자가 등 가동운동이 어깨 통증 및 기능에 미치는 효과 (The Effect of Self Thoracic Mobilization Exercise on Shoulder Pain and Function)

  • 본 연구는 자가등 가동운동(STME, Self Thoracic Mobilization Exercise)이 어깨 기능장애 환자에게 어깨통증과 장애정도, 어깨의 기능에 미치는 영향을 알아보고자 하였다. 어깨 기능장애 환자 32명을 대상으로 무작위로 STME와 대조군으로 할당하였으며, 시각 상사척도와 어깨기능장애, 어깨의 가동범위 평가를 중재 전-후 실시하였다. 16명의 STME 중재 그룹은 폼롤러를 활용하는 방법과 네발자세에서 이용하여 굽힘, 폄을 자가로 실시하였고, 16명의 대조군은 전기치료 패드를 이용한 플라세보 효과를 활용하였다. 두 집단은 20분 동안 주당 3회를 4주간 실시하였다. STME군이 대조군 보다 통증과 기능장애에서 모두 유의한 향상을 보였으며(p<0.05), 어깨의 가동범위는 STME을 이용한 그룹이 플라세보 그룹 보다 굽힘, 벌림, 가쪽 돌림에서 유의한 효과를 보였다(p<0.05), 하지만 폄과 안쪽돌림에는 유의한 효과를 보이지 못했다(p>0.05). 따라서 STME은 어깨통증과 기능장애 해결에 효과적이며, 향후 어깨 통증 환자에게 보다 쉬운 접근방법의 중재방법을 교육하고 효과적인 치료를 할 수 있을 것이다. 나아가 사회적 치료비용 또한 경감시킬 수 있을 것이다.

건강한 성인에게 가슴가동운동과 스트레칭운동이 최대들숨압 및 최대날숨압에 미치는 효과 (The Effects of Thoracic Mobilizing and Stretching Exercise on Maximal Inspiratory Pressure and Maximal Expiratory Pressure in Healthy Adults)

  • Purpose: The purpose of this study was to investigate the effect of chest mobilization and stretching exercises on maximal inspiratory pressure and maximal expiratory pressure in healthy adults who use computers for extended periods of time each day due to coronavirus disease 2019. Methods: Twenty-five healthy adults in their 20s and without respiratory disease (15 female, 10 male) took part in this study. Two types of thoracic mobilizing exercises using a Theraband and three types of stretching exercises using a foam roller were performed. Maximum inspiratory pressure and maximum expiratory pressure were measured three times each before and after the interventions. In terms of statistical methods, the maximum inspiratory pressure due to chest mobility and stretching was compared with the maximum expiratory pressure using parametric paired t-test and non-parametric Wilcoxon signed-rank test. Results: Maximum inspiratory pressure (p = .012) and maximum expiratory pressure (p = .006) showed significant differences before and after chest mobilization exercise and stretching among the participants. Conclusion: The results of this study suggest that chest mobilization and stretching exercises are effective exercise methods for improving maximal inspiratory and expiratory pressure. They suggest that these exercises can prevent respiratory muscle weakness and improve aerobic fitness in healthy people as well as those in need of cardiorespiratory physiotherapy.

등뼈 가동성 운동이 기계적 목통증 환자의 목등뼈부 기능 수준과 자세, 통증 수준에 미치는 영향 (Effects of Thoracic Mobility Exercise on Cervicothoracic Function, Posture and Pain in Individuals With Mechanical Neck Pain)

  • Background: Individuals with mechanical neck pain show biomechanical and neurophysiological changes, including cervical spine muscle weakness. As a result of deep muscle weakness, it causes stability disability and reduced upper thoracic spine mobility, which finally leads to functional movement restriction such as limited range of motion and dysfunction. Recent studies have shown that thoracic spine manipulation and mobilization could reduce symptoms of mechanical neck pain in patients. Objects: The purpose of this study was to investigate the effects of thoracic mobility exercise on cervicothoracic function, posture feature, and pain intensity in individuals with mechanical neck pain. Methods: The study subjects were 26 persons who were randomly assigned to the experimental (with thoracic mobility exercise) and control groups (without thoracic mobility exercise), with 13 subjects in each group. The cervicothoracic function (neck functional disability level and cervicothoracic range of motion), posture feature, and pain rating (using a quadrupled visual analogue scale [QVAS]) were measured before, after 3 weeks, and after 6 weeks. Results: Statistically significant group-by-time interactions were found with repeated analyses of variance for the Korean neck disability index (KNDI), all cervical range of motion (CROM), all thoracic range of motion (TROM), cranial rotation angle, sagittal shoulder posture (SSP), and QVAS (p<.05). All groups showed significant improvements from all times in all the evaluated methods. The KNDI, CROM, TROM of left rotation, and SSP in the experimental group showed significant improvements after 3 weeks, and the TROM of the right rotation and QVAS in the experimental group showed significant improvements after 6 weeks when compared with the control group. Conclusion: Thoracic mobility exercise during 6 weeks might be effective intervention to improve the functional level, posture feature, and QVAS pain rating for managing individuals with mechanical neck pain.

허리 불안정성을 가진 성인에게 적용한 복합운동프로그램이 정적균형능력에 미치는 영향 (The Effect of the Combined Exercise Program on the Static Balance Ability of the Lumbar Instability of Adults)

  • Background: The purpose of this study is to investigate the effect of the combined exercise program on the static balance ability of the lumbar instability of adults. Methods: The experimental group was divided into two groups and randomly. The experimental group 1 (Exp 1) applied the combined exercise program for the lumbar stabilization exercise and strengthening exercises and the experimental group 2 (Exp 2) applied the combined exercises program for the lumbar stabilization exercises and active stretching exercises. Each experiment group performed exercise program for 4 weeks, 3 times a week, 30 minutes a day. Static balance ability was measured using GOOD BALANCE system. Results: As the comparing results of static balance ability, normal standing eye open was Ant-Post and Med-Lat showed significant differences in Exp 1 (p<.05). One leg left eye open was velocity moment showed significant differences in Exp 1 (p<.05). and one leg right eye open was Med-Lat showed significant differences in Exp 1 (p<.05). Conclusion: These results show that thoracic self mobilization is immediate effective on balance and gait ability. Thus, thoracic self mobilization will help recovery of balance and gait ability in acute stroke patients.

  • The purpose of this paper was to provide the understanding of theory, technique, clinical use about arthro kinematic approach. Difference between AKA and Joint mobilization was seen through Table 1, and the relation between AKA and Athro kinematics was seen through Table 2. Examples of AKA techniques were as follow ; 1. Cervical intervertebral joint, left $C_{2/3}$ 2. Thoracic intervertebral joint, left $T_{5/6}$ 3. Sacroiliac joint, left (1) Nutation-upward gliding (2) Nutation-downward gliding (3) Superior distraction (4) Inferior distraction 4. 1st. costovertebral joint, left 5. 2nd. sternocostal joint, left 6. AKA-streching exercise 7. AKA - resistive exercise Symptoms, diagnosis, treatment were discribed for clinical use, and they were expected further that clinical application of AKA might clarify many of joint dysfunction.

깊은목굽힘근 강화운동과 등뼈 관절가동운동이 뇌졸중 환자의 전방머리자세와 목통증에 미치는 영향 (The Effect of Deep Neck Flexor Exercise and Thoracic Mobilization Exercise on Forward Head Posture and Neck Pain in Chronic Stroke)

  • 본 연구는 깊은목굽힘근운동과 등뼈 관절가동운동이 뇌졸중 환자의 통증, 전방머리자세에 미치는 영향을 알아보고자 한다. 연구대상자는 36명은 사전측정 후 무작위로 깊은목굽힘근군(DNFE), 등뼈 관절가동운동군(TROM), 대조군으로 각각 12명씩 배정되었다. 중재 전후에 통증(VAS), 두개척추각도(CVA), 두개회전각도(CRA), 전방머리내밈자세(FHP), 등뼈관절가동범위(TROM)를 측정하였다. 그 결과. DNFE 군과 TROM 군은 중재 전후 VAS, CVA, CRA, FHP, TROM에서 유의한 차이를 보였으며(p<.05), 두 군은 대조군에 비해 VAS, CVA, CRA, FHP, TROM에서 보였다(p<.05). 결론적으로 깊은목굽힘근운동과 등뼈 가동범위운동이 뇌졸중 환자의 전방머리자세와 목통증 회복에 효과적이었다. 그러므로 임상에서 뇌졸중 환자 중재프로그램에 깊은목굽힘근운동과 등뼈 가동범위운동을 적극적으로 활용되기를 바란다.
