유아의 점심 식사장소에 따른 영양소 및 식품 섭취량의 비교 : 제5기 국민건강영양조사 자료를 이용하여 (Comparison of nutrients and food intakes of young children according to lunch places: based on the fifth Korea National Health and Nutrition Examination Survey)

  • 본 연구는 유아의 점심 식사장소에 따라 영양소와 식품섭취량에 차이가 있는지를 조사하였다. 제5기 (2010 ~ 2012년) 국민건강영양조사 원시자료를 이용하여 개인별 24시간 회상에 의한 식품섭취조사 항목 중에서 점심 식사장소가 유아원/유치원일 경우 보육기관 점심식사군으로, 점심 식사장소가 가정과 이웃집/친척집일 경우 가정 점심식사군으로 나누어 복합표본설계 요소를 반영한 통계분석을 실시하였다. 대상자수는 1~2세에서 보육기관 점심식사군 179명, 가정 점심식사군 367명이었고, 3 ~ 5세에서 보육기관 점심식사군 549명, 가정 점심식사군 225명이었다. 연령군별 하루 총에너지섭취량, 점심과 저녁 식사로부터 에너지 섭취량은 두 집단 간 차이가 없었으나, 아침 에너지섭취비율이 보육기관 점심식사군에서 낮은 반면에 간식 에너지섭취비율은 더 높았다. 3 ~ 5세 유아에서는 나트륨, 칼륨 및 비타민 C 섭취량이 보육기관 점심식사군에서 많았으며, 리보플라빈 섭취량은 가정 점심식사군에서 유의하게 많았다. 점심으로부터 섭취한 식품군을 비교한 결과 1~2세 보육기관 점심식사군에서 채소류 섭취량이 유의하게 많은 반면에 과일류, 난류, 우유류 섭취량은 가정 점심식사군에서 많았다. 3 ~ 5세 보육기관 점심식사군의 점심으로부터 채소류, 두류, 어패류 섭취량이 유의하게 많은 반면에 우유류 섭취량은 가정 점심식사군에서 많았다. 3~5세 유아들이 보육기관 점심으로부터 채소류, 두류, 어패류를 더 많이 섭취하고 칼륨과 비타민 C를 많이 섭취하여 보육기관 점심이 영양소 균형과 식품 다양성 측면에서 가정 점심보다 나은 것으로 판단된다. 결론적으로 유아의 성장 발육을 위한 적절한 영양 공급은 물론, 올바른 식습관 형성을 위해서 보육기관 급식은 물론, 가정 식사에서도 채소류, 생선류 등 다양한 식재료를 활용하는 노력이 요구된다.

Comparison of Factors Affecting Perceived and Objective Dental Needs

  • Background: With increased interest in oral health, several efforts have been made to improve oral health conditions. To achieve this, needs for oral health must be precisely determined and accurately measured. Therefore, factors influencing both objective unmet dental needs, which were determined by experts, and perceived unmet dental needs, which were determined by patients, were examined in this study. Methods: Responses of 17,735 respondents aged greater than 19 years from the Korean National Health and Nutrition Survey collected using the fifth (2010~2012) rotation sample survey were analyzed. Based on the information collected from the survey and dental examination, we determined the associations between the independent (sex and socioeconomic level) and dependent variables using a chi-squared test. Moreover, ordinal logistic regression analyses on multiple categorical values were performed using perceived and objective dental needs as the dependent variables. Results: Generally, factors influencing both perceived and objective dental needs were similar. These included sex, household income, educational level, private insurance, and subjective oral health status. However, the high-income groups had lesser perceived and objective dental needs compared to the low-income groups. Furthermore, factors such as sex, educational level, and marital status had different influence on both needs. Conclusion: Generally, factors that affect perceived and objective dental needs were similar. To minimize unmet dental needs, factors influencing both perceived and objective dental needs should be examined for a broad dental insurance coverage, and efforts to prevent oral diseases are also required.

노인의 성별에 따른 자살생각 영향요인 비교 연구 (Comparative Study on Influencing Factors of Suicidal Ideation according to Sex in the Elderly)

  • Purpose: The purpose of this study was to investigate suicidal ideation and its associated factors according to sex in the elderly, by focusing on socio-demographic and health characteristics. Methods: Data were derived from the fifth Korean National Health and Nutrition Examination Survey (KNHANES V-2, 2011). The subjects were 1,464 elderly aged 65 years or more (620 male and 844 female). The SPSS/WIN 21.0 program was used for data analysis. Results: Suicidal ideation was positively associated with depression and stress in both male and female subjects. For elderly men, those aged 75 years or more, had no spouse, and had activity limitations showed a higher suicidal ideation risk. For elderly women, those who had low a education level and were perceived to be unhealthy showed a higher suicidal ideation risk. Conclusion: Nurses should consider sex differences when designing suicide prevention programs for the elderly.

제5기 국민건강영양조사 자료 중 남성에서 혈청 중 25-hydroxyvitamin D와 만성폐쇄성폐질환의 상관관계 (Relationship between serum 25-hydroxyvitamin D and chronic obstructive pulmonary disease in males in the fifth Korean national health and nutrition examination survey)

  • 제5기국민건강영양조사 자료중 40세이상 남성을 대상으로 혈액 중 25(OH)D와 COPD의 상관관계를 조사하였다. 25(OH)D는 사분위로 군을 나누어 적용하였고, 2012년 개정된 진료지침에 따라 $FEV_1/FEV_6$ < 0.73인 경우를 COPD로 판정하였다. 다른 요인을 보정하지 않았을 경우에는 25(OH)D와 COPD 사이에 상관관계가 없었으나 흡연상태, 가계소득, 교육수준, 직업, BMI, 만연령및흡연지수를 보정하였을 때는 25(OH)D와 COPD사이에 유의한 상관관계를 보였다. 25(OH)D농도 17~21 ng/mL인 3/4분위군을 기준으로 1/4분위의 경우의 OR 1.643 (95% CI 1.161-2.236), 2/4분위는 OR 1.453 (95% CI 1.045-2.020)으로 유의하게 증가하였다. 결론적으로 40세 이상 남성에서 혈중 25(OH)D 농도는 COPD 유병가능성과 유의한 상관관계를 보였다. 즉, 3/4 분위군 [25(OH)D 17 ng/mL 이상 21 ng/mL 미만]을 기준으로 혈중농도가 낮을수록 COPD 유병가능성이 높았다.

한국인의 비타민 D 부족 유병률에 관한 연구: 국민건강영양조사 2010~2011 분석결과 (Prevalence of vitamin D deficiency in Korea: Results from KNHANES 2010 to 2011)

  • Vitamin D deficiency (VDD) is becoming an epidemic and thereby a global health problem. Further, VDD adversely affects calcium metabolism and skeletal health, and is associated with increased risk of several diseases, e.g., autoimmune diseases, several types of cancers, type 2 diabetes mellitus, cardiovascular diseases, infectious diseases, asthma, psoriatic arthritis, and etc. To evaluate the prevalence of VDD in Korea, and then to evaluate the association of several factors with serum 25(OH)D level, the author analyzed the data of 14,456 individuals who were 10 years of age and over from the Fifth Korea National Health and Nutrition Examination Survey 1 & 2 (KNHANES V-1 & 2) conducted by the Korean Centers for Disease Control & Prevention. As a result, among Koreans (age $${\geq_-}$$ 10years), 65.9% of males and 77.7% of females were below optimum blood serum 25(OH)D (20 ng/mL). VDD is more severe in female than in male at all age groups. In addition, the younger generations had less 25(OH)D level than older generations in Korea. The analysis by complex sample general linear model (CSGLM) suggested that blood 25(OH)D concentration was related with gender (p < .001), residence (p = .030), occupation (p < .001), anemia (p < .001) and physical activity (p < .001). In conclusion, VDD is pandemic and it is more severe in younger generations in Korea. Further, from the results by CSGLM, serum 25(OH)D status is closely related with the life style of Koreans.

Dietary factors associated with high serum ferritin levels in postmenopausal women with the Fifth Korea National Health and Nutrition Examination Survey (KNHANES V), 2010-2012

  • BACKGROUND/OBJECTIVES: Serum ferritin levels are significantly increased after menopause and greatly affect women's health. The aim of this study was to investigate the dietary and non-dietary factors associated with high ferritin levels in postmenopausal women. SUBJECTS/METHODS: Among adult women in 2010-2012, qualified postmenopausal women (n = 3880) were separated into quartiles of serum ferritin. The variable differences among the quartiles of ferritin were determined using either procsurvey chi-square test (${\chi}^2$-test) among categorical variables, or GLM (Generalized Linear Model) among continuous variables. The odds ratio for high ferritin in relation to dietary factors was also determined using procsurvery logistic analysis. RESULTS: Age, obesity, drinking habit, and blood glucose levels were found to be significant indicators of high serum ferritin level after adjusting for all confounding factors. Among the food groups, grain, milk, vegetable, and seaweed intakes were significantly associated with high ferritin levels, but after adjusting for all confounding factors, only grains and vegetables remained significant factors. Among the nutrient groups, calcium, vitamin A, and vitamin C intake were significant factors, but after adjustment, none of the nutrient groups analyzed were associated with a high risk of ferritin. CONCLUSION: Age, obesity, drinking habit, and glucose levels, as well as inadequate intakes of grains and vegetables, were found to be significantly associated with high serum ferritin levels in postmenopausal Korean women.

에너지 및 단백질 섭취와 영구치 우식과의 관련성 (Relationship between intake of energy and protein and permanent teeth caries)

  • Objectives: This study aims to evaluate the association between intake of energy, calcium and protein and permanent teeth caries in Korean children. Methods: Research data were obtained from the Fifth Korean National Health and Nutrition Examination Survey (KNHANES) and 1,274 Korean children within 9 to 12 years were selected as subjects for the study. The DMFT index, nutrition factors including total energy and protein intake, and degree of calcium intake were identified Chi-square test was performed to assess the difference of distribution in socioeconomic status, oral health behavior and nutrition factors according to sex. Multilevel linear regression analysis was performed to identify the relationship between DMFT index and the related variables. Results: Significant differences in energy, protein and Ca intake were confirmed according to the sex. Among 10 year-old children who intake total energy less than the recommended had few dental caries (p=0.058, ${\beta}=-0.411$). In addition, 11-year-old children having protein intake more than the recommended experienced dental caries (p=0.02, ${\beta}=0.588$). Conclusions: These results suggest that, there is significant differences between intake of energy and protein and dental caries among Korean children. However, further researches is needed to confirm between intake of energy and protein as a related factor.

직장인 당뇨병 환자의 혈당조절 정도와 융합적인 영향요인 분석 : 2009-2013 국민건강영양조사를 바탕으로 (Convergence Factors Related to Glycemic Control in Workers with Diabetes Mellitus : using the Korean National Health and Nutrition Examination Survey, 2009-2013)

  • 본 연구는 직장인 당뇨병 환자의 혈당조절 정도와 융합적인 영향요인을 파악하여 당뇨병관리를 위한 중재방안을 모색하기 위한 서술적 조사연구이다. 연구 대상은 2009-2013년 국민건강영양조사를 활용하였으며, 만19세 이상 65세 미만의 성인 중 당뇨병을 진단받고, 직장을 가지고 있는 총 764명을 대상으로 하였다. 당화혈색소 7.0% 미만을 혈당조절군, 7.0% 이상을 혈당비조절군으로 분류하였으며, SPSS 18.0 프로그램을 이용하여 분석하였다. 일반적 특성 중 성별, 질병관련 및 건강행태 특성 중 당뇨병이환기간, 당뇨병치료여부, 고혈압진단여부, 흡연, 중등도 신체활동, 걷기 실천여부가 그룹간 통계적으로 유의하게 나타났으며, 로지스틱회귀분석결과 성별, 당뇨치료여부, 고혈압진단여부, 걷기 실천여부가 당화혈색소 조절에 영향을 주는 요인으로 나타났다. 이상의 결과를 바탕으로, 직장인 당뇨병 환자들의 적극적인 당뇨치료와 일상생활에서의 걷기실천이 중요하며, 이를 지지해 줄 수 있는 직장 내 환경조성이 이루어져야 할 것이다.

국민건강영양조사 제 5기(2010-2012)를 활용한 한국의 19세 이상 성인의 식습관에 따른 정신건강상태 (Eating habits in accordance with the Mental Health status : The 5th Korea National Health and Nutrition Examination Survey, 2010-2012)

  • 본 연구는 한국 성인의 식습관과 정신건강과의 관련성을 규명함으로써 올바른 식습관 형성의 중요성을 알리고 한국인의 정신건강증진을 위해 기초자료를 제공하는데 그 목적이 있다. 연구 대상자는 국민건강영양조사 5기(2010-2012)에 참여한 19세 이상 성인 15,848명(남자 6,348명, 여자 9,500명)을 대상으로 하였다. 자료분석에서 일반적은 특성은 기술분석을 활용하였으며, 식사와 정신건강과의 관련성은 카이제곱검정을 실시하였다. 그리고 다중 로지스틱 회귀분석과 선형경향성을 사용하여 식습관과 정신건강과의 관련성을 파악하였다. 연구결과, 남성은 식사횟수가 증가할수록 스트레스와 자살생각이 감소하는 경향을 보였다. 그리고 여성은 식사횟수가 증가할수록 스트레스, 우울, 자살생각이 모두 감소하는 경향을 보였다. 특히, 여성은 1식에 비해 2식을 할 경우 스트레스가 0.667배 감소하였다. 그리고 1식에 비해 3식을 할 경우 스트레스는 0.522배, 우울 0.605배, 자살생각 0.508배 감소함을 규명하여 식사횟수가 정신건강에 영향을 미치는 것을 확인할 수 있었다. 결론적으로 규칙적인 식습관이 정신건강에 영향을 줄 수 있기 때문에 올바른 식습관과 그로 인한 균형 잡힌 영양소 섭취가 정신건강에 중요함을 알리는 교육과 사회적 관심이 필요하다.

소아자녀를 둔 장년기 어머니의 결식빈도에 영향을 미치는 요인 : 2010-2011년 국민건강영양조사 자료 이용 (Factors Affecting the Frequency of Skipping Meals of Prime-Aged Mothers with Children : Data from the Fifth Korea National Health and Nutrition Examination Survey, 2010-2011)

  • Objectives: This study was designed with the goal of understanding the factors affecting the frequency of skipping meals of prime-aged mothers with children as well as their nutritional status. Methods: Utilizing data from the 2010-2011 Korea National Health and Nutrition Survey, the frequency of skipping meals of mothers aged between 30 to 49 years with children aged between 3 to 11 years during a two day period was statistically analyzed. The number of meals skipped calculated and categorized into skipping no meals, skipping one meal, skipping two meals or more. Results: Compared to subjects who corresponded to mean nutrient adequacy ratio(MAR) of 4 quartile, subjects who corresponded to MAR of 2 quartile had 2.766 (95% CI: 1.552-4.931) probability of being in the 1 meal skippers group, while the probability of being in the more than 2 meals skippers group was 2.743(95% CI: 1.353-5.564). Also, compared to subjects who corresponded to MAR of 4 quartile, subjects who corresponded to MAR of 1 quartile had 3.471 (95% CI: 1.871-6.442) probability of being in the 1 meal skippers group, while the odds ratio for being in the more than 2 meals skippers group was 5.258(95% CI: 2.642-10.466). Conclusions: The results have the advantage of being generalized because the study selected subjects from probability sampling of the female population of Korea. The research results showed that the elements influencing skipping meals of prime-aged mothers with children were mean nutrient adequacy ratio and the number of nutrients, under estimated average requirement intake, and others. Therefore, to encourage dietary behaviors in the right direction, an integrated approach that considers the associated factors must be realized. Future studies are needed to understand how the frequency of skipping meals of mothers affects their children.
