A Study on Depression and Activities of Daily Living among Elderly Patients in Geriatric Hospitals (요양병원 노인 환자의 우울과 일상생활 수행능력에 관한 연구)

  • Purpose: This study was conducted to examine the degrees of depression and activities of daily living (ADLs), and to identify factors that predict depression in elderly patients. Methods: The sample was149 patients (80% female; mean age 79.1) from three geriatric hospitals located in small cities and who agreed to participate in the study. Data were collected by staff nurses involved in direct patient care from October to September 2009. The level of depression was measured on a daily basis for seven days using a 13-item scale. Patients' ADLs was measured using a modified 10-item Bathel ADLs scale. Data were analyzed using SPSS/WIN 17.0. Results: Mean depression score was $10.05{\pm}4.77$ out of 26 points and about 27% was assessed as depressed with more than 13 points. Multiple regression analysis showed that low educational level, absence of spouse and fewer family visits predict depression. In addition, low ADLs and sleep disturbance predict depression. Conclusion: Nursing care providers need to be concerned about ADLs of older patients and should develop nursing activity programs to increase their in-hospital physical activities. It is also important care for older patients' sleep and encourages family visits to decrease their depression.

Anti-diabetic activities of catalpol in db/db mice

  • The objective was to investigate the hypoglycemic action of catalpol in spontaneous diabetes db/db mice. 40 db/db mice were randomly divided into five groups: model control gourp; db/db plus catalpol 40, 80, 120 mg/kg body wt. groups and db/db plus metformin 250 mg/kg group. Age-matched db/m mice were selected as normal control group. The mice were administered with corresponding drugs or solvent by gavage for 4 weeks. The oral glucose tolerance test was carried out at the end of $3^{rd}$ week. After 4 weeks of treatment, the concentrations of fasting blood glucose (FBG), glycated serum protein (GSP), insulin (INS), triglyceride (TG), total cholesterol (TC) and adiponection (APN) in serum were detected. The protein expressions of phosphorylation-$AMPK{\alpha}$1/2 in liver, phosphorylation-$AMPK{\alpha}$1/2 and glucose transporter-4 (GLUT-4) in skeletal muscle and adipose tissues were detected by western blot. Real time RT-PCR was used to detect the mRNA expressions of acetyl-CoA carboxylase (ACC) and Hydroxymethyl glutaric acid acyl CoA reductase (HMGCR) in liver. Our results showed that catalpol could significantly improve the insulin resistance, decrease the serum concentrations of INS, GSP, TG, and TC. The concentrations of APN in serum, the protein expression of phosphorylation-$AMPK{\alpha}$1/2 in liver, phosphorylation-$AMPK{\alpha}$1/2 and GLUT-4 in peripheral tissue were increased. Catalpol could also down regulate the mRNA expressions of ACC and HMGCR in liver. In conclusion, catalpol ameliorates diabetes in db/db mice. It has benefit effects against lipid/glucose metabolism disorder and insulin resistance. The mechanism may be related to up-regulating the expression of phosphorylation-$AMPK{\alpha}$1/2.

Geriatric Health Problems and Directions for Nursing Improvements (노인건강문제와 간호의 방향)

  • The elderly population will increase from 5.8% in 1996 to 12.5% in 2020. The related problems of health will also become a very important issue in the future. Therefore it is important to address the problems of geriatric nursing and geriatric health. 87.6% of the aged were ill with chronic degenerative diseases in 1994. In 1995, hospital admission rates (86.8) for those aged 60 or older were higher than that (56.3) of the total age group. Such high medical utilization will increase national health costs. For the development of geriatric nursing, active nursing intervention in various settings combined with education and research should be developed. Considering the health and welfare of the aged and the present status and views in Korea, I suggest the following: First, the health needs of the elderly in the institution, must be met by a plan that fosters geriatric nurses and programmed service development. Second, health service for the residential elderly must be provided in day care centers, short stays, nursing homes and geriatric hospitals. Geriatric nursing services should be provided in home residential areas, public health centers, public health subcenters as well as having, community health practitioners in primary health care posts and home health nurses. Third, geriatric nursing curriculum must be developed adjust to situations and culture of Korea and be included in the nursing curriculum. And gerontological nurse practitioner or geriatric specialist must be fosteraged to provide the professional care for the aged. Geriatric nursing research should be also achived for geriatric nursing improvements.

A Study on the Effective Method of Fire Safety Management of Geriatric Hospitals (요양병원 소방안전관리의 효율화 방안에 관한 연구)

  • Currently many geriatric hospitals have been built in Korea because increasing aging rapidly. However the increasing geriatric hospitals make the increasing safety accidents. Therefore this study is conducted by survey and face-to-face talk with the fire safety managers for analyzing safety problem of geriatric hospitals which number is twelve among 15 geriatric hospitals in the north of Chung-buk area. The result of this study is that geriatric hospitals are very old. Fire safety managers holding the fire safety license are a level 2 issued by the Korea Fire Safety Association(KFSA). And the fire safety inspection ability of them is normal (58%). Furthermore a fire safety budget ratio is 83% (A little) in the company yearly budget. And ratio having the fire inspection equipment in the building is 42% (A little) and 33% (Nothing). Satisfaction ratio of their duty is 66% and they also get stress from heavy duty and heavy responsibility. In conclusion, CEO needs to invest and interest the improvement of job performance, satisfaction ratio and fire safety facility. Also government needs the improvement of system to support and supervise the fire safety of geriatric hospitals consistently.

A Study on Dental Health and Chewing ability of Patients Hospitalized in Geriatric Hospital of Daegu (대구소재 한 노인요양병원 입원환자들의 구강상태와 저작능력에 관한 연구)

  • Purpose: This study investigates dental health and chewing ability of patients hospitalized in geriatric hospital, and compares the chewing ability for each factor that can be used as data for dental healthcare of senior citizens and basic data for denture insurance for the elderly. Methods: The 101 subjects of this study were selected from 178 patients hospitalized in a geriatric hospital located in Daegu Metropolitan City, excluding 77 patients who were being treated in the intensive care unit and who were unable to communicate. The chewing ability of the patients were measured using an evaluation scale based on foods consisting of 10 different hardnesses. Results: Patients with less than 21 teeth, those with dentures and patients who were less than satisfied with their dental conditions had difficulties in chewing hard food such as dried squid and radish kimchi, and the Chewing ability increased proportionally to the number of remaining teeth(p<0.001), appropriateness of the maxillary and mandibular dentures(p<0.005) and the level of dental satisfaction(p<0.001). Conclusion: This study is limited as the subjects were selected from a single hospital and the authors estimate that various studies will be necessary to investigate the Chewing ability of patients hospitalized in long-term hospitals. The subjects of this study did not receive any dental treatment while staying in the hospital and many of the subjects had bad fit denture or didn't have dentures or did not use dentures, although they have dentures, which calls for denture construction and prosthetics through dental treatment.

The Significance of Managing "Mibyeng" in Geriatric Health by Focusing on the Connection between Korean Medicine and Psychology: A Review (노년기 건강에 있어 미병(未病) 관리의 의의: 한의학과 심리학의 연관성을 중심으로)

  • Objectives: The aim of this study is to review the significance of managing "Mibyeong" in geriatric health in Korean medicine and psychology. Methods: We performed a literature search using OASIS, Koreantk, RISS, DBpia, and PubMed databases. We retrieved research articles using the keywords related to mibyeong (未病), sub-health (亞健康), Korean-medicine, psychology and geriatric health. Results: Both Korean medicine and psychology emphasize biological, psychological, and social balance and well-being. In addition, both value individual personality and constitution. Mibyeong is a Korean medical concept suggesting sub-health. Elderly individuals who are in a state of Mibyeong are frail and vulnerable to disorders and illness. Since depression, chronic diseases, low physical and mental activity have fatal outcomes in frail and elderly subjects, positive psychological support and active management of Mibyeong are very important. Conclusions: Geriatric health conditions are associated with chronic disease and multiple risk factors, including physiological, psychological, social, and economic conditions. A preventive and integrative approach is essential for geriatric health as prevention is better than cure for most geriatric diseases and illnesses.

Effect of Job Satisfaction and Self-Esteem on Psychological Burnout in Dietitians at Geriatric Hospital in Busan (요양병원 영양사의 직무만족, 자아존중감이 심리적 소진에 미치는 영향 -부산지역을 중심으로-)

  • This study aimed to investigate the effects of job satisfaction and self-esteem on psychological burnout in dietitians at a geriatric hospital in Busan. The survey was conducted from July 8 to August 31, 2014, and the data were analyzed using the SPSS program. The age group of over 36 years showed higher job satisfaction than the 'Under 25' group. Subjects that worked less than 2 years at their present jobs showed lower job satisfaction in job-itself than the 'Over 4 years' group did, and those who worked less than 1 year showed lower job satisfaction in terms of communication, appraisal compensation, and co-workers than did the other groups. Those who worked in '100~149 bed' hospitals showed higher job satisfaction than those in the 'Under 100 bed' group did. 'Over 200 bed' hospital dietitians showed higher satisfaction in supervision of superiors than did the other groups. Their experience of psychological burnout turned out to be 2.39 out of 5. Psychological burnout had a negative relationship with job satisfaction and self-esteem. Higher emotional exhaustion correlated with lower satisfaction in job-itself, less communication, and less supervision of superiors. Dehumanization correlated with lower satisfaction in job-itself, lower self-esteem, and worse communication, and while decreased personal achievement correlated with lower self-esteem, worse co-workers, lower satisfaction in job-itself, worse appraisal compensation, and worse communication. These results suggest that it would be effective for prevention and management of psychological burnout to mediate factors of job-itself, communication, supervision of superiors, co-worker, and appraisal compensation of dietitians in geriatric hospitals.

A Study on the Current Status and System of the Elderly Medical and Welfare Facilities with the Subjects in Busan (노인 의료복지시설의 실태분석 및 시설체계에 관한 연구 - 부산광역시 노인 의료복지시설을 대상으로 -)

  • This is the study on the current status and system of the elderly medical and welfare facilities with the subjects in Busan. I investigated the problems related to those by analysing the current status and implementing a survey and then suggested a new facility system as a salvation. At present, the elderly medical and welfare facilities can not receive variety of inmates and also they are operated separatedly so the elderly is not able to receive special treatment and nursing at the same time in one facility. Some nursing facilities can give special treatment by connecting specific hospital when it is necessary, but the connection is not activated than our expectancy at the present. On the base of these problems, this study suggested 'one-stop service system' that can afford to give nursing and special treatment in a place ranging wider areas as the solution. The five types of facility system by integrating the results of survey are as followings. 1) Free special nursing facility + The geriatric hospital. 2) The profit special nursing facility + The geriatric hospital. 3) The geriatric hospital + The hospital rooms with silver town residence type. 4) The actual expense special nursing facility + The geriatric hospital + The hospital rooms with silver town residence type. 5) The geriatric hospital + The residential welfare service. It seems that the preparation for the elderly is urgent when considering the facilities related to those are not special any more because every one is supposed to use them some day. The kinds and the degree of elderly's diseases will be more duplicated and significant afterwards. Therefore it is expected that more various and specialized facilities are to be required.

Evidence-based Clinical Practice Protocol of Physical Restraints by Adaptation Process for Patients in a Geriatric Hospital (요양병원 입원 노인을 위한 신체 억제대 프로토콜의 수용개작)

  • Purpose: This study was to develop an evidence-based clinical practice protocol of physical restraints by adaptation process for patients with a geriatric hospital. Methods: Protocol adaptation process was conducted in accordance with manual for guideline adaptation version 1.0 by ADAPTE collaboration. Results: The adapted physical restraint protocol was consisted of 3 domains and 37 recommendations. The number of recommendations in each domain were: 7 nursing assessment, 19 nursing intervention, and 11 nursing evaluation. More than half (56.8%) of the recommendations were rated as grade B, 37.8% as grade C, and 5.4% were rated as grade D. Conclusion: The adapted physical restraint protocol is expected to contribute as an evidence-based clinical practice protocol for healthcare workers in geriatric hospitals for reducing and improving efficiency of appropriate physical restraints use.
