Meta-analysis on the Efficacy of Glutamate Receptor Antagonists for Acute Migraine Treatment (급성 편두통 치료를 위한 글루탐산 수용체 길항제의 임상적 유효성에 대한 메타분석)

  • Background: Glutamate is implicated in the pathophysiology of migraine, a common neurological disorder. Therefore, glutamate receptor antagonists (GluRAs) have been suggested as a novel migraine treatment that are able to overcome the limitations of triptans. Objective: The aim of this study was to perform a meta-analysis to assess the efficacy of GluRAs for patients with migraine. Method: The PubMed, Cochrane Library, CINAHL, and Clinical Trial.gov databases were searched for randomized placebo-controlled trials of the efficacy of GluRAs for patients with migraine conducted up to August 2019. Two independent reviewers screened the literature according to inclusion and exclusion criteria and performed quality assessment and data extraction. Review Manager 5.3 software was used for the meta-analysis. Results: Three studies involving a total of 206 patients were included in the final analysis. Compared with placebo, GluRAs significantly improved the pain-free response at 2 hours (odds ratio [OR]=3.85, 95% confidence intervals [CIs]=1.63-9.09) and the 24-hour sustained pain freedom (OR=7.40; 95% CIs=2.36-23.20). The use of rescue medications with GluRAs was lower compared to that with placebo, but the difference was not significant (OR=0.39, 95% CI=0.10-1.47). Conclusion: Our meta-analysis showed that GluRAs were more effective than placebo for patients with migraine.

An Economic Feasibility Study for Construction and Use of Korea Ocean Research Stations (종합해양과학기지 구축 및 활용의 경제성 분석)

  • Korea ocean research stations manage the weather and environmental data collected from coastal and ocean areas to provide short-term and long-term ocean forecasts. The purpose of this paper is to analyze and quantify economic benefits of the ocean research stations with sensors to observe physical, chemical, and biological data. The construction and operation of an integrated ocean observation station is expected to reduce uncertainty about ocean and coastal areas and to improve the quality of ocean forecasts. The economic benefits are mainly come from improved search and rescue operations, ocean pollution management, yellow dust management, and improved productivity in ocean-related industries. In addition, an input-output analysis is performed to evaluate the economic impacts of ocean research stations nationwide. The analysis shows that the system can contribute to industries such as fishing, maritime and air cargo, medical and health care.

A study on the current status and the obstacles to prehospital spinal motion restriction performed by 119 paramedics to major trauma patients (중증외상환자에 대한 119구급대원의 척추고정 실태 및 장애요인)

  • Purpose: This study attempts to improve the status of emergency care for major trauma patients transferred by 119 paramedics by analyzing the status of emergency care and the obstacles to the spinal motion restriction (SMR) for major trauma patients. Methods: A total of 600 rescue logs were collected from major trauma patients transported by 119 paramedics in the C fire department from Jan. 1, 2015, to Dec. 31, 2017. And then, 280 questionnaires were collected from the 119 paramedics in C fire department from May 3 to Jun. 3, 2019. Data were analyzed using SPSS 24.0 version. Results: Among 499 spinal motion restriction adaptive patients, the spinal motion restriction rate was 51.1% (255 individuals). Lack of human resources and quality control problems were among the obstacles to spinal motion restriction. Conclusion: The 119 paramedics should improve their activeness and skills in performing emergency care, and since training and experience are of crucial importance, they should expand various education systematized according to demand.

  • Reliable and precise indoor localization is crucial for personal navigation, emergency rescue, and monitoring workers indoors. To use this technology in different applications, it is important to make it less dependent on infrastructure and to keep the error as small as possible. Fingerprinting stands out as a popular choice for indoor positioning because it leverages existing infrastructure and works with just a smartphone. However, its accuracy heavily relies on the quality of that infrastructure. For instance, having too few access points or beacons can greatly reduce its effectiveness. To reduce dependence on RF infrastructure, we have developed surface correlation (SC) using accumulated Received Signal Strength Indicator (RSSI) signals This approach constructs a user mask for radio map comparisons using an accumulated RSSI vector and the trajectory of the user, which is estimated through PDR. The location with the highest correlation is considered as the user's position after comparison. Through a simulation, the performance of short RSSI vector-based technology and SC is analyzed, and future directions for the development of SC are discussed.

Intraovarian vascular enhancement via stromal injection of platelet-derived growth factors: Exploring subsequent oocyte chromosomal status and in vitro fertilization outcomes

  • The inverse correlation between maternal age and pregnancy rate represents a major challenge for reproductive endocrinology. The high embryo ploidy error rate in failed in vitro fertilization (IVF) cycles reflects genetic misfires accumulated by older oocytes over time. Despite the application of different follicular recruitment protocols during IVF, gonadotropin modifications are generally futile in addressing such damage. Even when additional oocytes are retrieved, quality is frequently poor. Older oocytes with serious cytoplasmic and/or chromosomal errors are often harvested from poorly perfused follicles, and ovarian vascularity and follicular oxygenation impact embryonic chromosomal competency. Because stimulation regimens exert their effects briefly and immediately before ovulation, gonadotropins alone are an ineffective antidote to long-term hypoxic pathology. In contrast, the tissue repair properties (and particularly the angiogenic effects) of platelet-rich plasma (PRP) are well known, with applications in other clinical contexts. Injection of conventional PRP and/or its components (e.g., isolated platelet-derived growth factors as a cell-free substrate) into ovarian tissue prior to IVF has been reported to improve reproductive outcomes. Any derivative neovascularity may modulate oocyte competence by increasing cellular oxygenation and/or lowering concentrations of intraovarian reactive oxygen species. We propose a mechanism to support intrastromal angiogenesis, improved follicular perfusion, and, crucially, embryo ploidy rescue. This last effect may be explained by mRNA upregulation coordinated by PRP-associated molecular signaling, as in other tissue systems. Additionally, we outline an intraovarian injection technique for platelet-derived growth factors and present this method to help minimize reliance on donor oocytes and conventional hormone replacement therapy.

The Relationship Between Fire Service Demand and Project Budget (소방수요와 사업비예산과의 관계)

  • The environment of fire service is more uncertain as cities become complicated and new combustibles are used. If fire service's budget is not allocated sufficiently for fire facilities and equipment, fire service's quality and firefighter's safety are affected seriously. Therefore project budget must be increased to prevent fire and disaster effectively. This study investigate the change of fire service demands, the change and priority of fire service budget and project budget. And this study examined the relationship between fire service demand and project budget using correlation and regression analysis. Analysis showed that a number of rescue and emergency medical service were significant variable for project budget.

Differences in quality of sleep, job stress, fatigue, job satisfaction, and posttraumatic stress disorder according to the type of work in fire-fighting officers (소방공무원의 근무형태에 따른 수면의 질, 직무스트레스, 피로도, 직무만족 및 외상 후 스트레스장애 차이)

  • Purpose: The purpose of the study was to investigate the differences in quality of sleep, job stress, fatigue, job satisfaction, and posttraumatic stress disorder (PTSD) according to the type of work in fire-fighting officers. Methods: This study used an online questionnaire. The participants were 322 fire-fighting officers. The data were collected from May 2 to 20, 2018. Data were analyzed using SPSS 22.0. Results: According to the work styles of fire fighting officers, job stress, job satisfaction, and PTSD showed significant differences. However, There were no differences in sleep quality and fatigue. There were significant differences in job stress, job satisfaction, and PTSD in those in roles of fire suppression and rescue emergency medical technicians and this result suggests a way to improve the quality of sleep, job stress, job satisfaction and fatigue for fire-fighting officers who suffer from PTSD. Conclusion: Based on the results of this study, mental health of fire-fighting officers will be improved through basic data for psychotherapy which can overcome the quality of sleep, job stress, fatigue, job satisfaction and PTSD according to the work style of fire-fighting officers.

Caffeine Treatment during Oocyte Aging Improves the Developmental Rate and Quality in Bovine Embryos Developing In Vitro

  • In mammal, unfertilized oocytes remain in the oviduct or under in vitro culture, which is called "oocyte aging". This asynchrony negatively affects fertilization in pre- and post-implantation embryo development. Caffeine a phosphodiesterase inhibitor is known to rescue oocyte aging in several species. The objective of this study is to determine the cytoskeleton distribution in aged oocytes and the embryo developmental ability of aged oocytes in the present or absence of caffeine during maturation. Caffeine treatment increased the incidence of normal spindle assembly of aged oocytes (treatment, $67.57{\pm}4.11%$ aging, $44.61{\pm}6.4%$) and no significant differences compared to control group. Fluorescence values were compared using ROS (Reactive oxidation species) stain. Fluorescence values appear of control group intensity rate ($51.53.{\pm}3.80$), aging group ($68.10{\pm}5.54$) and treatment of caffeine ($45.04{\pm}2.98$). Aged oocytes that were derived from addition of caffeine to the IVM (in vitro maturation) medium had significantly increased 2-cell that developed to the blastocyst stage compared to the aging group. Blastocysts, derived from caffeine treatment group, significantly increased the total cell number compare aging ($90.44{\pm}10.18$ VS $67.88{\pm}7.72$). Apoptotic fragments of genomic DNA were measured in individual embryo using TUNEL assay. Blastocyst derived from caffeine treatment group decreased significantly the apoptotic index compared to blastocyst derived from aging group. In conclusion, we inferred that the caffeine treatment during oocyte aging can improve the developmental rate and quality in bovine embryos developing in vitro.

A study on problem solution method for works improvement and advancement quality of life on Job analysis of Emergency Medical Technicians (응급구조사의 직무분석에 따른 업무개선과 삶의 질 향상을 위한 문제점 해결 방법에 관한 연구)

  • This study is a descriptive survey research to know the levels of job satisfaction and influential factors for emergency medical technicians(EMT). This study sampled Level 1 and Level 2 Rescue Members from fire stations and EMT from general hospitals in the Gwangju City and JeollaNamdo area. Interviews for the 75 participants of this convenience sample were conducted for a two month period from July to September 2006. Data was collected on interviewing questionnaires. When looking at the results above internal factions were most influential towards satisfaction and self-responsibility, fulfillment, displays of Aptitude were highest for satisfaction. However, organizational factors were the lowest. There is a need for reform in promotion term periods and policy as well as promotion opportunities. Influential factors for job satisfaction were Role Performance Ability, Intentions for Changing Jobs, and Job Selection Motivation. Therefore, the more insufficient your ability to perform your job the less satisfied you are with it and the more fear you feel from experiences on the job the less satisfied you are with your job. Henceforth, there is a need to reform the educational programs offered to EMT for job training emphasizing job performance ability and the ability to judge for oneself on emergency situations, and reform the promotional system. Finally, there is a need for psychological consultation to offer stability to those after treating an emergency situation and encouragement for personal religion.

The Study on Improvement of Thermal Stress Breakage in Infrared Transmissive Glass for Aircraft Searchlight (항공기용 탐조등 적외선 투과 유리의 열응력 파손 현상 개선 연구)

  • Aircraft searchlights are used in search or rescue missions when performing night missions, and provide infrared light to keep operations in concealment. During aircraft operation, the infrared ray transmission filter glass for generating infrared rays can break, resulting in problems such as loss of concealment ability and maintenance. In this paper, we describe the procedure for testing the operating conditions of the searchlight when the defect occurs and finding the cause of the defect. We also summarize various methods to improve the cause and process of making the improved filter prototype. In addition, we also describe the results of verifying the performance requirements of the searchlight. As a result of the test and verification, the damage of the filter was improved and the improved filter was applied to the aircraft.
