3회 재발한 폐렴 구균성 뇌막염 1례 (A case of pneumococcal meningitis with 3 time recurrences)

  • Streptococcal pneumoniae is the second most coomon cause of meningitis in infancy and a major respiratory pathogen. It is the one of the most common cause of acquired pneumonia and otitis media in childhood. Intracranial extesion of acute otitis media occur somewhat more often from poorly pneumatized than from well-pneumatized temporal bones and in ears with a history of previous attacks of otitis media. We experienced a case of pneumococcal meningitis with 3 time recurrences in a 6 year-old male patient who had hospitalized with high fever, headache, vomiting and stupor mental state. He was diagnosed as CSF study and brain CT. Brain CT showed poorly pneumatized right mastoid region of temporal bone at that time. We report a case of pneumococcal meningitis with 3 time recurrences due to poorly pnematized mastoid region of temporal bone. A review of literatures was also presented briefly.

두개골 유아 근섬유증 - 증례보고 - (Infantile Myofibromatosis of the Skull - Case Report -)

  • Infantile myofibromatosis is a rare and benign myofibroblastic tumor that may occur in either solitary or multicentric form in the soft tissue of infants. A 13-month-old girl presented with a painless firm mass, measuring $2.5{\times}2.5cm$ in the right temporal area. Skull X-ray and CT scan revealed a well enhancing soft tissue tumor with a round skull defect and sclerotic margin. The tumor was totally excised with curettage of the skull defect followed by cranioplasty. Pathology was confirmed to be a solitary infantile myofibromatosis. We report this rare solitary infantile myofibromatosis of the temporal bone with review of the pertinent literature.

  • The tympanic plate is a small part of the temporal bone that separates the mandibular condyle from the external auditory canal. Fracture of this small plate is rare and usually associated with other bony fractures, mainly temporal and mandibular bone. There is a limited amount of literature on this subject, which increases the chance of cases being overlooked by physicians and radiologists. This is further supported by purely isolated cases of tympanic plate fracture without evidence of other bony fractures. Cone-beam computed tomography is an investigative three-dimensional imaging modality that can be used to detect fine structures and fractures in maxillofacial trauma. This article presents four cases of isolated tympanic plate fracture diagnosed by cone-beam computed tomography with no evidence of fracture involving other bones and review of the literature.

  • 축두골에 발생하는 단골섬유성리형성증은 극히 드문 이과학적 질환으로서 일반검사성적이 정상이며 병소에 무통성골조직증식을 주 소견으로 출현한다. 저자 등은 본증에 의하여 외이도 폐쇄와 심한 패쇄성외이도각화증을 동반하여 고도의 부음성난청을 일으킨 1례에 대하여 외이도형성술을 시행하여 치험하였기에 문헌고찰과 함께 보고하는 바이다.

전정질환의 측두골 조직병리 (Vestibular Histopathology in Temporal Bone)

  • There are a number of reports on the pathologies of vestibular disorders. However, these studies included only a few examples, which were not quantitative but merely anecdotal or descriptive. However, a single tissue section may be relevant to a specific disease in multiple ways. The histopathological characteristics of common peripheral vestibulopathies, including benign paroxysmal positional vertigo, $M{\acute{e}}ni{\grave{e}}re^{\prime}s$ syndrome, labyrinthitis, vestibular neuritis, and ototoxicity, have been described. A recent study validated a new quantitative method for determining vestibular otopathology. Detailed quantitative analyses of vestibular pathology are required to obtain a deeper understanding of the vestibular system. Such studies will likely reveal the pathophysiological causes of specific diseases by elucidating the correlations between structural and functional features. Therefore, histopathological studies of vestibular disorders should be performed.

  • A 65-year-old woman presented with a solid mass on the right temporal area. The mass had grown for over 2 years without any initiating event of trauma or inflammation. Before excision, the patient went through a computed tomography scan, revealing a calcified mass without bony connection. Under general anesthesia, an excisional biopsy was performed. Microscopic examination confirmed a diagnosis of soft tissue osteoma. Soft tissue osteoma is rare, especially in the head and neck region. Osteomas in the temporal region have not been reported yet. Due to its rarity, osteoma might be misdiagnosed as another soft tissue or bone origin tumor. Its treatment of choice is simple excision. In this review, we present an unusual clinical form of soft tissue osteoma.

Condylar jugular diverticulum: A report of 3 cases

  • Jugular bulb diverticulum is an irregular extension of the jugular bulb into the temporal bone that may be symptomatic or asymptomatic. The jugular bulb has rarely been reported to extend into the occipital condyle; such extension is termed a condylar jugular diverticulum and is characterized as a defect in the occipital condyle contiguous with the jugular bulb. This report details 3 cases of condylar jugular diverticulum. Extension of the jugular bulb into the ipsilateral occipital condyle was noted as an incidental finding on cone-beam computed tomographic (CBCT) images of 3 patients. All 3 patients were asymptomatic, and this finding was unrelated to the initial area of interest. CBCT use is becoming ubiquitous in dentistry, as it allows 3-dimensional evaluation, unlike conventional radiography. Proper interpretation of the entire CBCT is essential, and recognition of the indicators of condylar jugular diverticulum may prevent misdiagnosis of this rare entity.

갑상샘 여포암의 다발성 골전이 1예 (Follicular Thyroid Cancer with Multiple Bone Metastasis : A Case Report)

  • Follicular thyroid cancer(FTC) accounts for about 10-15% of thyroid cancer. Distant metastasis is common, usually to lung, bone and brain. 71-years-old man visited neurosurgery outpatient department. He complained of recent 6kg weight loss, left upper extremity pain with weakness and back pain. The radiologic findings showed multiple bone metastasis including thoracic spine and left scapular resulting from FTC. There was a probable brain metastatic lesion on right temporal fossa. The core biopsy of thyroid and thoracic spine(T11) confirmed metastatic follicular carcinoma. Radioactive iodine therapy and radiotherapy was done following total thyroidectomy. We report a unique case of multiple bone metastasis from follicular carcinoma of thyroid with literature review.

  • The purpose of this study is to examine the initial reaction of craniofacial complex to the traction of headgears which are frequent used in dinical practice by using holographic interference method. Changes in the fringe pattern according to the traction conditions (outer bow length, load amount, direction) were compared. Human dry skull was used as experimental material, the results were as follows. 1. The density of fringes increased with an increase in load. 2. Maxillary headgear affected circummaxillary bones-temporal bone, zygomatic bone, nasal bone, frontal bone, sphenoid bone as well as maxilla. 3. The most upward-backward displacement was observed in a high pull head gear $(30^{\circ})$, this is though to be a optimal direction for supressing the growth of the maxilla.

  • Severe temporal hollowing deformities can occur in patients who undergo craniectomy after intracranial hemorrhage. Reconstruction surgery for cosmetic purposes using silicone implants in patients with temporal hollowing deformities after craniectomy is advantageous because the procedure is simple and rapid, with a short recovery time, resulting in lower overall treatment costs. Of paramount importance, this option yields highly satisfactory results for patients. The authors present three cases of simple and fast surgery using silicone implants where highly satisfactory outcomes were obtained.
