Association between the Pattern of Prophylactic Antibiotic Use and Surgical Site Infection Rate for Major Surgeries in Korea (예방적 항생제 사용 양상과 수술부위감염률의 관련성)

  • Objectives : The purpose of this study was to analyze the association between the pattern of prophylactic antibiotic use(PAU) and the surgical site infection(SSI) rate for major surgeries in Korea. Methods : We retrospectively reviewed the medical records of patients who underwent cardiac, colon and gastric surgery, hysterectomies and hip/knee replacements at 20 hospitals, and inclusive of over 500 beds. We randomly sampled 60 cases per surgery type for patients discharged between September and November, 2006. A total fo 2,924 cases were included in our analysis. Cox's proportional hazard analysis was conducted to evaluate the association between the pattern of PAU and SSI rate. Results : The proportion of patients who received their first prophylactic antibiotics(PA) 1 hour before incision was 65.5%, who received inappropriate PAs was 80.8%, and the proportion of patients whose PA was discontinued within 24 hours of surgery was 0.5%. The average duration of PAU after surgery was 9 days. The relative risk(RR) of SSI in patients who received their first PA more than 1 hour before incision was significantly higher than for those who received it within 1 hour prior to incision(RR=8.20, 95% CI=4.81-13.99). Inappropriate PA selection increased SSI rate, albeit with marginal significance(RR=1.97, 95% CI=0.96-4.03). Also, prolonged PAU following surgery had no effect on SSI rate. Conclusions : These results suggest that the pattern of PAU in the surgeries examined was not appropriate. Errors in the timing of PAU and of PA selection increase SSI rate. SSI rate remained unaltered following prolonged PAU after surgery.

The effect of community based participatory obesity management program in obese school children (학령기 비만아동을 위한 지역사회 참여형 비만관리 프로그램의 효과)

  • The aim of this study was to develop a community based participatory obesity control program and measure the effects of the program on obesity, exercise habit, eating habit and life habits among obese elementary school children. The community based participatory obesity control program was provided for the experimental group of $4^{th}$ grade elementary school students for 10 weeks. The program included obesity related health educations and counseling once a week, jump rope classes twice a week and folk game classes held once a week. Method: There were 24 students with obesity in the experimental and 27 in the control groups. The collected data were analyzed by the Kolmogorov-Smirnov test, ${\chi}^2$-test, independent t-test and repeated measure ANOVA. Results: After 10weeks of participation in the program, the experimental group showed significantly lower Body Mass Index (p<.001) and waist hip ratio (p=.036), significantly higher exercise habit (p=.004), eating habit (p=.003) and life habit (p=.001) than those in the control group. Conclusion: This study provides evidence that community based participatory obesity management program is effective nursing intervention for managing child obesity.

Biomechanical Analysis of Lower Limb on Stance during Golf Swing (골프 스윙 시 스탠스에 따른 하지의 역학적 분석)

  • The purpose of this study was to investigate the body's strategy through kinematic variables of the lower extremities and ground reaction forces to maintain the club-head speed and ball accuracy despite the three stances during the golf swing. Ten male golfers who official handicap two were participate in the experiment. All subjects performed swing after maintaining the address posture according to stance conditions(square; SS, open: OS, closed: CS). Using a 3D motion analysis system and force plateform, the results were calculated with the 7-iron full swing each stance. In result, there was no difference in center of displacement, and left and right hip and knee joint angle displacement. Left ankle joint was largely plantar-flexed in OS, and right ankle joint was largely performed in CS from the address to the downswing. From address to take-back, right foot had a large left direction and the left foot had a right direction were greater in OS than in CS. Therefore, despite various stances, maintaining the same posture at impact is thought to have a positive effect on club head speed and ball direction.

Comparison of Brand-name School Uniform Patterns for Middle School Girls and Development of School Uniform Patterns by Students' Body Shape, Using 3D Virtual Clothing Simulation (3D virtual clothing simulation을 활용한 여자 중학생의 브랜드교복 패턴비교 및 체형별 교복 패턴개발)

  • In terms of junior high school girls' growth patterns during early adolescence, unlike childhood when relatively balanced growth patterns are found and high school years in which the normal adult body type is almost reached, junior high school girls display imbalanced and rapid growth. In fact, diverse size changes by body part occur with a significant difference among individuals. Therefore, it has been difficult for junior high school students to select their exact size when buying a school uniform. This study attempted to develop winter blouse and skirt patterns reflecting the latest comfortable and active school uniform trends, using middle school girl avatars of various body shapes. Skirt and blouse pattern-drawing methods and margins differed. Based on such results, research prototypes were prepared. Then, virtual wear prospective drawings, clothing pressure, and appearance were assessed by body shape. Skirts were assessed with 22 factors while blouses were analyzed with 25 factors. Then, correlations between skirts and blouses were analyzed. According to the analysis, the reason why the dart & pleats position and margin were rated low was confirmed. In a virtual wear assessment on skirt patterns by body shape, a significant difference was found in all categories except for position of the hip circumference, margin of the hips, width of the skirt, and appropriateness of waist line position. The virtual wear assessment on the blouse patterns by body shape also revealed a significant difference in all categories but fit and shape pf the back part. In blouses, a significant difference was observed around shoulders and waist in type 1 and around the belly in type 2. On the contrary, for skirts, a significant difference was found around the hips and waists in type 1 and type 2. Therefore, these factors should be considered in making blouses and skirts. The above results suggested that skirt and blouse patterns should vary by body shape. It is anticipated that there should be further studies comparing brand-name school uniforms for high school girls and school uniforms by body shape.

The relation of Dampness-Phlegm Diagnosis and interanl carotid artery stenosis by carotid artery sonography in cerebral infarction patients (경동맥 초음파를 이용한 뇌경색 환자의 내경동맥 협착도와 습담변증(濕痰辨證)의 관련성 연구)

  • Objectives : This study was aimed to clarify the relationship between the dampness-phlegm diagnosis and internal carotid artery stenosis by measuring carotid artery sonography in cerebral infarction patients. Methods : One hundred eighty subjects were recruited from the patients admitted to the Department of Internal Medicine at Kyunghee university oriental medical center from September 2008 to July 2010. We assessed one hundred eighty patients' carotid artery sonography data and diagnosed dampness-phlegm by oriental medical diagnosis. then, analyzed their characteristics, risk factor, lifestyle, metabolic syndrome, body mass index, Waist/Hip ratio(W/H ratio) and dampness-phlegm diagnosis. Results : On the demographic variables of the patients, age, smoking, W/H ratio and dampness-phlegm group were significantly higher in severe internal carotid artery(ICA) stenosis group than in the control group. According to the significant difference in dampness-phlegm group, we analyzed dampness-phlegm related index for pattern identifications by ICA stenosis. As a result, sputum, bowel sound, chest discomfort, slippery pulse were significantly higher in the severe ICA stenosis group than in the control group. In multivariate analysis, dampness-phlegm group showed close relationship with severe ICA stenosis group. Conclusion : According to the analysis, significance between dampness-phlegm diagnosed patients group and severe ICA stenosis were clarified. These results can be utilized in the future as a basis material.

Foot/Ankle Roll-Over Characteristics According to Different Heel Heights of Shoe during Walking (보행 시 신발 굴 높이어 따른 발/발목 복합체의 Roll-Over특성)

  • This study investigated the effects on foot and ankle roll-over characteristics according to different heel heights during walking. Fifteen female volunteers who have neither musculoskeletal nor foot problems were participated in gait analyses, wearing four different pairs of shoes in different heel heights. To obtain roll-over shape of foot/ankle complex, we used trajectories of knee and ankle joints as well as the renter of pressure between initial contact and opposite initial contact. Results revealed that the entire roll-over shape moved distally as the heel height increased but roll-over characteristics showed uniformly with different heel heights. In addition, we found that nondisablea persons automatically adapted to their foot/ankle complex to varying heel heights within 6cm, by moving the origin of roll-over shape distally to maintain roll-over characteristics. However, since the balance of the gait only by the ankle joint could not be achieved beyond the heel height of 6cm, compensations at the knee and the hip joints occurred simultaneously. Roll-over characteristics in human walking would provide simpler and wider understanding of human walking, and furthermore could be applied to the wide understanding of prosthetics and orthotics of the lower extremity as well as orthopaedic shoes.

A Novel COMP Gene Mutation in a Korean Kindred with Multiple Epiphyseal Dysplasia

  • Multiple epiphyseal dysplasia (MED) is a clinically and genetically heterogeneous chondroplasia, characterized by delayed development of the ossification centers and, deformities of the extremities that involve only the epiphysis and result in mild short stature. Mutations in the cartilage oligomeric matrix protein (COMP) gene are most commonly found, and most of the mutations are located in the calmodulin-like repeats and the C-terminal domain. We report a Korean kindred of 12 family members with MED in four generations who were found to have a novel mutation in the COMP gene. A pedigree showed early onset osteoarthritis requiring arthroplasty that was an autosomal dominant inherited trait. Radiological examinations demonstrated the presence of osteochondral defects in the medial femoral condyles, and the knee and hip joints showed variable degrees of precocious degenerative changes. Mutation analysis of the COMP gene in the proband and five other affected family members identified a novel missense mutation, c.1280G>C (p.Gly427Ala) in exon 12, which was not found in three unaffected family members. Direct sequencing of the COMP gene may yield pathogenic mutations in dominantly inherited MED cases, and may provide opportunities of carrier detection among high-risk family members, leading to genetic counseling for early diagnosis and intervention before the onset of complications.

Association of Metastasis with Clinicopathological Data in Mexican Patients with Osteosarcoma, Giant Cell Tumor of Bone and Chondrosarcoma

  • Background: Bone tumors are neoplasias with a high overall mortality; one of the main factors that reduce survival is their high capacity to develop metastases. It has been reported that finding lung metastases at diagnosis of osteosarcoma (OS), chondrosarcoma (CS) and giant cell tumor of bone (GCTb) is quite common. In this study, we inquire the relationship of metastases caused by these tumors with different clinical and pathological aspects, in order to guide medical personnel in the diagnosis and opportune treatment of metastases or micro metastases. Materials and Methods: We collected data of 384 patients with clinical, radiological and histopathological diagnosis of OS, GCTb and CS that attended the National Rehabilitation Institute (INR) during 2006 to 2014. Chi-square and Fisher's exact tests were performed for data analysis. Results: In the three tumor types, the presence of metastases at diagnosis was variable (p=0.0001). Frequency of metastases was 36.7%, 31.7% and 13.2% for OS, CS and GCTb respectively. The average age had no significant difference (p>0.05) in relation to metastases, even so, patients with OS and GCTb and metastases, were older while patients with CS and metastases were younger, in comparison to patients without metastases. Males had a higher frequency of metastases (68.2%, p = 0.09) in contrast to CS and GCTb, in which the metastases was more frequent in women with 51.9% (p = 0.44) and 57.9% (p = 0.56) respectively. Broadly, metastasis was associated with primary tumors located in the femur (44.4%), followed by the tibia (15.6%); metastases was more frequent when primary tumor of GCTb and OS were in the same bones, but were located in the hip (26.3%) for CS. Conclusions: The frequency of metastases in OS, GCTb and CS is high in our population and is determined by different clinicopathological variables related to the kind of tumor. Further studies are needed in order to evaluate metastases subsequent to diagnosis and associations with survival and clinicopathological factors, as well as to determine the sensitivity and specificity of current methods of detection.

Whole Genome Association Study to Detect Single Nucleotide Polymorphisms for Body Conformation Traits in a Hanwoo Population

  • A whole genome association (WGA) study was conducted to identify quantitative trait loci (QTL) for body conformation traits in Hanwoo cattle. The phenotypes of 497 steers were recorded from the Hanwoo Improvement Center of National Agricultural Cooperative Federation, Seosan, Korea, and analyzed using the Illumina Bovine 50 k SNP chip. A set of 35,987 SNPs that were available in the Hanwoo population was selected from the chip. After adjustments for the effects of year-season of birth, region and sire, phenotypes were regressed on each SNP using a linear regression model. Three hundred nineteen SNPs were detected for the ten conformation traits (p<0.003). For the significant SNPs, stepwise regression procedures were applied to determine best sets of markers. A total of 72 SNPs were selected (p<0.001), for which the sets of 5, 9, 10, 9, 8, 11, 4, 6, 3 and 7 SNPs were determined for height at withers, rump height, body length, chest depth, chest width, rump length, hip width, thurl width, pinbone width and heart girth, respectively. About 7-26% of the total phenotypic variation was explained by the set of SNPs for each trait. QTL for the conformation traits were harbored on most bovine chromosomes (BTAs). Four SNPs with pleiotropic effects on height at withers and rump height were detected on BTAs 3, 4, 6 and 16. A SNP with pleiotropic effects on chest width and rump length was also detected on BTA10. Two QTL regions, i.e. between 87 and 97 Mb in BTA3 and between 41 and 44 Mb in BTA7, were found, in which SNPs were detected for the five and three conformation traits, respectively. The detected SNPs need to be validated in other Hanwoo populations for commercial application to the genetic improvement of conformation characteristics in Hanwoo via marker-assisted selection (MAS).

Kinematical Analysis of Basket with 1/2 Turn to Handstand on Parallel Bars (평행봉 Basket with 1/2 Turn to Handstand 기술 분석)

  • The subject of this study was male apparatus gymnastics athlete who had scored high points doing basket with 1/2 turn on parallel bars. Then 3D motion analysis were used to calculate & analyse kinematic variables of Basket with 1/2 turn to Handstand. 1. The total average time spent for Basket with 1/2 turn took $2.16{\pm}.08sec$, at the downward upward phase took $.58{\pm}0.00sec$, $.23{\pm}.00sec$, at flight phase took $.28{\pm}.01sec$, at connected area phase took $.72{\pm}0.21sec$, at rotation area phase took $.35{\pm}.14sec$. To have a successful performance, there should be faster speed and velocity to rotate at the downward upward phase, then the upward velocity and height must be used adequately. Moreover, the speed must be faster at the flight connect phase to stabilize Center of Mass(CM) for the body, and must secure more time at the rotation area to have more stable performance. 2. After handstand on parallel bars while moving CM to right hand side, and It must be performed with big and magnificent performance with putting both hand's center to far away from the parallel bars. 3. Furthermore, CM must be moved fast from downwards to right hand side, and CM must be moved fast in vertical movement at upward and flight phase to avoid CM from moving back and forth, and left and right. 4. At downwards, the subject must rotate as bis as possible using hip-joint as wide as possible and at upwards, must put his body to vertical to have stable performance. While rotating or turning, it is better to do with bigger shoulder angle and have to make sure that trunk angle must be not scattered. To perform better and more positive in basket with 1/2 turn on parallel bars, the centrifugal force must be used big and fast at downward, and at upward and flight phase, downward movement must change to vertical movement as soon as possible while turning movement must happen at handstand position. Time spent must be shorten at connected area to stabilize CM and turning must be natural as possible while securing the necessary time of movement to well-balanced. Also, the body must be vertically closed from the ground.
