Systemic arterialization of lung without sequestration은 폐격리증 없이 정상적인 폐실질과 기관지를 가지고 기관지계와 정상적으로 소통이 되어 있는 폐분절이 대동맥으로부터 분지한 기형 체동맥에 의해 동맥혈을 공급받는 보기 드문 폐혈관 기형으로 저자들은 증상이 없이 폐종괴로 오인되어 내원하여 흉부 전산화 단층촬영 및 혈관 조영술을 통하여 확진된 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

폐에 체동맥이 분포하는 선천성 기형은 드문 질환으로 하폐엽의 기저분절에 폐동맥 대신 대동맥에서 1개 이상의 체동맥이 나오는데 정상적인 기관지와 연결 유무에 따라 폐 분획증과 좌측 폐동맥 이상 기시증(Anomalous Origin of Left Pulmonary Artery, AOLPA)으로 구분된다. 한일병원 흉부외과에서는 하행 흉부 대동맥에서 시작되는 비정상적인 체동맥을 가진 두 예의 폐동맥 기형을 경험하고, 1예는 기저분절절제술을, 다른 예는 동맥색전술을 시행하였고, 2년간 관찰한 결과 양호하였다. 이 결과를 바탕으로 외과적 수술이 이 기형의 근본적인 치료법이지만, 전신상태가 불량하거나 폐기능이 저하된 고위험군에서 동맥색전술도 선택적으로 시행할 수 있는 가능한 치료법으로 생각된다.

정상 폐로의 체순환 동맥 공급은 드문 것으로 알려져 있는데 저자들은 단순 흉부 X-선상 우연히 폐의 이상 음영을 발견하여 조영증강 전산화단층촬영상 좌하엽 저분절로 가는 체순환 동맥 분지를 진단한 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Pulmonary systemic arterialization to normal basal lung without sequestration is a rare congenital anomaly. In this rare abnormality, arterialization of the left lower lobe is the most common type. In general, surgical treatments have been performed. Recently, for reducing the complications and risks of surgery, embolization is mainly attempted by using coils. We report a case of 22-year-old male patient with a 10 mm anomalous arterial supply to his normal lung, which is being successfully treated by transcatheter embolization when using the Amplatzer Vascular Plug that has been adapted for the treatment of high-flows and large artery occlusions.
