부인과 복강경 수술 후 한의진료 방향에 대한 고찰 (Traditional Korean Medicine(TKM) Management for the Recovery after Laparoscopic Gynecological Surgery)

  • Purpose: The purpose of this study is to report the TKM Management for the recovery after laparoscopic gynecological surgery. Methods: The basic informations about laparoscopy and report 3 laparoscopic postoperative patients were managed with TKM. Patients had taken the surgery for the different gynecologic diseases. Results: The 3 patients' symptoms were improved gradually. TKM management is good for the recovery of laparoscopic surgery. And the management need to reflect postoperative complications. the cause and part of operation. and postoperative common symptoms. Conclusion: The TKM managements are effective in the postoperative recovery after laparoscopic gynecological surgery. And more study is needed for developing the model.

일부 한국농촌지역의 8대 한방공공보건사업에 대한 주민의 이용실태 및 개선방향;강원도 홍천군을 중심으로 (Utilization and Improvement of 8 Principle TKM Public Health Services in Rural Areas of Korea)

  • Health promotion program utilizing traditional korean medicine(TKM) is very unique modality uncommonly conducted throughout the world. Korea's TKM public health services went through initial stage from 2001 and spread throughout the country by 2003. 35 public health centers in the nation is appointed as 'TKM health promotion HUB' and in operation from 2007. But the program is still in the early state and evaluation of usage, satisfaction, and community service is still in demand. This study aims to verify current situations of TKM public health service by examining public awareness, usage, and satisfaction and suggest improvements based on findings. 2.5% of local residents were sampled and 1739 were chosen as subjects. Knowledge, attitude, satisfaction and others for TKM public health service were examined. TKM treatment service was most widely utilized(13.2%), followed by CVA prevention program(5.2%). For satisfaction level, CVA prevention program showed highest satisfaction(73.3%), closely followed by (Qi-gong program 63%). In general, awareness and utilization of TKM public health service were low but compensated by high satisfaction level. More active promotion and development of community specific programs should become available in the future.

한의약 공공보건사업에 대한 공중보건한의사들의 인식 (Recognition of Traditional Korean Medical Public Health Program in Public Health Oriental Medical Doctors)

  • Objective : This study aimed qualitative change of traditional korean medical public health programs (TKM-PHP) by the research of present condition, problems and improvement direction about TKM-PHP. Methods : Data were collected from 222 of 996 public health oriental medical doctors(PHOMD) using the structured questionnaire by e-mail. Collected data were analyzed through frequency analysis, T-test, and ANOVA using SPSS 12.0 and significant level was 0.05. Results : 59.5% of the respondents said that the TKM-PHP are not efficiently executed because there are not enough motivations to entice oriental medical doctors in charge of the programs and because PHOMD and the government officials in charge lack in relevant experiences and skills. A majority of the PHOMD recognize a need for activating the TKM-PHP but less actively participate in the programs since there is not a good rewarding system and there are neither standard manuals nor methodological guidelines for the programs. In order to activate the TKM-PHP, it is urgent to employ full-time oriental medical doctors and to continually and systematically appoint the professional manpower in charge. And it is also needed for the state to secure an adequate budget and prepare schemes for persistently train such professionals. Conclusion : Activating the TKM-PHP will lead to the development of TKM, but there are still such problems as lack of the professional manpower in charge, an excessive burden of the duties of PHOMD, lack of program manuals, and a poor system for evaluation. To solve these problems, it is advised to construct foundations for administerial supports, draw up a standard manual, prepare a system for evaluation in consideration of the characteristics of TKM, and hire professionals to ensure sustainable programs.

한의학 교육을 위한 필수한자 추출 및 분석연구 (Study on the prerequisite Chinese characters for the education of traditional Korean medicine)

  • There has been a need for an operational curriculum for teaching Chinese characters used by traditional Korean medicine (TKM), but the it was not thoroughly reviewed so far. We analysed the frequency of unicode Chinese characters with five textbooks of traditional Korean medicine used as a national standard. We found that 氣, 經, 陽, 陰, 不, 熱, 血, 脈, 病, 證, 寒, 中, 心, 痛, 虛, 大, 生, 治, 本, 之 are the 20 most frequently used Chinese characters, and also showed 100 frequently used characters for each textbook. We used a cumulative frequency analysis method to suggest a list of 1,000 prerequisite Chinese characters for the TKM education (TKM 1000). which represents the current usage of Chinese characters in TKM and covers 99% of all textbook use if combined with MEST 1800. This study showed prerequisite and essential Chinese characters for the implementation of evidence-based teaching in TKM. The TKM 1000, a prerequisite characters by this study based on the TKM textbooks can be used for the development of Korean Medicine Education Eligibility Test (KEET), entrance exam to the Colleges of Oriental Medicine or textbooks, and educational curriculum for premed students.

2, 30대 여성을 대상으로 한 대하(帶下)의 한방 치료에 대한 인식 조사 (A Survey on the Perception of the Women in Twenties and Thirties on Traditional Korean Medical Treatment on the Leukorrhea)

  • Objectives: The purpose of this study is to analyze the perception that women in 20s and 30s have on Traditional Korean Medical(TKM) treatment on the leukorrhea. Methods: We studied the perception of TKM treatment by questionnaire From October 1 to November 20, 2010. A questionnaire was given to 50 women and we studied 47 except 3 questionnaire which have inappropriate answer. Results: 1. In 47 women, 46(97.9%) women have heard about leukorrhea, 1(2.1%) woman hasn't heard about it. 2. In 32 women who suffered from the leukorrhea, 14(43.8%) women checked up for leukorrhea. 18(56.2%) women doesn't checked. 10 of 14 women took the medical treatment (drug or injection) and 2 of 14 women took the TKM treatment. 3. In 32 women, 16(50.0%) women know about TKM treatment for leukorrhea, 16(50.0%) women don't know. 4. In 32 women, 20(62.5%) women have intention of TKM treatment for leukorrhea, 2(6.2%) women don't have, and 10(31.3%) women don't know. The biggest obstacle was the high cost of TKM treatment. Conclusions: Many women are willing to take TKM treatment on the leukorrhea. We need more accumulation of data for demonstrating the efficacy and safety of TKM treatment.

부인과 수술 후 한방 조기 회복 치료를 받은 입원 환자에 대한 후향적 분석 (Traditional Korea Medicine for Enhanced Recovery after Surgery (ERAS) in Inpatients after Gynecological Surgery: Retrospective Analysis)

  • Objectives: The purpose of this study is to report the effect and safety of traditional Korean medicine (TKM) for enhanced recovery after surgery (ERAS) after gynecological surgery. Methods: The study is conducted by analyzing the medical records of the hospitalized patients in TKM hospital. Eighty-six patients were enrolled who received TKM for ERAS after gynecological surgery from January 2015 to April 2018. We performed statistical analysis by using SPSS ver. 25.0. To prove the effect and safety of TKM for ERAS, we analyzed symptoms and Hemoglobin (Hb), aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP) and serum creatinine (Cr) with glomerular filtration rate (GFR). In addition, we checked other adverse reactions that the patients complained to prove the safety of TKM for ERAS. Results: Among symptoms of pain, there was a statistically significant decrease in abdominal pain, lower back pain, and shoulder pain. Other symptoms showed statistically significant improvement in fatigue, abdominal tympanosis, operation-site discomfort, dizziness, urinary discomfort, throat discomfort, constipation, dyspepsia and colporrhagia. Anemia in both ferritin combination treatment group and herbal medicine only group showed significant improvement. There was no liver damage or renal damage by TKM. Conclusions: The result has shown that TKM for ERAS after gynecological surgery is effective and safe. The further study of TKM for ERAS with more patients is needed.

Life cycle Health Promotion Programs using Traditional Korean Medicine (HaPPs-TKM) and Activation Plan

  • Background : The Life cycle Health Promotion Programs using Traditional Korean Medicine (the Life cycle HaPPs-TKM) are the on-going 3rd stage projects that have centered on the development and dissemination of the standard life cycle HaPPs-TKM in the local community. The purpose of the study was to introduce the development background of the standard life cycle HaPPs-TKM and to suggest its activation plan. Methods : Academic and government research reports on the life cycle HaPPs-TKM were analyzed to introduce the development process, development backgrounds and the details of KM-HPP for each life cycle, such as infants and toddlers, adolescents, pregnant women, adults and the elderly. Results : We reviewed the development process of the standard life cycle HaPP-TKM consisted of a series of diagnosis on community members' health problems, establishment of project purpose, research on the involvement of KM intervention in a project, and final development of the project model. And we rediscovered that in the development backgrounds of KM-HPP, there were beneficial goals to manage and promote public health conditions for each life cycle. Conclusion : To activate life cycle HaPPs-TKM, we would recommend that activation plan should include six factors through systematic analysis of research reports. These factors consist of diversified goals for each life-cycle, competency reinforcement of local project manager, diversified Korean Medicinal modalities to enhance Sasang Constitution and Qigong, development of standard Outcome Index, periodical holding of performance contest, and improved guidance of government and associated entities through whole process of HaPP-TKM.

A Study on the Development of a Model for Providing Traditional Korean Medicine and Welfare Services for Community Care

  • Objectives: This study aims to develop a community care model in traditional Korean medicine (TKM) by developing a community care participation model for the health of the elderly and deriving tasks to implement it. Methods: This study implemented a group interview with experts. A fact-finding survey was conducted targeting 16 local governments that are implementing a leading project to identify the status of TKM service provision and welfare service linkage in all regions. An expert group interview (FGI) targeted public and private sector experts for each job role, the former represented by those in charge of the central government's health care policy and administrative delivery system, and the latter by professors majoring in social welfare, professors majoring in health, and local TKM societies. After forming the expert groups, three expert group interviews were conducted. Results: Through collective interviews with experts, a model for providing TKM and welfare services in community integrated care was derived by dividing it into local and central government levels. The strategies and tasks for promoting TKM-oriented health welfare services were derived from 3 strategies, 8 tasks, and 20 detailed tasks. Conclusion: The core direction of the TKM health care model is the region-centered provision of TKM and welfare services. To this end, policy support for the use and linkage of health care service resources is required at the central government level, and linkage and provision of health welfare services centered on TKM are necessary through linkage and convergence between service subjects and between government health care projects.

한 대학병원 산부인과 수술 환자의 수술 후 한방 치료에 대한 인식 조사 (Postoperative Traditional Korean Medicine Treatment - A Study on the Perception of Gynecological Surgery Patients on Traditional Korean Medicine Treatment in One University Hospital)

  • Purpose: The purpose of this study is to analyze the perception that gynecological surgery patients have on Traditional Korean Medicine(TKM) treatment. Methods: We studied the perception of TKM treatment by questionnaire from August, 2008 to Febrary, 2009 in Kyung-Hee University East-West Neo medical center. A questionnaire was given to 180 women and 150 women answered. Results: Out of 150 women, 93 women(62%) had been given TKM treatment during the last 3 years, while 57 women(32%) hadn't. 53 women(35.5%) had taken herbal medicine within the last 2 weeks, while 97 women(64.7%) hadn't. 71 women(47.3%) reported that they had been given TKM treatment related to the causative disease for the operation, while 22 women(14.7%) hadn't. And 97 women(64.7%) indicated that they were willing to use TKM as a part of their postoperative medical care, 53 women(35.3%) indicated that they weren't. Conclusion: Many patients are willing to take TKM treatment as postoperative care. We need more accumulation of data for demonstrating the efficacy and safety of TKM treatment.

Effectiveness of Home Visiting Treatment using Traditional Korean Medicine

  • Objectives: The study was investigated to assess efficacy for home visiting treatment using Traditional Korean Medicine (TKM). Methods: The traditional medical doctor of Um-Sung health center has treated the 109 patients and they also answered the questionnaires of home visiting treatment using TKM before and after. The questionnaires were composed of flexibility, pain, health behavior, perception of health, satisfaction, cognition of TKM, etc. The questionnaires were analyzed by use of paired t-test and chi-square test. Results: They have significantly improved the flexibility (p<.001), pain (p<.001), health behavior (p<.001), and the perception (p<.001) of health after home visiting treatment. It was also brought to improve about the perception of TKM, and showed a good satisfaction and the improvement of health. Conclusion: Home visiting treatment using TKM is helpful to improve the health based on these results. We need to systemize the research for the activation of home visiting treatment using TKM.
