A Case of Invasive Thymoma with Endotracheal Polypoid Growth

  • Thymomas are one of the most common neoplasms of the mediastinum derived from thymic epithelium. It is common that invasive thymoma invades the lung, pericardium, and great vessels. Airway compression by mass effect also occurs, but direct polypoid tumor growth into the airway is extremely rare. Only 20 cases of invasive thymoma with endobronchial polypoid growth have previously been reported globally. However, there is no case report of invasive thymoma with endotracheal growth. Herein, we report a rare case of invasive thymoma with endotracheal polypoid growth in a 28-year-old woman.

흉선종의 임상적 고찰 (A Clinical Analysis of Thymoma)

  • Thymoma is defined as a neoplasm of the epithelialreticular framework cells of the thymus. The treatment of choice and the prognosis for patients with thymoma are still controversial subjects due to lack of a uniform histological classification and standardized criteria for typing thymic tumors. Between June 1985 and May 1989, eight patients underwent thymomectomy at the Department of Thoracic and Cardiovascular Surgery, Korea University Medical Center. A clinical analysis was performed and the following result was obtained. In histologic cell type, epithelial type was 2 cases, lymphocytic type 3 cases, and mixed type was 3 cases. The clinical stage was classified by Masaoka’s classification that Stage I was 4 cases, Stage II; 2 cases, Stage III; 2 cases and Stage IV was none. Myasthenia Gravis was associated with thymoma in 5 patients. In 8 patients, thymomectomy were performed. Adjuvant radiotherapy was performed in one patient with Stage I due to suspicious tumor remnant and in Stage II, III patients routinely. Combined chemotherapy was performed in one patient with Stage III due to local recurrence. We concluded that the most important prognostic factor of thymoma is extent of tumor invasion. And the presence of Myasthenia Gravis is no more prognostic factor. Surgical resection of thymoma is treatment of choice, If local invasion or distant dissemination is present, postoperative radiotherapy and /or combined chemotherapy is recommended.

부갑상선 선종으로 오인된 흉선종 환자 1예 (A Case of Thymoma Mimicking Parathyroid Adenoma)

  • Thymoma is the most common anterior mediastinal mass in adult, and arises from thymic epithelial cells which includes consists various proportions of epithelial and lymphocytic aspect. Thymic epithelial cell arises from the ventral wings of the third and fourth branchial pouches in the embryo, and these cells are considered tumorous condition of thymoma. Surgical resection is main treatment of thymoma. And adjuvant chemotherapy or radiation is considered due to postoperative pathologic diagnosis. We experienced a tumor which located from left anterior neck along superior mediastinum on chest radiograph incidentally and diagnosed parathyroid adenoma clinically. After surgical removal, final pathologic report was thymoma. Here, we present the case with a review of the related literatures.

  • Thymoma is the most common primary tumor of anterior mediastinum, accounting for 20% to 30% of all mediastinal tumors. The recurrence rate after total resection of the thymoma ranges 8% to 18%. We reported one patient of recurrent malignant thymoma imaged with Tc-99m MIBI, Tc-99m Tetrofosmin and Tc-99m (V) DMSA. Early and delayed Tc-99m MIBI and Tc-99m Tetrofosmin scintigraphies showed an increased uptake in the mediastinal area. Also, Tc-99m (V) DMSA scintigraphy revealed an increased uptake tn the corresponding area. Coronal SPECT images of Tc-99m MIBI, Tc-99m Tetrofosmin and Tc-99m (V) DMSA revealed increased uptake of each radiopharmaceutical in the tumor lesion corresponding to the mediastinal lesion on the chest CT. However, the normal blood pool activities of the heart and great vessels of Tc-99m (V) DMSA obscured the recurrent malignant thymoma. Although Tc-99m (V) DMSA is a useful tumor seeking agent, we recommend Tc-99m MIBI and Tc-99m Tetrofosmin SPECT rather than Tc-99m (V) DMSA to detect primary and recurrent malignant thymoma.

흉막에서 발생한 침습성 흉선종,정상 흉선을 가진 예 (Invasive Thymoma Originating from Right Pleura with Normal Thymus A case Report)

  • 42세 남자는 약 10년간 석면에 노출되는 직업에 종사하다가 그만둔지 5년 후, 진행하는 호흡곤란을 주소로 검사를 받던중 우측 늑막에 다양한 크기의 종괴를 보이는 암종으로 입원하였다. 환자는 부분 마취하에 늑막 조직 생검을 받고 침윤성 흉선종의 의심하에 우 전늑막 및 우전폐 절제술을 받았다. 수술중 흉선의 상태는 정상이었으며 폐 첨부로부터 횡격막부위까지의 늑막과 폐엽간, 부분적인 폐 침습을 보이고 심낭의 침윤을 보이는 Masaoka 분류 Stage 로 보이는 침윤성 흉선종으로 진단되 었다. 환자는 술후 600의 방사선 조사를받고 술후 8개월인 현재까지 재발의 징후 없이 경과관찰중이다.

Paraneoplastic Encephalitis Associated with Thymoma: A Case Report

  • A 42-year-old woman with short-term memory loss visited Gangnam Severance Hospital, and her chest X-ray and computed tomography revealed a right anterior mediastinal mass. On hospital day two, she suddenly presented personality changes and a drowsy mental status, so she required ventilator care in the intensive care unit. She underwent thymectomy, and was pathologically diagnosed with thymoma, type B1. Her mental status eventually recovered by postoperative day 90. Paraneoplastic encephalopathy associated with thymoma is very rare, and symptoms can be improved by thymectomy. We report a case of paraneoplastic encephalopathy associated with a thymoma.

흉선종의 외과적 치료 (Surgical Treatment for Thymoma)

  • This report documents the clinical k pathologic features of 33 patients treated for thymoma for 11 years & 6 months between September 1977 and February 1989. At the Thoracic & Cardiovascular Surgery, Yonsei University, College of Medicine of the group, 31 patients treated with surgery were examined for the result of operation & prognosis. Mean age was 50 years. Thirteen were female and twenty were male. Dyspnea on exertion and chest discomfort were common in the patients without myasthenia gravis. Fourteen patients[42.6%] had myasthenia gravis and one patient had autoimmune thyroid disease. Four patients[12.1%] presented without symptoms attributable to their thymoma. Histologic review disclosed 12[36.4%] epithelial thymoma, 10[30.3%] mixed lymphoepithelial, 9[27.3%] lymphocytic, 1[3.0%] spindle cell and 1[3.0%] unknown cell thymoma. They were classified according to Masaoka`s clinical staging criteria; by these criteria, 8 patients were stage I, 5 patients were stage II, 15 patients were stage III and 3 patients were stage IV. Total excision of mass was possible in 20 patients. Partial excision of mass in 4 patients and biopsy in 7 patients were carried out during the operation. There was only one operation mortality. Follow-up was possible in 26 patients and follow-up ranged from 4 months to 10.5 years[mean 28.9 months]. One-year survival rates were 77.8% and eight patients expired during follow-up period. Eleven[78.6%] patients with myasthenia gravis were improved after the operation. This observation suggests that the most significant factor determining the survival is whether or not total surgical excision had been performed.

Good 증후군 치험 -1예 보고- (Good's Syndrome (Thymoma with Immunodeficiency) -A case report-)

  • Good증후군은 성인에서 발생하는 B세포 와 T세포 면역 결핍증의 드문 원인 질환으로 임상적인 특징은 세군 감염 빈도의 증가 및 바이러스, 진군의 기회 감염이다. 가장 일치하는 면역학적 이상은 저감마글로부린 혈증과 B세포가 감소되거나 소실되는 것이다. 이 증후군의 치료는 흉선종의 절제 및 적절한 면역글로부린 G 역가를 유지하기 위한 면역글로부린의 투여이다. 저자는 1예의 Good증후군을 경험하였는데 환자는 64세 여자로 빈번한 비강 폐 감염의 병력을 가지고 있다. 흉부전산화단층 촬영에서 폐렴과 전 종격동 종괴의 소견을 보였으며 경피적 침 생검에서 흉선종으로 진단되었다. 환자는 흉선절제술을 시행 받았으며 저감마 글로부린혈증을 치료하기 위해서 면역글로부린을 투여 받았다.

  • Malignant thymoma is usually manifested as a mediastinal mass. Occasiomally, these tumors may be associated with a variety of systemic syndromes, such as myasthenia gravis, pure red cell aplasia or hypogammaglobulinemia However, it is generally accepted that thymomas rarely metastasize and the frequency of extrathoracic metastasis has been reported as 1 to 15%. In general, nuclear changes of tumor cells such as enlarged size, irregular shape and altered textures are determining factors in the assessment of the grade of malignancy. We experienced a case of malignant thymoma of anterior mediastinum in a 35-year-old woman. After 3 years, she developed recurrent malignant thymoma in the right supraclavicular area, diagnosed by aspiration biopsy cytology.

갑상선 종괴로 오인된 이소성 경부 흉선 종양 (Ectopic Cervical Thymic Tumor Misdiagnosed as a Thyroid Mass)

  • Ectopic cervical thymic tumor, first described in 1941 by Boman, is a rare tumor of the neck displaying the same histologic features as mediastinal thymoma. It was classified into benign thymoma, invasive (or malignant) thymoma, thymic carcinoma histopathologically and clinically. The ectopic cervical thymic tumor is misdianosed as the thyroidal mass on radiologic examination and FNA cytology due to its rarity and unusual location. Recently, we have experienced two cases of ectopic cervical thymic tumor misdiagnosed as thyroid mass ; a case of thymic carcinoma;the other of invasive thymoma. So, we report these cases with review of the literature.
