측경부 낭종으로 발현된 유두상 갑상선암 (Lateral Neck Cyst as the Initial Presentation of Papillary Thyroid Carcinoma)

  • We have experienced 3 cases of papillary carcinoma of the thyroid gland seen as lateral neck cyst, Usually cystic lesions of the neck have been considered as benign lesions from clinical viewpoint. The usual differential diagnosis includes branchial cleft cyst, cystic hygroma, dermoid cyst, cold abscess(tuberculous lymphadenitis) and cavitating squamous cell carcinoma. A lateral neck cyst as the sole presenting complaint of the papillary thyroid carcinoma is very rare. Preoperative diagnosis of lateral neck cyst is often diagnostic dilemma. Fine-needle aspiration cytology can be helpful in detecting the cancer cells and in demonstrating the nature of the fluid component of the lateral neck cyst. Presence of brown murky fluid from the aspirated fluid is highly suggestive of thyroid carcinoma. The thyroid scans and B-mode ultrasonography mayor may not be helpful to detect the primary focus of the thyroid gland. We suggest that a patient with a lateral neck cyst in adult age group should be considered to be a possibility of underlying thyroid carcinoma presanting as lateral neck cyst.

  • Cystic lesions of the neck are frequently encountered on clinical basis. The usual differential diagnosis includes branchial cleft, cystic hygroma, dermoid cyst, tuberculous lymphadenitis, benign detached goiterous thyroid cyst, and Cavitation epidermoid carcinoma. Most of cysts are benign and malignant cysts are rare. The most common form of cystic malignancy in the lateral neck is cystic degeneration of epidermoid carcinoma metastatic to cervical lymph nodes. Cystic neoplasia may result from malingnant changes occurring within the wall of a previously benign cyst Complete head and neck examination on the primary focus is important Especially, when palpation of thyroid is negative, thyroid carcinoma is easily overlooked. Recently, we experienced papillary carcinoma of the thyroid seen as lateral neck cyst. So we report this case with review of literatures.

  • Thyroglossal duct cyst is the most common congenital neck mass. It develops from remnants of precursors of thyroid gland left behind during embryologic descent form the foramen cecum at the tongue base into the anterior neck during fetal development. An anterior midline neck mass presenting before the age of twenty and displaying vertical movement with tongue protrusion and swallowing is characteristic of this lesion. In this paper, we report on a case of TGDC without remnant duct that is presenting as thyroid cyst.

  • Lingual thyroid is the term applied to a mass of ectopic thyroid tissue located on the base of the tongue in the midline. It may be found anywhere between the circumvallate papillae and the epiglottis. It is believed to be caused by developmental anomalies involving the descent of the embryologic gland anlage from its position posterior to the tuberculum impar to its normal pretracheal location between week 3 and week 7 of embryologic development. Differential diagnosis of the lingual thyroid would include lingual tonsillar hypertrophy, vallecular cyst, thyroglossal duct cyst, epidermal cyst, lymphoma. Lingual thyroid is found in approximately 1 in 100,000 people, and affected individuals have no other thyroid tissue in 70% to 100% of cases. Recently, we have experienced a case of lingual thyroid with mild dysphagia in a 48-year-old male. Now we report the case with literature review.

비기능성 부갑상선 낭종 1예 (Non Functioning Parathyroid Cyst : A Case Report)

  • Parathyroid cyst are rare lesion of the neck and superior mediastinum. They are classified as either functional or nonfunctional based on the presence or absence of hyperparathyroidism. They typically present as a palpable neck mass, or as an incidental finding during neck surgery. So, it must be included within the differential diagnosis of a neck lump. A 48-year old woman was admitted to our hospital for evaluation of left anterior neck mass. Physical finding showed soft, movable, non-tender mass on the lower left third of the neck. Laboratory findings revealed normal thyroid function and normal serum calcium level. On previous history, patient underwent two times sono-guided fine needle aspiration at local clinic, and about 10cc clear watery aspirate was noted. Computerized tomography findings showed non-enhancing hypodense cystic lesion on left thyroid area with tracheal deviation to right side. Preoperative diagnosis was thyroid cyst, and thyroidectomy was planed. Intra-operative finding showed huge cystic mass occupying the left thyroid area and smooth, shiny, semitransparent thin cyst wall was noted, which was loosely attached to the thyroid. The cyst was easily dissected free from the thyroid and surrounding tissues. After cyst removal, pathological study confirmed as a parathyroid cyst.

  • Ectopic thyroid gland is a defined as thyroid tissue located in a site other than its usual pre-tracheal location. Ultrasound examination, CT scan, thyroid scan, and thyroid iodine uptake test are all valuable modalities for a differential diagnosis. Although most of cases are asymptomatic, treatment is indicated when there are symptoms related to progressive growth of mass. This article reports a ectopic thyroid cyst in the anterior neck that was similar to deep neck infection accompanied by acute airway obstruction.

갑상설관 낭종에서 발생한 유두상 선암 1예 (A Case of Papillary Thyroid Carcinoma Arising in Thyroglossal Duct Cyst)

  • Thyroglossal duct cyst is common midline mass, which is related to hyoid bone and may show signs of inflammation. Carcinoma arising in the thyroglossal duct cyst is rare, occuring in less than 1% of thyroglossal duct cyst. Papillary adenocarcinoma is the most common histologic type(75-85%). The initial treatement of choice is wide excision of the tumor bearing tissue(Sistrunk procedure), resection of associated lymph node alone is enough when they are small and isolated, but a modified neck dissection must be done if regional involvement is more extensive. Its prognosis is excellent(the incidence of regional lymph node metastasis is 7% compare to 89% for papillary cancer of thyroid gland proper).

제2형 새열 낭종으로 오인된 전이성 갑상선 유두상 암종 1예 (Metastatic Papillary Thyroid Carcinoma Masquerading as Type II Branchial Cleft Cyst : A Case Report)

  • In case of lateral cervical cystic lesions, the differential diagnoses include branchial cleft cyst(BCC), teratoma, dermoid, hemangioma and lymphangioma etc. But sometimes metastatic cystic lymph nodes may exist in lateral neck. In such circumstance, the primary lesions are known to stem from oropharynx, nasopharynx, salivary and thyroid gland etc. A-66-year-old-male came to our clinic, due to the lateral cervical mass for 5 years. We performed the neck CT, sonography and sono-guided FNAC. He was initially diagnosed with the benign cyst such as BCC. We performed the excisional biopsy on left level II, but the pathologic report was revealed as metastatic papillary thyroid carcinoma(PTC). And then he received the total thyroidectomy with neck dissection. The final diagnosis was cystic metastasis from PTC. We learn a valuable lesson form this case in the following. Even if the simple cervical cyst is presumed with radiology and clinical pattern, more careful considerations on the basis of history and radiologic findings are mandatory.

  • 아가미틈새(branchial cleft) 낭종은 흔히 경부측면에 위치하나, 아가미틈새 낭종의 조직학적 소견을 보이는 낭종이 비전형적인 위치에서 발견되기도 한다. 갑상샘의 아가미틈새양(branchial cleft-like) 낭종은 이제까지 보고된 14례 중 7세 여아에서 발견된 1례를 제외하고는 모두 성인에서 발견되었다. 저자들은 좌측경부의 종물을 주소로 내원한 신생아에서 초음파 검사, 컴퓨터 단층촬영, 병리조직학 검사를 통하여 진단된 갑상샘의 아가미틈새양 낭종 1례를 경험하였기에 보고하는 바이다.

  • An intrathyroid thyroglossal duct cyst (TGDC) presented as an anterior neck mass in a 62-year-old male without history of prior thyroid disease or infection. This cyst was clinically indistinguishable from a thyroid nodule. In addition to that, fine needle aspiration cytology revealed normal-looking squamous cells. Diagnosis, work-up, management approach, and treatment of intrathyroid TGDC are discussed as well as distinguishing features between intrathyrod TGDC with tract and without tract. This is the very rare case of intrathyroid TGDC in Korean population and the possibility of intrathyroid TGDC should remain in the differential diagnosis of thyroid nodule.
