Real-time PCR quantification of 9 periodontal pathogens in saliva samples from periodontally healthy Korean young adults

  • Purpose: Few studies have examined periodontal pathogens from saliva samples in periodontally healthy young adults. The purposes of this study were to determine the prevalence of periodontopathic bacteria and to quantify periodontal pathogens in saliva samples using real-time polymerase chain reaction (PCR) assays in periodontally healthy Korean young adults under 35 years of age. Methods: Nine major periodontal pathogens were analyzed by real-time PCR in saliva from 94 periodontally healthy young adults. Quantification of Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Tannerella forsythia, Treponema denticola, Prevotella intermedia, Fusobacterium nucleatum, Campylobacter rectus, Peptostreptococcus anaerobius, and Eikenella corrodens was performed by DNA copy number measurement. Results: F. nucleatum and E. corrodens were detected in all subjects; the numbers of positive samples were 87 (92.6%), 91 (96.8%), and 90 (95.7%) for P. gingivalis, P. anaerobius, and C. rectus, respectively. Other pathogens were also detected in periodontally healthy subjects. Analysis of DNA copy numbers revealed that the most abundant periodontal pathogen was F. nucleatum, which was significantly more prevalent than all other bacteria (P<0.001), followed by P. anaerobius, P. gingivalis, E. corrodens, C. rectus, and T. denticola. There was no significant difference in the prevalence of each bacterium between men and women. The DNA copy number of total bacteria was significantly higher in men than in women. Conclusions: Major periodontal pathogens were prevalent in the saliva of periodontally healthy Korean young adults. Therefore, we suggest that the development of periodontal disease should not be overlooked in periodontally healthy young people, as it can arise due to periodontal pathogen imbalance and host susceptibility.

IDENTIFICATION OF PUTATIVE PATHOGENS IN ACUTE ENDODONTIC INFECTIONS BY PCR BASED ON 16S rDNA (중합효소연쇄반응법을 이용한 급성 치수 및 치근단 질환의 병원성 세균의 동정)

  • The purpose of this study was to investigate the frequency of 7 putative pathogens in endodontic infections. The specimens were collected from infected pulpal tissue of patients who were referred for root canal treatment to the department of conservative dentistry, Chosun University Samples were collected aseptically using a barbed broach and a paper point. The cut barbed broaches and paper points were transferred to an eppendorf tube containing 500 ml of 1 X PBS. DNAs were extracted from the samples by direct DNA extraction method using lysis buffer (0.5% EDTA, 1% Triton X-100). Identification of 7 putative pathogens was performed by PCR based on 16S rDNA. The target species were as follows : Porphyromonas endodontalis, Porphyromonas gingivalis, Prevotella intermedia, Prevotella nigrescens, Bacteroides forsythus, Actinobacillus actinomycetemcomitans, and Treponema denticola. Our data revealed that the prevalence of P. endodontalis was found in 88.6% (39/54), P. ginivalis 52.3% (23/44), P. nigrescens 18.2% (8/44), P intermedia 15.9% (7/44) B. forsythus 18.2% (8/44), A. actinomycetemcomitans 3.3% (1/44), T. denticola 25% (l1/44) of the samples. The high prevalence of P. endodontalis and P. ginivalis suggests that they may play an important role in the etiology of endodontic infections.

Study on oral periodontal pathogens distribution and risk factors in college students (일부 대학생들의 구강 내 치주질환 세균 분포와 검출 위험요인 조사)

  • Objectives: This study attempted to provide basic data necessary for a prevention promotion program for oral health management in college students. Methods: This study investigated general characteristics and subjective periodontal health status using a structured questionnaire and examined the distribution of bacteria related to periodontal disease in oral cavity by real-time PCR in subjects composed of 57 male and female college students. Results: It was statistically significant that P. gingivalis was detected more frequently in smokers with 25% compared to non-smokers with 6.1%, not detected in subjects that engaged in tooth brushing more than three times a day, and was detected in subjects that engaged in tooth brushing fewer than three times a day with 21.1%. Pathogens in saliva had significant correlations with each other (p<0.05, p<0.01, p<0.001). P. gingivalis showed positive correlations with T. forsythia, T. denticola, P. intermedia, and A. actinomycetemocmitans, and T. forsythia with P. intermedia, and A. actinomycetemocmitans. P. intermedia had a positive correlation with A. actinomycetemocmitans, and F. nucleatum with P. intermedia. Conclusions: Bacteria related to periodontal disease in oral cavities in college students were distributed in various ways, and smoking and the frequency of daily toothbrushing were found to be risk factors for the detection of bacteria.

  • 용언은 그 어간이 여러 문법소와 결합하면서 자동적 음운 변동을 제외한 형태 변동이 있는가, 없는가에 의해 규칙 용언/불규칙 용언으로 구분할 수 있다. 이러한 불규칙 용언은 심성 어휘집에 어떤 형태로 저장되어 있으며, 규칙 용언과는 어떠한 관계가 있는지, 나아가 실어증 환자의 경우에는 정상인에 비해 어떤 행동장애를 보이며, 장애가 있다면 어느 경로의 손상으로 인한 장애인지를 알아보는 것이 본 연구의 목적이다. 이를 위해 이해성 실어증 환자 한 명과 음어적 실행증 현상을 동반한 경미한 정도의 실어증 환자를 피험자로 하였다. 실험 과제는 단어 채워 넣기 과제(word completion task)를 사용하였다. 즉 주어진 기본형 용언을 검사 문장의 문맥에 맞게 활용하여 채워 넣는 것이다. 실험 결과에 의하면 환자들은 규칙용언의 활용(예. 먹다/먹는)과 불규칙 용언 중 형태를 유지한 채로 활용하는 경우(예. 줍다/줍고)에는 거의 오류가 없었으나, 불규칙 용언이 형태 변화를 겪어야 할 경우(예. 줍다/주워)에는 대부분 오류를 보였다. 또 이때는 기본형(basic form)을 그대로 유지하는 오류 방향성을 관찰할 수 있었다. 이는 그간 문법으로 구분되어 오던 규칙 용언/불규칙 용언의 정보 처리보다는 형태 유지/형태 변화 정보 처리의 영향이 크다는 것을 알 수 있다. 특히 이해성 실어증 환자는 전체적인 오류율이 매우 높았는데, 규칙 용언의 경우에도 오류를 보였다. 이때, 용언의 어간에 해당하는 부분에는 오류가 없고, 뒤에 따르는 내용과의 관계를 파악해야 하는 문법 기능소, 즉 연결 어미에서 오류를 보여 정보의 유지, 통합에 문제가 있다는 기존의 연구와도 일치하는 결과를 나타냈다.환자는 시제 선어말 어미를 선택하는데도 어려움을 보임이 확인되었다. 실험 3 역시 실험 1과 실험2에서와 동일하게 처리의 어려움을 보였다. 이러한 실험 결과들은 국어의 존칭과 시제 선어말 어미가 통사부에서 구(XP)와 결합하여 새로운 구를 형성하는 통사적 접사로 해석할 수 있으며 Grodzinsky의 가설을 지지하는 결과를 보여 줌으로서 국어에서도 AgrP, TP, CP 사이의 통사적 위계가 있음을 뒷받침하는 증거가 된다.전처리한 Group 3에서는 IL-2와 IL-4의 수준이 유의성있게 억제되어 발현되었다 (p < 0.05). 이러한 결과를 통하여 T. denticola에서 추출된 면역억제 단백질이 Th1과 Th2의 cytokine 분비 기능을 억제하는 것으로 확인 되었으며 이 기전이 감염 근관에서 발견되는 T. denticola의 치수 및 치근단 질환에 대한 병인기전과 관련이 있는 것으로 사료된다.을 보였다. 본 실험 결과, $Depulpin^{\circledR}은{\;}Tempcanal^{\circledR}와{\;}Vitapex^{\circledR}$에 비해 높은 세포 독성을 보여주공 있으나, 좀 더 많은 임상적 검증이 필요할 것으로 사료된다.중요한 역할을 하는 것으로 추론할 수 있다.근관벽을 처리하는 것이 필요하다고 사료된다.크기에 의존하며, 또한 이러한 영향은 $(Ti_{1-x}AI_{x})N$ 피막에 존재하는 AI의 함량이 높고, 초기에 증착된 막의 업자 크기가 작을 수록 클 것으로 여겨진다. 그리고 환경의 의미의 차이에 따라 경관의 미학적 평가가 달라진 것으로 나타났다.corner$적 의도에 의한 경관구성의 일면을 확인

The distribution of red complex of implant sulcus (임플란트 치은열구내 red complex의 분포)

  • The aim of present study was to evaluate the influence of periodontal status of adjacent tooth and loading time to the prevalence of red complex of implant sulcus. In 97 patients, partially edentulous subjects with endosseous root-form implants were selected. All subjects were medically healthy and had not taken systemic antibiotics and professional plaque control 3 months before sampling. The number was as follows: clinically healthy implants:161, clinically unhealthy implants:22, clinically healthy adjacent teeth:73, clinically unhealthy adjacent teeth:38. All teeth and implants of each patient were examined probing depth(PD), modified sulcus bleeding index(mSBI), and modified plaque index(mPI), and samples of subgingival plaque were obtained at each site with sterile curet or fine paper points, then the plaque transferred to PBS. Obtained samples were examined for the presence of P. gingivalis, T. forsythensis, and T. denticola by the polymerase chain reaction (PCR). The relationship among clinical parameters and the colonizations by the 3 bacterial species from adjacent teeth and implants region were analyzed by student t-test (p<0.05). The results of this study were as follows: 1. mPI and mSBI of implant increased with increasing of same indices of adjacent tooth(p=0.03,0.001), but not in the PD . 2. The mPI, mBI, PD of implants was higher when red complex exist. 3. The prevalence of red complex was higher when the periodontal condition is unhealthy. 4. The prevalence of red complex of implants has no significant relation to the probing depth of adjacent tooth. 5. Prevalence of P. gingivalis, T. forsythensis of implants increased with loading time. (p=0.02,0.018) These results shows the importance of oral hygiene and supportive periodontal therapy.

Evaluation of periopathogenic bacteria of peri-implantitis using real-time PCR in Koreans: pilot study (한국인의 임플란트 주위 질환에서 real time PCR법을 이용한 치주세균 평가: pilot study)

  • Purpose: We performed quantitative and qualitative analysis of typical periodontal bacteria using real time PCR method to investigate the microbiological difference according to the severity of peri-implant disease in Koreans. Materials and Methods: Total of 60 implants were divided into three groups (healthy group, peri-implant mucositis group, peri-implantitis group) through periapical radiographs and clinical indices. The evaluated clinical parameters were pocket depth, plaque index, suppuration and bleeding on probing. Using a sterilized curette instrument, microbial samples were collected from the subgingival plaque and real-time PCR was performed on five periodontal bacteria. The relative expression levels of microorganisms were compared by comparative delta-CT method. Results: The relative expression levels of E. corrodens and T. denticola were significantly higher in the peri-implantitis group (P < 0.017). On the other hand, the relative expression level of F. nucleatum and P. gingivalis was relatively high in the healthy implant group regardless of the severity of disease. P. intermedia was significantly lower in the healthy implant group (P < 0.017). Conclusion: Periodontal bacteria were detected in Koreans with peri-implant diseases, but there was no microbiological distribution similar to periodontitis.

Analysis of periodontal pathogens in care facilities for the elderly with dementia (Real-time PCR을 이용한 요양보호시설 치매 노인의 구강 병원성 미생물 분석)

  • Objectives: The purpose of this research is to examine oral pathogen distribution among elderly with dementia in a care facility to understand the importance of preventively managing oral diseases in terms of preventively managing senile diseases. Methods: From 11th August 2015 to 11th October 2015, gingival crevicular fluid was collected from 130 subjects consisting of demented/non-demented elderly people aged above 65 in care facilities located in the regions of Busan/Gyeonggnam. Based on collected data, real-time PCR analysis on oral pathogen was conducted. Results: The demented elderly group consisting of female patients aged from 0 to 79 indicated higher ratios of T. denticola in comparison to the non-demented elderly group, and the demented elderly group consisting of female patients aged above 80 indicated a high ratio of S. mutans. It was confirmed that P. gingivalis and T. forsythensis categorized under the red complex are correlated, and that bacterial species categorized under the orange complex and bacterial species categorized under the red complex are correlated. Conclusions: Because eldery people with demntia are exposed to periodontal disease and dental caries, their oral environments are more vulnerable. In order to improve such environments, it is necessary to provide care facility personnel with an opportunity to receive education to become aware of the importance of oral health, and it is also necessary to compulsorily assign dental hygienists to care facilities so that they can serve as personnel specializing in oral health management.

Sealing capability and marginal fit of titanium versus zirconia abutments with different connection designs

  • PURPOSE. Limited data is available regarding the differences for possible microleakage problems and fitting accuracy of zirconia versus titanium abutments with various connection designs. The purpose of this in vitro study was to investigate the effect of connection design and abutment material on the sealing capability and fitting accuracy of abutments. MATERIALS AND METHODS. A total of 42 abutments with different connection designs [internal conical (IC), internal tri-channel (IT), and external hexagonal (EH)] and abutment materials [titanium (Ti) and zirconia (Zr)] were evaluated. The inner parts of implants were inoculated with $0.7{\mu}L$ of polymicrobial culture (P. gingivalis, T. forsythia, T. denticola and F. nucleatum) and connected with their respective abutments under sterile conditions. The penetration of bacteria into the surrounding media was assessed by the visual evaluation of turbidity at each time point and the number of colony forming units (CFUs) was counted. The marginal gap at the implant- abutment interface (IAI) was measured by scanning electron microscope. The data sets were statistically analyzed using Kruskal-Wallis followed by Mann-Whitney U tests with the Bonferroni-Holm correction (${\alpha}=.05$). RESULTS. Statistically significant difference was found among the groups based on the results of leaked colonies (P<.05). The EH-Ti group characterized by an external hexagonal connection were less resistant to bacterial leakage than the groups EH-Zr, IT-Zr, IT-Ti, IC-Zr, and IC-Ti (P<.05). The marginal misfit (in ${\mu}m$) of the groups were in the range of 2.7-4.0 (IC-Zr), 1.8-5.3 (IC-Ti), 6.5-17.1 (IT-Zr), 5.4-12.0 (IT-Ti), 16.8-22.7 (EH-Zr), and 10.3-15.4 (EH-Ti). CONCLUSION. The sealing capability and marginal fit of abutments were affected by the type of abutment material and connection design.

Association of lifestyle with periodontal pathogens on dental patients with periodontitis (retrospective study) (치주질환 환자의 생활양식과 치주 병원균의 연관성에 관한 후향적 연구)

  • This study aimed to investigate the association of lifestyle with the copy number of periodontal pathogens. This retrospective study collected electronic health records of 102 subjects with periodontitis, including reports of bacterial genetic tests and lifestyle questionnaires. The five pathogens were analyzed as follows: Porphyromonas gingivalis, Tannerella forsythia, Treponema denticola, Prevotella intermedia, and Fusobacterium nucleatum. The lifestyle questionnaire included age, sex, oral hygiene management, smoking, drinking, exercise, dietary, snacks, water intake, and sleeping time. An independent t-test or ANOVA was performed to compare the copy number of periodontal pathogens according to lifestyle (α=0.05). The copy numbers of P. gingivalis and F. nucleatum were significantly higher than those of other strains. The copy number of T. forsythia in patients who exercised was 54% lower than in those who did not (p=0.009). Other lifestyle factors did not affect the number of bacteria. Exercise habits among the lifestyles showed a association with the number of specific oral bacteria. This result suggests that a lifestyle questionnaire is essential in clinical situation and necessary to prevent and treat the periodontal disease effectively.

Effects of adjunctive daily phototherapy on chronic periodontitis: a randomized single-blind controlled trial

  • Purpose: The purpose of this randomized single-blind controlled trial was to elucidate the clinical and antimicrobial effects of daily phototherapy (PT) as an adjunct to scaling and root planing (SRP) in patients with chronic periodontitis. Methods: The study was conducted from December 2013 to May 2014 at Ewha Womans University Mokdong Hospital, Seoul, Korea. Forty-one patients with mild to moderate chronic periodontitis were randomly divided into two therapeutic groups in a 1:1 ratio: SRP+PT and SRP (control) groups. All participants underwent full-mouth SRP. PT was performed thrice a day for a month by using electric toothbrushes with embedded light-emitting diodes. Plaque index, gingival index, probing pocket depth (PPD), clinical attachment level (CAL), and bleeding on probing were assessed before (baseline) and four weeks after (follow-up) the treatment. Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Tannerella forsythia, Treponema denticola, Prevotella intermedia, Fusobacterium nucleatum, Parvimonas micra, Campylobacter rectus, Eikenella corrodens, Streptococcus mutans, and Streptococcus sobrinus levels were detected by a real-time polymerase chain reaction at the same points in time. Results: The clinical parameters improved in both the groups. At the follow-up assessment, PPD was significantly decreased in the SRP+PT group (P=0.00). Further, PPD and CAL showed significantly greater changes in the SRP+PT group than in the SRP group (PPD, P=0.03; CAL, P=0.04). P. gingivalis and T. forsythia levels decreased in this group, but no significant intergroup differences were noted. Conclusions: Adjunctive PT seems to have clinical benefits, but evidence of its antimicrobial effects is not sufficient. Long-term studies are necessary to develop the most effective PT protocol and compare the effectiveness of PT with and without exogenous photosensitizers.
