A Case of Pulmonary and Retroperitoneal Lymphangioleiomyomatosis (폐와 후복강에 발생한 림프관평활근종증 1예)

  • Lymphangioleiomyomatosis, a rare disease in women of childbearing age, is the result of benign nodular hypertrophy of the smooth muscle of the lypmhatics and other tissues of the abdomen and thorax. We report a 36-years-old woman with pulmonary and retroperitoneal lymphangioleiomyomat.osis who responded with hormone treatment. She developed vaginal pruritis and a pelvic ultraound was done given her significant past medical history. Ultrasound examination demonstrated a large mass in the right side of her pelvis. Therefore she was admitted to St. Michael's Hospital in Toronto for laparoscopy. Result of cytology was to be consistent with the diagnosis of retroperitoneal lymphangioleiomyomatosis. High resolution CT sacn of the thorax demonstrated multiple small cystic lesions, without associated nodularity compatible with a diagnosis of pulmonary lymphangioleiomyomatosis. She has been taking Provera tablets 100mg po tid since Dec. 15, 1993. We have given her a prescription for Depo provera 500mg IM monthly since she came back to Korea. and made arrangements for regular follow up monthly. We performed chest X-ray, CT of chest(high resolution), abdomen and pelvis, pulmonary function tests and arterial blood gas analysis. Chest X-ray and CT findings showed no significant change since July. 20, 1993.

Automatic Segmentation of Pulmonary Structures using Gray-level Information of Chest CT Images (흉부 CT 영상의 밝기값 정보를 사용한 폐구조물 자동 분할)

  • We propose an automatic segmentation method for identifying pulmonary structures using gray-level information of chest CT images. Our method consists of following five steps. First, to segment pulmonary structures based on the difference of gray-level value, we select the threshold using optimal thresholding. Second, we separate the thorax from the background air and then the lungs and airways from the thorax by applying the inverse operation of 2D region growing in chest CT images. To eliminate non-pulmonary structures which has similar intensities with the lungs, we use 3D connected component labeling. Third, we segment the trachea and left and right mainstem bronchi using 3D branch-based region growing in chest CT images. Fourth, we can obtain accurate lung boundaries by subtracting the result of third step from the result of second step. Finally, we select the threshold in accordance with histogram analysis and then segment radio-dense pulmonary vessels by applying gray-level thresholding to the result of the second step. To evaluate the accuracy of proposed method, we make a visual inspection of segmentation result of lungs, airways and pulmonary vessels. We compare the result of the conventional region growing with the result of proposed 3D branch-based region growing. Experimental results show that our proposed method extracts lung boundaries, airways, and pulmonary vessels automatically and accurately.

  • Trauma is an injury to the body that involves multiple anatomical and pathophysiological changes caused by forces acting from outside the body. The number of patients with trauma is increasing as our society becomes more sophisticated. The importance and demand of traumatology are growing due to the development and spread of treatment and diagnostic technologies. In particular, damage to the large blood vessels of the chest can be life-threatening, and the sequelae are often severe; therefore, diagnostic and therapeutic methods are becoming increasingly important. Trauma to nonaortic vessels of the thorax and aorta results in varying degrees of physical damage depending on the mechanism of the accident and anatomical damage involved. The main damage is hemorrhage from non-aortic vessels of the thorax and aorta, accompanied by hemodynamic instability and coagulation disorders, which can be life-threatening. Immediate diagnosis and rapid therapeutic access can often improve the prognosis. The treatment of trauma can be surgical or interventional, depending on the patient's condition. Among them, interventional procedures are increasingly gaining popularity owing to their convenience, rapidity, and high therapeutic effectiveness, with increasing use in more trauma centers worldwide. Typical interventional procedures for patients with thoracic trauma include embolization for non-aortic injuries and thoracic endovascular aortic repair for aortic injuries. These procedures have many advantages over surgical treatments, such as fewer internal or surgical side effects, and can be performed more quickly than surgical procedures, contributing to improved outcomes for patients with trauma.

CLINICAL STUDY OF MIDFACE FRACTURE (안면 중앙부 골절에 관한 임상적 연구)

  • The purpose of this study was to investigate the epidemiology of midface fractures. We observed clinically 71 patients with midface fracture who visited the Department of oral and maxillofacial-surgery, Chosun University, Dental Hospital, from 1991 to 1994. The results obtained were as follows. 1. There was the highest age incidence in the third decade(29.6%). 2. There was the highest incidence in the summer(36.6%). 3. The most etiologic factor of midface fracture was traffic accidents(43.6%) and next factor was fall down(38.0%). 4. The highest incidence fracture was zygomatic arch(38.7%) and next fracture was ZMC fracture(31.5%). 5. Most midface fracture was treated within 2 weeks(86.7%). 6. Midface fractures was most frequently combined with mandibular fracture(15.5%), head injuries(14.1%), orthhopedic injuries(8.5), thorax & abdomen(5.6%). 7. The highest complication was the nerve injury(8명), and next complications were infection(3명), epiphora(3명) etc.

  • Objectives: Due to advance of science and IT technology, high tech imaging equipment like ultrasonography, CT, MRI and PET scan is constantly being developed and progressed; use of these techniques is needed for Korean medicine. Methods: Historical study was undertaken on the evidence of using ultrasonography. Normal organs and various sorts of diseases were also observed by ultrasonography. Results: Korean medicine judges disease of internal organs and condition of health by symptoms of functional disease and organic disease based on anatomical theory. Ultrasonography based on anatomical theory is non-invasive and free from radiation exposure and can be performed directly from clinical practice in real-time. Ultrasonography can be a big help for securing the stability of the internal organs in inserting needles in the thorax and abdomen as well as diagnosing functional and organic diseases based on anatomical theory. Conclusion: We look forward to a great development of scientification and objectification of Korean medicine by using and researching imaging equipment based on anatomical theory as well as ultrasonography.

A Case of Measles Pneumonia Complicated by Subcutaneous Emphysema and Pneumomediastinum in an 18-year-old Patient (피하기종과 종격기종을 동반한 홍역성 폐렴 1예)

  • Background : A 18 year-old man presented with dyspnea and a swelling of the neck. On physical examination, maculopapular rashes were noted on the face and the whole body and crepitus was noted at the thorax and upper arms. His chest X -ray showed bilateral interstitial infiltrates of the lung, pneumomediastinum and subcutaneous emphysema. On serologic examination, measles IgM was positive. Under the diagnosis of measles pneumonia, the patient was treated with oral ribavirin, which resulted in a complete resolution of the pneumomediastinum, subcutaneous emphysema, pneumonic infiltrate, and subjective symptoms of dyspnea and swelling of the neck in 7 days. Here we report this case with a brief review of the relevant literature.

  • Objectives : The purpose of this study is to find out relationship between Trigger points and Acupoints. Methods : We have researched some of oriental and western medical books about Trigger points and Acupoints from head to trunk. Conclusions : 1. In the head, there are 46 trigger points Among them, 19 trigger points correspond to Acupoints and 25 trigger points are similar to them 2. In the thorax, there are 33 trigger points Among them, 17 trigger points correspond to Acupoints and 20 trigger points are similar to them 3. In the upperimb, there are 20 trigger points Among them, 9 trigger points correspond to Acupoints and 13 trigger points are similar to them. 4 In the trunk, there are 11 trigger points. Among them, 5 trigger points correspond to Acupoints and 11 trigger points are similar to them.

  • The impedance pneumography which is widely used in monitoring respiration is simple to use and noninvasive, but it is sensitive to motion artifacts and insensitive to detect obstructive apnea. A 3-channel respiratory inductive plethysmography (RIP) developed in this study detects inductance change of the inductance band induced by cross-sectional area change of thorax or abdomen as one breathes. It was confirmed that RIP was less sensitive to various motion artifacts but more sensitive to detection of obstructive apnea than impedance pneumography.

Hemangiosarcoma in a Female Pointer (개에서 발생한 혈관육종 일례)

  • A 12-year old female Pointer was referred to the Veterinary Teaching Hospital, College of Veterinary Medicine, Kyungpook National University for evaluation of emaciation. The owner had noted a dysuria and dyschesia 2 months before admitting to the hospital. The patient showed anorexia, depression and mild anemia. Radiographic findings indicated the mass below the urinary bladder, which pushed the intestine up to the spine. The patient was necropsied and grossly, two large masses were found on the heart and cranial thorax. Congestive nodules of 0.5-1.5 cm in diameter were scattered on both sides of the lungs. The left ventricular wall was hypertrophied and adult heartworms were found in right atrium. A large mass, filled with blood, 9-10 cm in diameter, originated from the rectum, compressed urethra. The gall bladder was filled with bile juice and the spleen was mild atrophied. The cortex of the kidney and the intestine were congested. On microscopic observation, there were neoplastic cells forming irregular vascular channels in the lung, kidney and heart. The vascular channels contain variable amount of blood. In the lung, the cells were almost elongated and the nuclei of neoplastic cells were round or ovoid and hyperchromatized. Based on the observation, the case was diagnosed as a hemangiosarcoma.

A Case of Pulmonary Lymphangioleiomyomatosis Associated with Tuberous Sclerosis and Renal Angiomyolipoma (결절성 경화증과 동반된 폐의 임파관평활근종증 1예)

  • Lymphangioleiomyomatosis(LAM) is rare and essentially limited to women in the reproductive ages. A 39-year-old female was admitted due to progressive exertional dyspnea and intermittent productive cough. Chest PA showed marked hyperinflation of the lung associated with a diffuse reticulo-nodular pattern. High resolution CT scan of the thorax demonstrated that diffusely scattered thin-walled cysts were distributed throughout the bilateral lung fields. Abdominal CT scan showed variable sized multiple angiomyolipoma of both kidney. By open lung biopsy, she was diagnosed as pulmonary LAM associated with Tuberous sclerosis and renal Angiomyolipoma. We present the case and discuss the connection between pulmonary LAM and Tuberous sclerosis.
