Background/aim : Gallbladder stone is the most common disease of biliary tract. It is lead to acute abdominal pain. The cholecystitis which is caused by in gallbladder stone occurs frequently and the possibility of the gallbladder cancer comes to be high. Our study was designed to determine the prevalence and risk factors gallbladder stone in a population who community. Materials and Methods : We enrolled a total of 826 (male : 297, female : 529) persons who 60 or older in Jeju-do 10 places from 2008 July one months. The height, weight, liver function tests, lipid profile, fasting blood sugar, were all measurement. They had their gallbladder examined with using ultrasonography. Statistical significance was defined as a p-value less than 0.05. Results : Among 826 persons, gallbladder stone was found in 49 persons (5.9%). The male is 6.1% and the female is 5.8%, the male 1.03 times appeared more highly the female. The body mass index above of 23.0 kg/$m^2$ with the gallbladder stone was 69.3% (p<0.047). Conclusion : The gallbladder stone appeared highly according to age increases. The obesity is the risk factor of gallbladder stone. The weight management and diet control is a possibility of reducing a gallbladder stone creation.
This study was conducted in order to examine the effects of horticultural therapy on stress in demented elders, and for this purpose, we analyzed stress-related physiological changes in demented elders under the care of an elderly facility who had not taken medication and consented to blood tests. The horticultural therapy program included only flower decoration, which was preferred by the participants, aiming ultimately to reduce stress. First, a one-to-one session was given for forming rapport and the horticultural therapist had many conversations about plants with the elders. Furthermore, the participants were induced to be absorbed into the horticultural activities, though a short while, so that they might forget troubles. Evaluation was made by measuring subjective stress and the blood cortisol hormone concentration before and after the horticultural therapy and examining differences between them. Subjective stress increased from 12.88 to 17.88 in the control group, but decreased significantly from 13.88 before the therapy to 6.38 after in the experimental group (p = 0.007). When the blood cortisol hormone level was measured as a physiological indicator of stress, it decreased significantly from $11.33{\mu}g{\cdot}L^{-1}$ before the horticultural therapy to $9.85{\mu}g{\cdot}L^{-1}$ after (p = 0.037). These results suggest that horticultural therapy reduces the blood cortisol hormone level and is effective in easing stress.
Arthritis is one of the most common chronic degenerative joint disease in elderly. Osteoarthritis is a widespread, slowly developing disease, with a high prevalence increasing with age in women. The large joints mostly involved by the disease are the knees. But there are no treatments available that cure the underlying process of osteoarthritis diseases. Physical exercise helps in increasing cartilage nutrition and remodeling, increases the synovial blood flow, decrease swelling, and improves muscle strength. Thus, exercise has been suggested as an important nursing strategy in osteoarthritis. Purpose: The purpose of this study were to compare muscle strength between Tai-Chi exercise and aquatic exercise for women with knee osteoarthritis. Methods: A quasi-experimental study with pretest and posttest measures was used. The study subjects were those who had been enrolled in a community health center, and agreed to participate in the study for eight weeks, signed the consent form, and obtained the physicians approval. The study dropout rates were 13.2% with the final study subjects of 17 on Tai-Chi exercise, 16 on aquatic exercise program. The collected data were analyzed using SPSS for Window (version 12.0). Independent sample t-test and paired t-test was performed to compare of muscle strength for women with osteoarthritis after 8-week Tai-Chi exercise and aquatic Exercise. Results: The homogeneity tests of demographic characteristics and study variables at the pretest data revealed no significant differences between two groups. After 8-week Tai-Chi and aquatic exercise, there was significant result in pre-post test comparison on muscle strength on Tai-Chi group, but no significant in aquatic group. There were no significant differences of knee extensor (p=.078), and hand grip(p=.118) in group comparisons on muscle strengths. But there were significant differences of knee flexor(p=.024). Conclusion: Tai-chi exercise was effective in improving knee flexor. So, it seems that Tai-chi exercise may be more suitable for aquatic exercise in osteoarthritis exercise programs. Further studies with other comparisons in physical and psycho social outcomes are necessary to confirm the more effects of exercise.
Average life expectancy is getting longer due to medical developments and improvements in living standards. So much so that the elderly have an increased risk of developing osteoporosis. Therefore, it is important to prevent, diagnose, and treat the senile disease at an early stage through a bone density test. Bone density is measured by dual energy X-ray absorption (DXA). In this study, while using DXA, in cases when the measurements for both the lumbar and the femur could not be taken simultaneously, the correlation between both measurements were known, and the measurement of one area was used to make a clinical inference for the value of the other. Measurements were taken using Lunar Prodigy Advance (GE) for 43 participant with clinically significant fractures. Statistical calculations were produced and analysed regarding bone density. In case of T-score, lumbar spine produced a statistical result of $-2.112{\pm}1.836$ and femur neck was $-1.716{\pm}1.565$. In case of Z-score lumbar spine produced a statistical result of $-0.151{\pm}1.513$, and femur neck $-0.026{\pm}1.283$. It is indicated that the pearson correlation coefficient of T-score between lumbar spine and femur neck is high at 0.699, and the pearson correlation coefficient of Z-score is considered relatively high at 0.503. The correlation of bone density between lumbar spine and femur neck is shown to be statistically meaningful in T-score's p-value at 0.000 and Z-score's p-value at 0.001. In conclusion, it seems to have clinical usefulness that we can infer the result of one measurement through that of the other part tested, based on the knowledge of the correlation coefficients between lumbar spine and femur neck.
Background: Since people move through microenvironments rather than staying in one place, they may be exposed to both indoor and outdoor PM2.5 concentrations. Objectives: The aim of this study was to assess the exposure level of each sub-population group and evaluate the contribution rate of the major microenvironments. Methods: Exposure scenarios for sub-population groups were constructed on the basis of a 2019 Time-Use survey and the previous literature. A total of five population groups were classified and researchers wearing MicroPEM simulated monitoring PM2.5 exposure concentrations in real-time over three days. The exposure contribution for each microenvironment were evaluated by multiplying the inhalation rate and the PM2.5 exposure concentration levels. Results: Mean PM2.5 concentrations were 33.0 ㎍/m3 and 22.5 ㎍/m3 in Guro-gu and Wonju, respectively. When the exposure was calculated considering each inhalation rate and concentration, the home showed the highest exposure contribution rate for PM2.5. As for preschool children, it was 90.8% in Guro-gu, 94.1% in Wonju. For students it was 65.3% and 67.3%. For housewives it was 98.2% and 95.8%, and 59.5% and 91.7% for office workers. Both regions had higher exposure to PM2.5 among the elderly compared to other populations, and their PM2.5 exposure contribution rates were 98.3% and 94.1% at home for Guro-gu and Wonju, respectively. Conclusions: The exposure contribution rate could be dependent on time spent in microenvironments. Notably, the contribution rate of exposure to PM2.5 at home was the highest because most people spend the longest time at home. Therefore, microenvironments such as home with a higher contribution rate of exposure to PM2.5 could be managed to upgrade public health.
Purpose: In general, traffic accidents occur sporadically, so there are various limitations in terms of time and cost when conducting field investigations to prepare prevention and prevention measures. In particular, with the transition to a non-face-to-face society after the COVID-19, there is a greater need to prepare a replacement for field surveys. Therefore, in this study, Roadview provided by various websites was used as an alternative to field surveys in Sasang-gu, Busan City. The possibility was evaluated. Method: The research method was to extract vulnerable points for traffic accidents that occurred between 2016 and 22 and analyze road views based on the field survey evaluation items provided in the Traffic Safety Diagnosis Guidelines. Result: The main result was that Sasang-gu was most vulnerable to accidents involving elderly pedestrians at Sasang-ro, Daedong-ro, and Hakjang-ro. As a result of a detailed analysis of vulnerable points through Road View, Sasang-ro needed control of commercial vehicles and protection of the transportation vulnerable in the market commercial area. Daedong-ro was vulnerable to illegal on-street parking and slope merging sections, and Hakjang-ro was vulnerable to roads that were prone to speeding. When evaluating the possibility of replacing Roadview's field survey based on the results of this analysis, Roadview was able to effectively evaluate most items, such as separation of sidewalks and the location and spacing of safety facilities. However, there were limitations in items such as actual measurement performance. Conclusion: In other words, the road view can replace most field surveys, and the actual measurement evaluation items can be judged to be useful as auxiliary data, resulting in time and cost savings and high efficiency.

Background and Purpose: In this study we aimed to find the association between neuropsychological performance and body mass index (BMI) in patients with mild cognitive impairment (MCI). In addition, we investigated the effects of the apolipoprotein E (APOE) genotype in the relationship between the BMI and cognition in MCI. Methods: We enrolled a cohort of 3,038 subjects with MCI aged 65-90 from the Clinical Research Center for Dementia of South Korea and a dementia cohort of the Ewha Womans University Mokdong Hospital. MCI patients were classified into three subgroups according to the Asian standard of BMI. We compared cognitive performances between groups by one-way analysis of variance. To investigate the effects of the APOE genotype, we used multivariate linear regression models after adjusting for possible confounders. Results: Even though normal BMI groups were younger, had more females, and had less comorbidities, the higher BMI groups had better cognitive functions. Among subjects with APOE ε4 carriers, there was a positive relationship between the BMI and the memory task alone. Conclusions: Our findings suggested that higher BMI in patients with MCI were associated with better cognitive performance. The effects of the APOE ε4 genotype in the associations between BMI and cognition were distinguishing. Therefore, according to physical status, APOE ε4 genotype-specific strategies in the assessments and treatments may be necessary in elderly patients with MCI.

Objectives: This study was performed to determine the levels of depression symptoms among the community elderlies and to reveal its related factors, specifically aimed at revealing psycho-social factors such as social support net-work, sense of self-esteem, state anxiety, interpersonal behavior trait and locus of control.
Methods: The interviews were performed during the period from April 1st to June 30th, 2007 to the 790 elderlies in urban and rural areas.
Results: The mean scores of depression (CES-D) among all subjects were 16.65±10.97(urban: 18.81±10.31, rural: 14.49±11.20) and they were higher in urban than rural. The level of depression symptoms was influenced by the variables of educational level, with or without spouse, bear for living expenses, sense of satisfaction in daily life. As for health related behaviors, they were influenced by the variables of with or
without regular exercise. As for health status, they were influenced by the subjective health status, with or without disability of body, visual acuity, hearing ability, ability of food mastication, urinary incontinence, with or without help in activity of daily living (ADL) and instrumental activity of daily living (IADL). As for social support net-work and social activities, they were influenced by number of offspring, frequency of offspring contents, frequency of going out and activity of hobbies. As for psycho-social factors, they were influenced by sense of self-esteem, state anxiety and social support, especially, the level of depression symptoms was higher influenced by the variables of psycho-social factors than other variables. Conclusions: Lower of the level of depression symptoms among the community elderlies would require development and application of programs to keep under management psycho-social factors as well as looking for ways to promoting the present health status.
Tooth loss in elderly is mainly caused by alveolar bone loss via severe periodontitis. Although the severity of periodontitis is known to be affected by age, the aging process or the genetic changes during the aging of periodontal tissue cells are not well characterized. In this study, we investigated the effect of in vitro aging on the change of gene expression pattern in periodontal fibroblasts. Gingival fibroblasts (GF) and periodontal ligament fibroblasts (PDL) were obtained from two young patients and replicative senescence was induced by sequential subcultivation. When more than 90% cells were positively stained with senescence-associated ${\beta},-galactosidase$, those cells were regarded as aged cells. In aged GF and PDL, the level of phosphorylated retinoblastoma (RB) and $p16^{INK4A}$ protein was significantly decreased and increased, respectively. However, the protein level of p53 and p21, well known senescence-inducing genes, did not increase in aged GF and PDL. Although $p27^{Kip1}$ and $p15^{INK4B}$, another cyclin-dependent kinase inhibitors, were reported to be involved in replicative senescence of human cells, they were decreased in aged GF and PDL. Because senescent cells showed flattened and enlarged cell shape and are known to have increased focal adhesion, we examined the protein level of several integrins. Aged GF and PDL showed increased protein level of integrin ${\alpha}2$, ${\alpha}v$, and ${\beta}1$. When the gene expression profiles of actively proliferating young cells and aged cells were compared by cDNA microarray of 3,063 genes and were confirmed by reverse transcription-polymerase chain reaction, 7 genes and 15 genes were significantly and commonly increased and decreased, respectively, in aged GF and PDL. Among them, included are the genes that were known to be involved in the regulation of cell cycle, gene transcription, or integrin signaling. The change of gene expression pattern in GF and PDL was minimally similar to that of oral keratinocyte. These results suggest that $p16^{INK4A}/RB$ might be involved in replicative senescence of periodontal fibroblasts and the change of gene expression profile during aging process is cell type specific.

Objective: This study aimed to report the relationship between walking activity and perceived physical activity barriers after classifying the age group(≤64, 65-74, ≥75). Methods: The subjects were conducted on 1500 residents of two areas, and the walking activity was defined as five or more times a week, 10 minutes or more at a time, and 30 minutes or more per day. Perceived physical activity barriers were lack of time, lack of social support, lack of energy, lack of willpower, and fear of falling. Results: In the group under 64 year, the lack of time (exp(B)=0.819, p=0.002) and the lack of willpower (exp(B)=0.656, p<0.001) were significantly associated. In the 65-74 year old group, the lack of willpower (exp(B)=0.714, p<0.001) and fear of falling (exp(B)=0.787, p=0.003) were a significant association. In the group aged 75 years and over, lack ofwill power (exp(B)=0.734, p<0.001) and fear of falling (exp(B)=0.807, p=0.003) were significantly associated. Conclusion: In order to solve the lack of willpower and lack of time, a walking activity should be performed together at a certain time in the village, and the elderly in the late ages should reduce the fear of falling by performing a fall prevention exercise in parallel.
