Treatment Outcomes of Mandibular Advancement Devices in Obstructive Sleep Apnea Patients

  • Mandibular advancement devices (MAD) are therapeutic options for obstructive sleep apnea (OSA). The aim of study was to investigate treatment outcomes of before and after insertion of MAD in OSA patients. We retrospectively selected a total of 13 patients who were diagnosed with OSA syndrome. All sleep-related parameters including apnea-hypopnea index (AHI), oxygen desaturation index (ODI), wake after sleep onset (WASO), total arousal were measured by before and after MAD. The use of MAD proves to be efficient in reducing snoring, apnea-hypopne index ($17.2{\pm}14.6$ vs $20.9{\pm}14.6$), WASO ($27.4{\pm}28.8$ vs $47.9{\pm}43.6$), oxygen desaturation index ($9.0{\pm}11.6$ vs $16.4{\pm}11.7$), stage N3 ($54.8{\pm}45.2$ vs $36.6{\pm}22.0$), REM sleep times ($73.3{\pm}19.4$ vs $66.0{\pm}31.0$) and increases sleep efficiency ($92.6{\pm}6.6$ vs $87.2{\pm}11.2$). The decreases in apnea index based on a reduction in the overall and supine AHI values after MAD therapy were significantly greater for the positional OSA than nonpositional OSA patients. The use of MAD proves to be efficient in snoring, WASO, sleep efficiency, reduced AHI and associated with good compliance of patients.

Visuomotor Coordination Deficits of Ipsilateral Upper Limb in Stroke Patients with Shoulder Pain

  • Purpose: The purpose of this study was to investigate whether ipsilateral shoulder pain affects the sensorimotor function of the same side shoulder in patients with stroke. Methods: Thirty stroke patients, who were divided into the ipsilateral shoulder pain group (n=15) and the ipsilateral shoulder non-pain group (n=15). Subjects were evaluated on performance of a tracking task, joint reposition test and 9-Hole pegboard test for sensorimotor functions, and Fugl-Meyer test and Motricity Index for functional ability of the contralateral side. Results: In comparison of the two groups, significant differences in performance on functional ability, including the Fugl-Meyer test(both upper and lower limb) and Motricity Index(only lower limb) were observed (p<0.05). With regard to sensorimotor functions, the ipsilateral shoulder pain group were observed significantly poor scores on the Accuracy Index, joint reposition score and 9-Hole pegboard test, when compared with the ipsilateral shoulder non-pain group (p<0.05). Conclusion: We found that ipsilateral shoulder pain could impede accurate performance of a movement and result in deteriorated proprioception of the ipsilateral shoulder. Therefore, careful evaluation and appropriate therapeutic intervention are essential for stroke patients who suffer from ipsilateral shoulder pain.

A Retrospective Study on 15 Patients of Chronic Prostatitis/ Chronic Pelvic Pain Syndrome (만성 전립선염/만성 골반통증 증후군 환자 15례에 대한 후향적 연구)

  • Objective : The purpose of this study is to report the clinical effect of herb medicine (Indongsoyeom-bang) and acupuncture therapy on Chronic Prostatitis/Chronic Pelvic Pain Syndrome. Methods : 15 patients diagnosed with Chronic Prostatitis/Chronic Pelvic Pain Syndrome were enrolled in this study. They were treated with herb medicine (Indongsoyeom-bang) and acupuncture therapy. To evaluate the therapeutic effect, before and after we measured Extract Prostatic Secretion (EPS), National Institutes of Health Chronic Prostatitis Symptom Index (NIHCPSI) and International Prostatitis Symptom Score (IPSS). Result : After treatment WBC numeral index by EPS test and NIH-CPSI and IPSS decreased. Conclusion : Herb medicine (Indongsoyeom-bang) and acupuncture therapy are effective in treating Chronic Pelvic Pain Syndrome. Further study of mechanism and efficacy is needed.

Daily Physical Functioning and Quality of life for Stroke (뇌졸중 환자의 일상적인 신체적 기능과 삶의 질 수준)

  • Purpose: It remains controversial for the effect of daily functioning and quality of life on therapeutic exercise after stroke. The purpose of this study was to describe the effects on daily functioning and QOL. Methods: Outcome measures of daily functioning included, such as the Functional Independence Measure (FIM), Barthel index. Outcome measures of QOL included, such as Stroke Impact Scale(SIS) and the Medical Outcomes Study short-form 36-item questionnaire(SF-36). Results: 125 stroke patients were recruited, who were in or outpatients. The average age was 55.4 years. 64.8% were male. The mean Bathel index and FIM score was 63.7 and 87.5. The mean SIS score were higher in communication and mean SF-36 score were higher in physical pain. In/out patients are associated with SIS (communication, emotion) and SF-36(social function, energy or fatigue). Sex are associated with SF-36 (physical function). Other disease state are associated with SIS(hand function) and SF-36 (physical function). Paralysis portion are associated with SIS(communication, daily activity). Barthel index are associated with SIS(communication, mobility) and SF-36(social function, physical function, role limits due to emotional problems). Conclusion: These findings may provide the useful with rehabilitation professionals, who specilalized in the importance of QOL in designing treatment modalities.

The Effects of Zanthoxylum schinifolium Extract in a Dextran Sulfate Sodium-induced Rat Colitis Model (Dextran Sulfate Sodium 유도 흰쥐 궤양성 대장염에 대한 산초 추출물의 효과)

  • The aim of this study was to evaluate the anti-inlfammatory effect of the extract of Zanthoxylum schinifolium on ulcerative colitis induced by 3% dextran sulfate sodium in the rat. The experiment animals were divided into six groups: control (normal), DDS-induced colitis, 1 mg/kg, 15 mg/kg and 150 mg/kg of the extract of Z. schinifolium and 150 mg/kg of 5-aminosalicylic acid (5-ASA) as comparative drug. Anti-ulcerative colitis effect was evaluated pathologically by disease activity index (DAI), the change of weight and myeloperoxidase (MPO) in colon mucosa. Treatment with 15 mg/kg and 150 mg/kg of the extract of Z. schinifolium significantly improved the gain of body weight and DAI as clinical symptoms, and reduced MPO and $PGE_2$ level as biochemical index. Especially, 150 mg/kg of the extract of Z. schinifolium showed markedly more improvement than the same dose of 5-ASA in all kind of index such as MPO, $PGE_2$ and DAI. These results suggest that Z. schinifolium mediated anti-inflammatory action on colorectal sites may be a useful therapeutic approach to ulcerative colitis.

Therapeutic Effect of Tetrax based on Visual Feedback Training on Balance Dysfunction due to Ataxia in Subjects with Cerebellar Stroke: A Retrospective Study (소뇌 뇌졸중 환자에서 실조로 인한 균형장애에 대한 시각적 피드백 훈련 기반 테트락스의 치료적 효과: 후향적 연구)

  • PURPOSE: This study is to investigate the therapeutic effect of Tetrax on balance dysfunction caused by ataxia in cerebellar stroke. METHODS: A total of thirty subjects with cerebellar stroke were recruited. The participants was divided into two groups, the experimental (n=15) and the control group (n=15). Tetrax training and conventional physical therapy (CPT) were performed in experimental group, whereas the patients in the control group were treated with CPT twice a day. Each session of the Tetrax and CPT was carried out for 30 minutes, 5 times per week for 4 weeks. Korean version of the Scale for the Assessment and Rating of Ataxia (K-SARA) was the primary outcome measure, and the secondary outcomes covered Berg balance scale (BBS), falling index (FI), Timed up and go (TUG), and modified Barthel index of Korean version (K-MBI). All outcome measures were evaluated before and after 4 weeks. RESULTS: K-SARA was decreased significantly after 4 weeks intervention in both the experimental (p<.05) and the control group (p<.05). Furthermore, the experimental group produced significantly better outcomes in K-SARA, BBS, FI, and TUG compared with the control group (p=.012, p=.027, p=.008, and p=.048). There were significant correlations between K-SARA and BBS, FI, TUG, and K-MBI (p<.001, p<.001, p=.004, and p<.001). CONCLUSION: The restoration of ataxia was related with the improvement of the balance, falling risk, mobility, and activity of daily living. Tetrax training was effectively aided recovery of ataxia after cerebellar stroke.

A home-based exercise program for temporomandibular joint osteoarthritis: pain, functionality, and joint structure

  • Objectives: Osteoarthritis (OA) is the most prevalent and disabling joint disease in the world. Temporomandibular joint (TMJ) exercise is a widely used treatment and could be a beneficial and long-term tool for treating TMJ OA. The present study aims to evaluate the effects of therapeutic exercise in the conservative treatment of TMJ OA. Materials and Methods: A single-group experimental pre-post test was performed. We included patients who met the diagnostic criteria for TMJ OA. Outcome variables were pain intensity (visual analogue scale), functionality (Helkimo index), and structural changes (ultrasound). Follow-up periods were at months 1, 3, and 6. The intervention included a home-based program with thermotherapy, manual therapy, and therapeutic exercise during the entire follow-up period. Results: We included 15 patients and 26 joints, all women with a median age of 57 years (range, 49-62 years). Median change in pain intensity on joint palpation, mouth opening, and at rest at the first month was 47.5 mm, 51 mm, and 60 mm, respectively, and 48 mm, 49.5 mm, and 42.5 mm, at six months (P=0.001). The Helkimo index showed significant improvement in medians from baseline severe dysfunction (17 points) to minimal dysfunction at three and six months (2 points) (P=0.001). Ultrasound showed improved disc position. Conclusion: This study demonstrated significant improvements in pain, function, and joint disc position and represents a valuable tool for the long-term treatment of patients with TMJ OA.

The Impact of Program Improvement Using Forest Healing Resources on the Therapeutic Effect: Focused on Improving Index of Greenness for Adolescents

  • This study is to examine the effect of improving the forest therapy program for adolescents using forest healing resources (focused on improving index of greenness for adolescents). The participants were 30 students from in the control group that participated in the 2018 program, and 51 students in experimental group that participated in the improved program in 2019. The questionnaire, developed by Korea Forest Welfare Institute, was comprised of items on general matters, index of greenness, restorative environment, positive emotion, negative emotion, facial expression and psychological assessment. The control group had 30 and the experimental group had 49 valid copies of the questionnaires. As a result of the paired sample t-test for each group, the control group showed a significant increase in all categories except restorative environment. In the experimental group, all categories significantly improved to a higher level (p <.01). An independent sample t-test (one-tailed test) was performed to test the effect of the forest therapy program with improved index of greenness. As a result, the index of greenness increased by 0.73 points(t=2.555, p <.01) and restorative environment by 1.01 points (t=2.567, p <.01), showing statistical significance. Negative emotion increased by 0.04 points (t=0.183, p >.05), which was not significant. On the other hand, positive emotion decreased by 0.42 points (t=-1.918, p <.05), facial expression by 0.57 points (t=-1.775, p <.05), and psychological assessment by 0.29 points (t=-0.981, p >.05), showing significance in positive emotion and facial expression. However, all the decreased items showed significant improvements between the pretest and posttest scores of the experimental group.

The Clinical Studies about improvement rate in managering patients who had lumbago and pain in loin & feet with herbal-acupunture therapy (요통(腰痛) 및 요각통(腰脚痛) 환자에 있어 약침치료(藥鍼治療)의 호전도에 대한 임상적 고찰)

  • Objectives : The purpose of this study is to compare the improvement rate in managering patients who had lumbago and pain in loin & feet by herbal-acupunture therapy with conservative treatment. Methods : 40patients who invited from March. 2003 to May. 2004 with lumbago and pain in loin & feet inve, were divided into two classes. A class was managed with conservative treatment(acupuncture, herb med, negative therapy, bed rest and physical therapy) and herbal-acupuncture therapy, the other class was managed with conservative treatment only. Two classes were distributed by sex and age, therapeutic periods, radiolographic results and therapeutic results that checked with VAS(visual analog scale) and ODI(Oswestry disability index). Results : According to VAS and OBI results, A class was managed with conservative treatment and herbal-acupuncture therapy shows the improvement in controling the pain of lumbago etc. comparing with the other class.

Effect of Royal Jelly on Therapy and Prevention of Streptozotocin Induced Diabetic Rats (로얄제리가 Streptozotocin 유발 당뇨 흰쥐의 치료 및 예방에 미치는 영향)

  • In order to investigate the preventive and therapeutic of royal jelly on diabetes, the levels of blood glucose and serum lipids as well as the number of blood cells were determined in streptozotocin(STZ) diabetic rats. Rats were divided into seven groups. The RJ group was administered royal jelly and the STZ group was treated with streptozotocin to induce diabetes. To determine the preventive effect, diabetes was induced after administration of royal jelly for 2 weeks in group RS1/RS2. In group SR1/SR2 diabetic rats were administered royal jelly for 2 weeks to investigate the therapeutic effect. After 3 weeks, the body weight was reduced in STZ and SR1 groups and food intake was increased in the STZ, RS1 and SR1 groups. The blood glucose level was similar to the control group in the RJ, RS1 and RS2 groups and there was no effect in the other groups. The total lipid and triglyceride level were lower in the SR1 group as compared to STZ, and the total cholesterol level was higher in the STZ group. The index of atherogenesis was lower in the RJ and SR1 groups compared to the normal group. The number of red blood cells and hemoglobin was higher in the RJ and SR1 groups and the number of white blood cells was higher in the RJ and SR2 groups.
