인체에서 Pick 원리 및 열희석법에 의한 심박출량 측정의 비교 연구 (Comparison of Cardiac Output Determination by Direct Fick and Thermodilution Method in Man)

  • Measurement of cardiac output by thermodilution method is easy to perform and can be repeated at very short intervals and offers a number of theoretical and practical advantages. The purpose of this paper is to reevaluate the accuracy and reproducibility of the thermodilution cardiac output by comparing the results with those obtained by the direct Fick technique. From January through April 1981 [4 months], 26 simultaneous cardiac output measurements were made using thermodilution and direct Fick methods in 12 patients in the intensive care unit of Severance Hospital following open heart surgery. The average age of the patients was $34.8{\pm}12.0$ [SD] years [range; 17-57 years]. For cardiac output determination by thermodilution technique [$CO_TD$], a Swan-Ganz Thermodilution Catheter, Model 93A-131-7F [Edwards Laboratories], a Cardiac Output Computer, Model 9520A, and Strip Chart Recorder, Model 9810 [Edwards Laboratories] were used. For the determination of cardiac output by direct Fick principle [$CO_Fick$], a Van Slyke Manometric Apparatus [Thomas] for the measurements of arteriovenous oxygen content difference and a Collin`s Respirometer with Kymograph for the measurement of oxygen consumption were used. The correlation between the simultaneously measured $CO_TD$ and $CO_Fick$ was found to be $CO_Fick$ = 0.95.$CO_TD$ + 0.1073, [r= 0.9777, P < 0.001 ]. For 26 double or triple determinations of $CO_TD$, the values ranged from 1.77 L/min to 6.66 L/min, with a reproducibility of 2.18%. The maximum difference of $CO_TD$ for $CO_Fick$ was between +11.99% and -12,99% [$Mean{\pm}SD$ = $6.07{\pm}3.97$%]. Measurement of cardiac output by thermodilution method provides a simple, rapid, reproducible and highly accurate method for multiple cardiac output measurements suitable for use at the bedside. 신규 살조물질인 Thiazolidinedione 유도체 (TD49)의 해양생태계에 대한 급성독성평가 (Acute Toxicity Assessment of New Algicides, Thiazolidinedione Derivative (TD49) to Marine Ecosystem)

  • A thiazolidinedione derivative, TD49 with the highly selective algicide to red tide was newly synthesized and its acute toxicity was examined in order to evaluate the effect on aquatic ecosystems of coast. Major three species having important role in the food chain of marine ecosystem, such as Skeletonema costatum of microalgae, Daphnia magna of crustacea, Paralichthys olivaceus of flatfish fingerling were employed for the acute toxicity assessment. $EC_50$ or $LC_50$ as the assessment criterion was investigated to each specie, and NOEC (No Observed Effect Concentration) and PNEC (Predicted No Effect Concentration) from most sensitive specie to toxicity of TD49 were further calculated. $EC_50$ of S. costatum in 96-hour, $EC_50$ of D. magna in 48-hour, and $LC_50$ of P. olivaceus in 72-hour for TD49 were $0.34\;{\mu}M$, $0.68\;{\mu}M$, and $0.58\;{\mu}M$, respectively. NOEC from the results of S. costatum was estimated to be $0.20\;{\mu}M$ and PNEC was estimated as 3.40 nM by applying factor value of 100 to $EC_50$ $0.34\;{\mu}M$ of S. costatum. In addition, it was revealed that Solutol used as the dispersing agent of TD49 had very little toxic influence under the concentration range of $0{\sim}0.4\;{\mu}M$ used in TD49 toxicity experiment. Although the estimated concentration of TD49 that will be sprayed onto the coastal field for the algicide is higher than NOEC value, it is considered that the spraying concentration would not be a considerable problem due to a dilution effect by tide at the opened coast.

수치지질도의 세계측지계 좌표변환 (ArcToolbox를 이용한 충주 및 황강리 도폭의 사례) (The Coordinate Transformation of Digital Geological Map in accordance with the World Geodetic System (A Case Study of Chungju and Hwanggang-ri Sheets using ArcToolbox))

  • 우리나라에서는 2010년부터 국제적으로 통용되는 세계측지계의 사용을 의무화하였다. 이에 따라 현재 국토지리정보원에서 발행되는 수치지도는 세계측지계 기준으로 제공되고 있다. 그러나 현재도 대부분의 수치지질도는 동경측지계 기준으로 제공되고 있다. 따라서 사용자들은 수치지질도를 이용한 2D/3D 지질공간분석에 있어서 수치지질도의 좌표를 세계측지계로 변환하여 사용할 수밖에 없는 실정이다. 따라서 본 논문에서는 수치지질도의 좌표정의 및 변환에 익숙하지 않은 사용자들을 위해 ArcToolbox를 이용한 동경측지계로의 좌표정의 및 세계측지계로의 좌표변환 방법을 소개하고자 한다. 이때 좌표정의 및 변환의 객관성을 도모하기 위해서 인접한 1:5만 축척의 충주와 황강리의 지질도폭을 예를 들어 사용하였다. 동경측지계 좌표정의에 있어 충주와 황강리 도폭을 각각 동경측지계 중부원점과 동부원점으로 정의하였고, 황강리 도폭을 다시 동경측지계 중부원점으로 변환하여 충주 도폭과 병합하였다. 세계측지계 좌표변환에 있어 병합된 충주 및 황강리 도폭은 다시 세계측지계 중부원점으로 변환하였다. 변환 결과, 병합된 충주와 황강리 도폭은 수치지도(대소 367041) 위치와 정확히 일치하였다. 이와 같은 방법으로 과거에 만들어진 수치지질도의 좌표문제를 효과적으로 해결 할 수 있었다. 따라서 본 논문에서 제시한 좌표정의 및 변환 방법은 향후 다양한 수치주제도들의 2D/3D 지질공간분석시 주제도들의 전처리 과정으로 유용하게 사용될 수 있을 것으로 기대된다.

Effect of different arch widths on the accuracy of three intraoral scanners

  • PURPOSE. The purpose of this study was to compare the accuracy of three intraoral scanner (IOS) systems with three different dental arch widths. MATERIALS AND METHODS. Three dental models with different intermolar widths (small, medium, and large) were attached to metal bars of different lengths (30, 40, and 50 mm). The bars were measured with a coordinate measuring machine and used as references. Three IOSs were compared: TRIOS 3 (TRI), True Definition (TD), and Dental Wings (DW). The relative length and angular deviation of both ends of the metal bars from the scan data set (n = 15) were calculated and analyzed. RESULTS. Comparing among scanners in terms of trueness, the relative length deviation of DW in the small (1.28%) and medium (1.08%) arches were significantly higher than TRI (0.46% and 0.48%) and TD (0.33% and 0.18%). The angular deviation of DW in the small (1.75°) and medium (1.83°) arches were also significantly greater than TRI (0.63° and 0.40°) and TD (0.55° and 0.89°). Comparing within scanner, the large arch of DW showed better accuracy than other arch sizes (P < .05). On the other hand, the larger arch of TD presented a greater tendency of angular deviation in terms of trueness. No significant differences were found in terms of trueness between the arch widths of TRI group. CONCLUSION. The different widths of the dental arches can affect the accuracy of some intraoral scanners in full arch scan.

  • The Bank of Korea raised the benchmark interest rate by a quarter percentage point to 1.75 percent per year, and analysts predict that South Korea's policy rate will reach 2.00 percent by the end of calendar year 2022. Furthermore, because market volatility has been significantly increased by a variety of factors, including rising rates, inflation, and market volatility, many investors have struggled to meet their financial objectives or deliver returns. Banks and financial institutions are attempting to provide Robo-Advisors to manage client portfolios without human intervention in this situation. In this regard, determining the best hyper-parameter combination is becoming increasingly important. This study compares some activation functions of the Deep Deterministic Policy Gradient(DDPG) and Twin-delayed Deep Deterministic Policy Gradient (TD3) Algorithms to choose a sequence of actions that maximizes long-term reward. The DDPG and TD3 outperformed its benchmark index, according to the results. One reason for this is that we need to understand the action probabilities in order to choose an action and receive a reward, which we then compare to the state value to determine an advantage. As interest in machine learning has grown and research into deep reinforcement learning has become more active, finding an optimal hyper-parameter combination for DDPG and TD3 has become increasingly important.

Analysis of C5G7-TD benchmark with a multi-group pin homogenized SP3 code SPHINCS

  • The transient capability of a SP3 based pin-wise core analysis code SPHINCS is developed and verified through the analyses of the C5G7-TD benchmark. Spatial discretization is done by the fine mesh finite difference method (FDM) within the framework of the coarse mesh finite difference (CMFD) formulation. Pin size fine meshes are used in the radial fine mesh kernels. The time derivatives of the odd moments in the time-dependent SP3 equations are neglected. The pin homogenized group constants and Super Homogenization (SPH) factors generated from the 2D single assembly calculations at the unrodded and rodded conditions are used in the transient calculations via proper interpolation involving the approximate flux weighting method for the cases that involve control rod movement. The simplifications and approximations introduced in SPHINCS are assessed and verified by solving all the problems of C5G7-TD and then by comparing with the results of the direct whole core calculation code nTRACER. It is demonstrated that SPHINCS yields accurate solutions in the transient behaviors of core power and reactivity.

지연성 진전 1례 (A Case of Tardive Tremor as A Varient of Classic Tardive Dyskinesia)

  • Tardive dyskinesia(TD), typically appearing as an undesirable side effect of a long term antipsychotic drug treatment has gained increased attention in recent times due to the discovery of many TD variants. This is a single case study of a patient who has undergone more than 8 years of high dosage antipsychotic treatment. After altering the type and dosage of antipsychotic medication 3 months prior to visit, the patient showed relatively abrupt onset symptoms of severe tremor and dystonia. These symptoms, appearing in clear consciousess, got better to a certain degree after 48 hours, worsened for 12 hours, and then improved again. Subsequently there was no continuing movement disorder. Several tests and consultation were carried out. However except for the medication factor, no other possible causes for such disabling symptoms were found. This clinical condition was thought to be akin to tardive tremor, a variant of TD. Furthermore, the course was atypical.

Microwave-assisted Solvent-free Synthesis of Some Dimethine Cyanine Dyes, Spectral Properties and TD-DFT/PCM Calculations

  • A series of dimethine cyanine dyes were synthesized in a fast, efficient and high yield by the condensation of quaternary salts with 1H-indole-3-carbaldehyde in the presence of piperidine under solvent-free microwave irradiation. The products were identified by 1H NMR, IR, UV-Vis spectra and elemental analysis. The absorption and fluorescence properties of these dyes were investigated both experimentally and theoretically. Calculations performed at a combination of time-dependent density functional theory (TD-DFT) and the polarizable continuum model (PCM) reproduced the π-π* type absorption bands of the dyes. Regression analysis was used for studying theoretical results of the absorption maxima in different solvents. Compared with experimental counterparts, estimated overall uncertainties in the absorption maxima were about ±2%.

간질 치료제 복용으로 인한 음성학적인 변화에 대한 연구 (Acoustic Variations in Epileptic Patients with Topiramate)

  • Topiramate (TPM) is a new antiepileptic drug characterized by a clinical effective reduction in seizure frequency and it represents a useful drug effective in a wide range of epileptic patients. Known side effects are represented by weight loss, hypohidrosis, anorexia, sedation, nephrolithiasis, cognitive complaints and language disorders. This study is to examine acoustic characteristics of patients with TPM. 15 patients were assessed through a Computerized Speech Lab (CSL) applied before the beginning of therapy with TPM and 3 months after medication had been stabilized. Tests had been chosen to assess voice onset time (VOT), total duration (TD), vowel formants, loudness, pitch, speaking rate, and articulation patterns. We compared the data from patients and healthy volunteers. The statistical analysis of the results did not show changes in acoustic tests, except for TD which was increased. The increase of the TD is evaluated as a deterioration of fluency. Our results suggest that patients with TPM did not experience acoustic speech changes except that fluency was declined. Unlike previous studies, the medication of TPM has nothing to do with speech problems in patients with epilepsy.
