Partial Budget Modeling of Economic Losses of Aujeszky's Disease (부분예산분석을 이용한 오제스키병 발생 농가의 경제적 손실 추정)

  • Aujeszky's disease (AD) is a respiratory, infectious viral illness associated with high mortality, especially in neonatal piglets and has frequently been considered an economically important disease in many endemic countries. Although AD is still occurring in a geographically defined region in Korea, little attention has been paid to the economics of AD. In this study, partial budget technique was used to develop a simulation model to measure financial losses following the disease epidemic in a swine operation utilizing stochastic or deterministic parameters from the literatures and the index case herd of AD occurred in 2005, where available and applicable. For the infected case herd with a 12500-pig, the total economic loss for this operation was estimated to be about 199 million Korean won (95% confidence interval [CI] 148,645,000-250,741,000). Given net loss due to death of a pig at sow level was 119,000 won, total loss for the case herd with 1200 sows accounted for 143 million won (95% CI 92,599,000-193,729,000). The net loss of the death of one pig at growing and fattening level resulted in loss of 46,000 won (95% CI 40,000-53,000) and 126,000 won (95% CI 122,000-131,000), respectively. Taking into account for the number of pigs raised in the case herd, total loss amounted to 8 million won (95% CI 7,167,000-9,347,000) and 12 million won (95% CI 11,959,000-12,891,000), for growers and fatteners, respectively, assuming 63% of saved feed intake when a pig dies halfway through the respective period. Under the model's assumptions, suckling pig mortality was the major factors of loss in estimating the economic consequences (approximately 71.8% of the total loss). The high economic losses of a herd infected with AD suggest that the effective and region-specific control measures should be implemented in disease endemic foci.

A Study on the Factors which influenced the Performance of Community Health Practitioners' Function -Around the CHPs in Kyonggi-province Area- (보건진료원 직무수행에 영향을 미치는 요인에 관한 연구 - 경기도 관내 보건진료원을 중심으로 -)

  • This study was done in order to analyze the factors which influenced the performance level of community health practitioners' task. Interview survey was done during the period from August to October, 1986. Interviewee were 166 CHPs among total of 217 CHPs in Kyonggi province area. Multiple stepwise regression and canonical correlation analysis were used to identify major factors influenced to perform community health practitioners' task. The results of this study were summarized as follows: 1. General characteristics of CHPs 1) Personal characteristics The average age of CHPs was 37.8 years and their marital status was $77.6\%$ of married, educational back-ground was $65.3\%$ of junior college graduation. Their job career was $38.6\%$ of between 1-3 years, $33.3\%$ of between 3-5 years, $22.2\%$ of less than 1 years. Most of CHPs$(62.8\%)$ were fully satisfied with their job, $33.3\%$ were moderately, and $3.8\%$ were not satisfied. 2) Working environmental condition Only $31.7%$ of CHPs were satisfied with their working condition of primary health post, $26.6\%$ were not satisfied. Half of CHPs$(52.5\%)$ replied having good cooperation with health center, $10.1\%$ replied bad. Cooperation with health subcenter was good in $32.9\%$, and bad in $21.9%$. Cooperation with private health institutions was good in $34.2\%$, bad in $21.6%$. 2. Performance level of community health practitioners' task Among a total of 52 contents of their functions medical history taking. physical examination, referral of diagnostic laboratory work-up($(86.4\%)$, health assessment of pregnant women$(82.1\%)$, development of health information system$(79.4\%)$, supervision of health workers $(78.4\%)$, follow-up of family planning acceptors$(77.3\%)$, and follow-up of family planning acceptors' side effects$(77.3\%)$ were actively performed. Diagnosis of pregnancy$(62.1\%)$, sampling of drinking water for quality test$(52.5\%)$, making list of equipment' & supplies $(51.5\%)$, evaluation of primary health post activities $(37.6\%)$, organization of village health workers$(32.4\%)$ and management of village health workers $(30.1\%)$ were poorly performed. 3. Stepwise multiple regression analysis of job function The factors which influenced the performance level of community health practitioners' function were age, marital status, educational level, job career, job satisfaction, satisfaction of working environment of primary health post, cooperation of health center, cooperation of health center, cooperation of private health instiutions in orders. These 9 variables were able to explain job function from $25.7\%$ of program planning to $6.7\%$ of management of common disease. 4. Canonical correlation analysis between the performance of function and general characteristics of CHPs. Cooperation of private health institutions was found to be the factor influencing task performance of community organization, management of primary health post, technical supervision of health personnels. Job satisfaction of CHPs was also found to be the factor influencing task performance of family planning, management of common disease and maintenance of health information system.

A Dog Park Planning Model for Promoting Animal Welfare Culture in Regional Community, Korea (지역사회의 동물복지문화 증진을 위한 도그파크 계획 모형)

  • Korea's pet-related society has been fast growing without the base and preparations of socio-cultural animal welfare. For the most part, Korea's current public parks, in particular, have a space structure in which a lot of friction between the general public who are not fond of dogs and dog owners in public places has been happening unavoidably. A lot of countries that are taking a lead role in animal welfare have built dog parte that the general public and dogs can enjoy together. This research starts with the realization that the concept of 'dog park' needs to be introduced in Korea as more and more Koreans own dogs and other pets. This research aims to provide guidance for dog park planning by drawing a dog park planning model and to promote basics for co-existence of dogs and people in a park and set in place animal welfare early and raise the level of awareness. Total 7 types of dog put 56 planning items including dog zone, dog playground, and agility, and a selective matrix by which it is possible to select essential recommendatory optional items by each type of dog park were drawn in this research. Overall there were the highest needs in separated area for dogs in park. A dog park planning model is designed as follows. 1) To select a dog park type that was introduced. 2) To set up facilities applicable in common and planning items desired in advance by each type. 3) To zone dog park areas by the priority order of planning items that were introduced and to position necessary facilities. 4) To plan program that should be were introduced. A dog park was expected to take some positive roles such as providing opportunities to socialize dogs, activating social communities, more effective park management by participating local community members and so on.

Studies on the Similar Effects between Jwahun(坐薰) and Sojucheon(小周天)_Practice (좌훈(坐薰)과 소주천(小周天) 수련의 상관성에 관한 연구)

  • objectives : This research aimed to study the similar effects between Jwahun and Sojucheon_practice, as both Induces to SooSengg-HwaGang(the coordination of water and fire). methods : 141 recipients of the survey was conducted in the Jwahun room of Korean medicine Hospital, which contained to body reaction between the moment of the Jwahun and after the Jwahun. And the result of them compared with the body's response of Sojucheon_practice results : 1. In a survey, 78% of the patients had responses that were sweating. The area sweating a lot in the survey is the face, abdomen, chest and was, in this order, the ratio was high. About feeling of warmth and coolness, survey of those who felt the warmth ratio was 79%, those who did not feel the coolness ratio was 77%, most of the responses were warm bodies. 46% of patients replied that feels warm in the belly. It said the fact that Jwahun directly to the lower body is to warm. 2. Symptoms of congestion of the head on the survey, 78% of investigators did not appeal. Other investigators were responding they were dizzy. Research on pain and itching, but no response was at 84%. Among the responses to the itchiness, itching perineum was 11% of the respondents. The rest was weak. 3. Associated with defecation in the survey, the investigators found that 33% of respondents said fart. Urine is 22% of respondents said. Abdominal reactions, 33% of respondents answered was taking a tone in the abdomen. In response to the whole body reaction, the most relaxing 39% of the respondents said, 22% of respondents replied that the body feel lighter. 4. After Jwahun, belly is warm and cathartic reactions were the most responsive. This said that Jwahun even after, in the lower abdomen lasting warmth as can be seen. Smooth bowel movements disproves the abdominal organs is a good move. It will also be seen in the same context of bowel movements, abdominal response when just doing Jwahun. conclusions : Jwahun activate the meridians and through the perineum stimulation induced imdokmaek(任督脈) SooSengg-HwaGang(水升火降) to enable the training and some Sojucheon can have similar effects.

Korea Academy of Prosthodontics criteria for longevity studies of dental prostheses (보철물 수명 연구를 위한 대한치과보철학회 표준 방안: KAP Criteria)

  • Purpose: The most important factor in longevity studies of dental prostheses is objective and consistent evaluation of the prosthesis. The Korean Academy of Prosthodontics suggested developing a standardized method for longevity studies of dental prostheses. The purpose of this study is to evaluate previously-used criteria and to develop new criteria, in the form of a procedure flowchart and an evaluation sheet. These new criteria may be able to provide a unified standard for future longevity studies of dental prostheses. Materials and methods: A literature review was performed about the evaluation of dental prostheses. Taking into account the strengths and weaknesses of previously used criteria, a novel, intuitive and objective method was developed for assessment of dental prostheses. Then, a pilot survey was performed with the newly developed flowchart and evaluation sheet to determine problems and implement possible improvements. Results: Thirty cases of fixed dental prosthesis (FDP), 25 cases of removable dental prosthesis (RDP), and 13 cases of implant supported prosthesis (ISP) were evaluated. The average life expectancy estimate was 12.82 years for FDP, 5.96 years for RDP, and 4.82 years for ISP with Kaplan-Meier survival analysis. Additionally, possible improvements discovered by the pilot survey were reflected in the flowchart and evaluation sheet. Conclusion: The newly developed KAP criteria, flowchart and evaluation sheet enabled objective and consistent results in trial longevity studies of dental prostheses. It is expected that future studies will not only use the KAP criteria but also further improvement will be made on them.

Predictive Value of IHC4 Score for Pathological Response to Neoadjuvant Chemotherapy in Hormone Receptor-Positive Breast Cancer

  • Purpose: This study aimed to explore the value of IHC4 in predicting pathological response after neoadjuvant chemotherapy in patients with hormonal receptor (HR)-positive breast cancer (BC). Materials and Methods: In this retrospective exploratory study, data for 68 HR-positive BC patients who received neoadjuvant chemotherapy were recorded. IHC4 scores were calculated based on estrogen receptors/progesterone receptors, Ki-67 and HER2 status. Logistic and ordinal regression analyses in addition to likelihood ratio test were used to explore associations of IHC4 scores and other clinico-pathological parameters with pathological complete response (pCR) and pathological stage. Results: Taking the 25th percentile as the cut-off, a lower IHC4 score was associated with an increased probability of pCR (low; 52.9% vs. High; 21.6%, OR=4.1, 95% CI=1.28-13.16, p=0.018) and a lower pathological stage (OR=3.9, 95% CI=1.34-11.33, p=0.012). When the IHC4 score was treated as a continuous variable, a lower score was again associated with an increased probability of pCR (OR=1.010, 95% CI=1.001-1.018, p=0.025) and lower pathological stage (OR=1.009, 95% CI=1.002-1.017, P=0.008). Lower clinical stage was associated with a better pCR rate that was of borderline significance (P=0.056). When clinical stage and IHC4 score were incorporated together in a logistic model, the likelihood ratio test gave a P-value of 0.004 after removal of the IHC4 score and 0.011 after removal of the stage, indicating a more significant predictive value of the IHC4 score for pCR. Conclusions: This study suggests that the IHC4 score can predict pathological response to neoadjuvant chemotherapy in HR-positive BC patients. This finding now needs to be validated in a larger cohort of patients.

Automatic Meniscus Segmentation from Knee MR Images using Multi-atlas-based Locally-weighted Voting and Patch-based Edge Feature Classification (무릎 MR 영상에서 다중 아틀라스 기반 지역적 가중 투표 및 패치 기반 윤곽선 특징 분류를 통한 반월상 연골 자동 분할)

  • In this paper, we propose an automatic segmentation method of meniscus in knee MR images by automatic meniscus localization, multi-atlas-based locally-weighted voting, and patch-based edge feature classification. First, after segmenting the bone and knee articular cartilage, the volume of interest of the meniscus is automatically localized. Second, the meniscus is segmented by multi-atlas-based locally-weighted voting taking into account the weights of shape and intensity distribution in the volume of interest of the meniscus. Finally, to remove leakage to the collateral ligaments with similar intensity, meniscus is refined using patch-based edge feature classification considering shape and distance weights. Dice similarity coefficient between proposed method and manual segmentation were 80.13% of medial meniscus and 80.81 % for lateral meniscus, and showed better results of 7.25% for medial meniscus and 1.31% for lateral meniscus compared to the multi-atlas-based locally-weighted voting.

Association between Scrub Typhus Outbreaks and Meteorological Factors in Jeollabuk-do Province (전북지역 쯔쯔가무시증 발생과 기후요소의 상호 관련성)

  • Objectives: Scrub typhus is one of the most prevalent vector-borne diseases. It is caused by Orientia tsutsugamushi, which is transmitted when people are bitten by infected chigger mites. This study aims at quantifying the association between the incidence of scrub typhus and meteorological factors in Jeollabuk-do Province over the period 2001-2015. Methods: Reported cases of scrub typhus were collected from the website of the Disease Web Statistical System supported by the Korea Centers for Disease Control and Prevention (KCDC). Simultaneous meteorological data, including temperature, rainfall, relative humidity, and sunshine duration were collected from the website of the National Climate Data Service System by the Korea Meteorological Administration. Correlation and regression analyses were applied to identify the association between the incidence of scrub typhus and meteorological factors. Results: The general epidemiological characteristics of scrub typhus in Jeollabuk-do Province were similar to those nationwide for sex, age, and geographical distribution. However, the annual incidence rate (i.e., cases per 100,000) of scrub typhus in Jeollabuk-do Province was approximately four times higher than all Korea's 0.9. The number of total cases was the highest proportion at 13.3% in Jeonbuk compared to other regions in Korea. The results of correlation analysis showed that there were significant correlations between annual cases of scrub typhus and monthly data for meteorological factors such as temperature and relative humidity in late spring and summer, especially in the case of temperature in May and June. The results of regression analysis showed that determining factors in the regression equation explaining the incidence of scrub typhus reached 46.2% and 43.5% in May and June. Using the regression equation, each 1oC rise in the monthly mean temperature in May or June may lead to an increase of 38 patients with scrub typhus compared to the annual mean of incidence cases in Jeollabuk-do Province. Conclusion: The result of our novel attempts provided rational evidence that meteorological factors are associated with the occurrence of scrub typhus in Jeollabuk-do. It should therefore be necessary to observe the trends and predict patterns of scrub typhus transmission in relation to global-scale climate change. Also, action is urgently needed in all areas, especially critical regions, toward taking steps to come up with preventive measures against scrub typhus transmission.

Novel DOX-MTX Nanoparticles Improve Oral SCC Clinical Outcome by Down Regulation of Lymph Dissemination Factor VEGF-C Expression in vivo: Oral and IV Modalities

  • Background: Oral squamous cell carcinoma (OSCC) remains as one of the most difficult malignancies to control because of its high propensity for local invasion and cervical lymph node dissemination. The aim of present study was to evaluate the efficacy of novel pH and temperature sensitive doxorubicin-methotrexate-loaded nanoparticles (DOX-MTX NP) in terms of their potential to change the VEGF-C expression profile in a rat OSCC model. Materials and Methods: 120 male rats were divided into 8 groups of 15 animals administrated with 4-nitroquinoline-1-oxide to induce OSCCs. Newly formulated doxorubicin-methotrexate-loaded nanoparticles (DOX-MTX NP) and free doxorubicin were IV and orally administered. Results: Results indicated that both oral and IV forms of DOX-MTX-nanoparticle complexes caused significant decrease in the mRNA level of VEGF-C compared to untreated cancerous rats (p<0.05). Surprisingly, the VEGF-C mRNA was not affected by free DOX in both IV and oral modalities (p>0.05). Furthermore, in DOX-MTX NP treated group, less tumors characterized with advanced stage and VEGF-C mRNA level paralleled with improved clinical outcome (p<0.05). In addition, compared to untreated healthy rats, the VEGF-C expression was not affected in healthy groups that were treated with IV and oral dosages of nanodrug (p>0.05). Conclusions: VEGF-C is one of the main prognosticators for lymph node metastasis in OSCC. Down-regulation of this lymph-angiogenesis promoting factor is a new feature acquired in group treated with dual action DOX-MTX-NPs. Beside the synergic apoptotic properties of concomitant use of DOX and MTX on OSCC, DOX-MTX NPs possessed anti-angiogenesis properties which was related to the improved clinical outcome in treated rats. Taking together, we conclude that our multifunctional doxorubicin-methotrexate complex exerts specific potent apoptotic and anti-angiogenesis properties that could ameliorate the clinical outcome presumably via down-regulating dissemination factor-VEGF-C expression in a rat OSCC model.

Effects of a Song Psychotherapy Intervention on the Change Readiness of Alcoholics (알코올 중독의 변화단계를 반영한 노래심리치료가 알코올 중독자의 변화 준비도에 미치는 효과)

  • This study investigated whether a song psychotherapy based on the change stages of alcohol addiction impacted the change readiness of alcoholics. The song psychotherapy intervention consists of six activities (listening to a song, singing, songwriting, song sharing, lyrics analysis, and discussion and analysis of a song), which considers the change stages of alcohol addiction. A total of 64 inpatients diagnosed with alcoholism were randomly assigned to either the control (n = 33) or experimental group (n = 31). The experimental group took part in the intervention program for 45 minutes twice a week for 6 weeks (total of 12 sessions). The intervention effect was evaluated with SOCRATES-K. The results showed that the SOCRATES-K score significantly increased by 14.6% (p < .001) for the experimental group, whereas it remained almost unchanged (0.8% increase, p = .141) in the control group. For subcategory scales, the experimental group showed the highest increase in the ambivalence factor, followed by the recognition and taking-steps factors. Meanwhile, no significant changes in all three scales were found in the control group. Therefore, the song psychotherapy intervention based on the change stages of alcohol addiction was found to significantly increase the change readiness of alcoholics.
