잠재적인 무릎넙다리 통증 증후군 환자에게 미세전류를 이용한 IASTM과 플로싱 밴드 적용이 하지 근막의 두께 변화에 미치는 즉각적 효과 (Immediate Effect of the Application of IASTM Using Microcurrent and a Flossing Band and on Changes in the Thickness of the Lower Extremity Fascia in Patients with Intrinsic Patellofemoral Pain Syndrome)

  • Background: This study examined the Immediate effects of IASTM using microcurrent and the flossing band on the lower extremity fascia thickness in subjects with Intrinsic patellofemoral pain syndrome. Methods: Sixty-six subjects with patellofemoral pain syndrome were randomized into three groups (22 each in the microcurrent IASTM (instrument assisted soft-tissue mobilization) group, and flossing band group, and combined microcurrent IASTM and flossing band group) to evaluate the immediate effects of the lower extremity fascia thickness before and after intervention. The thickness of the lower extremity fascia was measured using an ultrasound machine. Using SPSS Window. 22.0, a Shapiro Wilk was conducted to test the normality of all variables; within-group comparisons were made with a paired-samples t-test, and between-group interventions were subjected to a one-way analysis of variance. Results: Changes in the thickness of the fascia in the thigh area were observed before and after intervention in all three groups. There was a significant decrease, and in the combined group, there was a significant decrease in fascia thickness compared to when the IASTM group and the flossing band group were applied separately (p<.05). Conclusion: Through this study, the effect on fascia thickness was confirmed when IASTM and flossing band intervention were combined, and it is believed that it can be used as basic clinical data for patients with knee-thigh pain syndrome.

Successful Treatment of Rectus Femoris Calcification with Ultrasound-guided Injection: A Case Report

  • Painful periarticular calcification most commonly occurs within the rotator cuff of the shoulder and rarely around the elbow, hip, foot, and neck. As acute inflammatory reaction develops, severe pain, exquisite tenderness, local swelling, and limitation of motion with pain occur. In case of calcific tendinitis of the shoulder, it can be easily diagnosed according to the symptoms and with x-ray. However, in lesions of the hip, as it is a rare location and usually involves pain in the posterolateral aspect of the thigh, which can simulate radicular pain from a lumbar intervertebral disc, it could be difficult to diagnose. Hence, physicians usually focus on lumbar lesions; therefore, misdiagnosis is common and leads to a delayed management. Here, we report the case of a 30-year-old female patient with calcific tendinitis of the rectus femoris that was successfully managed with ultrasound-guided steroid injection. This study offers knowledge about the rectus femoris calcification.

  • Piriformis Syndrome is characterized by pain of the low back, groin, perineum, buttock, hip, posterior thigh, leg and foot. Symptoms are aggravated by sitting, prolonged combination of hip flexion, adduction, the medial rotation, or by activity. In addition, patient may complain of painful swelling of the limb and sexual dysfunction-dyspareunia in female, and impotence in male. It currently appears that three specific conditions may contribute to the piriformis syndrome: (a) myofascial pain; (b) nerve and vascular entrapment; (c) dysfunction of the sacroiliac joint. The important keys of diagnosis are history and physical examination. There is no known objective diagnostic method. We described the clinical features of four cases of piriformis syndrome and reviewed foreign literature.

Extended duration pulsed radiofrequency for the management of refractory meralgia paresthetica: a series of five cases

  • Meralgia paresthetica (MP) is a sensory mononeuropathy, caused by compression of the lateral femoral cutaneous nerve (LFCN) of thigh. Patients refractory to conservative management are treated with various interventional procedures. We report the first use of extended duration (8 minutes) pulsed radiofrequency of the LFCN in a case series of five patients with refractory MP. Four patients had follow up for 1-2 years, and one had 6 months follow up. All patients reported remarkable and long lasting symptom relief and an increase in daily life activities. Three patients came off medications and two patients required minimal doses of neuropathic medications. No complications were observed.

  • Epidural adhesiolysis is a convenient and safe method for the management of back pain. However, we experienced a case where a patient developed sensory change to S2 area after epidural adhesiolysis. Male patient, 43 years old, was admitted to our pain clinic for epidural adhesiolysis for back pain. Patient was experiencing pain radiating to left thigh, and sensory change and motor disturbance to the S1 area. Patient's symptoms and signs were much improved on the first day of epidural adhesiolysis. Patient, however, complained of numbness of perineal and S2 areas after the next day of injections. We postulated the cause of this complication was due to: compression of nerve root by the large volume of injectate and hematoma, and the side effect of local anesthetic, hypertonic saline and steroid.

다양한 도구의 적용이 뒤넙다리근의 뻣뻣함과 근 긴장도에 미치는 영향 (The Effect of Applying Various Tools to the Stiffness and Muscle Tone of Hamstring Muscles)

  • Purpose : The purpose of this study was to examine the effects of tools (i., extracorporeal shock wave therapy, massage gun, and foam roller) on range of motion, muscle tone and pain threshold among patients with hamstring stiffness. Methods : Fourteen participants with hamstrings stiffness were recruited. Interventions were performed 6 times, and each session was for 30 seconds using the three tools. The range of motion, muscle tone, and pain threshold were measured. The order of the use of the three tools was randomly determined. The foam roller was made to move from the bottom of the hip crease to the upper part of the back of the hamstring. Additionally, velocity 5 vibration stimulation was performed on the hamstring using a massage gun. Moreover, vibration stimulation was performed on the hamstring with extracorporeal shock wave therapy 5 minutes, 5 Hz, and 1,500 strokes. The flexibility of the posterior thigh muscle was based on maintaining the knee and hip joints in a 90 ° bend in the supine position. The joint angle of the knee was measured, when the knee was actively extended, at the maximum point where the posterior thigh muscle was stretched. The elasticity of the posterior thigh muscle was measured while the subject was prone and in a relaxed state without any force. Measurements were made at the muscle abdominal area of the semitendinosus muscle of the posterior femur, and the area to be measured was marked with a pen. The measurement of the tenderness threshold of the posterior femur was measured using a tenderness meter(Commander Algometer, J-Tech, USA). The force value at the point at which the pressure sensation change to pain was measured after applying vertical pressure to the posterior femur muscle, which was the halfway point between the ischial tuberosity and the popliteal surface of the subject lying on their stomach. Results : The extracorporeal shock wave therapy increased stiffness and, muscle tone, and caused changes in the pain threshold, whereas the other two tools had no effect on these indices. Conclusion : Extracorporeal shock wave therapy has important effects on range of motion and muscle stiffness and can be used in warmup protocols.

전기 자극이 여성 노인의 만성 무릎통증, 운동자기효능 및 삶의 질에 미치는 효과 (Effects of Electrical Stimulation Therapy on Chronic Knee Pain, Exercise Self-Efficacy, and Quality of Life in Korean Elderly Women)

  • Purpose: This study was to examine the effects of electric stimulation therapy on chronic knee pain, exercise self-efficacy, and quality of life in Korean elderly women. Methods: The design was an unequivalent control pretest-posttest study. Samples were total of 60 (experimental: 30, control: 30) elderly women with healthy cognitive and communication abilities aged 65 years old and above. The experimental treatment involved was the electric stimulation of both thigh quadriceps muscles for 15 minutes per treatment, 3times per a week, for a total of 12 weeks. Measurements taken were S-F MPQ and AIMS for chronic knee pain, exercise self-efficacy measurement for exercise self-efficacy, and S-F 36 scale for quality of life. Data were analyzed using SPSS PC+ 12. Results: Chronic knee pain according to the Short-Form McGill Pain Questionnaire (t=43.563, p=.000) and Arthritis Impact Measurement Scale (t=31.364, p=.000) were significantly decreased in the experimental group by the application of electrical stimulation therapy. Exercise self-efficacy (t=107.116, p=.000) and quality of life (t=76.429, p=.000) were significantly increased in the experimental group by the application of electrical stimulation therapy. Conclusion: Electrical stimulation therapy could be a more effective primary nursing intervention in decreasing chronic knee pain, and on increasing exercise self-efficacy and quality of life for Korean elderly women.

근위부 대퇴직근의 자연 파열 - 증례보고 - (Spontaneous Rupture of Proximal Rectus Femoris: A case report)

  • Rupture of the rectus femoris at the musculotendinous junction is very rarely reported in the literature, especially which is caused by chronic repetitive stimulation. A 21 year old soldier complained of progressing right thigh pain and mass for 8 weeks after military training. MRI of the right thigh showed inhomogenous mass lesion suspecting soft tissue tumor. However ultrasonographic examination showed torn retracted rectus femoris tendon at musculotendinous junction. One year after primary repair of ruptured tendon, the patient is free from symptoms and returned to full previous activities. Knowledge of this sonographic and MRI finding will help the surgeon to diagnose and treat this rare injury.

Awake intubation in a patient with huge orocutaneous fistula: a case report

  • Mask ventilation, the first step in airway management, is a rescue technique when endotracheal intubation fails. Therefore, ordinary airway management for the induction of general anesthesia cannot be conducted in the situation of difficult mask ventilation (DMV). Here, we report a case of awake intubation in a patient with a huge orocutaneous fistula. A 58-year-old woman was scheduled to undergo a wide excision, reconstruction with a reconstruction plate, and supraomohyoid neck dissection on the left side and an anterolateral thigh flap due to a huge orocutaneous fistula that occurred after a previous mandibulectomy and flap surgery. During induction, DMV was predicted, and we planned an awake intubation. The patient was sedated with dexmedetomidine and remifentanil. She was intubated with a nasotracheal tube using a video laryngoscope, and spontaneous ventilation was maintained. This case demonstrates that awake intubation using a video laryngoscope can be as good as a fiberoptic scope.

Effect of Acute Aquatic Plyometric Training on Muscle Strength, Edema and Pain

  • The purpose of this study was to investigate the pre- and post-exercise performance, edema, and pain of plyometrics in water and land environments. Twelve males in their 20s were selected as subjects and performed 10 sets of squat jumps 10 times in 2 environmental conditions (water and ground). There was no significant difference in iEMG of vastus medilais according to exercise conditions and time. In MPV of CMJ, there was no significant difference according to exercise conditions and time. The thigh circumference showed a significant difference according to the exercise condition and time, and was higher in the ground condition after exercise. There was a significant difference in pain according to the exercise condition and time, and it was found to be high after exercise, 48 hours, and 72 hours in the ground condition. We believe that plyometric training in an aquatic environment will have less swelling and pain compared to plyometric training conducted in a land environment, and the pain will improve quickly, so we think that training can be conducted in a relatively shorter period than in the land environment.
