In this paper many vulnerable areas of the present fire PSA methodology were revealed to apply risk-informed fire protection to nuclear power plants. The results and insights from the fro PSA should be used as a part of a risk-informed decision making process rather than the complete technical basis for decision making. The degree of support and scope of applications is dependent on the accuracy and validity of the model used in the fire PSA. Accordingly; the usefulness of the fire PSA will increase as ongoing research and development efforts lead to improvements in the state of the art technology and as improvements in the implementation of the state of the art technology lead to more consistent results.

In the current technology-driven era, Small and medium enterprises (SMEs) have been deemed as one of the leading and prominent drivers of sustainable economic progress in emerging and developing economies such as in the Bangladesh context. Hence, it is of significance to understand what might fuel accelerating performance of SME business as increased SME performance will bring about more sustainability and strong development of SME sector within the country, through which more employment is anticipated to be generated. Therefore, the current study examines the impact of three factors: risk-taking, innovativeness, and proactiveness on SME performance in Bangladesh. The study has gathered data from SME entities in Dhaka city of Bangladesh, by applying a non-probability sampling strategy. 250 SME owners were contacted to act as respondents and finally, 180 SME owners fully completed the survey questionnaire, indicating that the final sample size is n=180. SPSS is used as a purpose of testing the hypotheses by considering a 5% significance level as acceptance criteria of the hypothesis. Hierarchical regression analysis was run to understand the impact of control variables and independent variables on SME performance and found that age of business, risk-taking, innovativeness, and proactiveness have an important impact on SME performance in Bangladesh.
Limiting conditions for operations (LCOs) are evaluated dynamically using the tool of system dynamics. The LCOs de-fine the allowed outage times (AOTs) and the actions to be taken if the repair cannot be completed within the AOT. System dynamics has been developed to analyze the dynamic reliability of a complicated system. System dynamics using Vensim software have been applied to LCOs assessment for an example system, the auxiliary feed water system of a reference nuclear power plant. Analysis results of both full power operation and shutdown operation have been compared for a measure of core damage frequency. The framework developed in this study has been shown to be very flexible in that it can be applied to assess LCOs quantitatively under any operational context of the TS in FSAR.
This study focuses on assessing risks which might occur in operation stage of CAES storage cavern and analyzing fire scenarios for the risk that have been assessed with highest risk level. Risks in operation stage were categorized into upper risk group and lower risk group. Components of upper risk group are technical risk, facility risk and natural disaster risk. Lower risk group is composed of 11 sub-risks. 20 experts were chosen to survey questionnaires. ANP model was applied to analyze the relative importance of 11 sub-risks. Results of risk analysis were compared with risk criterion to set risk priorities, and the highest risk was determined to be 'occurrence of the fire within the management opening'. Three fire scenarios were developed for the highest risk level and FDS (Fire dynamics Simulator) was used to analyze these scenarios. No. 3 scenario which air blows from tunnel into outside atmosphere represented that a rate of smoke spread was the fastest among three fire scenarios and a smoke descended most quickly below the limit line of breathing. Thus, No. 3 scenario turned out to be the most unfavorable condition when operating staffs were evacuated from access tunnel.
The types and quantities of Hazmat and Hazmat transportation are gradually increasing, keeping pace with industrialization and urbanization. At present the safety management for Hazmat transportation only considers reducing accident probability, but even when an accident involving Hazmat-carrying vehicles occurs, that is not regarded as a Hazmat-related accident if the Hazmats do not leak out from the containers carrying them. Thus the methods to reduce risk (Risk=Probability$\times$Consequence) have to be developed by incorporating accident probability and consequence. By using Geographic Information System (GIS), a technical method is invented and is automatically able to evaluate the consequence by different types of Hazmat. Thus this study analyzed the degree of risk on the links classified by the Hazmat transport pathways. In order to mitigate the degree of risk, a method of 7-step risk management on Hazmat transportation in railway industries can be suggested. (1st step: building up GIS DB, 2nd step: calculating accident probability on each link, 3rd step: calculating consequence by Hazmat types, 4th step: determination of risk, 5th step: analysis of alternative plans for mitigating the risk, 6th: measure of effectiveness against each alternative, and 7th step: action plans to be weak probability and consequence by the range recommended from ALARP). In conclusion, those 7 steps are used as a standardization method of optimum transportation routing. And to increase the efficiency of optimum transportation routing, optional route can be revise by verification.
A substantial literatures on environmental risk perception have emerged since the late 1960s. Most these works focus on how people form risk perception, on what people believe, on difficulties in communicating information about health, safety, or environmental risks to non-experts. In this study, questionnaire surveys were conducted to sample from stakeholders(general public, environmental specialist, governmental official, non-government officials, journalist) during March and Aprils, 2000. Total number of responses was 1,803 including 773 persons of general public, 353 experts, 390 governmental officials, 111 journalists and 176 NGO members. Risk perception on 26 environmental issues were statistically analyzed to relate with the need to regulate each issues, interest and knowledge, experience of hazard, satisfaction in environmental situation, etc. This research aims to aid risk analysis and policy-making by providing a basis for understanding and anticipating group responses to environmental issues and improving the communication of risk information among general public, lay-people, technical experts, and decision-makers. This study concludes that those who are in charge of promoting and regulating health and safety of citizens should understand how people perceive about and respond to environmental risk. Without such understanding, well-intended environmental policies of governments would be ineffective.

This study identifies various risk factors associated with activities of early construction stage, then establishes the Risk Breakdown Structure(RBS) by classifying the risks into the three groups; Common risks, risks for Earth works, and risks for Foundation works. The Common risks are identified and classified by considering various aspects of the early construction stage such as financial, political, constructional aspects, etc. The risks for Earth works and Foundation works are identified in detail by surveying technical specifications, relevant claim cases and interviewing with experts. These risks are classified based on the Wok Breakdown Structure(WBS) of the early construction stage. The WBS presented in this study classifies the works of early construction stage into four categories; excavation, sheeting works, foundation works, footing works. This study suggests a risk analysis method using fuzzy theory for construction projects. Construction risks are generally evaluated as vague linguistic value by subjective decision making. Fuzzy theory is a proper method to quantify vague conditions of construction activities. Therefore, this study utilizes fuzzy theory to analyse construction risks. The weight of risks is estimated by reflecting the interrelationship among risk factors from absolute weights obtained by fuzzy measure into the relative weights by Analytical Hierarchy Process(AHP). The interrelationship is estimated by Sugeno-fuzzy measure.

The Railway Safety Act of 2014 strengthened the 'Railway Safety Management System' to establish autonomous safety management for railway operators and railway facility managers. Accordingly, it is required to establish and implement risk assessment and safety measures for risk management. However, the current risk assessment system is carried out at the fragmented safety management level within individual fields, which has caused difficulties in establishing and implementing risk assessment and safety measures. In addition, the technical standards of the safety management system stipulate that risk assessment of railway operators is mandatory, so standardized standards for risk assessment of railway facilities and railway vehicle maintenance are needed. Therefore, in this paper, we first verified railway risks by analyzing railway accident data for the last 10 years, and proposed a standardized framework to effectively assess and manage risks through a case study of a condition-based smart maintenance system developed based on railway vehicle maintenance data.
The reliability-based seismic design of steel frames is a complex process that incorporates seismic demand with a structural capacity to attain safe buildings aligned with specified constraints. This paper introduces an efficient base shear force formulation to support the reliability-based design process of steel frames. The introduced base shear force equation combines the seismic demand statistics with the reliability objective to calculate a fictitious base shear force for linear static analysis. By concentrating on the seismic demand and promising to meet a certain level of reliability, the equation converts the reliability-based seismic design problem to a deterministic one. Two code-compliant real-size steel moment frames are developed according to different reliability objectives to demonstrate the competency of the proposed formula. The nonlinear dynamic analysis method is used to assess the seismic reliability of the constructed frames, and the numerical results validate the credibility of the suggested formulation. The base shear force calculation method regarding seismic reliability is the main finding of this study. The ease of use makes this approach a potent tool for design professionals and stakeholders to make rapid risk-informed decisions regarding steel moment frame design.
The Propulsion Test Facilities for the development of Korea Space Launch Vehicle-II are being built, some test facilities are completed and various combustion tests are running. The Propulsion Test Facilities consists test-stand, which carries out tests for engine development model, and various sub-systems and vessels containing LOX and Jet A-1 as propellant. There are always risks of fire and explosion at the test-stand since engine development model is conducted at test-stand with real combustion test with very high pressure, mixed propellant and high energy. In this paper, in order to establish the consequence analysis and risk reduction measures in the Propulsion Test Facilities, followings are considered. 1) a propellant leak accident scenario is assumed in test-stand. 2) TNT equivalent model equation based on blast wave of the explosion was used to analyze blast overpressure and impacts. Also, technical, systematic and managemental measure is described to ensure risk reduction for propulsion test facility.
