For about half a century. dental implants made of titanium have developed as a method of restoration for the tooth loss. In these days. the titanium implants seem to be considered as the alternative for the conventional prosthodontics. But its hard to say that the titanium implants are superior to the treatments that preserve the natural tooth. As this is a general opinion among dentists. the implant will not be able to be the alternative for all the prosthetic treatments. Clinically, there are many causes for extracting tooth. The severe destruction of the tooth structure or periodontal diseases leads to inevitable tooth extraction. When the complete cure is doubtful because of narrow intraoral visibility and improper accessibility in approaching to the tooth and periodontal lesion, we. clinicians often inevitably extract tooth. Passive treatments like conventional restoration, curettage or surgical flap cant be the perfect treatments for the tooth that has subgingival root caries or severe periodontal diseases involved furcation. Many clinicians might have been forced to pull out the relatively healthy tooth by the difficulties of approaching to the lesions and poor prognosis. Though the intentional tooth replantation is performed sometimes. as it doesnt have enough scientific foundation. it has not been considered as a popular treatment method yet. I have been felt keenly the necessity of positive tooth preservation, so I have been attempting the treatment that has new concept. calling Natural Tooth Implantation (NTI) clinically. NTI differs from the tooth replantation in the goal for the treatment and biological healing process. Now. I confirm that NT! is a very positive and valid method of tooth preservation. Like you can get from the name. NTI is the dental implant procedure using natural teeth and similar to the healing process of the titanium implants in many aspects. Lesch-Nyhan syndrome is rare X-linked genetic disorder involving absence of the enzyme hypoxanthine guanine phosphoribosyl transferase (HGPRT) related to purine metabolism. The deficiency of HGPRT activity leads to an excesscive uric acid production and consequent hyperuricemia. It occurs almost exclusively in males, and the incidence is estimate to be 1/100,000~380,000. Clinical presentation is characterized by developmental delay, mental retardation, choreoathetosis, spastic cerebral palsy, nephrolithiasis, obstructive nephropathy and acute gouty arthritis. A characteristic feature of Lesch-Nyhan syndrome is the appearance of intractable self-mutilation behavior. Self-mutilation behavior is complicated by secondary infection and tissue loss as well as pain. The dental management of self-mutilation includes presertive methods of using appliances such as lip bumper or soft mouthguard and radical methods such as extraction of all teeth or orthognathic surgery. A case of Lesch-Nyhan syndrome patient with self-mutilation and severe lower lip injury is presented. He was treated successfully with soft mouthguard.
We described two cases of odontogenic keratocysts of the maxilla. Odontogenic keratocysts (OKCs) are jaw cysts with a proclivity for local invasion and recurrence. Clinically, OKCs are characterized by aggressive, local growth. The diagnosis of mandibular OKC is based on physical examination and plain film radiographic findings. However, histopathological confirmation is required to make the diagnosis with certainty. The molar regions of the mandible and maxilla are the principal primary locations. The maxillary antrum is also a common site. Initial therapy is typically enucleation with or without extraction of the associated teeth. Long-term follow-up is necessary because of the aggressive nature and recurrence rate of OKCs.
This study was undertaken to analyze the displacement and stress distribution in the mandible according to the pulling directions during mandibular first molar cervical traction after mandibular second molar extraction. The 3-dimensional finite element method(FEM) was used for a mathematical model composed of 594 elements and 1019 nodes. An orthodontic force, 450 gm, was applied to the each mandibular first molar in parallel, and below the occlusal plane by $7^{\circ}\;and\;25^{\circ}$ and meet the midsagittal plane by $40^{\circ}$ toward posterior direction. The results were as follows: 1. Mandibular teeth were displaced in more downward, posterior and lateral direction. Especially high stress was noted in case of parallel pull than in case of below the occlusal plane by $7^{\circ}\;and\;25^{\circ}$. 2. Mandibular first molar was moved bodily. 3. Generally, alveolar bone, mandibular body, ascending ramus and mandibular angle portion were displaced in downward, posterior and lateral direction. But coronoid process was displaced in downward, forward and lateral direction, and anterior and inner middle portion of condyle head and neck were displaced in downward, forward and medial direction, and posterior and outer middle portion of condyle head and neck were displaced in upward, forward and medial direction. 4. Maximum stress was observed at the condyle head and neck portion. With steeper direction of force, condyle head and neck showed more stress than parallel relation to the occlusal plane.

The aim of this clinical report is to describe the complex treatment of an adult Class III malocclusion patient who was disappointed with the outcome of a previous oral rehabilitation. Interdisciplinary treatment planning was performed with a primary indication for implant removal because of marginal bone loss and gingival recession, followed by orthodontic and surgical procedures to correct the esthetics and skeletal malocclusion. The comprehensive treatment approach included: (1) implant removal in the area of the central incisors; (2) combined orthodontic decompensation with mesial displacement and forced extrusion of the lateral incisors; (3) extraction of the lateral incisors and placement of new implants corresponding to the central incisors, which received provisional crowns; (4) orthognathic surgery for maxillary advancement to improve occlusal and facial relationships; and finally, (5) orthodontic refinement followed by definitive prosthetic rehabilitation of the maxillary central incisors and reshaping of the adjacent teeth. At the three-year follow-up, clinical and radiographic examinations showed successful replacement of the central incisors and improved skeletal and esthetic appearances. Moreover, a Class II molar relationship was obtained with an ideal overbite, overjet, and intercuspation. In conclusion, we report the successful esthetic anterior rehabilitation of a complex case in which interdisciplinary treatment planning improved facial harmony, provided gingival architecture with sufficient width and thickness, and improved smile esthetics, resulting in enhanced patient comfort and satisfaction. This clinical case report might be useful to improve facial esthetics and occlusion in patients with dentoalveolar and skeletal defects.

In the management of ankylosed primary molars, early diagnosis, proper treatment, and thorough follow-ups are very important. Untreated infraocclusion due to ankylosis has a negative impact on normal occlusal development, and may cause problems. There are many treatment options on infraoccluded deciduous molars, such as periodic observation, conservative method, restoration, and space regaining via extraction of the teeth. In this case report, two 6-year-old girls were diagnosed with ankylosed maxillary second primary molar and displaced tooth germ of the second premolar. Early surgical removal of the ankylosed primary molar was considered as a treatment approach. The long-term follow-up shows normal eruption of a succeeded permanent premolar.

Hemophilia, the most common of the inherited bleeding disorder, is the result of a deficiency of clotting factor. Since bleeding after dental treatment may cause severe or even fatal complications, people with hemophilia must be given special dental care. We report on the diagnosis and treatment of a 9-year-old boy having severe hemophilia visited our department with the chief complaints of pus discharge on the left lower molar region. In the clinical and radiographic examination, periapical abscess and dental caries were diagnosed. Considering complexity of the treatment and complication in the coagulation, it was decided to carry on the treatment under general anesthesia. Clotting factor IX concentrates were intended to provide 50-70% plasma level. Pulpectomy, resin restoration and Stainless steel crown were given under general anesthesia. Several teeth were extracted and the sockets were packed with Surgicel$^{(R)}$ (Oxidized Regenerated Cellulose, Johnson and Johnson Co. Neuchatel, Switzerland) under general anesthesia. Transpalatal arch and lingual arch were given for maintaining the extracted space before discharged. For people with severe hemophilia, factor replacement is necessary before scaling, surgery or regional block injections. Therefore, if several extractions are needed, dental care under general anesthesia would be effective and efficient management.

Mandibular symphyseal distraction osteogenesis is an alternative approach for correcting mandibular transverse deficiencies and dental crowding. The traditional approaches for these are extraction of teeth and arch expansion with traditional orthodontic treatment. Also extractions are usually unavoidable in patients with severe crowding. The purpose of this study is to evaluate the effect of mandibular symphyseal distraction osteogenesis by use of tooth-borne expansion appliance. All of 12 patients had been performed distraction osteogenesis. The surgical procedures were accomplished under local anesthesia and intravenous sedation in an ambulatory surgical setting using a routine distraction protocol. The latency period was 5 days or 7 days after symphyseal osteotomies. The rate & rhyth is a intermittent, 0.75mm or 1.0 mm per day and stabilized for 6, 8 weeks after distraction. The time of orthodontic tooth movement after distraction was variable from 2 weeks to 8 weeks (mean 3 weeks). All patients had been evaluated with study casts, plain periapical films, panorama radiograms before & after surgery. Mandibular symphyseal distraction osteogenesis increased mandibular arch width and corrected dental crowding, with paralleling tooth-borne movement, without proclination of the mandibular incisors.
The purpose of this study was to evaluate the change of before and after treatment of anterior openbite malocclusions treated by Multiloop Edgewise Archwire technique. The openbite sample consisted of 4 male and 12 female adults, treated with nonextraction or third molar extraction. The normal sample consisted of 58 subjects, which have pleasing facial profile and normal occlusion and no experience of orthodontic or prosthodontic treatment. The 58 subjects of normal sample were subdivided by cephalemetric vertical relationship of face. The 40 subjects, cephalometric vertical relationship of face was in normal range, classified as Normal Sample group 1. The 18 subjects, increased cephalometric vertical relationship of face, classified as Normal Sample group 2. The computerized cephalometric analysis was accomplished with 50 reference points for 22 skeletal measurements, 46 dentoalveolar measurements, 8 soft tissue measurements. Statistical analysis of the data was carried out with paired t-test, Student's t-test, and DUNCAN test using SAS(PC version), The results were as follows : 1. There were no statistically significant differences in skeletal measurement between before and after treatment. The major changes were in dentoalveolar region. 2. After treatment, the long axis of maxillary and mandibular posterior teeth were distally tipped-back, and uprighted to bisected occlusal plane. The interincisal angle was increased. 3. There were no statistically significant increase in the upper posterior dental height and statistically significant decrease in the lower posterior dental height. The upper anterior dental height was increased, but there was no statistically significant increase in the absolute upper anterior dental hight. The lower anterior dental height was increased. 4. After treatment, the maxillary occlusal plane to palatal plane angle and the mandibular occlusal plane to mandibular plane angle were statistically significant increased. Then, there were no statistically significant difference between after treatment group and normal sample group 2. 5. After treatment, the percentage of upper lip length to upper anterior dental height was decreased. Then, There were no statistically significant difference between after treatment group and normal sample group 2.

Sturge-Weber syndrome is a rare nonhereditary developmental condition that is characterized by a hamartomatous vascular proliferation involving the tissue of brain and face. The clinical features are characterized by port wine nevus following one or more divisions of trigeminal nerve, ocular involvement and neurologic involvement such as epilepsy, mental retardation, and contralateral hemiplegia. Oral manifestations include unilateral blood vessel expansion of the oral mucosa, vascular hyperplasia of gingiva, pyogenic granuloma-like massive hemangiomatous proliferation of oral mucosa, macrodontia, ipsilateral macroglossia, blood vessel anomaly of maxilla or mandible and abnormal tooth eruption sequence. This case report is about 11-year-old Sturge-Weber syndrome patient presented port wine nevus on the face, venous malformation on soft plate and buccal mucosa. In this case we performed simple extraction of several deciduous teeth and periodic oral hygiene management. If a patient with Sturge-Weber syndrome has to undergo dental surgery in affected areas of the mouth, great care must be taken to prevent severe hemorrhage.
