  • Williams-Beuren syndrome (WS) is a rare genetic disorder that results from microdeletion at chromosome 7, which harbors the elastin gene. Clinical findings include arteriopathy, aortic stenosis, hypertension, and laxities and contractures in different joints throughout the body. While many components of the temporomandibular joint (TMJ) normally contain elastin, there are few reports on TMJ manifestations of WS. This study reports a TMJ ankylosis case in a WS patient and shares insight on a possible link between development of TMJ ankylosis and elastin deficiency in WS patients. A WS patient presented with bilateral TMJ ankylosis and was successfully treated with TMJ gap arthroplasty. Hypermobility of TMJ and lack of elastin in retrodiscal tissue can induce anterior disc displacement without reduction. Due to lack of elastin, which has a significant role in the compensatory and reparatory mechanism of TMJ, WS patients might be prone to TMJ ankylosis.

  • Introduction : Patients with temporomandibular disorder may have various ear-related symptoms. If an excessive load is applied to the ear area due to the skeletal shape of the mandibular condyle or malposition of the disc, an auditory problems may occur. Case report : The patient was referred to our clinic due to the suspicion of temporomandibular disorder from the local otorhinolaryngology clinic. A few days ago, his right ear could not be heard. MRI showed that the left TMJ disc was anterior displacement with reduction, the right TMJ disc was anteromedial displacement without reduction. Also Right mandibular condyle showed sclerotic bone change, subchondral cyst and was compressing the frontal wall of the ear on MRI view. Right TMJ arthroplasty was done under the diagnosis of right TMJ osteoarthritis and osteochondroma. Postoperative intermaxillary fixation was done with SAS screw and elastics for 2 weeks. One month after the operation, hearing and TMJ discomfort were recovered without any complications. Conclusions As seen in this case, hearing loss due to benign tumor-like lesions of the temporomandibular joint should be treated surgically to restore the TMJ function and hearing.

  • Synovial chondromatosis (SC) is an uncommon progressive cartilaginous metaplasia of residual mesenchymal cells in synovial tissue. This disorder usually affects large joints and is rarely observed in the temporomandibular joint (TMJ). SC in TMJ is difficult to diagnose early owing to non-specific clinical symptoms. In this article, we report a patient with SC on the right TMJ, who presented with pain in the right TMJ and progressive occlusal changes, not responsive to conventional conservative temporomandibular disorder treatment for several months. This case emphasizes the importance of an accurate specific diagnosis for TMJ problems before the delivery of any treatment.

  • Purpose : Authors attempted to evaluate the effect on explanation of pathogenesis and stress management as the initial care of temporomandibular disorders. Materials and methods : The materials were 634 patients with temporomandibular disorder, who had been referred to our Department of Dentistry, Wonju Christian Hospital during recent 5 years. We examined about clinical aspects of temporomandibular disorders, such as, major signs of temporomandibular joint disorder, life environment and habits, radiological findings of temporomandibular joint and electromyography of masseter muscle. The patients were treated by explanation of TMJ pathogenesis and stress management. After that, the patient were evaluated about the effect in third week. Results : The result was more favorable (96.5% success rate) without intolerable signs of temporomandibular joint disorder. Conclusion : The explanation of TMJ pathogenesis and stress management were thought as the very effective care in management of patients with temporomandibular disorder.

  • Epidermoid cyst in bony wall of external auditory canal (EAC) is a very rare pathologic condition. In addition to its extreme rarity, its positional proximity to temporomandibular joint (TMJ) might lead to clinical misdiagnosis as TMJ disorders, especially when it is accompanied by temporomandibular disorders-like symptoms. In this article, we report a patient with epidermoid cyst in anterior bony wall of EAC, which was misdiagnosed as TMJ disorder because the left TMJ pain aggravated during mastication.

  • Objectives : The purpose of the study is to investigate the relationship between job-stress and temporomandibular joint(TMJ) disorder in dental hygienists. This study will provide the basic data to improve the working condition and the quality of life. Methods : The subjects were 229 dental hygienists at general hospitals in Seoul, Korea. A self-reported questionnaire was filled out from May 20 to June 20, 2013. The questionnaire consisted of 4 questions of demographic features, 11 questions for TMJ symptoms and 5 questions for job stress. The data were analyzed by frequency analysis, chi-square test, Mann-Whitney U test and multiple job-stress logistic regression analysis using SPSS version 21.0. Results : During the last six months, 53.3%(122 persons) of the dental hygienists had TMJ disorder symptoms including joint noise(40.6%, 93 persons), TMJ pain(31.4%, 71 persons) and limitation of TMJ(21.8%, 50 persons). Job-stress is divided into two ranges including high stress group(4.3-5.0 points) and low stress group(0.0-3.6 points) in TMJ pain and joint noise(p<0.05). TMJ pain was closely related to low back pain, pelvis pain and tension headache arising from the uncomfortable working posture. Conclusions : It is necessary to prevent the job stress in the dental hygienists by the improvement of working condition, emotional stability, and frequent postural change.

  • The temporomandibular joint (TMJ) is one of the most unique joints in the human body that orchestrates complex movements across different orthogonal planes and multiple axes of rotation. Comprising the articular eminence of the temporal bone and the condylar process of the mandible, the TMJ integrates five major ligaments, retrodiscal tissues, nerves, and blood and lymph systems to facilitate its function. Cooperation between the contralateral TMJ and masticatory muscles is essential for coordinated serial dynamic functions. During mouth opening, the TMJ exhibits a hinge movement, followed by gliding. The health of the masticatory system, which is intricately linked to chewing, energy intake, and communication, has become increasingly crucial with advancing age, exerting an impact on oral and systemic health and overall quality of life. For individuals to lead a healthy and pain-free life, a comprehensive understanding of the basic anatomy and functional aspects of the TMJ and masticatory muscles is imperative. Temporomandibular disorders (TMDs) encompass a spectrum of diseases and disorders associated with changes in the structure, function, or physiology of the TMJ and masticatory system. Functional and pathological alterations in the TMJ and masticatory muscles can be visualized using various imaging modalities, such as cone-beam computed tomography, magnetic resonance imaging, and bone scans. An exploration of potential pathophysiological mechanisms related to the TMJ anatomy contributes to a comprehensive understanding of TMD and informs targeted treatment strategies. Hence, this narrative review presents insights into the fundamental functional anatomy of the TMJ and pathological changes that evolve with TMD progression.

  • The literature on alloplastic total temporomandibular joint (TMJ) replacement is encouraging, with acceptable improvement of treatment outcomes in terms of both pain level and jaw function. This is a case report on patients who suffered from degenerative joint disease and ankylosis after mandibular condyle fracture or prior TMJ surgery and were treated by TMJ replacement with condyle prosthesis. We obtained good results from the procedures, including total TMJ replacement.
