흉선종의 흉막전이에 동반된 Group D Salmonella 농흉 및 심낭염 -1예 보고- (Empyema and Pericarditis by Salmonella Group D Complicating Malignant Thymoma with Pleural Metastasis)

  • Non-typhoid salmonella감염질환은 악성 종양, 당뇨병, 스테로이드의 장기사용 등 면역 기능이 저하된 환자에서 흔히 균혈증과 동반되어 드물게 보고되고 있다. 그중에 non-typhoid salmonella에 의한 농흉과 심낭염의 합병 발생은 극히 드물다. 저자들은 흉막 전이가 동반된 악성 흉선종 환자에서 Group D salmonella 감염으로 인한 농흉과 심낭염이 합병되었던 예를 치험하였기에 문헌 고찰과 함께 보고한다.

  • Pure red cell aplasia (PRCA) and hypogammaglobulinemia are paraneoplastic syndromes that are rarer than myasthenia gravis in patients with thymoma. Good syndrome coexisting with PRCA is an extremely rare pathology. We report the case of a 50-year-old man with thymoma and PRCA associated with Good syndrome who achieved complete PRCA remission after thymectomy and postoperative immunosuppressive therapy, and provide a review of the pertinent literature.

  • 전종격동 하부에 심장과 연접한 이소성 흉선종은 폐암뿐 아니라 흉선종, 생식세포암, 림프종, 전이성 종양 등의 여러 질환과 감별이 필요하다. 병리조직상으로는 이소성 흉선종에 대한 주의가 없는 경우, 미분화상피암, 암성병변의 림프절 전이, 또는 림프구가 우세하게 관찰되는 경우 림프종 등으로 오진될 수도 있으므로 감별진단에 있어 주의가 필요하다. 또한 낭성 흉선종은 이러한 감별진단을 더욱 어렵게 한다. 저자들은 하부 종격동에 심장 우측과 연접하여 발생한 이소성 낭성 흉선종 환자 1예를 경험하였기에 보고하는 바이다.

  • Objective: Improvement of myasthenia gravis through Ortho-Cellular Nutrition Therapy (OCNT). Methods: A 52-year-old patient with myasthenia gravis was prescribed 21 different OCNT treatments, including Cyaplex. Results: Following the administration of OCNT, symptoms such as eyelid ptosis, masticatory dysfunction, diplopia, and fatigue showed improvement. Conclusion: OCNT can assist in alleviating and mitigating symptoms for patients suffering from thymoma and myasthenia gravis.

흉선종의 수술적 치료 및 예후인자의 평가 (Surgical Treatment and Assessment of Prognostic Factors of Thymoma)

  • 1981년 1월부터 1994년 12월까지 흉선종으로 세브란스병원에서 수술한 65명의 환자를 대상으로 하였다. 환자중 28명(43.1%)이 중증근무력증을 동반하고 있었으며 마사오카 병기에 의하면 1기 28명(43. 11%), 2기 13명 (20.0%), 3기 22명 (33.8%), 4기a와 4기b가 각각 1명 (1.5%) 이었다. 수술사망은 없었고 완전절제가 48명 (73.8%)에서 가능했으며 수술 후 보조치료는 10명에서 시 행하여졌다. 병리학적분류로 24.6%가 상피세포형, 27.7%가 림파구형, )3.9%가 혼합형이 었다. 5년과 10년 생존율은 각각 87%, 82% 이었으며 흉선종의 수술 후 예후인자는 종양의 국소침윤여부, 종양의 완전절제여부, 흉선암, 병기, 중증 근무력증의 동반 여부였다. 1기 흉선종 환자에서는 재발이 없었다.

중증 근무력증의 외과적 치료 (Surgical treatment of myasthenia gravis)

  • Myasthenia gravis is a rare condition, affecting only 1/75000. It is characterized by weakness and fatigue of the voluntary muscles due to impaired neuromuscular transmission at the acetylcholine receptor site. It is probably caused by an autoimmune mechanism leading to reduction of the available nicotinic acetylcholine receptors at the neuromuscular junction. A relationship between the thymus and myasthenia gravis was suggested by Weigert in 1901, and Blalock was the first to report the efficiency of thymectomy in the treatment of myasthnia gravis in 1939. Since that time, thymectomy has become increasingly important in managing patients with myasthenia gravis. Fourteen patients with myasthenia gravis underwent thymectomy between September 1987 and March 1994 and got the following results; 1. The sex distribution was 10 females and 4 males with ages ranging from 15 to 51 years[mean, 34.8 years]. 2. Clinical manifestation of ocular symptoms were seen in all cases, dysphagia in 12[85.7%], extremity weakness in 11 [78.6 %], and dyspnea in 3 [21.4 %]. 3. According to the modified Osserman`s classification, 8 patients were in group IIb, 3 in IIa, 2 in I and 1 in IIc, respectively. 4. Histopathologic examination of the resected thymuses revealed hyperplasia in 8 patients, benign thymoma in 3, and malignant thymoma in 3. 5. There were no perioperative or immediate postoperative deaths but one patient died about two months after the thymectomy due to respiratory failure. 6. During the follow-up period, 11 patients [78.6 %] showed clinical improvement. Among them, 4 cases [28.6 %] showed complete remission, 3 [21.4 %] showed marked improvement, and 4 [28.6 %] showed subjective improvement. 7. Among the 8 patients with thymic hyperplasia, clinical improvement was noticed in 7 patients 5 %]. Among the 6 patients with thymoma, 4 patients [66.7 %] showed improvement.

Tc-99m pertechnetate/Tl-201 감영 신티그라피에서 부갑상선 선종으로 오인된 흉선종 1예 (A Case of Thymoma Misdiagnosed as Parathyroid Adenoma on Tc-99m pertechnetate/Tl-201 Subtraction Scintigraphy)

  • A large superior mediastinal mass was found incidentally by ultrasonography in a 60-year-old man. There was an abnormal accumulation of Tl-201 in the lower pole of left thyroid gland, extending into left superior mediastinum on Tc-99m pertechnetate/Tl-201 subtraction scan. Laboratory findings relating thyroid and parathyroid were all within normal range. We considered the mass as a non-functioning parathyroid adenoma tentatively. However, subsequent surgery and pathologic examination revealed the mass to be a benign mixed thymoma. We report a case of patient with thymoma showing unusual Tc-99m pertechnetate/Tl-201 subtraction imaging and laboratory findings, and suggest to consider the possibility of other mediastinal tumors rather than parathyroid adenoma.

An Unusual Case of Superior Vena Cava Syndrome Caused by the Intravascular Invasion of an Invasive Thymoma

  • Superior vena cava syndrome (SVCS) is usually caused by extrinsic compression or invasion of the superior vena cava (SVC) by malignant tumors involving mediastinal structures. Although thymomas are well-known causes of SVCS, cases of SVCS caused by malignant thymomas protruding into adjacent vessels draining the SVC with thrombosis have been very rarely reported worldwide. We experienced a 39-year-old female patient with SVCS that developed after the direct invasion of the left brachiocephalic vein (LBCV) and SVC by an anterior mediastinal mass with a high maximum standardized uptake value on the chest computed tomography (CT) and positron emission tomography-CT. Based on these results, she underwent en bloc resection of the tumor, including removal of the involved vessels, and was eventually diagnosed as having a type B2 thymoma permeating into the LBCV and SVC. We present this case as a very rare form of SVCS caused by an invasive thymoma.

Meta-Analysis of Limited Thymectomy versus Total Thymectomy for Masaoka Stage I and II Thymoma

  • Background: This meta-analysis aimed to evaluate the incidence of tumor recurrence, postoperative myasthenia gravis, postoperative complications, and overall survival after limited versus total thymectomy for Masaoka stage I and II thymoma. Methods: A systematic search of the literature was conducted using the PubMed, Embase, MEDLINE, and Cochrane databases to identify relevant studies that compared limited and total thymectomy in Masaoka stage I-II patients. The quality of the included observational studies was assessed using the Newcastle-Ottawa Scale. The results of the meta-analysis were expressed as log-transformed odds ratios (log ORs), with 95% confidence intervals (CIs). Results: Seven observational studies with a total of 2,310 patients were included in the meta-analysis. There was an overall non-significant difference in favor of total thymectomy in terms of tumor recurrence (pooled log OR, 0.40; 95% CI, -0.07 to 0.87; p=0.10; I2=0%) and postoperative myasthenia gravis (pooled log OR, 0.12; 95% CI, -1.08 to 1.32; p=0.85; I2=22.6%). However, an overall non-significant difference was found in favor of limited thymectomy with respect to postoperative complications (pooled log OR, -0.21; 95% CI, -1.08 to 0.66; p=0.64; I2=36.1%) and overall survival (pooled log OR, -0.01; 95% CI, -0.68 to 0.66; p=0.98; I2=47.8%). Conclusion: Based on the results of this systematic review and meta-analysis, limited thymectomy as a treatment for stage I and II thymoma shows similar oncologic outcomes to total thymectomy.

  • Tetrofosmin is a ligand that forms a lipophilic, cationic complex with Tc-99m. Tc-99m tetrofosmin was developed as a myocardial perfusion imaging agent and also used to depict tumors. Mediastinal tumors is also detected by Tc-99m tetrofosmin. We report a case of extracardiac mediastinal activity detected by Tc-99m tetrofosmin scintigraphy, which revealed thymoma.
