A Histopathological Study on Suture Granuloma in Neutralized Dogs (개에서 거세 후 발생한 봉합사 육아종의 병리조직학적인 연구)

  • Postoperative suture granuloma have rarely been reported in animals. Eight biopsy masses from testes areas of neutralized dogs were diagnosed as suture granuloma. The suture granuloma occurred at any time from several weeks to a few years after surgery and appeared to be testicular tumors by macroscopic examination. The granulomas were classified into three types based on the histopathological findings. The first type of pyogranuloma was mainly composed of neutrophils, macrophages, and suture fragments. The second type was chronic necrotizing granuloma which was well demarcated by fibrous connective tissues and was composed of a few suture fragments, macrophages and central fibrinoid necrosis. The third type of granuloma had a poorly defined margin with scarcely observed suture fragments and central necrosis in the tissue. These histopathological findings suggested that various types of suture granuloma may be caused by suture material and could even appear long after surgery.

  • Precocious puberty is difficult to define because of the marked variation in the age at which puberty begins normally, onset of puberty before 8 years of age in girls and 9 years in boys may be considered precocious. The etiology of precocious puberty in boys is usually idiopathic, but can result from adrenal and testicular tumors. The hepatoblastoma that produces hCG is a very rare functioning tumor known to cause precocious puberty in boys. Recently, author experienced one case of virilizing adrenal cortical adenoma in 22 month-old boy, one case of adrenal cortical carcinoma in 28 month-old boy, and one case of virilizing hCG-producing hepatoblastoma in 7 year-old boy and reviewed literatures.

Clinical, Morphological and Immunohistochemical Characteristics of Canine Lipid-Rich Sertoli Cell Tumor

  • Some canine typical Sertoli cell tumors (SCT) induce signs of hyperestrinism. However, whether variant lipid-rich SCTs have signs of hyperestrinism remains largely unknown. In the present study, an 11-year-old male Pekingese dog showed significantly elevated serum estrogen and characteristic signs of hyperestrinism such as gynecomastia and alopecia. Cytological, radiological and ultrasound examinations found testicular mass, prostatitis with squamous metaplasia, and cystitis. Pathologically, the tumor lesions consisted mainly of lipid-rich tumor cells with signet-ring appearance, which were immunohistochemically positive for vimentin and anti-$M{\ddot{u}}llerian$ hormone. Based on the findings, a diagnosis of lipid-rich Sertoli cell tumor was established. In conclusion, a canine lipid-rich SCT induced signs of hyperestrinism and caused prostatitis via squamous metaplasia due to its excessive secretion of estrogen.

  • In children and adolescents, inguinal and scrotal diseases are relatively common, and imaging is very useful for the diagnosis and differential diagnosis of these diseases. Therefore, it is important to understand the imaging findings of these diseases. In this article, we classify these diseases into small testes, cryptorchidism, patent processus vaginalis, acute scrotum pain, trauma, testicular tumors, and others and describe their characteristic findings.

Fine Needle Aspipration Cytology of Metastatic Pulmonary Seminoma - A Cese Report - (세침흡인 세포검사로 진단된 폐에 전이한 정상피종 - 1예 보고 -)

  • Fine needle aspiration cytology of a pulmonary mass was performed on a 51-year-old man who had a left testicular mass. Cytologic features were composed of a homogeneous population of malignant cells associated with a background of foamy and lacelike material. The cellular features were characterized by monomorphous cell proliferation of relatively regular large cells, generally isolated or grouped. Occasionally, fine blanching stroma with large tumor cells and scanty lymphocytes were noted. The tumor cells had a round, regular nucleus, prominent round nucleoli, and a thin rim of cytoplasm containing large vacuoles or lacunae filled with glycogen. The fine needle aspiration cytologic diagnosis was highly consistent with metastatic seminoma from testis and less likely primary or other metastatic carcinoma. The diagnosis of resected testicular mass was classic seminoma. Despite the fact that cytopathologists were not familial with diagnosis of seminoma due to clinician's lack of interest in fine needle aspiration cytology of germ cell tumors including seminoma, it appears that a diagnosis of this tumor should not be problematic in cytologic material if specific histologic criteria are applied.

Apoptotic Response of Human Oral Squamous Carcinoma Cells to Etoposide (Etoposide에 대한 사람구강편평상피암종세포의 세포자멸사 반응)

  • Anti-cancer drugs have been shown to target diverse cellular functions in mediation cell death in chemosensitive tumors. Most antineoplastic drugs used in chemotherapy of leukemias and solid tumors induce apoptosis in drug-sensitive target cells. However, the precise molecular requirements that are central for drug-induced cell death are largely unknown. Etoposide is used for the treatment of lung and testicular cancer. This study was performed to examine whether etoposide promote apoptosis in human oral squamous carcinoma cells (OSC9) as well as in lung and testicular cancer. Etoposide had a significant dose- and time-dependent inhibitory effect on the viability of OSC9 cells. TUNEL assay showed the positive reaction on condensed nuclei. Hoechst stain demonstrated that etoposide induced a change in nuclear morphology. The expression of p53 was increased at 48 hour, suggesting that the nuclear of OSC9 cell was damaged, thereby inducing apoptosis. Etoposide treatment induced caspase-3 cleavage and activation. Intact PARP protein 116-kDa and 85-kDa cleaved product were observed. The activated caspase-3 led cleavage of the PARP. These results demonstrate that etoposide-induced apoptosis in OSC9 cells is associated with caspase-3 activation.

Comparative Effectiveness of Risk-adapted Surveillance vs Retroperitoneal Lymph Node Dissection in Clinical Stage I Nonseminomatous Germ Cell Testicular Cancer: A Retrospective Follow-up Study of 81 Patients

  • Purpose: To retrospective assess the potential predictors for relapse and create an effective clinical mode for surveillance after orchidectomy in clinical stage I non-seminomatous germ cell testicular tumors (CSI-NSGCTs). Materials and Methods: We analyzed data for CSI-NSGCTs patients with non-lymphatic vascular invasion, %ECa < 50% (percentage of embryonal carcinoma < 50%), and negative or declining tumor markers to their half-life following orchidectomy (defined as low-risk patients); these patients were recruited from four Chinese centers between January 1999 and October 2013. Patients were divided into active surveillance group and retroperitoneal lymph node dissection (RPLND) group according to different therapeutic methods after radical orchidectomy was performed. The disease-free survival rates (DFSR) and overall survival rates (OSR) of the two groups were compared by Kaplan-Meier analysis. Results: A total of 121 patients with CSI-NSGCT were collected from four centers, and 81 low-risk patients, including 54 with active surveillance and 27 with RPLND, were enrolled at last. The median follow-up duration was 66.2 (range 6-164) months in the RPLND group and 65.9 (range 8-179) months in the surveillance group. OSR was 100% in active surveillance and RPLND groups, and DFSR was 89.8% and 87.0%, respectively. No significant difference was observed between these two groups ($X_2=0.108$, P=0.743). No significant difference was observed between the patients with a low percentage of embryonal carcinoma (<50%) and those without embryonal carcinoma (87.0% and 91.9%, $X_2=0.154$, P=0.645). No treatment-related complications were observed in the active surveillance group whereas minor and major complications were observed in 13.0% and 26.1% of the RPLND group, respectively. Conclusions: Active surveillance resulted in similar DFSR and OSR compared with RPLND in our trial. Patients with low-risk CSI-NSGCTs could benefit from risk-adapted surveillance after these patients were subjected to radical orchidectomy.

Predictors of Outcome in Patients with Advanced Nonseminomatous Germ Cell Testicular Tumors

  • Background: Predictor factors determining complete response to treatment are still not clearly defined. We aimed to evaluate clinicopathological features, risk factors, treatment responses, and survival analysis of patient with advanced nonseminomatous GCTs (NSGCTs). Materials and Methods: Between November 1999 and September 2011, 140 patients with stage II and III NSGCTs were referred to our institutions and 125 patients with complete clinical data were included in this retrospective study. Four cycles of BEP regimen were applied as a first-line treatment. Salvage chemotherapy and/or high-dose chemotherapy (HDCT) with autologous stem cell transplantation were given in patients who progressed after BEP chemotherapy. Post-chemotherapy surgery was performed in selected patients with incomplete radiographic response and normal tumor markers. Results: The median age was 28 years. For the good, intermediate and poor risk groups, compete response rates (CRR) were, 84.6%, 67.9% and 59.4%, respectively. Extragonadal tumors, stage 3 disease, intermediate and poor risk factors, rete testis invasion were associated with worse outcomes. There were 32 patients (25.6%) with non-CR who were treated with salvage treatment. Thirty-one patients died from GCTs and 94% of them had stage III disease. Conclusions: Even though response rates are high, some patients with GCTs still need salvage treatment and cure cannot be achieved. Non-complete response to platinium-based first-line treatment is a negative prognostic factor. Our study confirmed the need for a prognostic and predictive model and more effective salvage approaches.

Fine Needle Aspiration Cytology of Metastatic Seminoma in Cervical Lymph Node - A Case Report - (경부 림프절에 전이한 정상피종의 세침흡인 세포학적 소견 - 1예 보고 -)

  • Fine needle aspiration cytology of the cervical lymph node was performed in a 63-year-old man who had had an orchiectomy for seminoma one year ago. The tumor cells were arranged in loose clusters, occasional sheets, or single cells. The nuclei were round to ovoid with fine or reticular chromatin, and had one or more prominent nucleoli. These cells were intermingled with lymphocytes in a characteristic foamy, lacelike background. Documented reports of the cytologic appearance of the seminoma are rare, especially in the metastatic lesion. The diagnosis of primary gonadal seminoma by fine needle aspiration cytology is probably not indicated since the treatment of primary gonadal tumor requires surgical resection. Because of the characteristic cytologic features, fine needle aspiration cytology may be helpful in evaluation of the extent of tumor spread in the patients with testicular tumors.
