측두하악장애증상 및 기여요인과 성격유형과의 관련성을 규명하고자 경기도 지역 대학에 재학중인 학생 199명(평균연령 $21.0{\pm}2.9$세, 남자 73명, 여자 126명)을 대상으로 MBTI 검사와 설문조사를 실시하여 다음과 같은 결과를 얻었다. 1. 내향성(I)이 외향성(E)보다, 감각형(S)이 직관형(N)보다, 사고형(T)이 감정형(F)보다, 인식형(P)이 판단형(J)보다 한 가지 이상의 주관적 측두하악장애증상을 가지고 있는 자의 비율 및 한가지 이상의 기여요인을 갖는 자의 기여요인 수 평균치가 높은 경향을 보였다. 2. ISTP와 ISFP는 다른 성격유형들에 비해 한 가지 이상의 주관적 측두하악장애증상을 가지고 있는 사람의 비율과 한 가지 이상의 기여요인을 갖는 사람의 비율에서 높은 경향을 보였다. 3. 내향성(I)이 외향성(E)보다, 감각형(S)이 직관형(N)보다, 감정형(F)이 사고형(T)보다, 판단형(J)이 인식형(P)보다 개구시 악관절부 동통 증상의 빈도가 높은 경향을 보였다. 4. 이악물기 습관과 스트레스는 내향성(I)이 외향성(E)보다 높은 빈도를 보였고(p<0.05), 껌 씹는 습관은 외향성(E)이 내향성 (I)보다 높은 빈도를 보였다(p<0.05). 5. 편측저작습관은 판단형(J)이 인식형(P)보다 높은 빈도를 보였다(p<0.05). 6. 성격이 느긋한 편이거나 보통인 경우보다는 예민한 편이거나 신경질적인 경우가 측두하악장애 증상수의 평균치가 높게 나타났다(p<0.001). 7. 보통 성격이 다른 성격들에 비해 기여요인 수의 평균치가 가장 낮게 나타났다(p<0.0001). 결론적으로 측두하악장애는 개인의 성격과 관계가 있으므로 환자의 성격 유형을 고려한 상담과 교육의 활용이 측두하악 장애 치료에 도움이 되리라 사료된다.
본 연구는 측두하악장애와 연관된 두통의 유병율을 평가하고 측두하악장애와 연관된 두통의 양상을 평가함으로써 두통과 측두하악장애와의 관계를 평가하기 위하여 시행되었다. 2008년 1월부터 2008년 6월까지 측두하악장애 관련 증상을 주소로 조선대학교 치과병원 구강내과에 내원한 환자 중 일차성 두통을 가지고 있는 환자 66명을 대상으로 하여, 측두하악장애, 두통에 관한 평가를 시행하였다. 연구결과는 다음과 같다. 1. 측두하악장애을 기준으로 분류하였을 경우, 측두하악관절 및 저작근이 동시에 이환된 경우가 66명 중 36명(54.5%)로 가장 많았다. 2. 두통을 기준으로 분류하였을 경우, 긴장성 두통이 66명중 59명(89.39%)로 가장 많았다. 3. 66명 중 36명(54.5%)이 측두하악장애와 연관된 두통을 호소하였다. 4. 측두하악장애와 연관된 두통을 호소한 36명 중 19명(52.78%)이 본래 존재한 일차성 두통과는 다른 양상이 측두하악장애와 연관되어 나타난다고 하였다. 즉 측두하악장애와 일차성 두통을 동시에 가지고 있는 경우, 측두하악장애와 연관된 두통이 상대적으로 흔하게 발생된다고 볼 수 있을 것이다. 또한 그 중 과반수 이상이 기존 두통양상과는 다른 증상을 호소하는 것으로 보아, 측두하악장애와 두통의 연관성을 연구하는데 있어 새로운 두통양상에 대한 연구가 추가적으로 이루어져야 할 것이다. 또한 측두하악장애와 두통과의 연관성을 평가하기 위해서는 보다 많은 모집단을 대상으로, 보다 세분화된 기준을 가지고 대조군을 설정하여 평가하는 것이 추후에 필요할 것으로 사료된다.
To compare the effect of hyaluronate and dexamethasone on the temporomandibular joint arthrocentesis the author investigated 22 temporomandibular joint disorder(TMD) patients with pain and limitation of mouth opening who visited at the Department of Oral and Maxillofacial Surgery. Chosun Dental Hospital and were made a diagnosis as Wilkes stage III or IV of TMJ internal derangement clinically and radiographically. The two groups consisted of 10 patients with injection of sodium hyaluronate 10mg$(Artz^{(R)})$(hyaluronate group) on the upper joint space of the affected temporomandibular joint 5 times at intervals of a week after arthrocentesis, and 12 patients with injection of dexamethasone$(Oradexon^{(R)})$ at a time(dexamethasone group). Maximum mouth opening, pain value and satisfaction value during mastication were assessed on a visual analog scale before arthrocentesis and after 6 months. Then the within-group and between-group differences were evaluated in the obtained data and the clinical success rate of each group was calculated according to our success criteria. The results were as follows. 1. the mean of maximum mouth opening before arthrocentesis and after 6 months in the hyaluronate group were 24.9mm and 39.0mm respectively, and those before arthrocentesis and after 6 months in the dexamethasone group were 25.7mm and 41.3mm respectively. 2. The mean of pain value on a visual analog scale in the hyaluronate group before arthrocentesis and after 6 months were 6.7 and 1.8 respectively, and those in the dexamethasone group before arthrocentesis and after 6 months were 7.0 and 1.8 respectively. 3. The mean of satisfaction value during mastication on a visual analog scale in the hyaluronate group before arthrocentesis and after 6 months were 2.8 and 7.7 respectively, and those in the dexamethasone group before arthrocentesis and after 6 months were 3.1 and 7.8 respectively. 4. There were statistically significant differences between all measurements before arthrocentesis and after 6 months(P<0.001), but no difference between all measurements in the hyaluronate group and those in the dexamethasone group. 5. The over all success rate of the hyaluronate group and the dexamethasone group were 60.0% and 63.6% respectively. In summary, there was significant difference between the effect of hyaluronate and dexamethasone on the temporomandibular joint arthrocentesis but hyaluronate is better than corticosteroid as the injection drug in consideration of the side effect related with repeated injection.
이 논문의 목적은 류마티스성 관절염 환자에게서 나타나는 측두하악관절장애에 대하여 임상적 및 방사선학적 특징을 평가하는 것이다. 경북대학교병원 류마치스내과에서 류마치스성 관절염을 가지고 있는 것으로 판명된 환자 중에 측두하악관절 기능장애의 증상을 나타내는 환자 15명을 실험군으로 하고, 경북대학교병원 구강내과에서 측두하악관절장애로 진단된 환자 20명을 대조군으로 하였다. 류마티스성 관절염 환자에서 근막동통, 아침경직감, 측두하악관절 운동범위의 감소, 염발음의 발생빈도가 각각 86.7%, 73.3%, 80.0%, 73.3%이었으나, 대조군에서는 각각 55.0%, 15.0%, 40.0%, 30.0%로 나타나 류마티스성 관절염 환자에서 측두하악관절장애의 증상의 발생 빈도가 훨씬 높게 나타났다. 방사선학적 특징 중에 두 군 간에 발생빈도에 있어서 유의한 차이를 나타내는 소견은 과두의 평탄화와 경화, 변연골증식, 관절와의 침식 등이었으며 모두 류마티스성 관절염 환자에게서 많이 나타났다. 또한 MRI 사진에 의해서 확인된 바에 의하면, 관절원판의 변위와 관절원판의 파괴, 피질골의 침식이나 증식 등의 소견이 류마티스성 관절염 환자에게서 유의하게 높은 빈도로 나타났다. 결론적으로 류마티스성 관절염 환자에서 측두하악장애의 발생빈도가 매우 높고 관절의 구조적 변화도 일반적인 측두하악장애 환자에 비해 더 심하게 나타남을 알 수 있었다.
2006년 한 해 동안 경북대학교 구강내과에 측두하악장애를 주소로 내원한 전체 환자 중에 파노라마 방사선 사진 상에서 인정될만한 하악과두흡수가 관찰된 환자를 대상으로 측모두부방사선 사진을 촬영하였으며, 측두하악장애의 임상적 평가와 악안면 골격형태를 분석하여 다음과 같은 결론을 얻었다. 1. 임상적 평가 1) 측두하악장애를 주소로 내원한 총 환자 수는 2419명 이었고 그중 224명 약 9.3%가 과두흡수를 나타내었다. 과두흡수가 있는 환자군 중 여자는 183명(81.7%)이고 남자는 41명(18.3%)으로 여자에서 월등히 많이 유발되었다. 환자의 연령은 12세에서 70세에 걸쳐 분포되었으며 평균 연령은 30.6세였다. 연령분포는 10대가 26.3%(59명), 20대가 34.8%(78명), 30대가 13.8%(31명), 40대가 11.2%(25명), 50대가 7.1%(16명), 60대가 6.3%(14명), 그리고 70대가 0.4%(1명)로 나타났다. 2) 과두흡수 부위의 통증을 호소한 경우는 145명, 통증을 호소하지 않은 경우가 79명이었다. 3) 과두흡수 환자 중 이갈이 습관을 53명, 이깨물기 습관은 80명이 호소하였으며, 다른 기타 습관들을 보고한 경우가 204명이었다. 4) 과두흡수 부위의 통증을 호소한 환자의 치료기간은 1개월 미만이 81명으로 전체적으로 치료기간이 짧게 나타났다. 2. 악안면 골격 형태 평가 전체 과두흡수 환자 224명중 18세 이상의 186명(여자 155명, 남자 31명)만이 한국 성인 정상교합자의 측모두부방사선사진 계측연구 결과와 비교하였다. 1) SNA, SNB는 남녀 모두에서 유의한 작은 값을 나타냈고, ANB항목에서는 남자는 평균 3.57 여자는 5.05으로 여자 환자에서 큰 값을 보여 남자에 비해 하악이 더 후퇴되어 있음을 알 수 있다. 2) 남녀 모두 SN-GoMe, FMA의 각도가 과두흡수 환자에서 유의하게 상당히 커져 있음을 보였다. FMA는 남자는 평균 30.44, 여자는 31.69로 여자에서 수직적 성장이 더 크게 나타났다. 3) 총후안면고경(posterior facial height)이 과두흡수 군에서 모두 작은 것이 특징으로 보였으며, 총전안면고경(Anterior facial height)은 과두흡수 환자군에서 남녀 모두 정상군에 비해 차이가 없었다. 저자의 연구에서 얻어진 결과에 의해 하악과두흡수는 정형외과적으로 퇴행성관절염과는 달리 젊은 여성에서 유병률이 높았으며 이는 남성보다 여성에서 수직적 골격 성장 양상으로 인해 하악과두 전방부의 압축응력을 증가시키는 위험요인을 가진 환자에서 발현이 높은 것으로 생각되며 그리고 노년층보다는 젊은 사람에서 이상기능활동으로 인한 부하의 증가 때문이라 생각한다. 하악과두흡수를 보이는 환자들의 치료기간이 대체적으로 짧으며, 나이가 증가함에 따라 내원하는 환자수가 증가하지 않는다는 것은 이 질환이 자기한정적임을 알 수 있고, 생리적 내성 범위 이상의 부하가 가해져 발생한 하악과두의 재형성 과정으로 생각한다.

The purpose of this study was the long-term clinical and radiological evaluation of conservatively treated condylar fractures in children. This study was therefore undertaken to analyze the long-term effects of treating condylar fractures in children with conservative therapy in order to resolve the controversial question of whether complete remodeling occurs in this age or, if not, whether it is more likely to be associated with certain types of fracture or with other factors. This study was based on a series of 11 consecutive children and adolescents, aged between 3 and 15 years, with fractures of the condylar process who had been treated with conservative therapy at the Department of Oral & Maxillofacial surgery, Inha University Hospital, Inchon, Korea. All patients underwent a clinical investigation with special emphasis on Temporomandibular joint function and facial asymmetry. The patients also underwent a radiologic investigation, focusing on fracture remodeling and symmetry of mandible. It consists of panoramic radiograph, PA & lateral cephalogram and 3-D CT. No patient described impaired temporomandibular joint(TMJ) function or pain on the affected side. 2 of 8 (25%) unilateral and 1 bilateral fractures show slight facial asymmetry. Despite apparently excellent recovery of function, there were marked remodeling changes evident on the CT scan. Such changes are not usually evident on panoramic radiograph. Radiologic investigation showed incomplete remodelling(54.5%) and asymmetry of the mandible (27.3%) in some patients. Conservative treatment of condyle fracture in children results in satisfactory long-term outcome of jaw function despite a relative high frequency of radiologically noted aberrations.

Treatment methods of mandibular condylar fracture were conservative and surgical method. Surgical method of mandibular high condylar fracture was very difficult because approach and internal fixation of small size fracture fragment were difficult. So there is a tendency to select conservative method over surgical method for guiding a stable occlusion and avoiding TMJ disorder and growth disturbance, minimizing pain and deviation during function. But, in case of mandibular high condylar fracture patient who has no biting teeth on posterior teeth area, guiding a stable occlusion and conservative functional treatment were very difficult. In this case, patient was 62years old male. He had fracture of mandibular symphysis, right mandibular body, left mandibular high condyle. We treated the patient for mandibular symphysis and right mandibular body fracture area with surgical method. But left mandibular high condylar fracture area was difficult to treat with surgical method. So we selected a conservative functional method on left mandibular high condylar fracture area. We intended recovery of vertical dimension and stable occlusion with implantation of immediate provisional implant on maxillar and mandibular posterior teeth area, and temporary crown. And then patient did mandibular functional movement and his mandibular function was recoverd.
This study conducted 2 types of occlusal splint therapy to eliminate clicking sound. 15 patients who had clicking on their joints were selected, and divided, at random, into 3 groups. In the first group, 4 persons put on stabilization splint and in the second group, 6 persons put on anterior repositioning splint which had made condyle to protrude 3mm, and in the last group, 5 persons put on anterior repositioning splint which had made condyle to protrude 6mm. Patients who wore anterior repositioning splint were instructed to use the splint for all days. The evaluation of clicking was measured by occlusal soundscope. The clicking was converted to aucostic signal by the attached microphone, instead of vibrating sensor The in-put aucostic signal on the occlusal soundscope made it possible to observe the experiment's result. Anterior repositioning splint was produced in the centric occlusion state, when the model was mounted to articulator and inserted two pieces of 3mm and 6mm resin blocks each into the rear wall of articulator fossa. The observation of the patients who wore three different splints for 3 weeks has reached the following conclusions: 1. Stabilization splint produced no effect in eliminating the clicking sound. 2. Anterior repositioning splint therapy with 3mm condylar protrusion produced significant improvement in eliminating the clicking sound. 3. The 6mm protruded anterior repositioning splint caused pain on affected TMJ area as well as the clicking on unaffected joint.
Ultrasound images are noninvasive, can be observed in real time, have no radiation exposure, do not cause pain, and are not restricted in use depending on the patient's prosthetic implant or medical condition. Since the use of ultrasound in the dental field was first applied for tooth preparation in 1957, the use of diagnostic ultrasound for the first time in 1963 has been reported. Currently, it is used in the diagnosis of soft tissue lesions such as malignant tumor or salivary gland disease, fine needle aspiration test, temporomandibular joint disease, lymph node metastasis, measurement of muscle thickness and inflammatory diseases, differentiation of periapical cyst and granulation tissue, measurement of periodontal tissue thickness. The ultrasound image can be visualized in real time. The clinician can explain the structure to the patient while consulting the patient and consult the patient. When injecting the drug into a specific site or aspirating a specific site or substance, So that it can be confirmed and practiced. Recently, ultrasonic equipment specialized in the dental field has been developed and marketed, and it is expected that the use of ultrasonic waves will become active in the dentistry. In the future, development of popular equipment with size and frequency suitable for dental diagnosis and various researches on maxillofacial ultrasonic anatomy. If clinical studies are continuously carried out to demonstrate efficacy, ultrasound is expected to aid in accurate diagnosis and treatment throughout the dentistry.
This is case report of open reduction of condylar fractures with or without discal injury. Many articles described open reduction and internal fixation of condylar fractures emphasize the method of bony reduction and fixation without mention of the position of the disc. So we like to present our cases of open reduction of condylar fractures in conjunction with reconstruction of the disc and associated structures. The pupose of this presentation is to emphasize several well - established principles of trauma management and management of temporomandibular joint injuries, such as 1) in situations of traumatic injury, anatomic restoration is the goal of treatment and, 2) anatomic alignment of the TMJ disc over the condyle is preferable to disc subluxation because the latter may lead to chronic pain, limitation of opening and degenerative arthritis. Although our case is small with short term follow up, we believe that open reduction and internal fixation of condylar fractures in conjunction with disc repair is a biologically sound approach to those fractures indicated for open surgery. Long term follow up will allow better judge the validity of this treatment approach to us.

제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
