Planning and decommissioning of a disused Theratron- 780 teletherapy machine and the dose assessment methodology for normal and radiological emergency conditions

  • The present work represents a technical guideline for decommissioning a disused teletherapy machine model Theratron-780 and contains category one 60Co radioactive source. The first section predicts the dose rate from the source in case of normal and radiological emergency situations via FLUKA-MC simulation code. Moreover, the dose assessment for the occupational during the whole process is calculated and compared to the measured values. A suggested cordoned area for safety and security in a radiological emergency is simulated. The second section lists the whole process's technical procedures, including (preview, dismantle, securing, transport and storage) of the disused teletherapy machine. Results show that the maximum obtained accumulated dose for occupational were found to be 24.5 ± 4.9 μSv in the dismantle and securing process in addition to 3.5 ± 1.8 μSv during loading on the transport vehicle and unloading at the storage facility. It was found that the measured accumulated dose for workers is in good agreement with the estimated one by uncertainty not exceeding 5% in normal operating conditions.

High Dose Rate Cobalt-60 After Loading Intracavitary Therapy of the Uterine Cervical Carcinoma in Srinagarind Hospital, Analysis of Residual Disease

  • Objectives: To evaluate residual disease in uterine cervical cancer patients treated with teletherapy using combined high dose rate Cobalt-60 brachytherapy. Materials and Methods: A retrospective study of uterine cervical cancer patients, FIGO stages IB-IVB (International Federation of Gynecologists and Obstetricians recommendations), treated by radiotherapy alone between April 1986 and December 1988 was conducted and the outcomes analysed. The patients were treated using teletherapy 50 Gy/25 fractions, five fractions per week to the whole pelvis together with HDR Cobalt -60 afterloading brachytherapy of 850 cGy/fraction, weekly to point A for 2 fractions. Results: The study covered 141 patients with uterine cervical cancer. The mean age was 50.0 years with a range of 30-78 years. The mean tumor size was 4.1 cm in diameter (range 1-8 cm). Mean follow - up time was 2.94 years (range 1 month-6.92 years). The overall incidence of residual locoregional disease was 3.5%. Residual disease, according to stage IIB, IIIB and IVA was present in 2.78%, 3.37% and 50.0%. It was noted that there was no evidence of residual disease in stage IB and IIA cases. Conclusion: Combined teletherapy along with high dose rate Cobalt -60 brachytherapy of 850 cGy/fraction, weekly to point A for 2 fractions resulted in overall 3.5% residual disease and a 96.5% complete response. The proposed recommendation for improving outcome is initiation of measurements for early detection of disease.

A Method for Evaluation of Mechanical Accuracy of a Teletherapy Machine Using Beam Directions (방사선 진행방향을 이용한 원격치료장치의 기계적 정확성 평가방법)

  • Purpose: The purposes of this paper are to develop a theoretical basis that the beam directions should be considered when the mechanical accuracy of teletherapy machine is evaluated by the star pattern test, to develop methods using asymmetric field in length to simulate beam direction for the case that beam direction does not appear on film. Method: In evaluating mechanical rotational accuracy of the gantry of teletherapy unit by the star pattern test, the direction of radiation beams was considered. A star pattern using some narrow beams was made. Density profiles at 10cm far from estimated gantry axis on the star pattern were measured using an optical densitometer. On each profile, one coordimate of a beam axis was determined. A pair of coordinates on a beam axis form an equation of the axis. Assume that a unit vector equation omitted is with same direction as radiation beam and a vector equation omitted is a vector directing to the beam axis from the estimated gantry axis. Then, a vector product equation omitted ${\times}$ equation omitted is an area vector of which the absolute value is equal to the distance from the estimated gantry axis to the beam axis. The coordinate of gantry axis was obtained by using least-square method for the area vectors relative to the average of whole area vectors. For the axis, the maximum of absolute value of area vectors would be an accuracy of the gantry rotation axis. For the evaluation of mechanical accuracies of collimator and couch axes for which beam direction could not be depicted on a star pattern test film, narrow beams asymmetric in field length was used to simulate beam direction. Result: For a star test pattern to evaluate the mechanical accuracy of rotational axes of a telectherapy machine, the result considering beam direction was different from that ignoring beam direction. For the evaluation of mechanical accuracies of collimator and couch axes by means of a star pattern test, narrow asymmetric beams could simulate beam direction. Conclusion: When a star pattern test is used to evaluate the mechanical accuracy of a teletherapy unit, beam direction must be considered or simulated, and quantitatively evaluated.

Penumbra Effect on Integral Absorbed Dose in Co-60 Teletherapy

  • Due to the Co-60 source size, the penumbra in Co-60 teletheraphy poses a serious problem, even if the extended collimators are used, Here an empirical formula for the calculation of integral absorbed dose in the penumbra region was derived. Through a numerical calculation, the penumbra effect on integral absorbed dose was investigated. The longer the source-to-skin distance, the larger the integral absorbed dose of penumbra region, and the larger the source diameter, the larger the integral absorbed dose of penumbra region. It was also found that in some case the integral absorbed dose in penumbra region becomes several times larger than the integral absorbed dose of treatment region itself if the source-to-skin distance becomes greater. Therefore, one must consider the penumbra effect in Co-60 teletherapy.

Trouble in Source Driving System of a $^{60}Co$ Teletherapy Unit ($^{60}Co$ 치료장치의 선원 구동상의 문제점)

  • An asymmetry in dose profile of a $^{60}Co$ teletherapy unit was found by means of water-phantom measurement. The reason of that trouble was confirmed to be the abnormal 'ON' position of the source, which is resulted from the high friction between contiguous surfaces of the spring for driving the source to 'OFF' position. Lubrication in the spring improved the mobility a little, but was not a radical repair. The radical repair was to replace the old spring by new one. Periodic maintenance for source driving system and periodic measurement of field symmetry are required for prevention of abnormal 'ON' position of $^{60}Co$ source.

Study on the Reduction of Electron Contamination with A Cobalt-60 Gamma Ray (코발트-60 감마선의 전자 오염에 관한 연구)

  • Electron contamination due to the interaction between radiation beam and material was analyzed for the factors such as source-skin distance (SSD), field size, tray characteristics and position of filter, which can affect the surface dose in Cobalt teletherapy. Surface dose in open beam was more influenced by SSD with increasing field size. Relative surface charge (RSC) increased with the use of tray (solid, circular hole, slotted), compared with open beam, which is thought to be due to increased electron contamination of the tray. To reduce the surface dose, 0.4mm thick Lipowitz metal filter was used. Compared with open beam, RSC decreased by 8.8%, 11.3%, 13.3%, 16.6%, 19.3% and 21.7% for the field size of $5{\times}5$, $10{\times}10$, $15{\times}15$, $20{\times}20$, $25{\times}25$ and $30{\times}30cm^2$, respectively. On the contrary, use of Lipowitz metal filter increased RSC at 60cm or less SSD. Surface dose was effectively reduced with Lpowitz metal filter placed right below solid tray in Cobalt teletherapy.

  • High energy radiation therapy using accelerator or radioioisotope teletherapy unit is now one of the most important modality in the field dealing with human malignant tumor. It's successful technology overcomes incurable disease to change into curable disease not only by the improvement of clinical technique but also by the development of radiation physics and biology. The author presented the principles of radiation therapy by means of basic knowledge of medicine. physics and biology, described the various ways to improve the result of radiation therapy, and reviewed recent status of radiotherapy field in Korea.

  • In recent years there has been a growing interest in total body, hemibody, total lymphoid irradiation. For refractory leukemia or lymphoma patients, various techniques and dose regimens were introduced, including high dose total body irradiation for destruction of leukemic or bone marrow cells and immunosuppression prior to bone marrow transplantation, and low dose total body irradiation for treatment of lymphocytic leukemia or lymphomas. Accurate provision for specified dose and the desired homogeneity are essential before clinical total body irradiation. Purposes of this paper are to discuss calibrating Cobalt Unit in 3m distance using Rando Phantom, to compare calculated dose, calibrated dose, and compensating filters for homogeneous dose distribution in the head and neck, the lung, and the pelvis. Results were following. 1. Measured dose on the lung was 6% higher than on the abdomen. Measured dose on the head (10%) and neck (18%) were higher than the abdomen because of thinness. Pelvic dose was measured 12% less than the abdomen. Those data suggest that compensating filter was essential. 2. Measured dose according to distance was 3% less than calculated dose which suggest that all doses in clinical use should be compared with calculated dose for minimizing error.

  • This study was undertaken to observe the salivary gland of the white rat when exposed to single and fractionated doses of Cobalt-60 irradiation. One hundred fifty white rats of the experiment were devided into control and 3 experimental groups. In experimental groups, group I receivcd 1200 rads everyweek untill 4800 rads reached, group II received 1500 rads and group III received 2000 rads with single dose. irradiation was carried out using a RAC-120 Cobalt-60 Teletherapy Unit with a dose rate 84.3 r/min, field size 4×5 cm measured at 80 from source. Rats were serially sacrificed at the following postirradiation time intervals: 1, 3, 5, 7, 10, 14, 21, 28, 35, and 42days. At sacrifice, the parotid and submandibular glands were dissected out in toto, and stained with: 1) hematoxylin and eosin; 2) periodic acid Shiff; 3) toludine blue.
