안구 건조증 환자의 치험 3례 (Clinical Observation on 3 Cases of Dry Eye Syndrome)

  • Objective : The Dry Eye Syndrome is characterized by reduction of the stability of the tearfilm and a set of alterations of the eye surface which could relate to tear quality, normal makeup of tearfilm and alterations in blinking or regular closing of eyelids. We treated 3 patients who was suffering with severe Dry Eye Syndrome and assessed the effectiveness of Oriental Medicine in the treatment of Dry Eye Syndrome. Methods : The patients were treated by acupunture, Gi Guk Yang Hyeol-decoction(Qijuyangxie-decoction). The improvement of the symptom was judged the subjective symptoms by the change of Ocular Surface Disease Index. Result : We observed in 2 cases of the patient significant decrease in the frequency of subjective symptoms, 1 case was not treated. Conclusion: The Oriental Medicine had effective results in Dry Eye syndrome. We need to have more observation and examination of Dry Eye syndrome.

  • This study was carried out to investigate sheet properties of Indian mallow hanji, made by different pulping methods such as alkali and sulfomethylated pulpings, and different stock compositions, various mixing ratios of bast fiber and woody core fibers. Effect of morphological properties of pulp stocks on the sheet formation and their optical properties were also evaluated using an image analyzer and confocal laser scanning microscope(CLSM). In addition, the effect of fiber distribution index(FDI), which was calculated based on the image in a z-direction of a sheet from CLSM, on the sheet properties of Indian mallow hanji was discussed. The proposed FDI had a good correlation with various properties of paper, such as apparent density, opacity, tear index, breaking length and zero-span tensile strength. Especially, sulfomethylated pulp sheets'FDI was higher than alkali pulp sheets.

조절펄스광선조사가 눈물막 지질층 두께 및 안구건조증에 미치는 효과 (Effect of Intense Regulated Pulse Light on Thickness of Tear Film Lipid Layer and Dry Eye Syndrome)

  • 목적: 건성안 환자에서 조절펄스광선조사가 미치는 임상 효과 및 눈물막지질층 두께 변화에 대하여 알아보고자 하였다. 대상과 방법: 건성안에 대한 주관적인 불편감을 호소하는 환자 중 마이봄샘기능부전 소견을 보이며, 눈물막파괴시간이 10초 이하인 25명을 대상으로 후향적 연구를 시행하였다. 모든 대상자에 대한 조절펄스광선조사는 시술 당일, 15일, 45일 총 3회에 걸쳐 시행되었으며, 시술 전후 눈물막파괴시간, 쉬르머검사, 안구표면염색점수, 안구표면질환지수(Ocular Surface Disease Index, OSDI) 및 눈물막 지질층의 두께를 측정하여 비교 분석하였다. 결과: 조절펄스광선조사 시술 후 눈물막파괴시간은 $3.7{\pm}1.2$초에서 $4.4{\pm}2.1$초로 증가되어 통계적으로 유의한 차이를 보였으며(p=0.03), 쉬르머 검사 및 안구표면염색 점수 결과는 통계적으로 유의한 차이를 보였다(p<0.01, p=0.01). OSDI는 $39.5{\pm}17.5$점에서 $30.4{\pm}18.7$점으로 통계적으로 유의한 향상을 보였다(p=0.01). 눈물막지질층 두께는 통계적으로 유의한 변화를 보이지 않았다(p=0.49). 결론: 조절펄스광선조사 시술은 눈물막파괴시간, 쉬르머검사, 안구표면염색점수 및 주관적인 건성안 증상을 개선시키는 효과적인 치료 방법이라 생각된다.

Bio-Soda Pulping of Rice Straw with Pleurotus cornucopiae under Atmospheric Pressure

  • This study was carried out to develop the bio-chemical pulping method to enhance the energy saving and decrease the capital cost through the soda pulping under atmospheric pressure ($100^{\circ}C$). Nonwood substrates, rice straw, were pretreated by white-rot fungi, Pleurotus cornucopiae. Several basic pieces of data that can be applied in soda pulping were acquired. The results of this study were as follows. Under the conditions without any nutrients or with glucose, N and glucose + N, the weight losses of rice straws inoculated by Pleurotus cornucopiae were 12.1∼32.6 %, 12.0∼26.3 %, 13.0∼25.4 % and 15.3∼24.7 % for 5, 10, 15, 20, 25 and 30 days incubation periods respectively. The more the fungal incubation was extended, the more the weight losses were gained. The yield of untreated rice straw was 54.8 % after pulping. When any nutrients was not added or glucose, N and glucose + N were added for the pretreatment, the total yields were ranged to 57.3∼42.9 %, 51.0∼43.3 %, 51.7∼43.9 % and 52.1∼46.1 % for 5 different incubation periods respectively. The yields were gradually decreased based on the extending of the incubation periods. The physical properties of the rice straw soda pulp without fungal treatment, the density, breaking length, burst index, tear index and folding endurance were 0.24g/㎤, 2.32 Km, 0.91 kPaㆍ$m^2$/g, 46.7 mNㆍ$m^2$/g and 21 times, respectively. In the case of pretreatment without any nutrients or with glucose, N and glucose + N as nutrients, the density was 0.24g/㎤, the breaking length was 3.30∼6.46 Km, the burst index was 1.36∼3.01 kPaㆍ$m^2$/g, the tear index was 33.0∼57.0 mNㆍ$m^2$/g and the folding endurance was 14∼381 times at most incubating periods, when pulping was done. The physical properties were increased as the incubation duration was extended. Especially, when N and glucose + N were added, the physical properties showed superior results during each incubation period.

초음파처리에 의한 KOCC의 개질 (Modification of KOCC with Ultrasonic Wave Treatment)

  • Ultrasonic treatment of KOCC was tried to explore the possibility for improvement of KOCC quality as a raw material for making industrial packaging paper and paperboard. The effect of pH, stock consistencies and treatment times on the ultrasonic response of KOCC were investigated. WRV, apparent density, tensile index and burst index were improved as pH of KOCC slurry is increased. However highest tear index and stiffness were obtained at pH 8. It was also found that the highest WRV and strength properties could be obtained at 0.5% of KOCC consistency during ultrasonic treatment. WRV and most strength properties were improved with ultrasonic treatment time. It was found that ultrasonic treatment can be used as a useful mean for KOCC modificaton.

TMP의 LB 공융용매 처리 시 젖산/베타인 몰 비율이 리그닌 함량 및 수초지 특성에 미치는 영향 (Effect of Molar Ratios of DES on Lignin Contents and Handsheets Properties of Thermomechanical Pulp)

  • This study was conducted to investigate the effects of deep eutectic solvent (DES) treatment on properties of TMP fibers and handsheets. DES was prepared by mixing of lactic acid and betaine (L and B), and the molar ratio of these two components mixtures was controlled to 2:1 (L:B=2:1) and 5:1 (L:B=5:1). As results, lignin was partly extracted from the TMP fibers. Especially, the delignification of TMP samples was promoted according to the increase of the molar ratio of lactic acid. Except for tear index, both tensile index and burst index of handsheets were increased when higher molar ratio of lactic acid was mixed for DES preparation. However, the LB DES treatment of TMP fibers didn't give any effect on the optical properties of TMP handsheets. The plausibility of TMP fiber was to be enhanced by LB DES treatment.

O, Eo 및 C/D단계를 이용한 신갈나무 크라프트펄프표백 (Bleaching of Oak Kraft Pulp by O, Eo and C/D Stage)

  • The purpose of this study was to decrease pollutants of effluents from KP bleaching. Multistage bleaching that combined with O, Eo, and C/D stage was conducted, after optimum bleaching conditions of O, Eo, and C/D stage were determined. Moreover the properties for bleached pulps and pollution level of bleaching effluent were measured by each standard method. Due to increase on the brightness and delignification of pulps by O, Eo, and C/D stage bleaching. It was possible that bleaching sequences were reduced from five stage to four stage. In addition to, OC/DEoD and OC/DEopD bleaching sequences had higher brightness than CEDED bleaching sequence. The pulps bleached by O and Eo stage had higher in tensile index than conventional bleaching sequences with the similar results in burst and tear index. While each bleaching sequences had little differences, bleaching sequences with Eo stage had higher in physical properties than with Eop stage. The pollution level measured by Chemical Oxygen Demand(COD), Biochemical Oxygen Demand(BOD) and Color of bleaching effluents were much lower in combined bleaching sequences of O, Eo and C/D stage than that of CEDED sequence. Particularly, a pollution index of bleaching effluents generated by C/DEoDEB and OC/DEopDED bleaching sequences were lower than that from the other bleaching sequences.

Decay-Accelerating Factor Differentially Associates With Complement-Mediated Damage in Synovium After Meniscus Tear as Compared to Anterior Cruciate Ligament Injury

  • We have reported that anterior cruciate ligament (ACL) injury leads to the differential dysregulation of the complement system in the synovium as compared to meniscus tear (MT) and proposed this as a mechanism for a greater post-injury prevalence of post traumatic osteoarthritis (PTOA). To explore additional roles of complement proteins and regulators, we determined the presence of decay-accelerating factor (DAF), C5b, and membrane attack complexes (MACs, C5b-9) in discarded surgical synovial tissue (DSST) collected during arthroscopic ACL reconstructive surgery, MT-related meniscectomy, osteoarthritis (OA)-related knee replacement surgery and normal controls. Multiplexed immunohistochemistry was used to detect and quantify complement proteins. To explore the involvement of body mass index (BMI), after these 2 injuries, we examined correlations among DAF, C5b, MAC and BMI. Using these approaches, we found that synovial cells after ACL injury expressed a significantly lower level of DAF as compared to MT (p<0.049). In contrast, C5b staining synovial cells were significantly higher after ACL injury (p<0.0009) and in OA DSST (p<0.039) compared to MT. Interestingly, there were significantly positive correlations between DAF & C5b (r=0.75, p<0.018) and DAF & C5b (r=0.64 p<0.022) after ACL injury and MT, respectively. The data support that DAF, which should normally dampen C5b deposition due to its regulatory activities on C3/C5 convertases, does not appear to exhibit that function in inflamed synovia following either ACL injury or MT. Ineffective DAF regulation may be an additional mechanism by which relatively uncontrolled complement activation damages tissue in these injury states.

토착한지의 특성 - 외발 초지법 분류를 중심으로 - (Properties of Indigenous Korean Paper(Hanji) - Classification of Oebal(single frame)Papermaking Methods -)

  • 본 시험은 외발 뜨기 한지와 쌍발 뜨기 한지를 구분하고 각각의 물성 차이 중 특히 4가지 강도에 대해 상대적 우위와 가로 세로 강도의 차이를 파악했다. 그 결과, 섬유 배향성을 특별히 고려하지 않은 외발 뜨기는 쌍발 뜨기와 비교해 가로, 세로 강도 차이가 다소 적다는 것 외에 특별히 뛰어난 점이 없다는 것을 알 수 있었다. 외발 뜨기 한지가 질기다고 느껴지는 것은 좋은 원료와 토착식 제법으로 여러 장(2~8장) 겹쳐서 절대적인 강도를 향상시켰기 때문이며 상대적인 강도인 지수비교에서는 큰 차이가 없었다. 또한 부가적으로 도침의 효과를 파악할 수 있었다. 도침은 내절도를 두드러지게 향상시켰으며 인열지수(인열강도)를 뺀 열단장과 파열지수(파열강도)를 높이는 역학을 했다. 특히 열단장(인장강도)의 향상을 가져왔다. 그리고 토착식 제법의 외발 뜨기는 여러 겹을 겹칠수록 인열지수가 증가(다른 강도에 비교해 상대적인 증가임)하는 경향을 보였다. 그러므로 도침과 토착식 한지는 서로 부족한 강도를 훌륭히 보완하는 역할을 했음을 알 수 있었다. 따라서 섬유 배향성을 고려하여 초지하고 도침을 실시해야 외발 뜨기 고유의 강도적 특징을 살릴 수가 있음을 알 수 있었다. 그리고 오늘날에 토착 외발 뜨기 방식으로 제조된 전통 한지들은 전체적인 강도면에서 토착 한지류와 비슷했으며 2겹지의 경우엔 가로 세로 강도차이도 토착한지 수준이었다. 그러나 강도들간의 우열 특성에서는 인열강도가 특히 낮았던 쌍발 뜨기 한지와 전체적으로 고른 강도를 보인 토착 외발 뜨기 한지의 중간에 위치하여 개선의 여지를 찾을 수 있었다.

외상성 견갑하건 파열의 관절경하 봉합술 (Arthroscopic Repair of Traumatic Subscapularis Tendon Tear)

  • 목적: 회전근 개 파열 중 견갑하건의 파열은 극상건이나 극하건의 파열 보다는 드물다. 저자들은 외상 후 발생한 견갑하 건의 단독 파열에 대해 관절경하 봉합술 후 기능적 결과 및 구조적 연속성에 대해 보고하고자 한다. 대상 및 방법: 2003년 2월부터 2008년 10월까지 외상 후에 발생한 편측 견갑하건 파열로 관절경하 봉합술을 시행한 15예(남자 13예, 여자 2예, 평균 연령 46.2세, 범위: 35~52세)를 후향적으로 분석하였다. 전 예에서 견갑하건 이외의 다른 회전근 개의 손상의 동반은 없었고 견갑하건의 완전 파열이 9예, 상부 2/3의 파열이 6예였으며 술 후 최소한 2년간 추시 하였다(평균 28개월, 범위: 25~38개월). 술 후 최종 추시 상에서 기능적 평가는 Constant 점수와 미국 견주관절 학회 점수(ASES index)로 판정하였고 자기 공명 영상을 이용하여 봉합부의 연속성을 판정하였다. 결과: 최종 추시 결과 평균 기능적 결과 지수와 근력에 있어서 유의한 호전을 보였으며 Constant 점수는 술 전 41.5점에서 술 후 81.3점으로(P<0.05) ASES 점수는 술 전 46.4점에서 89.6점으로 향상되었다(P<0.05). 13예(87%)에서 술 후 결과에 만족하였으며 완전 파열의 경우가 부분 파열보다 기능적 점수가 높은 경향을 보였다. 술 후 평균 13개월에 촬영한 자기공명영상 검사에서 15예 중 12예(80%)에서 봉합부가 치유된 것으로 판정되었고 봉합부의 연속성이 있는 경우보다 재파열된 예에서 술 후 견관절 기능적 점수가 의미 있게 감소되었다(P<0.05). 결론: 외상성 견갑하건 단독 파열환자에서 관절경하 봉합술을 시행한 결과 동통, 견관절 운동역 및 근력의 유의한 향상 을 보였다. 술 후 봉합부의 연속성은 견관절의 기능과 연관되어 있으며, 비록 완전파열의 경우가 부분파열보다 기능적 점수가 높은 경향을 보였으나 이에 대한 추가적 연구가 필요할 것으로 사료된다.
