A Case of Anaplastic Carcinoma Arising Ectopic Thyroid (이소성 갑상선에 발생한 역형성 암종 1예)

  • Ectopic thyroid glands generally occur in the midline as a result of abnormal median migration, and their presence lateral to the midline is extremly rare. We report a 83-year-old male with anaplastic carcinoma admix papillary carcinoma in the extrathyroid area. We suspected left lateral neck metastasis on preoperative fine needle aspiration result and computed tomography. the patient underwent total thyroidectomy and left selective neck dissection(level II, III, IV, V). The patient was diagnosed as having an ectopic thyroid gland on the lateral neck with anaplastic carcinoma admix papillry carcinoma. The patient is alive without incident of tumor recurrence at 5month after surgery and radiotherapy.

  • The major limitation of fine needle aspiration of the thyroid is in the evaluation of follicular tumors. it may be difficult or occasionally, impossible to distinguish on a cytologic basis, between hyperplastic nodular getter, follicular adenoma and well differentiated follicular carcinoma. We reviewed cytologic presentations of 15 histologically confirmed follicular neoplasms of the thyroid. Aspiration smears of 6 follicular adenomas were cell-rich with ball-like or syncytial aggregates of monotonous follicular cells, in contrast to honeycomb-like flat sheets of adenomatous goiter Mild nuclear pleomorphism and a small visible nucleolus were noted in 2 cases, respectively. Nine cases of follicular carcinoma showed very high cellularity, irregularly shaped cell clusters, and numerous isolated tumor cells. Nuclear pleomorphism and visible nucleoli were noted in 6 and 5 cases, respectively. Another characteristic finding, entrapped endothelial cells within the ceil clusters, was noted in 7 carcinomas and 1 adenoma. The background of the smears of all follicular neoplasms was hemorrhagic, with no or scanty colloid. We confirmed that the cytologic features of follicular adenoma and carcinoma were similar in general, with subtle differences in cell morphology, but the presence of isolated tumor cells and entrapped endothelial cells was suggestive of malignancy.

Posterior Mediastinal Goiter - A Case Report - (후종격동 갑상선종;1례 보고)

  • Posterior mediastinal goiter extending to carotid sheath posteriorly is rare repoted case. Recently we experienced huge posterior mediastinal goiter with compressin of trachea in 57-year old male, The patient that complained of dyspnea referred to our hospital for further evaluation of mediasitnal tumor. We confirmed huge secondary posterior mediastinal intrathoracic goiter with diagnostic methods following by chest X-ray, thyroid scan, chest CT, and CT guided fine needle aspiration biopsy in this patient, and performed operation for excision Exision of posterior mediasitnal goiter performed through initial transeverse cervical incision and additional median sternotomy, and the mass removed completely without any complications. The postoperative course were uneventful.

  • Papillary thyroid carcinoma (PTC) is the most prevalent type of thyroid cancer. In contrast, thyroid lymphoma is a very rare disease. Concurrent onset of both is very rare in the thyroid gland. Ultrasound (US)-guided Fine needle aspiration (FNA) is a useful diagnostic tool, but occasionally pathology results may change after the surgery. A 56 years old woman visited with Hashimoto's thyroiditis and nodule on the thyroid gland isthmus on US exam. US-guided FNA was performed at thyroid nodule and diagnosed as PTC. The patient underwent total thyroidectomy. The pathological findings revealed a mucosa associated lymphoid tissue (MALT) lymphoma accompanied with PTC. Authors report this unusual case with a review of literature.

Simultaneous Occurrence of Medullary and Papillary Thyroid Carcinoma with Lymph Node Metastasis: A Case Report (림프절 전이를 동반한 갑상선에 동시 발생한 수질암과 유두상 암종 1례)

  • Simultaneous occurrence of medullary and papillary thyroid carcinomas in the same gland is very rare. In fact. there are only 18 cases of simultaneous occurrence of medullary and papillary thyroid carcinomas in the literature. We report a case of simultaneous medullary and papillary carcinoma of thyroid gland. A 67-year-old woman was diagnosed with medullary carcinoma of right lobe of thyroid gland and papillary carcinoma of left lobe of thyroid gland by fine needle aspiration cytology. Total thyroidectomy, anterior neck dissection, bilateral modified radical neck dissection and tracheotomy was undertaken. The tumor metastasized to regional lymph node and extrathyroidal muscle invasion of left papillary carcinoma was also revealed by pathological report. This report describes a case of thyroid carcinoma that demonstrated both medullary carcinoma and papillary components in the thyroid with lymph node metastasis.

Clinical implications of anti-thyroglobulin antibody measurement before surgery in thyroid cancer

  • Thyroglobulin antibody (TgAb) is a class G immunoglobulin and a conventional marker for thyroid autoimmunity. From a clinical perspective, TgAb is less useful than thyroid peroxidase antibodies for predicting thyroid dysfunction. However, TgAb is found more frequently in differentiated thyroid cancer (DTC) and can interfere with thyroglobulin (Tg) measurements, which are used to monitor the recurrence or persistence of DTC. Recent studies suggested a small but consistent role for preoperative TgAb in predicting DTC in thyroid nodules, and in reflecting adverse tumor characteristics or prognosis, including lymph node metastasis, but this is still controversial. Postoperative TgAb can serve as a biomarker for remnant thyroid tissue, so follow-up measures of TgAb are useful for predicting cancer recurrence in DTC patients. Since high serum TgAb levels may also affect the fine needle aspiration washout Tg levels from suspicious lymph nodes of DTC patients, it is important to use caution when interpreting the washout Tg levels in patients who are positive for TgAb.

A Case of Mixed Papillary Thyroid Tumor and Squamous-Cell Carcinoma (갑상샘 유두암과 혼재하여 발견된 편평세포암 1예)

  • The occurrence of a mixed tumor containing papillary thyroid carcinoma (PTC) and primary squamous-cell carcinoma (SCC) is rare because there is no squamous epithelium in the thyroid gland. Reported herein is a 30-year-old female with mixed PTC and primary sec of the thyroid presented as thyroid incidentaloma. Fine-needle aspiration biopsy of the thyroid nodule revealed the presence of malignant thyroid cells. The histopathological examination following total thyroidectomy yielded two mixed, morphologically distinct histotypes that included PTC and sec. After total thyroidectomy, the patient underwent radioactive iodine therapy. No recurrence or metastasis occurred during the 20-month follow-up period after the operation.

Lateral Neck Cyst as the Initial Presentation of Papillary Thyroid Carcinoma (측경부 낭종으로 발현된 유두상 갑상선암)

  • We have experienced 3 cases of papillary carcinoma of the thyroid gland seen as lateral neck cyst, Usually cystic lesions of the neck have been considered as benign lesions from clinical viewpoint. The usual differential diagnosis includes branchial cleft cyst, cystic hygroma, dermoid cyst, cold abscess(tuberculous lymphadenitis) and cavitating squamous cell carcinoma. A lateral neck cyst as the sole presenting complaint of the papillary thyroid carcinoma is very rare. Preoperative diagnosis of lateral neck cyst is often diagnostic dilemma. Fine-needle aspiration cytology can be helpful in detecting the cancer cells and in demonstrating the nature of the fluid component of the lateral neck cyst. Presence of brown murky fluid from the aspirated fluid is highly suggestive of thyroid carcinoma. The thyroid scans and B-mode ultrasonography mayor may not be helpful to detect the primary focus of the thyroid gland. We suggest that a patient with a lateral neck cyst in adult age group should be considered to be a possibility of underlying thyroid carcinoma presanting as lateral neck cyst.
