Nuss Operation with Simultaneous Intracardiac Repair -A case report- (선천성 심기형에 동반된 누두흉에서 동시에 시행안 심기형 교정 및 Nuss 수술 -1예 보고-)

  • There are still some controversies regarding one stage repair of pectus deformity with congenital heart disease due to probable complications after extensive resection of deformed cartilages. We performed Nuss operation with simul-taneous intracardiac repair in 8 year old patient with pectus excavatum and partial atrioventricular septal defect. The result of operation was satisfactory without prolongation of operation time, bleeding or instability of chest wall.

  • Persistent left superior vena cava (PLSVC) is a common venous anomaly of the thorax and usually drains into the right atrium. Less often it drains into the left atrium and has previously been related to ischemic stroke. We report a case of PLSCV that founded during ischemic stroke evaluation in a 77-year-old woman which was detected on transesophageal echocardiography (TEE) and transcranial Doppler ultrasonography (TCD) with saline agitated test and computed tomography.

  • MADYMO human model with the detail neck was used to investigate the reaction force of neck and neck injury from rear impact directions. In the validation simulation, head acceleration, thorax acceleration and the global kinematics of the head and neck were correlated well with experimental data. Acceleration data from three 15 km/h low speed car rear impact pendulum tests(rear-end, offset, oblique) were used to simulate the model. In the simulation results, the reaction force on the facet joint and discs in the oblique rear impact were higher than rear-end, offset rear impacts. Further research is still needed in order to neck injury analysis about different crash parameters.

  • We studied the characteristics of the compound electrode and found that the compound electrode provides the four-electrode method in a compact form. We developed a new method of measuring the skin impedance using simple electrodes at low frequencies. At high frequencies where the effect of internal tissue impedance is not negligible, we used the compensation method using compound electrodes since they measure the voltage right under the skin. At 50 kHz, we measured the real part of the skin impedance of less than $80{\Omega}$ on the thorax. We propose a simple instrument which can measure accurate skin impedance at various frequencies.

  • Holoprosencephaly of unknown definite causes, has been associated with several chromosome abnormalities involving the autosomes and the sex chromosomes. The most commonly reported associations include dup(3p), del(7q), deletions of chromosome 13, trisomy 13, trisomy 18, and triploidy. In previously reported cases in Korea, none were associated with chromosome 21 anomalies. In conclusion, we reported the first case of holoprosencephaly(semilobar type) associated with pure monosomy 21. We experienced a semilobar type holoprosencephaly with monosomy 21 in a neonate who had multiple congenital anomalies, including an abnormal face, a small thorax with widely spaced hypoplastic nipples and nail hypoplasia, lung hypoplasia with severe scoliosis and cardiac abnormalities. Chromosomal analysis revealed a 45, XY, -21.

  • Real-world accident cases were investigated to understand injury characteristics of the elderly driver. A total 10 cases of car-to-car frontal crash accidents from passenger car including SUV claimed to domestic car insurance company were reviewed. The injury characteristics of the elderly were analyzed from personal information (gender, age), medical treatment record (medical certificate, curative days), vehicle information (model, air-bag, seatbelt) and damage information. This study showed that elderly driver has higher possibility of thorax injury than non-elderly's. Moreover, Injury type and severity were more severe than non-elderly driver at similar type accident conditions. Also, elderly driver's medical treatment period needs 3 times more than non-elderly driver's.

  • A capitellid species, Heteromastus filiformis ($Clapar\grave{e}de$, 1864) is newly reported from Korean waters with the description and illustrations. Korean materials of the present study show several characteristics generally agreed well with the previous descriptions of H. filiformis as follows: the thorax is composed of the first achaetigerous peristomium and following 11 thoracic chaetigers; the capillary chaetae are present on the thoracic chaetigers 1-5 and narrowly bilimbate; the thoracic hooded hooks are appeared on the chaetigers 6-11 and have indistinct nodes on the shaft and several teeth above the main fang; the abdominal hooded hooks possess distinct nodes on the shaft and a few teeth above the main fang; the branchiae appearing on the posterior abdominal segments are the broadly-based and rounded lamellae projecting posteriorly. The authors reviewed the taxonomy of Heteromastus with a comparison of morphological characteristics among worldwide species, and provided a key of them.

  • Benign hyperplasia of hilar lymph glands is rare. Most of the lesions were intrathoracic cavity. The lesions were discovered most often on routine roentgenograms of the chest or because. of pressure symptoms or the presence of a palpable mass if outside the thorax. Diagnosis is. made after removal of gland, a procedure which may also have therapeutic value. They have been divided into 2 histol0gic types: the hyaline-vascular lesions, which were, most numerous, were characterized by small hyaline-vascular follicles and interfollicular capillary proliferation ;the plasma cell lesions were characterized by large follicles with! intervening sheets of plasma cells. We experienced one case of benign hyperplasia of lymph gland in left hilum, which were. most numerous, characterized by small hyaline-vascular follicles and interfollicular capillary proliferation-hyaline-vascular type. This 29 years old male patient was treated by right upper lobectomy with excision of lesion. The postoperative courses was uneventful.

  • We report here on a very rare case of granulocytic sarcoma of the pectoralis muscle on the left chest wall of a patient with chronic myelogenous leukemia, and this malady presented as a very rapidly growing hematoma-like mass.

  • The thoracic diaphragm is a muscle tissue separating the thorax and abdomen of a man. In "Somun(素問)", Gyeok(鬲) means diaphragm. Its meaning comes from Gyeok(鬲), a tripod boiler with three separated parts for cooking. In "Yeongchu", Gyeok(膈) is a pictophonetic character which takes the place of Gyeok(鬲). The different uses of the two words probably does not come from the original writing, but result from longtime transformation.
