간질 치료에서 뇌파의 임상적 유용성에 관한 논란: 부정적 관점에서 (Controversies in Usefulness of EEG for Clinical Decision in Epilepsy: Cons.)

  • Electroencephalogram (EEG) is a representative diagnostic tool in epilepsy. However, there are several points of debate on the role of EEG in diagnosis and management of epilepsy. We suggest that EEG has some limitations for differential diagnosis from nonepileptic episodic diseases, classification of epilepsy, prediction of recurrence, and evaluation of treatment response. Interictal EEG cannot diagnose or exclude epilepsy because interictal epileptic discharge (IED) is frequently absent in epilepsy and can appear in nonepileptic conditions. Although EEG is helpful in classification of epilepsy, focal spikes in generalized epilepsy and secondary bilateral synchrony in localization related epilepsy cause interrater disagreement. It is controversial whether EEG predicts recurrence after the first seizure in adults. The predictive value of EEG in antiepileptic drug (AED) withdrawal is not absolute. The prognosis after AED withdrawal depends on epilepsy syndrome. Many studies could not confirm the value of EEG in assessing the treatment response. After all, epilepsy is clinically diagnosed and assessed. Interictal EEG alone does not provide decisive information and routine follow-up of EEG is not recommended.

Antiepileptic Drugs in Children : Current Concept

  • An epileptic seizure is defined as the transient occurrence of signs and/or symptoms due to abnormally excessive or synchronous neuronal activity in the brain. The type of seizure is defined by the mode of onset and termination, clinical manifestation, and by the abnormal enhanced synchrony. If seizures recur, that state is defined as epilepsy. Antiepileptic drugs (AEDs) are the mainstay of treatment. Knowledge about initiating and maintaining adequate AEDs is beneficial for the clinician who treats children with epilepsy. This article will delineate the general principles for selecting, introducing, and discontinuing AEDs and outline guidelines for monitoring adverse effects. In general, AED therapy following a first unprovoked seizure in children is not recommended. However, treatment should be considered after a second seizure. In children and adolescents, if they are seizure-free for at least 2 years, attempts to withdraw medication/s should be made, taking into account the risks vs. benefits for the individual patient. The decision on when and what AED to use should be tailored according to the patient. For optimal treatment, the selection of adequate AEDs can be achieved by considering the precise definition of the patient's seizure and epilepsy syndrome. Continuous monitoring of both therapeutic and adverse effects is critical for successful treatment with AEDs.

Coherence Analysis of Jaw and Neck Muscle Coordination during Chewing in Healthy Adults

  • Purpose: Coordinated activity between the jaw and neck muscles is important in oral motor tasks such as chewing. This study examined coherence between the jaw and neck muscles during chewing in healthy adults. Methods: A total of 12 healthy adults underwent electromyography (EMG) of the jaw and neck muscles during right-sided chewing at a frequency of 1 Hz. Surface electrodes were placed over the temporalis (TA), masseter (MS), anterior digastric (DA), and sternocleidomastoid (SM) muscles on the right side. EMG signals were processed for coherence and phase analysis using advanced signal processing techniques. Results: The MS and TA muscle pair exhibited high synchronization when chewing (median coherence=0.992). Contrarily, the coherence values between the MS and DA, as well as the MS and SM muscle pairs, were relatively low (median coherence=0.848 and 0.957, respectively). Phase analysis revealed minimal temporal differences between the MS and TA muscle pair and the MS and SM muscle pair, whereas substantial phase shifts were observed between the MS and DA muscle pair. Conclusions: During chewing in healthy adults, the TA muscle works synergistically whereas the DA muscle antagonistically with the MS muscle, and the SM muscle supports the activity of the MS muscle. The observed synchrony and coordination provide insights into the intricate interplay among these muscles during oral motor tasks.

In Vivo Development of Vitrified Rat Embryos: Effects of Timing and Sites of Transfer to Recipient Females

  • In cryopreserved rat embryos, survival rates obtained in vitro are not always consistent with the rates obtained in vivo. To determine the optimal conditions for in vivo development to term, rat embryos at the 4-cell, 8-cell and morula stages were vitrified in EFS40 by a 1-step method and transferred into oviducts or uterine horns of recipients at various times during pseudopregnancy. Vitrified and fresh 4-cell embryos only developed after transfer into oviducts of asynchronous recipients on Day -1 to -2 of synchrony, i.e., at a point in pseudopregnancy that was 1-2 days earlier than the embryos. However, although about half the vitrified embryos transferred into oviducts on Day -1 developed to term, only a minority of embryos transferred at later times did so, whether vitrified (10-34%) or fresh (24-33%), suggesting that this may not be the most suitable stage for cryopreservation. Very few 8-cell embryos, either vitrified or fresh, developed when transferred into oviducts on Day 0 to -0.5. However, when transferred into uterine horns, high proportions of vitrified 8-cell embryos (-63%) developed to term in reasonably synchronous recipients (Day 0 to -0.5) but not in more asynchronous ones (6%; Day-1). A majority of vitrified morulae also developed to term (52-68%) in a wider range of recipients (Day 0 to -1), the greatest success occurring with recipients on Day -0.5. Similar proportions of vitrified and fresh 4-cell embryos, 8-cell embryos and morulae developed to term when there was appropriate synchronization between embryo and recipient. Thus vitrification of preimplantation stage rat embryos does not appear to impair their developmental potential in vivo.

수정란이식에 있어서 수란우의 준비와 관리 (Preparation and Management of Recipients in Bovine Embryo Transfer)

  • Recipients are an integral part of embryo transfer and they are expensive to maintain as a good recipient. Recipient management is one of the most important components in a successful embryo transfer program. Management includes selection and subsequent care of the animals. A good recipient is basically on "open" cows or heffers whose reproductive tract is capable of receiving one or two embryos and incubating it to term. Potential recipients should be always be healthy and cycling normally ranging from 18 to 23 days. A thorough veterinary examination is recommended for candidate of recipients and cattle for questionable health should be eliminated from the recipient herd. Age and size of recipients are particularly important considerations when heifers are used, because of most embryos available for transfer are from large dams and sires. Body condition can influence a recipient's production, reproduction and health. Obese and underconditioned cattle should be avoided for use. Transfer of fresh embryos especially requires precise synchronization of donors and recipients. For estrus synchronization, PGF$_2$$\alpha$ is injected twice 10 to 12 days apart and short4erm progestagen treatment is applied to potential recipient cattle by coil into vagina (PRID) or ear implant (Synchro-Mate-B). The highest pregnancy results are achieved in recipients at exact synchrony with donors or 12 to 24 hr earlier than donors. Estrus detection is a major factor in breeding efficiency. High accuracy can be achieved by use of heat mount detection alds or by obserbing cattle for 30-minute peroids 3 times daily. Assay progesterone in milk can be used to discrIminate between pregnant and nonprenant recipients. Rectal palpation on day 35 to 70 after is an accurate and safe method of pregnancy diagnosis. Embryonic mortality in recipients may be associated with factors such as high environmental temperature and nutritional or lactational stress in early lactation period. Achievement of short calving interval requires concentrated management activity during the first 90 days following calving. Acceptable candidate for a recipient should be routinely vaccinated for infectious diseases. Proper nutritional programs according to NRC requirements and body condition scoring system for recipient cattles are vital to the ultimate success of an embryo transfer program.r program.

소의 수정란이식에 관한 연구 (Studies on Embryo Transfer in Cattle)

  • This study was carried out to produce superior dairy cattle by embryo transfer. Seven dairy cows were superovulated with divided injection of FSH 4Omg for 5 days started on day 9 to 14 of the estrus cycle and injection of PGF$_2$$\alpha$ 45mg on day 4 of FSH injection. Donor cows were flushed to collect embryos on day 7 or 8 of the estrus cycle. Fresh embryos collected were transferred to synchronized dairy recipients or frozen using glycerol 3 step method to he equilibrated. And 35 embryos which were frozen using glycerol 6 step method were imported from U.S.A. After glycerol dilution of frozen embryos was done by reverse density during freezing. frozen-thawed embryos were transferred to synchronized dairy or beef recipients. The results obtained were as follows; 1. Total of 24 embryos were collected from 7 donor cows flushed and transferable embryos were 18 (75.0%). 2. Among 24 embryos. morula, early blastocyst, blastocyst, expanded blastocyst and unfertilized ova were 3 (12.5%), 1 (4.2%), 10 (41.6%), 4 (16.7%) and 6 (25.0%), respectively. 3. Heat inducing rate after 1st and 2nd injections of PGF$_2$$\alpha$ in Holstein and beef cattle was 83.3% and 71.4% and 62.5% and 69.2%, respectively. 4. Among 56 recipients, 23 head were pregnant (41.1%). The pregnancy rate of fresh embryos was 50.0% (1/2 heads) and the pregnancy rate of frozen embryos which were frozen using glycerol 3 step and using glycerol 6 step imported from U.S.A. was 52.6%(l0/19 heads) and 34.3%(12/35 heads), respectively. 5. The pregnancy rate of blastocyst (60.0%) was higher than that of morula (39.0%), early blastocyst (25.0%) and expanded blastocyst (0%). 6. The pregnancy rate of grade I embryos (52.2%) was higher than that of grade 2 (34.6%) and grade 3 (28.6%). 7. The pregnancy rate according to synchrony of recipient with donor was higher in simultaneous recipient (55.0%) and +l2hrs' (53.8%) than -24hrs' (23.5%), -l2hrs' (20.0%) and +24hrs' (0%).

Timed Artificial Insemination or Embryo Transfer using CIDR, Estradiol Benzoate and Prostaglandin $F_{2{\alpha}}$ for the Rebreeding of Korean Native Donor Cattle

  • This study compared the pregnancy rates of Korean native donor cattle after either a timed artificial insemination (TAI) or embryo transfer (TET) following the synchronization of ovulation using a controlled internal drug release (CIDR) device together with estradiol benzoate (EB) and prostaglandin $F_{2{\alpha}}$ ($PGF_{2{\alpha}}$). Fifty five cows and 8 heifers which had been previously used for embryo production were assigned to two treatments: (1) Thirty-two cattle received a CIDR device and 2 mg EB (Day 0), 25 mg $PGF_{2{\alpha}}$ injection at the time of CIDR removal on Day 7, and 1 mg EB injection on Day 8. All of the cattle received a TAI 30 h (Day 9) after the second EB injection (TAI group). (2) Thirty-one cattle received the same hormonal treatments as in the TAI group. The cattle with corpus luteum (CL) received a TET on Day 16 using frozen-thawed embryos (TET group). Ultrasonographic observations demonstrated that the proportion of cattle with synchronized ovulation on Day 10 and the concomitant formation of new CL on Day 13 did not differ between groups (p>0.05); the overall mean rates were 65.1 and 73.0%, respectively. The conception and pregnancy rates did not differ (p>0.05) between the TAI (12.5% and 12.5%) and TET groups (13.0% and 9.7%), respectively. We conclude that the pregnancy rate following TAI or TET in Korean native donor cattle was poor, which might be due in part to a poor synchrony of ovulation and concomitant CL formation.

  • 라이브 시네마 연극(Live cinema theatre)은 연극과 영상 매체가 결합된 공연 형태로서 하나의 극영화를 만드는 과정이 라이브 이벤트로서 무대화 된 것이다. 여기서 배우는 카메라를 보며 연기하며 모든 시청각적 효과는 라이브로 조합된다. 무대는 실시간으로 장면이 구성 편집되고 있는 프로덕션 상황으로서 제시되며 연극은 카메라를 매개로 스크린을 통해 관객 앞에 생중계된다. 영국 연출가 케이티 미첼(Katie Mitchell)은 이야기 전달 및 공유 과정을 효율화한다는 목적의식 아래, '라이브 시네마 연극'이란 특정용어로 작업 형태를 구별 지칭하며 이를 장르문법으로 이끌어내고 있다. 여기서 '시네마'와 '연극'은 사이성의 실험을 넘어 동기화(synchronization)를 통해 수평적 관계를 맺음으로써 각자의 위상을 유지한다. 이는 서로 다른 영역에서 독립적으로 존재하는 연극 영화의 결합을 통해 기술매체 시대에 총체의 의미를 재규정하려는 시도를 보여준다. 여기서 카메라는 무대 안팎에 현존하고 몸은 이러한 상황 안에 실존하면서, 카메라-몸-무대는 서로 불가분한 관계를 구축한다. 무대 배우들은 기술적이고 심리적인 상태를 넘나들며 카메라의 현존을 체화하고, 스텝들은 몸에서 내면성을 분리하여 외현화하며, 연출은 이를 프레임 단위로 조직함으로써 무대 위 존재양식은 뉴미디어와의 관계 안에서 재구성되고 있다.

불면증에서 순환교대파형의 의미 (Cyclic Alternating Pattern : Implications for Insomnia)

  • The cyclic alternating pattern (CAP) is a periodic EEG activity in NREM sleep, characterized by sequences of transient electrocortical events that are distinct from background EEG activities. A CAP cycle consists of two periodic EEG features, phase A and subsequent phase B whose durations are 2-60 s. At least two consecutive CAP cycles are required to define a CAP sequence. The CAP phase A is a phasic EEG event, such as delta bursts, vertex sharp transients, K-complex sequences, polyphasic bursts, K-alpha, intermittent alpha, and arousals. Phase B is repetitive periods of background EEG activity. The absence of CAP more than 60 seconds or an isolated phase A is classified as non-CAP. Phase A activities can be classified into three subtypes (A1, A2, and A3), based on the amounts of high-voltage slow waves (EEG synchrony) and low-amplitude fast rhythms (EEG desynchrony). CAP rate, the percentage of CAP durations in NREM sleep is considered to be a physiologic marker of the NREM sleep instability. In insomnia, the frequent discrepancy between self-reports and polysomnographic findings could be attributed to subtle abnormalities in the sleep tracing, which are overlooked by the conventional scoring methods. The conventional scoring scheme has superiority in analysis of macrostructure of sleep but shows limited power in finding arousals and transient EEG events that are major component of microstructure of sleep. But, it has recently been found that a significant correlation exists between CAP rate and the subjective estimates of the sleep quality in insomniacs and sleep-improving treatments often reduce the amount of CAP. Thus, the extension of conventional sleep measures with the new CAP variables, which appear to be the more sensitive to sleep disturbance, may improve our knowledge on the diagnosis and management of insomnia.

  • 뇌하수체의 성선자극 홀몬 분비 세포는 혈액내 estradiol양의 변화에 따라 LH와 FSH를 분비하므로써 주기적인 변화를 보인다. 성선 자극 홀몬의 분비는 성선 자극 홀몬을 합성하고 보유하는 두가지 형태의 능력의 크기에 의하여 조절되며 이들을 조절하는 것은 시상하부에서 분비되는 황체형성 홀몬-분비 홀몬(LH-RH)과 난소에서 분비되는 estradiol이다. LH-RH는 성선 자극 홀몬 분비세포에 작용하여 성선 자극 홀몬 합성, 저장 및 분비를 촉진시키며 estradiol은 LH-RH의 기능을 확대하고 LH-RH가 self-priming효과를 나타내도록 유도하기도 하며 LH-RH의 성선 자극 홀몬 분비 기능을 저해하기도 한다. Estradiol은 기저성 성선 자극 홀몬을 분비시키기 위하여 negative feedback작용을 하고 배란성 성선 자극 홀몬을 분비시키기 위하여는 positive feedback작용을 하며 feedback작용 부위는 시상하부 및 뇌하수체 전엽이다. 또한, estradiol이 feedback작용을 하여 성선 자극 홀몬의 분비를 조절하는 데는 LH-RH뿐만 아니라 중추신경-시상하부에서 분비되는 dopamine, norepinephrine, prostaglandin등이 참여한다.
