Imaging Features of Solitary Spinal Plasmacytoma in a Dog (개 척추에서 발생한 고립성 형질세포종의 자기공명영상 증례)

  • A 12-year-old, intact, female Alaskan malamute presented with severe spinal pain and hind limb lameness. On radiographs, a round, demarcated lytic lesion was identified in the central fifth lumbar vertebra. On magnetic resonance imaging (MRI), the lesion involving the spinal cord appeared hypointense on T1 weighted, hyperintense on T2 weighted, heterogeneously enhanced on post-contrast T1 weighted, and hypointense on GE images. A focal, small, ill-defined, lytic lesion was also observed radiographically in the sixth lumbar vertebra, it appeared as a focal hyperintense lesion on T1 weighted, T2 weighted, and GE images and showed focal enhancement on post-contrast T1 weighted images. She was euthanized owing to extreme pain and severe and progressive clinical signs; a plasmacytoma was histopthologically diagnosed. This report presents an unusual type of spinal tumor, plasmacytoma. MRI is a useful modality to evaluate the anatomic location and extension of spinal lesions.

Susceptibility Weighted Image for Stem Cell Tracking in Rat Photothrombotic Infarction (흰쥐 광 혈전 뇌경색 모델에서 줄기세포 추적을 위한 자화강조영상)

  • To the detect of SPIO-labelled hMSC, in vitro study on various cell concentration and in vivo molecular magnetic resonance imaging(MRI) technique using T2, $T2^*$ and SWI are compared with pathology. Cell concentration was $1.56{\times}10^4$, $3.13{\times}10^4$, $6.25{\times}10^4$, $1.25{\times}10^5$, $2.5{\times}10^5$, $5{\times}10^5\;cells/m{\ell}$ and for control $5{\times}10^5\cells/m{\ell}$. MRI technique using T2, $T^2$ and SWI. Photothrombotic infarction was located 2.5mm from bregma right, posterior. Cell injected through the tail vein of rat for 8 rats. MRI performed pre injection and post injection of 1, 3, 7 and 14days and sacrifice for pathology. MRI analysed on quantitatively. In vitro result, SWI was highest CNR as compared with $T2^*WI$, T2WI and $2.5{\times}10^5\;cells/m{\ell}$ cell concentration. In vivo result among the T2WI, $T2^WI$, SWI, T2WI is highest CNR between normal and infarction. CNR in normal-SPIO and infarction-SPIO is high score in SWI. Therefore, T2WI is good distinguish between normal and infarction, SWI are well detect SPIO-labelled hMSC from normal and infarction. Nowaday, SWI are mostly used on hemorrhage, calcification etc. in clinically, but for the future, stem cell therapy is commonly application at all disease which is good observing tool for SPIO-labelled stem cells.

Assessment of Magnetic Resonance Image Quality For Ferromagnetic Artifact Generation: Comparison with 1.5T and 3.0T. (강자성 인공물 발생에 대한 자기공명영상 질 평가: 1.5T와 3.0T 비교)

  • In this research, 15 patients were diagnosed with 1.5T and 3.0T MRI instruments (Philips, Medical System, Achieva) to minize Ferromagnetic artifact and find the optimized Tesla. Based on the theory that the 3.0T, when compared to 1.5T, show relatively high signal-to-ratio(SNR), Scan time can be shortened or adjust the image resolution. However, when using the 3.0T MRI instruments, various artifact due to the magnetic field difference can degrade the diagnostic information. For the analysis condition, area of interest is set at the background of the T1, T2 sagittal image followed by evaluation of L3, L4, L5 SNR, length of 3 parts with Ferromagnetic artifact, and Histogram. The validity evaluation was performed by using the independent t test. As a result, for the SNR evaluation, mere difference in value was observed for L3 between 1.5T and 3.0T, while big differences were observed for both L4, and L5(p<0.05). Shorter length was observed for the 1.5T when observing 3 parts with Ferromagnetic artifact, thus we can conclude that 3.0T can provide more information on about peripheral tissue diagnostic information(p<0.05). Finally, 1.5T showed higher counts values for the Histogram evaluation(p<0.05). As a result, when we have compared the 1.5T and 3.0T with SNR, length of Ferromagnetic artifact, Histogram, we believe that using a Low Tesla for Spine MRI test can achieve the optimal image information for patients with disk operation like PLIF, etc. in the past.

  • An experimental study was to use the parameter to determine the MRI artifact of chemical shift that occurs in water and fats. Scanning the image according to the encoding parameter and the bandwidth and change in 1.5T and 3.0T MRI to the SNR, we compared the CNR. In the image was confirmed that the occurrence of artefacts in the chemical shift of the water and oil. 3.0T more image artifacts in the 1.5T was confirmed that the relatively reduced. The width of the bandwidth it could be confirmed that according to the honeycombs artifacts decrease. Therefore, in order to reduce the artifacts in the MRI scan of the chemical shift runners weak field strength, is thought to be appropriate to widen the width of the bandwidth.

  • 목적: 현재 3.0T MRI system은 세계적으로 개발이 진행되고 있는 가운데, 3.0T에서 사용할 수 있는 RF coil의 개발이 시급한 상황이다. 1.0T 및 1.5T MRI 와는 달리 3.0T에서 사용할 수 있는 Body coil 및 그에 따른 High power RF amplifier 제작에 많은 제약이 있다. 작은 용량의 RF amplifier를 이용하여 신체의 부분을 촬영 하고자 한다면, Tx/Rx 가능한 coil을 이용하면 가능할 것이다. 이러한 이유로 본 연구에서는 Tx/Rx 가능한 Quadrature type C-spine RF coil을 설계, 제작하여 3.0T 고자장 자기공명 영상장치에서의 임상진단 활용범위를 확대하였다.

Diffusion-weighted MR imaging findings of intracerebral hematoma (뇌실질내의 확산강조영상 소견)

  • Purpose : To evaluate diffusion-weighted imaging findings of intracerebral hematoma according to the time sequence. Materials and methods : Seventeen patients with intracerebral hematoma were studied. Diffusion weighted images using 1.5 tesla MRI machine were obtained with b-value of $1000{\;}sec/\textrm{mm}^2$. The patients were grouped as hyperacute stage(within 12 hours, 5 patients), acute stage(within 3 days, 4 patients), subacute stage(within 3 weeks, 4 patients), and chronic stage(after 3 weeks,4 patients). The signal intensities were analysed as bright, high, iso, low and dark at the central and peripheral portions of the hematoma in each stage, and compared with those of T2 and T1 weighted images. Results : The signal intensities of the central and peripheral portion of the intracerebral hematoma on diffusion-weighted images were high and dark in hyperacute stage, dark and high-bright in acute stage, and high-bright and dark in subacute and chronic stages. The patterns of signal change of hematoma on diffusion-weighted image according to the time sequence were similar to those on T2-weighted image, but changed early and prominently. Conclusion : The intracerebral hematoma on diffusion-weighted image showed unique central and peripheral signal intensity according to the time sequence. Central portions show high to bright signals in hyperacute, subacute and chronic stage, and dark signal in acute stage, and peripheral portions show dark signals in hyperacute, subacute and chronic stage, and high to bright signal in acute stage.

Medical Image Encryption using Non-linear MLCA and 1D CAT (비선형 MLCA와 1D CAT를 이용한 의료영상 암호화)

  • 본 논문에서는 비선형 MLCA(Maximum Length Cellular Automata)와 1D CAT(One-Dimensional Cellular Automata Transform)를 이용하여 의료 영상 암호화 방법을 제안한다. 암호화 방법은 먼저, Wolfram Rule 행렬에 의해 전이행렬 T를 생성한다. 그 후, 암호화하려는 원 영상에 생성된 전이 행렬 T를 곱하여 원 영상의 픽셀 값을 변환한다. 또한 변환된 원 영상을 여원 벡터 F와 XOR 연산하여 비선형 MLCA가 적용된 영상으로 변환한다. 다음, 게이트웨이 값을 설정하여 1D CAT 기저함수를 생성한다. 그리고, 비선형 MLCA가 적용된 영상에 생성된 1D CAT 기저함수를 곱하여 암호화를 한다. 마지막으로 키 공간 분석을 통하여 제안한 방법이 높은 암호화 수준의 성질을 가졌음을 검증한다.

Multi-task Deep Neural Network Model for T1CE Image Synthesis and Tumor Region Segmentation in Glioblastoma Patients (교모세포종 환자의 T1CE 영상 생성 및 암 영역분할을 위한 멀티 태스크 심층신경망 모델)

  • Glioblastoma is the most common brain malignancies arising from glial cells. Early diagnosis and treatment plan establishment are important, and cancer is diagnosed mainly through T1CE imaging through injection of a contrast agent. However, the risk of injection of gadolinium-based contrast agents is increasing recently. Region segmentation that marks cancer regions in medical images plays a key role in CAD systems, and deep neural network models for synthesizing new images are also being studied. In this study, we propose a model that simultaneously learns the generation of T1CE images and segmentation of cancer regions. The performance of the proposed model is evaluated using similarity measurements including mean square error and peak signal-to-noise ratio, and shows average result values of 21 and 39 dB.

The Relationship between Image Parameters and SAR for Each Sequence of MRI (MRI 검사의 시퀀스 별 영상 변수와 SAR의 관계)

  • This study analyzed the relationship between image parameters and specific absorption rate (SAR) in various sequence environments to optimize SAR. For this purpose, image parameters were adjusted for T2, T1, STIR, T1 FLAIR, and T2 FLAIR sequences in a 3.0T MRI, and the whole body (WB) SAR and head SAR calculated by the device were measured. Then, the SAR was evaluated by adjusting the number of images and the flip angle (FA) of the refocusing RF. As a result, SAR increased as the number of image increased in all sequences. T1 and T1 FLAIR had correlation coefficients (r) of 0.876, 0.876 (WB SAR, head SAR), 0.867, 0.867 (WB SAR, head SAR), respectively, and STIR had the highest correlation with 0.898 and 0.899 (WB SAR, head SAR). showed (p<0.05). When applied by increasing the refocusing FA, WB SAR and head SAR increased overall in all sequences. The T1 and T2 sequences showed high correlation with correlation coefficients (r) of 0.897, 0.898 (WB SAR, head SAR) and 0.914, 0.915 (WB SAR, head SAR), respectively, while the sequences to which the inversion recovery technique was applied had relatively low FA, showed less sensitivity to increase. Therefore, in a sequence with a relatively low TR, minimizing the number of image and applying the fast spin echo to reduce the refocusing FA in a sequence with a high duty cycle are effective in reducing SAR.

Evaluation of Usefulness of IDEAL(Iterative decomposition of water and fat with echo asymmetry and least squares estimation) Technique in 3.0T Breast MRI (3.0T 자기공명영상을 이용한 유방 검사시 IDEAL기법의 유용성 평가)

  • The purpose of this study was to examine the usefulness of IDEAL technique in breast MRI by performing a quantitative comparative analysis in patients diagnosed with DCIS. On a 3.0T MR scanner, fat-suppressed T2-weighted images and T1-weighted images before and after contrast enhancement were obtained from 20 patients histologically diagnosed with ductal carcinoma in situ (DCIS). The findings from the quantitative image analysis are the following: 1) On T2-weighted images, SNR were not significantly different in the lesion area itself between the CHESS and IDEAL groups, while the IDEAL group showed higher SNR at the ductal area and fat area than the CHESS group. In addition, the CNR were higher for the IDEAL group in those regions. 2) On T1-weighted images before enhancement, SNR were not significantly different in the lesion area itself between the CHESS and IDEAL groups, while the IDEAL group showed higher SNR at the ductal area and fat area than the CHESS group. In addition, the CNR were higher for the IDEAL group in those regions. 3) On T1-weighted images after enhancement, SNR were not significantly different in the lesion area itself between the CHESS and IDEAL groups, while the IDEAL group showed higher SNR at the ductal area and fat area than the CHESS group.
