Objectives: This study was conducted by clinical dentistry hygienists to develop knowledge measurement tools and evaluate knowledge necessary to cope with emergencies related to systemic diseases occurring in dentistry. Methods: Basic questions were developed on the six most frequently occurring diseases in the dental office: diabetes, syncope, respiratory disease, coronary artery disease, cerebrovascular disease, and epilepsy. Fifty-eight preliminary questions were composed in a survey format through expert validation. The survey was then conducted on 290 dental hygienists to verify the composition validity and reliability. Factor and reliability analyses were performed using the SPSS 23.0 program, and the correct answer rate for each question was calculated. Results: 49 items of a total of 13 factors were adopted as the final items for signs, symptoms, prevention and treatment of five diseases. The question with the highest percentage of correct answers for each question was "Slowly set up the unit chair when raising the patient"(97.9%) during the prevention of unconsciousness and question with the lowest correct answer rate was "soft substances such as cloth should be put in the mouth to prevent trauma while the seizure persists" (16.6%). Conclusions: A total of 49 questions have been developed as tools to measure the knowledge of dental hygienists' first aid related to systemic diseases and it is expected that related curriculum can be operated or evaluated by using them.

Purpose: This study utilizes big data from the 8th (2021) National Health and Nutrition Examination Survey to determine first, the relationship between chewing discomfort in the elderly and some systemic diseases and second, whether oral diseases and oral health problems are related to systemic diseases. Since this may have an impact, we aim to provide basic data to facilitate the expansion and emphasize the importance of integrated health management education. Methods: Original data from the 8th (2021) National Health and Nutrition Survey, conducted by the Korea Centers for Disease Control and Prevention, were analyzed using SPSS Version 21.0 (IBM). A complex sample frequency analysis was conducted to confirm the general and health-related characteristics of the study subjects, and a complex sample cross-analysis was conducted to determine chewing discomfort according to both general and health-related characteristics. Complex sample multiple logistic regression analysis was conducted to determine the effect on chewing discomfort. Results: In order to analyze the factors that affect chewing discomfort, the general characteristics that showed significant differences in chewing discomfort were adjusted for age, personal income, education level, basic livelihood security, high blood pressure, subjective health status, and subjective oral health. It was found that the condition had a statistically significant effect on chewing discomfort. Conclusion: The findings of this study demonstrate that high blood pressure, subjective health status, and subjective oral health status affect chewing discomfort; hence, measures such as developing and operating programs to improve national oral health are needed. We hope that our study will be used as basic data for research into chewing discomfort and systemic diseases in the elderly.
Background: The purpose of this study was to investigate the distribution of systemic diseases and medications in patients older than 65 years of age who visited the hospital for implant treatment, as well as to investigate basic information about surgical complications that may occur after insured implant treatment. Method: A total of 126 patients over 65 years of age were treated for implant surgery from October 1, 2013 to October 30, 2016. Electronic chart review was conducted to obtain medical records, which included sex, age, systemic diseases, medication(s) being taken, and control of the medications. Five patients were excluded due to lack of medical records giving information about systemic disease. Results: Of the 126 patients, 112 (88.9%) were taking medication due to systemic disease and 9 patients (7.1%) were not. The sex distribution was 71 women and 55 men and the highest proportion of patients were between 65 and 69 years old. The most common diseases were, from most to least frequent, hypertension, cardiovascular disease, diabetes, and osteoporosis. The drug groups that can cause major complications after dental treatment were classified as hemorrhagic, osteoporotic, and immunosuppressive agents, and were taken by 45 (35.7%), 22 (17.5%) and 4 (3.2%) patients, respectively. Conclusion: Given that 88.9% of the elderly patients who were eligible for insurance implant treatment had systemic disease, it is necessary to carefully evaluate patients' medical histories and their general conditions in order to prevent emergencies during implant surgery.
Sleep plays a critical role in homeostasis of the body and mind. Insomnia is a disease that causes disturbances in the initiation and maintenance of sleep. Insomnia is known to affect not only the sleep process itself but also an individual's cognitive function and emotional regulation during the daytime. It increases the risk of various neuropsychiatric diseases such as depression, anxiety disorder, and dementia. Although it might appear that insomnia only affects the nervous system, it is also a systemic disease that affects several aspects of the body, such as the cardiovascular, endocrine, and immune systems; therefore, it increases the risk of various diseases such as hypertension, diabetes mellitus, and infection. Insomnia has a wide range of effects on our bodies because sleep is a complex and active process. However, a high proportion of patients with insomnia do not seek treatment, which results in high direct and indirect costs. This is attributed to the disregard of many of the negative effects of insomnia. Therefore, we expect that understanding insomnia as a systemic disease will provide an opportunity to understand the condition better and help prevent secondary impairment due to insomnia.

Although the relationship between malignancy risk with systemic sclerosis (SSc) has been inconclusive, there are some previous studies for a positive correlation. Most patients with SSc have some degree of lung parenchymal involvement in the form of interstitial thickening and fibrosis. Interstitial lung disease is the most common pulmonary manifestation of SSc. Interstitial lung disease following chemotherapy (5-fluorouracil, leucovorin, and oxaliplatin [FOLFOX]) is an uncommon life-threatening complication and it is induced by oxaliplatin. We report a case of multiple cancers in a patient with SSc and aggravated interstitial lung disease by chemotherapy.
Objectives: The purpose of the study is to investigate the awareness toward oral health and plaque removal in the elderly people in Korea. Methods: An interview method was carried out to 308 elderly people from a senior welfare facility in Mokpo, Jeonnam from May 1 to December 30, 2011 after receiving informed consents. Except 4 incomplete answers, data were analyzed using SPSS 18.0 program. The questionnaire consisted of demographical characteristics of the subjects, experience of plaque removal, recognition towards plaque removal, and correlation to systemic diseases. Results: Most of the elderly people experiences plaque removal(63.4%) and 55.9% had revisit intention. Those having plaque removal accounted for 47.8% and they had good subjective oral health recognition. Those not having plaque removal accounted for 61.8% and 61.8% having average systemic health condition and 53.3% having poor oral health recognition. The plaque removal was closely related to subjective oral health recognition. Conclusions: It is necessary to develop the preventive oral health program for the elderly people. The plaque removal in the elderly people is the most important services to prevent the systemic diseases including hypertension, arthritis, diabetes, nervous disease, and gastrointestinal diseases.
Chronic obstructive pulmonary disease (COPD) is a chronic airway inflammation characterized by fixed airflow limitation and chronic respiratory symptoms, such as cough, sputum, and dyspnea. COPD is a progressive disease characterized by a decline in lung function. During the natural course of the disease, acute deterioration of symptoms leading to hospital visits can occur and influence further disease progression and subsequent exacerbation. Moreover, COPD is not only restricted to pulmonary manifestations but can present with other systemic diseases as comorbidities or systemic manifestations, including lung cancer, cardiovascular disease, pulmonary hypertension, sarcopenia, and metabolic abnormalities. These pulmonary and extrapulmonary conditions lead to the aggravation of dyspnea, physical inactivity, decreased exercise capacity, functional decline, reduced quality of life, and increased mortality. In addition, pneumonia, which is attributed to both COPD itself and an adverse effect of treatment (especially the use of inhaled and/or systemic steroids), can occur and lead to further deterioration in the prognosis of COPD. This review summarizes the long-term outcomes of patients with COPD. In addition, recent studies on the prediction of adverse outcomes are summarized in the last part of the review.
The purpose of laboratory tests in the field of oral medicine can be divided into two categories: (1) medical evaluation of patients with systemic diseases that are planning to receive dental care and (2) diagnosis of patients with certain oral diseases. First, laboratory tests are commonly used to evaluate patients with systemic diseases who need dental management. A combination of multiple tests is usually prescribed as a test panel to diagnose and assess a specific disease. Test panels closely related to oral medicine include those for rheumatoid arthritis, connective tissue disease/lupus, liver function, thyroid screening, anemia, and bleeding disorders. Second, laboratory tests are used as auxiliary diagnostic methods for certain oral diseases. They often provide crucial diagnostic information for infectious diseases caused by bacteria, fungi, and viruses that are associated with pathology in the oral and maxillofacial regions. Laboratory tests for infectious diseases are composed of growth-dependent methods, immunologic assays, and molecular biology. As the field develops, further application of laboratory tests, including synovial fluid analysis in temporomandibular joint disorders, salivary diagnostics, and hematologic biomarkers associated with temporomandibular disorders and orofacial pain conditions, is currently under scrutiny for their reliability as diagnostic tools.

Herein, we report a case of multiple systemic arteries to pulmonary artery fistulas without any underlying causes, presenting recurrent hemoptysis. Transcatheter embolization was successfully performed several times on multiple systemic feeding arteries. Multiple systemic arteries to pulmonary fistulas can be a source of uncontrolled bleeding, and embolization may be a reasonable therapeutic option to control the bleeding.
