The Study on the Energy and Fluid Metabolism and the Pathology and Symptomatology of Taeeumin based on The Discourse on Viscera and Bowels of Donguisusebowon (『동의수세보원(東醫壽世保元)』 「장부론(臟腑論)」에 근거한 기액대사(氣液代謝)와 태음인(太陰人) 병리병증(病理病證) 고찰(考察))

  • Objectives : We aimed to analyze the meanings of the energy and fluid metabolism in the Discourse on Viscera and Bowels of Donguisusebowon, and to find the clues for the explanation of the pathology and symptomatology of Taeeumin. Methods : The Discourse on Viscera and Bowels of Donguisusebowon was reviewed and examined for relevant information on the energy and fluid metabolism from the structural and the functional point of view respectively. And, based on the derived meanings of the energy and fluid metabolism, the pathology and symptomatology of Taeeumin were analyzed. Results and Conclusions : 1. The meanings of the energy and fluid metabolism can be explained by the different attributes of the energy and fluid produced from the esophagus and the small intestine, and the different function of exhaling-dispersing and inhaling-concentrating in the different tract of circulation such as Lung affiliation (esophagus, skin, ear and lung) and Liver affiliation (small intestine, flesh, nose and liver). 2. The Exterior disease of Taeeumin starts with the weakness of exhaling-dispersing function at the skin, and leads to the dysfunction of the esophagus and the lung sequently. The dysfunction of the lung aggravates that of the skin and the esophagus. 3. The Interior disease of Taeeumin begins with excess of the inhaling-concentrating functions at the flesh and the small intestine, and leads to the dysfunction of the lung, which induces the dysfunction of exhaling-concentration at the skin and esophagus. And, this disparities between exhaling-dispersing and inhaling-concentrating functions exasperate the problem at the flesh and the small intestine.

The Study on the Development of Diagnosis Algorithm of Taeeumin Symptomology (태음인(太陰人) 병증(病證) 진단 알고리즘 개발 연구)

  • Objectives : This study is aimed to develop the algorithm to diagnose Taeeumin's symptomology, by the method of literature research on Sasang Constitutional Medicine. Methods : Applying the sequential differentiations of Taeeumin's symptomology, or exterior-interior disease differentiation, favorable-unfavorable pattern differentiation, and mild-severe-dangerous-urgent pattern differentiation, "Donguisusebowon" and related literatures have been reviewed. Results and Conclusions : 1) 1st step: Taeeumin's exterior pattern and interior pattern are differentiated by the indexes of whole-body cold or heat pattern, sweating, and facial complexion. 2) 2nd step: The favorable pattern of the Taeeumin's exterior disease can be detected by indexes of the existence of fever, generalized pain while the unfavorable one by indexes of the abnormal condition of digestion and feces, and fearful throbbing. The favorable pattern of the Taeeumin's interior disease can be diagnosed based on indexes of eye pain, dry nose, dry throat, and heat symptoms that occur in various parts of the body, while the unfavorable one by indexes of thirsty, urination, feces and specific symptoms which can be induced by dryness. And in the both unfavorable patterns the dark complexion on the faces is revealed. 3) 3rd step: The mild-severe patterns of the favorably exterior disease are differentiated in terms of the condition of fever, while the mild-severe patterns of the favorably interior disease are in differentiated based on whether abnormal symptoms are revealed in the gastrointestinal tract. Both of the unfavorably dangerous-urgent patterns in exterior and interior diseases are differentiated by the symptoms such as tinnitus, dim vision, weakness of legs and back pain, and lack of strength in legs and thighs.

  • 1. Backgrounds: There are a lack of comprehensive study on Taeeumin Exterior Heat syndrome diarrhea(表熱證泄瀉). Thus, The author examines the content of various literatures related to Tae-eumin Exterior Heat syndrome diarrhea. 2. Methods: Literature examination is used Lee, Je-ma's writings and Sasang Constitutional Medical doctor's writings. Syndrome examination is used Tae-eumin's study on Syndrome of DonguisusebowonGaboGu-bon("咸山沙村東醫壽世保元甲午舊本") and Donguisusebowon("東醫壽世保元") and treatises of the academic world. 3. Results: and Conclusions: 1) "Profuse sweating and reddish and difficult urination"(汗多而小便赤澁) is regard as Taeeumin Interior Febrile Disease(裏熱病). 2) There is no possibility of a misprint about Taeeumin Exterior Heat syndrome diarrhea. Taeeumin Exterior Heat syndrome diarrhea is engaged in both the Exterior and Interior(表裏俱病). 3) It is difficult to prescribe GalgeunHaegi-Tang(葛根解肌湯) instead of Galgeunnabokja-Tang(葛根蘿葍子湯). 4) Tae-eumin Exterior Heat syndrome diarrhea is formed in the early period and absorbed into Donguisusebowon ("東醫壽世保元") in the latter period. "Profuse sweating and reddish and difficult urination" is estimated to be added in the latter period.

A Case Study of a Taeeumin Patient with Advanced Parkinson's Disease Having Orthostatic Hypotension (기립성 저혈압이 있는 진행된 파킨슨병 태음인 환자 치험 1례)

  • Objectives This case study is about a Taeeumin patient with advanced Parkinson's disease identified as Dry-heat (Joyeol) pattern. In this study, we report significant improvement of non-motor and motor symptoms after treatment with Cheongsimyeonja-tang.Methods The patient was identified as Taeeumin Dry-heat (Joyeol) pattern and treated with Cheongsimyeonja-tang and acupuncture. The Unified Parkinson Disease Rating Scale (UPDRS) was used to assess the overall functions of the patient. And the global assessment scale (GAS) was used to assess the improvements of dizziness, bradykinesia and tremor after the treatment.Result and Conclusion The UPDRS total score decreased from 138 points to 86 points after 5 weeks treatment. And symptoms of dizziness caused by hypotension, bradykinesia, and tremor showed significant improvement in GAS after the treatment. Furthermore, sleep disturbance and constipation were reported to be improved after the treatment. In conclusion, this study shows that Sasang constitutional medicine can be effective treatment for Taeeumin patient with advanced Parkinson's disease having orthostatic hypotension.

A Case Study of a Taeeumin Patient with Paresthesia and Weakness in the Limbs of Unknown Cause Diagnosed as Dry-Heat (Joyeol) Pattern (태음인 조열병증(燥熱病證)으로 진단한 원인불명의 팔다리 저림 및 위약감 환자 치험 1례)

  • Objectives The aim of this study was to report significant improvement of paresthesia and weakness in the limbs of unknown cause after treatment with Mangeummoonmu-tang and Cheongeummoonmu-tang in a Taeeumin patient. Methods The patient was diagnosed as Taeeumin Dry-Heat (Joyeol) pattern and treated with Mangeummoonmu-tang, Cheongeummoonmu-tang and acupuncture. The patient's subjective symptoms of paresthesia and weakness were observed using Global Assessment Scale (GAS) during the treatment period. Results and Conclusion The symptoms of paresthesia and weakness decreased from GAS 100 to GAS 0 for two weeks. Furthermore, the patient's symptoms of fatigue and xeroderma were reported to be improved after treatment. In conclusion, this study shows that Sasang constitutional medicine can be effective treatment for Taeeumin patient with paresthesia and weakness in the limbs of unknown cause.

A Case Study of a Taeeumin Patient with Meniere's Disease Treated with Cheongsimyeonja-tang (청심연자탕으로 호전된 메니에르병 환자 치험 1례)

  • Objectives The aim of this study was to report significant improvement of vertigo, tinnitus and aural fullness after treatment with Cheongsimyeonja-tang in a Taeeumin patient with Meniere's disease. Methods The patient was diagnosed with Taeeumin Dry-heat symptomatic pattern and treated with Cheongsimyeonja-tang and acupuncture. The patient's subjective symptoms of vertigo, tinnitus and aural fullness were observed using Global Assessment Scale (GAS) during the treatment period. Results & Conclusions The symptoms of vertigo, tinnitus and aural fullness decreased from GAS 100 to GAS 0 for seven weeks. Furthermore, the patient's symptoms of insomnia, urinary frequency, chest discomfort and fatigue were reported to be improved after treatment. In conclusion, this study shows that Sasang Constitutional Medicine can be effective treatment for Taeeumin patient with Meniere's disease diagnosed as Dry-heat symptomatic pattern.

  • Objectives This case study was about a Taeyangin patient with transient ischemic attack(TIA) due to vertebrobasilar insufficiency(VBI). In this study, we report the progress of mistreat with Taeyangin as Taeeumin and the improvement of symptoms after rediagnosis as Taeyangin. Methods Patient's chief complain was weakness of lower limbs, dysarthria and dizziness. Patient first time took Taeeumin prescription such as Chunghyulganggi-tang, Chunsimyonja-tang and Sanyakbopaewon-tang. After occuring side effects we changed the prescription to Taeyangin herbal medicine such as Ogapijangchuk-tang gagam and Mihudeungsikjang-tang gagam. Result and Conclusion Patient suffered from diarrhea by taking Taeeumin prescription and symptom was continue. After changing of prescription to Taeyangin herbal medicine, not only chief symptom but also original symptom was improved.

BMI Characteristics and Food Preference of Taeeumin and Soeumin According to Sasang Constitution Analysis Method (체질분석 방법에 따른 BMI 특성 및 태음인과 소음인의 음식선호도)

  • n this study, the BMI characteristics and food preference were examined according to the Sasang constitution typology. The constitution type of the subjects was judged by SCAT2 (SC) and Sasang specialists (SP), and the data were compared with the group (SS) in which the two results coincided. The results of SC and SP were consistent with 55 (38.2%) out of 144 subjects. Among the 55 subjects, there were 36 (65.5%), 15 (27.2%), and 4 (7.3%) Soeumin, Taeeumin, and Soyangin, respectively. The BMI of Taeeumin was significantly higher than that of Soeumin (p< .001) in all analytical methods. On the other hand, there was a difference in determining the body shape of Soyangin between the SCAT2 and specialists. The Taeeum-Soeum Food Preference Index was applied to compare the food preference to 41 types of food. In SS analysis, 13 kinds of foods preferred by Taeeumin or Soeumin were found, of which 8 (19.5%) were consistent with the existing food data. Taeeumin preferred 6 kinds of food, such as cold soybean-soup noodles, wild sesame seaweed soup, pan-fried tofu, Yeongun-jorim, Doraji-namul, and soy milk. In contrast, the favorite foods of Soeumin were black rice and Dak-galbi.

The Comparison and Analysis on Prescription of Taeeumin's Mongsul Disease between the Gabo Edition and the Sinchuk Edition of Donguisusebowon (『동의수세보원 갑오구본』과 『동의수세보원 신축본』의 태음인 몽설병 치방 비교·분석 연구)

  • Objective This study was aimed to research how the recognition of Taeeumin's Mongsul disease was changed from the Gabo edition to the Sinchuk edition of Donguisusebowon, and to present reasonable explanation to the prescription suggested in the Sinchuk edition. Method The original text about Taeeumin Mongsul disease and corresponding prescription in Donguisusebowon Sinchuk edition written in 1901 and Donguisusebowon Gabo edition written in 1894 were compared and analyzed. Furthermore, the literature and articles which are relevant to the prescription and the difference of pathology between the Gabo edition and the Sinchuk edition of Donguisusebowon were searched. Results & Conclusion Sinchuk editon compared to Gabo edition differentiates prescriptions according to the condition of stool, which means that Mongsul disease can occur both in Lung-dryness of Esophagus-Cold pathology and that of Liver-Heat pathology. The prescriptions presented in Sinchuk edition are more specified and in agreement with pathology newly proposed in Sinchuk edition than the prescriptions in Gabo edition. Although Cheongsimyeonja-tang is not mentioned on the original text about Taeeumin Mongsul disease in Sinchuk edition, it can be prescribed for the disease with Lung-dryness of Liver-Heat pathology as Yuldahanso-tang can be. However, Yuldahanso-tang focuses more on Liver-Heat symptoms, while Cheongsimyeonja-tang focuses more on Lung-dryness.

A Case Study of Taeeumin Patient with Diabetes Treated with Jowiseungchungtang-Gamibang (태음인 당뇨 환자의 조위승청탕가미방 치험 1례)

  • Objectives The purpose of this study was to report the clinical effect on a 54-year-old diabetes patient, categorized into taeeumin exterior disease, after taking herbal medicine. Methods The patient was diagnosed as taeeumin's esophagus-cold pattern and type 2 diabetes symptoms in taeeumin patient were indigestion, fatigue, and headache. We assessed the changes of fasting blood sugar(FBS), hemoglobin A1c(HbA1c), and total cholesterol. Results and Conclusions FBS, HbA1c, and total cholesterol reduced after treated by Jowiseungchungtang-gamibang three times per day for 5 months. These results show that treating a diabetic patient, accompanied by esophagus-cold symptoms, with Jowiseungchungtang-gamibang particularly improves high blood glucose after orthopedic surgery in practicing healthy eating habits.
