The Korean Academy of Orofacial Pain and Oral Medicine inaugurated Temporomandibular Joint Day in November 9th, 2018 and aimed to emphasize the importance of the temporomandibular joint in maintaining overall orofacial health and celebrates the benefits of proper treatment of its related disorders including temporomandibular disorders to raise awareness of the public on significant temporomandibular joint-related conditions. Three essential statements were made on this memorable day as below: 1) Dentists of Korea are responsible of maintaining a healthy status of the temporomandibular joint of the people. 2) Dentists of Korea strive for the research and management of temporomandibular joint related conditions 3) November 9th is annually celebrated as the Temporomandibular Joint Day in Korea.
Objective: In patients with temporomandibular joint disorders, air insoles are used to investigate functionality and pain changes in the temporomandibular joint when walking in daily life. Intervention: Sixty-five patients with temporomandibular joint disorder were recruited: 34 as a control group who walked more than 7,000 steps a day in daily life, and 31 as an experimental group who were instructed to take at least 7,000 steps every day while wearing their air insoles. Measurements: To determine the effects of air insoles on temporomandibular joint pain, steady-state pain, maximum mouth opening, average pain, and the most severe pain were measured before and after the experiment. In addition, to evaluate functionality, the ability to open the mouth in a comfortable state, pain when opening the mouth, and the point of sound and maximum degree to which the mouth could be opened were evaluated before and after the experiment. Results: Pain, mouth openness, and sound points showed significant differences from the control group after the experiment, and the maximum mouth opening range showed no significant difference. Conclusion: When air insoles were used by patients with temporomandibular joint disorder, the functionality of the temporomandibular joint was improved and pain was decreased.

Some treatment methods have been proposed for patients with chronic closed lock of temporomandibular joint. We report a conservative treatment for patients who had chronic closed lock of temporomandibular joint and who did not want surgical treatment. Two patients who had been treated in the Template clinic, Soonchunhyang University Bucheon Hospital, are the subjects of this report. The subjects had chronic closed lock symptom for over 3 months after an onset of locking; conventional therapies had no effect. The subjects were treated by making them wear a Template appliance while sleeping and exercise for 10 hours a day. After periodic follow-up, significant improvement was observed for Template treatment in terms of the maximal mouth opening range. When conventional therapy is expected to be ineffective, The Template appliance can be used as conservative treatment for temporomandibular disorders patients with chronic closed lock of temporomandibular joint.

This study was performed to observe the effects of arthrography on the tissues of temporomandibular joint histopathologically. Among 17 rabbits (34 joints), 2 (4 joints) were used as normal control group, others (30 joints) were experimentally arthrographed using 0.03㎖ diatrizoate meglumine (Hypaque meglumine 60, Wintrop, U.S.A). The rabbits were sacrificed at 2-hour, 1-, 2-, 4- and 7-day after experiment and the tissues of temporomandibular joint were prepared according to the usual method for light microscopic examination. The results were as follows: 1. There were no changes of the fibrous connective tissues of the condylar fossa, the fibrous connective tissues and hyaline cartilages of the condyle, and the articular dislc 2. In 2-hour experimental group, the proliferation of the surface synovial membrane cells, the congestion and hemorrage of the vessels were observed. 3. In 1 and 2-day experimental group, minimal lymphocytes infiltration was observed with the proliferation of the surface synovial membrane cells, the congestion and hemorrage of the vessels. 4. In 4-day experimental group, the diminution of the proliferation of the surface synovial membrane cells was seen, but there was no vascular changes. 5. In 7-day experimental group, the tissues showed similar appearance to the normal control group.
Temporomandibular joint(TMJ) disorders have characteristic clinical findings such as pain, joint sound, and abnormal jaw function. With the rapid progress in TMJ imaging techniques, magnetic resonance imaging(MRI) especially provides the clinician and researcher with the ability to evaluate TMJ disorders(effusion, anterior disk displacement) and the hard and soft tissue of the TMJ. In order to manage the internal derangement of TMJ such as anterior disk displacement, the so-called appliance therapy is commonly used. This technique became the treatment of choice before surgical intervention by many practitioners. The purpose of the present study was to evaluate and determine the efficacy of anterior repositioning splint by means of the change of the position of the disk through pre- and post-treatment MRI. MRI and clinical symptom were carefully recorded in TMJ disorder patients. The relationship between TMJ symptoms such as pain, clicking sound, and disk displacement and effusion in MRI were analyzed. Then, splint therapy was applied for 3 months according to our protocol; at day 1, patients should wear 24 hours, in order to accomodate the splint and then, wearing time is reduced to 12 hours per day for one month. During next 2 months, it is tapered to 8 hours per day according to the evaluation of the clinical symptoms. Post-treatment MRI was obtained after 3 month protocol. This treatment strategy improved the clinical symptoms of TMJ disorder, and effusion and articular disk position showed a significant change. In conclusion, a treatment modality using an anterior repositioning splint therapy is an appropriate method for temporomandibular disorder patients.
Objectives : The purpose of this study was to investigate the relationship between masseter and cervical muscle activity and temporomandibular disorder in female office workers. Methods : Experimental group of 24 healthy subjects complained of temporomandibular joint related to computer use which lasted more than 3 months in the past year and was present in the past 7 days as well as on the day of test. Control group of 20 healthy subjects had no complaints of minimal discomfort on the day of test, and had no discomfort in the past 7 days. If they had reported discomfort in the past 12 months, it was of a short duration(<3 months) and resolved at least 3 months prior to participation. Outcomes were assessed by meridian-electromyography(MEMG), whole spin x-ray, mandibular function impairment questionnaire(MFIQ), neck disability index(NDI), visual analog scale(VAS), Beck depression inventory(BDI), stress reaction inventory(SRI) and Holmes & Rahe social readjustment rating scale(SRRS). Results : The contraction power of masseter muscle, upper trapezius, sternocleido-mastoid muscle and erector spinae by MEMG was significantly higher in the experimental group. The muscle fatigue of masseter muscle and sternodeido-mastoid muscle by MEMG was significantly higher in the experimental group. SRI was significantly higher in experimental group. There was no significant difference between two groups in the Jackson's angle, Cobb's method and cranio-cervical posture. Conclusions : The results suggest that temporomandibular disorder related mental stress but physical stress does not change cervical structure significantly.
Background: The purpose of this study was to determine the effect of Postural Yinyang correction of the temporomandibular joint (functional cerebrospinal therapy) on temporomandibular disorder. Methods: Medical records of 21 outpatients were reviewed who were diagnosed with temporomandibular joint disorder, unspecified (K0769) and treated at the Department of Acupuncture & Moxibustion, Dong-Eui University Korean Medicine Hospital from May $1^{st}$, 2017 to April $30^{th}$, 2018. Patients received more than 10 treatments of upper cervical manipulation and performed self-exercise therapy more than 3 times a day and wore an accurate balancing appliance in the oral cavity for more than 8 hours per day. To estimate the efficacy, visual analogue scale (VAS), numerical rating scale (NRS), maximum mouth opening (MMO), symptom intensity scale (SIS), max SIS (MSIS), symptom frequency scale (SFS), mandibular function impairment questionnaire (MFIQ) and 5-point Likert scale were used. Results: NRS and MSIS were significantly improved during each period. VAS, MMO, SIS, and SFS were significantly improved during each period, except the period from the $8^{th}$ to $10^{th}$ visit. MFIQ score was significantly improved during the period from the $1^{st}$ to $10^{th}$ visit. In the 5-point Likert scale, the results showed a high patient satisfaction with the treatment. Conclusion: These results showed that functional cerebrospinal therapy using an accurate balancing appliance, may be useful for reducing the symptoms of temporomandibular disorder.
The purposes of this study were to assess the effect of therapeutic exercise and to offer an approach to the physical therapy and rehabilitation procedure of the temporomandibular joint (TMJ) following surgery. In this research, 42 patients with TMJ surgery were assigned one of two groups. The experimental group included 21 patients who performed therapeutic exercise, and the control group included 21 patients who did not perform therapeutic exercise. Conservative therapy such as an ice pack, a hot pack, and pulsed ultrasound was applied to both groups. Treatment was applied twice a day during the admission period and, after discharge, everyday for six weeks. Visual analogue scale (VAS), incisal biting force, and joint ROM were measured before surgery and at 30 days after surgery. The results were as follows: VAS (p<0.05), mouth opening (p<0.01), lateral excursion to unaffected side (p<0.05), and protrusion (p<0.05) between experimental group and control group showed statistically significant differences. Incisal biting force and lateral excursion to affected side between experimental group and control group showed no statistically significant difference.
