed useful test to evaluate the clinical course of the hyperthyroidism.

Synergistic interactions of Aegle marmelos leaf, Emblica officinalis fruit and Ocimum sanctum leaf extracts in the regulation of hyperthyroidism and / or hyperglycaemia

  • The effects of Aegle marmelos (Rutaceae) leaf, Emblica officinalis (Euphorbiaceae) fruit and Ocimum sanctum. (Labiateae) leaf extracts were studied in L-thyroxine (0.5 mg/kg) induced hyperthyroidic mice. Separately combined effects of these three plant extracts and of a commonly used antithyroidic drug, Propyl thiouracil (PTU) were investigated for comparison. Serum concentration of thyroxine $(T_4)$, triiodothyronine $(T_3)$, glucose and the activity of hepatic Glucose 6-Phosphatase (G-6-Pase) were considered as main parameters. Hepatic lipid peroxidation (LPO), superoxide dismutase (SOD) and Catalase (CAT) activities were also studied to reveal the toxic effect of the plant extracts, if any. While exogenous $T_4$ enhanced serum concentration of $T_4$, $T_3$, glucose and the activity of hepatic G-6-Pase, a simultaneous administration of either A. marmelos leaf (1.0 mg/kg), E. officinalis fruit( 30 mg/kg) and O. sanctum leaf (50 mg/kg) extracts, to hyperthyroidic animals decreased all these parameters. However, the effects were more pronounced, as nearly normal thyroid function and serum glucose concentration were exhibited when all three plant extracts were administered together. A decrease in LPO and a concomitant increase in SOD and the CAT activities indicated the safe and antiperoxidative nature of the plant extracts, administered either alone or in combination. Our findings reveal that the three test plant materials exhibit synergistic effects without any hepatotoxicity, suggesting their potential use in the amelioration of hyperthyroidism and/ or hyperglycaemia.

Nonautoimmune congenital hyperthyroidism due to p.Asp633Glu mutation in the TSHR gene

  • Most cases of congenital hyperthyroidism are autoimmune forms caused by maternal thyroid stimulating antibodies. Nonautoimmune forms of congenital hyperthyroidism caused by activating mutations of the thyrotropin receptor (TSHR) gene are rare. A woman gave birth to a boy during an emergency cesarean section at 33 weeks of gestation due to fetal tachycardia. On the 24th day of life, thyroid function tests were performed due to persistent tachycardia, and hyperthyroidism was confirmed. Auto-antibodies to TSHR, thyroid peroxidase, and thyroglobulin were not found. The patient was treated with propylthiouracil and propranolol, but hyperthyroidism was not well controlled. At 3 months of age, the patient had craniosynostosis and hydrocephalus, and underwent a ventriculoperitoneal shunt operation. Direct sequencing of the TSHR gene showed a heterozygous mutation of c.1899C>A (p.Asp633Glu) in exon 10. No mutations were discovered in any of the parents in a familial genetic study. We have reported a case of sporadic nonautoimmune congenital hyperthyroidism, by a missense mutation of the TSHR gene, for the first time in South Korea.

방사성동위원소옥소(放射性同位元素沃素)($^{131}I$)의 치료량(治療量)이 갑상선종환자(甲狀腺腫患者)의 혈액상(血液像)에 미치는 영향(影響)에 관(關)한 연구(硏究) (A study on the Hematologic Effects of Radioactive Iodine($^{131}I$) Therapy on Various Thyroid Diseases)

  • To clarify the hematologic effects of the radioiodine ($^{131}I$) in therapeutic doses ($5{\sim}10$ mCi) on the various thyroid patients, authors studied the peripheral blood pictures of 396 goitrous patients before and after radioiodine ($^{131}I$) administrations in the Isotope Clinic of Seoul National University Hospital. Among these 396 cases of goiters, we gave 5 to 10 mCi of radioiodine ($^{131}I$) with single or fractionated administrations. The blood pictures of peripheral blood were repeated after 3 months in 40 cases of 65 cases who had been treated with $^{131}I$. The blood pictures of non-treated thyroid patients were compared with that of normal Korean values to clarify any difference between normal and goiter. The blood pictures of hyperthyroid patients treated with $^{131}I$ therapy were compared with the blood pictures of non-treated thyroid patients. The results were as following: 1) The incidence according to type: Toxic diffuse goiter: 35.4% Nontoxic nodular goiter: 29.7% Euthyroid: 13.8% Nontoxic diffuse goiter: 12.6% Hypothyroidism: 4.3% Thyroiditis($\bar{s}$ subacute form): 1.8% Toxic nodular goiter: 1.4% Malignancy: 1.0% 2) Age incidence: The range of distribution was 11 to 71 years. The peak incidence was found in the 4th decade of life. $80.6{\sim}82.6%$ of those 396 cases were found among the 3rd, 4th and the 5th decades of life. 3) Sex incidence: Sex ratio of male:female was 1:7.8. 4) The most outstanding findings in peripheral blood before treatment were decreased erythrocyte count and hemoglobin value in all types of thyroid diseases, especially in. the cases of hypothyroidism and thyroiditis. Hook worm-infested patients showed no significant difference in erythrocytes and hemoglobin values from those of other hook worm free patients. 5) Total leukocytes count was within normal range. Differential count of W.B.C. showed increased percentile of lymphocyte in diffuse toxic goiter and thyroiditis. 6) 39 cases of diffuse goiter treated with $^{131}I$ toxic showed amelioration in the anemia and restoration to normal range of lymphocyte count in association with increased percentile of neutrophiles 3 months after administration, except a case of toxic nodular goiter. One can observe anemia in slight degree, and increased lymphocytes count in hypothyroidism. Therapeutic dose of radioiodine ($^{131}I$) does not result any residual effect on the hematopoietic function. Radioiodine ($^{131}I$) therapy resulted in improvement of thyroid function in association of amelioration of pevious abnormal blood pictures. 7) Authors did not observe any myxedema resulted from radioiodine therapy during the 3 months period in this study.

  • 본(本) 실험(實驗)은 thyroxine 및 propylthiouracil의 투여(投與)에 의(依)한 갑상선(甲狀腺)의 기능변화(機能變化)와 아울러 체성장(體成長)에 미치는 영향(影響)을 구명(究明)코저 실시(實施)하였는 바, 25일령(日齡)의 암 흰쥐를 갑상선척출군(甲狀腺剔出群), propylthiouracil 처리군(處理群), Thyroxine 처리군(處理群) 및 정상대조군(正常對照群)의 사개군(四個群)으로 나누어 배치(配置)하고 각군(各群)을 처리후(處理後) 시간경과(時間經過)에 따라 도살(屠殺)하여 갑상선(甲狀腺)의 중량측정(重量測定) 및 조직학적(組織學的) 검색(檢索)과 아울러 체중(體重)의 변화(變化)를 조사(調査)하였다. 1. 갑상선(甲狀腺)의 중량(重量)은 모든 관찰시간(觀察時間)에서 비교군간(比較群間)에 유의성(有意性)이 인정(認定)되었는데 PTU 처리군(處理群)은 정상대조군(正常對照群)에 비(比)하여 높은 중량(重量)을 나타냈고, Thyroxine 처리군(處理群)은 낮은 값을 나타냈다. 2. 갑상선(甲狀腺)의 조직상(組織像)은 PTU 처리군(處理群)에서는 처리(處理) 1주후(週後)부터 여포상피세포(濾胞上皮細胞)가 증식(增殖) 비대(肥大)를 동반(同伴)하는 원주화현상(圓柱化現象)을 나타내기 시작하여 실험기간(實驗期間)이 경과(經過)될 수록 더욱 진행(進行)되는 조직소견(組織所見)을 보였으며, Thyroxine 처리군(處理群)에서는 여포상피세포(濾胞上皮細胞)가 도평화(屠平化)되는 퇴행성(退行性) 변화(變化)가 인지(認知)되었다. 3. 체중(體重)은 처리(處理) 2주후(週後)부터 비교군간(比較群間)에 유의성(有意性)이 인정(認定)되는 변화(變化)를 보였는데 PTU 처리군(處理群)과 갑상선척출군(甲狀腺剔出群)은 정상대조군(正常對照群)에 비(比)하여 낮은 값을 나타냈고 Thyroxine 처리군(處理群)은 높은 체중치(體重値)를 보였다. 그러나, 다중검정결과(多重檢定結果) PTU 처리군(處理群)과 갑상선척출군(甲狀腺剔出群) 상호간(相互間)에는 유의성(有意性)이 인정(認定)되지 않았다.

개심술 후 갑상선 호르몬치 변화에 대한 연구 (Alterations in Thyroid Hormone Levels After Open Heart Surgery)

  • 갑상선 호르몬은 체내에서 기초대사량을 증가시키고, 교감신경 흉분작용을 나타내며,심장에 대해서는 심근수축력, 심박수, 심근 산소소모량 등에 영향을 미친다. 개심술과 같은 대수술시나 심한 전신질환이 있을 때 흔히 갑상선 호르몬치의 변화를 보이게 되는 바 본 연구에서는 인공 체외순환이 갑상선 기능에 어떤 영향을 미치는지, 그리고 변화된 갑상선 기능이 개심술후 회복에 영향을 미치는 지에 대해 알아보고자 하였다. 1995년 4월부터 1996년 2월까지 전남대학교 의과대학 흉부외과학교실에서 인공 체외순환 하에 개심 술을 받은 20명의 환자를 대상으로 갑상선 기능을 평가하였다. 대상 환자 모두 술전에 갑상선 질환을 앓은 병력이나 갑상선 기능에 영향을 미칠 수 있는 어떤 형태의 시술, 검사, 및 약물치료를 받은 적은 없었다. 대상 환자를 두 그룹으로 나누었으며, 술후 회복에 특별한 문제가 없었던 환자군(Group 1, n=10), 술후 합병증(호흡부전, 저심박출증, 다장기 기능부전 등)이 발생한 환자군(Group 2, n=10)으로 강심제를 고용량으로 3일 이상 유지하였거나, 4일 미상 중환자실 체류를 필요로 한 경우로 하였다. 갑상선 기능의 평가는 말초\ulcorner액에서 측정한 triiodothyronine(73), thyroxine(74), free thyroxine(n4), 및 thyroid stimulating hormone(TSH) 농도를 기준으로 하였다. 인공 체외순환이 갑상선 호르몬 변화에 미치는 영 향을 알기 위해 술전 검사치와 비교분석하였으며, 갑상선 호르몬의 변화가 술후 예후에 미치는 영향을 알아보기 위해 두 그룹간의 검사치를 비교 분석하여 보았다. 혈청 T)는 인공 체외순환 후 10분에 최저 치(30.05*17.5ng/dl, p(0.001)로 감소하였다가 점차 증가하였으나 전체적으로 술전에 비해 의미 있게 낮았으며(p<0.05), 혈청 T4, fT4, TSH는 다소간의 변동을 보여, 체외순환을 사용한 개심술이 갑상선 기능에 변화를 초래하며, 모두 그룹간의 비교에서도 혈청 T3는그룹 1에 비해 그룹2에서 의미 있게 낮았으며(P<0.05), 술후 4일째 그룹 1에서는 정상수준으로 회복되었으나 그룹 2에서는 정상수준으로 회복되지 못한 점으로 미루어 이처럼 변화된 갑상선 기능이 술후 환자의 회복에 영향을 미침을 알 수 있었으며, 이는 집중 치료 중인 가사 상태의 환자에서 흔히 볼 수 있는 "euthyroid sick syndrome"과 유사한 양상이었다. 이상의 결과로 인공 체외순환이 갑상선 기능을 변화시키며, 갑상선 기능의 변화가 술후 환자의 예후에 영향을 미칠 수 있음을 시사하였다. 앞으로 더욱 정선되고 선택된 경우에 대한 많은 연구와 더불어 다음 단계로 갑상선 호르몬(73)의 투여가 실제로 술후 환자의 회복에 관련이 있는 지의 연구가 필요하리라 사료된다.요하리라 사료된다.

한국의 건강검진 현황 (Current Status of Health Screening in Korea)

  • Along with a development of medical technology, a variety of tests, such as laboratory tests, x-ray and endoscopies are being used in health screening tests. As the tests determine the quality of health screening, test items and methods should be carefully selected. This study was to get hold of the test items of major health screening programs in Korea Most of the health screening programmes focused upon detection of risk factors and diagnosis of life - style related diseases (diabetes, hypertension, cardiovascular diseases, hypercholesterolemia, overweight, drinking, smoking, cerebrovascular diseases, osteoporosis) ,cancers (stomach, cervix, lung, breast, liver, colon, prostate, ovary, pancreas, thyroid, esophagus) , infectious diseases (hepatitis, tuberculosis, sexually-transmitted diseases, parasites) , chronic obstructive respiratory diseases, chronic renal diseases (bacteriuria hematuria, proteinuria) , anemia, glaucoma, hearing loss, Alzheimer disease, stress, early psychiatric diseases. The health screening tests were basic physical examination, basic laboratory tests (CBC, urinalysis, liver function tests, lipid tests, glucose, HbAlc, uric acid, electrolytes, serological tests (HBsAg, HBs-Ab, HCV-Ab, HIV-Ab, VDRL) EKG, x-ray (chest PA, CT) , endoscopy (gastroscopy, colonoscopy) , sonography(abdomen, thyroid, pelvis, breast) ,cytology (cervix) , bone density, tumor markets (NMP22, alpha-FP, CEA, CA-19-9, CA12S, PSA) and eye tests. Advanced technologies, like CT, PET, MRI, MRT/Angio, molecular testings) were widely used in hospital health screening programmes .In summary, a variety of tests were utilized in health screening in Korea. Those tests were utilized by stages or according to sex and age in most of health screening programmes, however a few program used tests to excess disregarding health screening subjects.
