Reevaluation of Midline Malignant Reticulosis with Systemic Manifestation after Irradiation (방사선치료 후 전신적증세의 발현을 나타낸 Midline Malignant Reticulosis 환자군에 대한 재고)

  • During a 10 year periods, 42 patients with well-documented Midline Malignant Reticulosis were treated with local irradiation and followed for extended periods of time. 13 cases with systemic manifestation after irradiation illustrate the protean features of this disease. Although it commonly Presents in upper airway tracts such as nasal cavity and/or septum, soft palate and palatine tonsil, the lesion may be localized as well as diffuse. 2 cases among them showed systemic relapse on lung, pancreas and scrotum probably from either a multifocal or metastatic deposits. Another 3 eases were associated with systemic diseases such as stomach carcinoma, typhoid enteritis and CNI of fat tissue. Remained cases were diagnosed by clinical or radiological examination. Possibility of close relation to lymphoma with Midline Malignant Reticulosis are also suggested in 1 case. In Midline Malignant Reticulosis, the best results of treatment are obtained in localized lesion of the upper airway treated early with irradiation: A poorer outcome is associated with multifocal systemic involvement, which necessitates a systemic therapy.

Effect of Blanching Time on Changes in Vitamin and Mineral Contents in Leafy Vegetables Treated by Pesticides (데치는 시간에 따른 침투성 및 비침투성 농약 처리 엽채류의 비타민 및 무기질의 함량 변화)

  • The present study was conducted to investigate effect of blanching time on changes in vitamin and mineral contents in leafy vegetables treated by different pesticides. Vitamin A content of fresh leafy vegetables was high whorled mallow > chard > spinach in order and vitamin C content of those was high spinach > whorled mallow > chard in order. Vitamin A and C contents of leafy vegetables treated by non-systemic pesticide were higher than those of leafy vegetables treated by systemic pesticide. Changes in vitamin A and C contents of leafy vegetables treated by systemic pesticide and non-systemic pesticide showed a similar trend. Vitamin A content significantly increased, while vitamin C content significantly decreased by conventional blanching. Changes in mineral contents in leafy vegetables treated by systemic pesticide and non-systemic pesticide showed a similar trend. Upon conventional blanching, Mg in spinach, Mg and Ca in chard and whorled mallow slightly increased, while K in all leafy vegetables remarkably decreased, and Se in spinach slightly decreased and Se in other vegetables remarkably decreased. On the other hand, Na, P, Fe, Cu and Zn in all leafy vegetables slightly decreased or did not show any change.

  • Nowadays, medically compromised patients who could not receive dental treatments in the past are able to go through minor oral surgeries with adequate preoperative measures. Thorough understanding of the systemic disease and its complications is needed as well as the management them. Frequent complications of surgical procedures are bleeding, infection, delayed healing, systemic reactions by stress and they can be aggravated due to the patients' systemic conditions. Therefore, understanding of the systemic disease of patient visiting dental office and treatment modification according to the systemic status is needed. Also consultation to the medical doctor is imperative, through which perioperative risk and complications can be reduced. Among the high frequency complications of dental treatment of medically compromised patients, bleeding, infection, delayed healing, systemic reactions by stress will be discussed with the management of each one.

Cardiovascular effect of epinephrine in endodontic microsurgery: a review

  • Epinephrine is one of the most widely-used vasoconstrictors in dental treatment including endodontic microsurgery. However, the systemic safety of epinephrine has been in debate for many years because of its potential risk to cause cardiovascular complications. The purpose of this review was to assess the cardiovascular effect of epinephrine use in endodontic microsurgery. Endodontic microsurgery directly applies epinephrine into the bone cavity, and the amount is reported to be much larger than other dental surgeries. Moreover, when considering that systemic potency of intraosseous application is reported to be comparable to intravenous application, the systemic influence of epinephrine could be increased in endodontic microsurgery. Besides, pre-existing cardiovascular complications or drug interactions can enhance its systemic influence, resulting in increased susceptibility to cardiovascular complications. Although clinical studies have not reported significant complications for patients without severe systemic complications, many epinephrine-induced emergency cases are warning the cardiovascular risk related with pre-existing systemic disease or drug interactions. Epinephrine is a dose-sensitive drug, and its hypersensitivity reaction can be fatal to patients when it is related to cardiovascular complications. Therefore, clinicians should recognize the risk, and the usage of pre-operative patient evaluation, dose control and patient monitoring are required to ensure patient's safety during endodontic microsurgery.

  • Purpose: The purpose of this study was to investigate the relationship between systemic diseases and denture wear status in elderly. The subjects of this study were 4,340 seniors aged 65 or older who conducted screenings and health surveys using the National Health and Nutrition Survey data during the sixth period (2013, 2014 and 2015). Methods: For the general characteristics and denture wear status according to general systemic diseases, complex sample frequency analysis and cross-analysis were performed. After controlling the general characteristics, complex sample logistic regression analysis was performed to determine the relationship between systemic diseases and denture wear status. Statistical software was used for SPSS (SPSS 23.0 for Windows, SPSS Inc, USA). Significance level was considered as 0.05. Results: A multisample logistic regression analysis was performed to determine the relationship between systemic diseases and denture wear status. The results showed that the dentured state had a significant effect on dyslipidemia and diabetes. Conclusion: Dyslipidemia and diabetes, which are related to denture wearing condition, need to be prevented and treated with regular checkups. In addition, based on the results of the study, it is thought that the elderly need denture prosthetic treatment after tooth extraction. In addition, it can be used as basic data for oral health business plan that can maintain and manage oral health.

A Novel Approach to the Discovery of Non-systemic Anti-inflammatory Steroids; Antedrug

  • Therapeutic use of anti-inflammatory steroids is limited due primarily to their systemic suppressive effects on pituitary function and the immune system.. To overcome the clinical limitation, a new approach toward the discovery of non-systemic anti-inflammatory steroids is based upon the antedrug concept introduced by this laboratory. The new concept describes locally active agents which are designed to undergo a predictable biotransformation to inactive metabolites upon entry into systemic circulation from the applied site. Thus, true antedrugs are devoid of systemic adverse effects. In a continuing effort, 16$\alpha$-carboxylate and isoxazoline derivatives of prednisolone have been synthesized and screened. In the croton oil-induced ear edema bioassay, the following relative potencies were obtained setting hydrocortisone=1.0; 3a, 1.5; 3b, 3.1; 4a, 4.0; 4b, 12.2; 5b, 8.2; 6b, 11.2; 7a, 1.9; 7b, 4.1; 8a, 3.3; 8b 6.8; 9a, 0.7; 9b 8.6; 10a 2.6; 10b, 7.4. Results of the five-day bioassay indicated that, in contrast to the parent compound, the novel steroidal antedrugs did not significantly alter body weight gain, thymus weights, adrenal weights or plasma corticosterone levels. Taken together, the antedrug concept appears to be a fundamentally sound strategy for the separation of local anti-inflammatory activity form systemic adverse effects.

Comparison of Systemic Accident Investigation Techniques Based on the Sewol Ferry Capsizing

  • Objective: This study aims to survey and compare three systemic accident investigation techniques of Accimap, STAMP and FRAM, based on the application studies of the Sewol ferry accident. Background: Traditional accident investigation methods such as domino models, FTA, etc. work well for losses caused by physical component failures or actions of human in relatively simple systems, but are unable to depict mechanisms generating errors and violations in the current complex socio-technical systems. For better understanding the structure and behavior of the socio-technical systems, systemic techniques have been developed and used. Method: This study was mainly based on survey of literatures through surfing webpages of ScienceDirect and Google, and ergonomics relevant journals. The key words of Sewol, Sewol ferry, Sewol ferry accident, etc. were used in the survey. Results: Three systemic accident investigation methods included similar actors in the Sewol ferry accident including government, Ministry of Ocean and Fisheries, Korean Coast Guard, Korean Register of Shipping, Korea Shipping Association, Chonghaejin Marine Company, crew members. The methods graphically represented each level's failures or performance variabilities of relevant functions and relationships between them. It was shown that the systemic methods consider the entire system, ranging from the environment in which the accident occurred, to the role of government in shaping the system of work. Each method has its own comparative pros and cons, but the Accimap has advantages in terms of time of analysis, data required, model complexity and degree of comprehensiveness. Conclusion: This study reviewed and compared three systemic accident investigation methods, which showed that there are systemic characteristics and pros and cons in the methods. Application: The results would be used as a guideline when selecting accident investigation methods.

  • Recurrent transverse myelitis is a rare manifestation of systemic lupus erythematosus. Recurrent transverse myelitis presents the biggest diagnostic problem, since it is common manifestation of multiple sclerosis. But it can also be the only feature or first manifestation in systemic lupus erythematosus. Neurological manifestations and magnetic resonance imaging can be indistinguishable, and there are no specific diagnostic tools. Here we describe a 59-year-old female having a systemic lupus erythematosus with recurrent transverse myeltitis. No uniform therapeutic protocol exists for systemic lupus erythematous with transverse myelitis, and the prognosis is usually poor. We suggest that aggressive treatment (usually with pulses of methylprednisolone and cyclophosphamide) might improve the prognosis of systemic lupus erythematosus with transverse myeltis.

System dynamics simulation of the thermal dynamic processes in nuclear power plants

  • A nuclear power plant (NPP) is a highly complex system-of-systems as manifested through its internal systems interdependence. The negative impact of such interdependence was demonstrated through the 2011 Fukushima Daiichi nuclear disaster. As such, there is a critical need for new strategies to overcome the limitations of current risk assessment techniques (e.g. the use of static event and fault tree schemes), particularly through simulation of the nonlinear dynamic feedback mechanisms between the different NPP systems/components. As the first and key step towards developing an integrated NPP dynamic probabilistic risk assessment platform that can account for such feedback mechanisms, the current study adopts a system dynamics simulation approach to model the thermal dynamic processes in: the reactor core; the secondary coolant system; and the pressurized water reactor. The reactor core and secondary coolant system parameters used to develop system dynamics models are based on those of the Palo Verde Nuclear Generating Station. These three system dynamics models are subsequently validated, using results from published work, under different system perturbations including the change in reactivity, the steam valve coefficient, the primary coolant flow, and others. Moving forward, the developed system dynamics models can be integrated with other interacting processes within a NPP to form the basis of a dynamic system-level (systemic) risk assessment tool.

Comparison of Residue Patterns for Systemic and Non-systemic Pesticides in Strawberry (딸기 중 침투성 및 비침투성 농약에 따른 잔류특성 연구)

  • BACKGROUND: The healthy food trend has encouraged the consumption of natural products, including berries. This trend is expected to increase the strawberry consumption. There has been a concern about the exposure of pesticides approved for use on strawberry. In this study, the dissipation patterns of systemic and non-systemic pesticides were evaluated in strawberry under plastic-covered greenhouse conditions. METHODS AND RESULTS: Cyflumetofen and dimethomorph were applied on strawberry in the critical GAP (Good Agricultural Practices). Strawberries were harvested at 0, 1, 2, 3, 5, 7 and 10 days after final application of the pesticides. The analyses of the residual pesticides were performed by HPLC-DAD with C18 column. The limits of quantitation (LOQ) of cyflumetofen and dimethomorph were 0.04 and 0.02 mg/kg, respectively. The recovery of cyflumetofen and dimethomorph were 88.1 ~ 103.3% and 79.0 ~ 110.2% for the spiked two levels (LOQ and 10LOQ), respectively. The biological half-lives of cyflumetofen and dimethomorph werer 7.5 and 8.9 days, respectively. The dissipation rates in strawberry were calculated by the statistics method at a 95% confidence level. The distribution showed that pesticides with low log Pow were indicated by the decreased dissipation rate and pesticides with similar log Pow and low solubility also showed the decreased dissipation rate. CONCLUSION: The residues of cyflumetofen and dimethomorph in strawberry at time 0 after the final application were below the established MRL in Korea. The dissipation behavior of systemic and non-systemic pesticides in strawberry is affected by their log Pow and water solubility values.
