Odontoid Synchondrosis Fracture Treated by C1-2 Polyaxial Screw-Rod Fixation

  • The synchondrosis between the dens and the body of axis normally fuses between 5 and 7 years of age. Until this age, synchondrosis fractures can occur in children. Most synchondrosis fractures are conventionally treated by external immobilization alone. We report a 10-year-old child with odontoid synchondrosis fracture who was treated by C1 lateral mass and C2 pars screw rod fixation with a successful outcome and discuss the possible reasons for occurrence of odontoid synchondrosis fracture in this older child as well as the indications for surgery in this condition.

Fabrication of Stable Cartilage Framework for Microtia in Incomplete Synchondrosis

  • The synchondrosis between the sixth and seventh costal cartilage is usually used for the base frame in autogenous ear reconstruction. If the synchondrosis is loose, a variety of modifications can be devised. This report introduces new methods for these problems. In cases of incomplete synchondrosis, only the surface of the base block margin was smoothly tapered without carving for the removal of the conchal deepening. The secure fixation of the two segments (helix and antihelix) to the base block using fine wire sutures gave stability to the unstable basal frame. After confirming that all the segments were assembled in one stable piece, the remaining conchal deepening of the basal framework was removed, and the outer lower portion of the basal cartilage was trimmed along its whole length. A total of 10 consecutive patients with microtia, ranging from 8 to 13 years old, were treated from 2008 to 2009. The follow-up period was 6 months to 2 years. Despite incomplete synchondrosis, the stable frameworks were constructed using the authors' method and aesthetically acceptable results were achieved. The proposed method can provide an easy way to make a stable cartilage framework regardless of the variable conditions of synchondrosis.

하악두연골의 성장에 관한 시험관적 연구 (GROWTH OF MANDIBULAR CONDYLAR CARTILAGE OF THE RAT IN VITRO)

  • The purpose of this study was to evaluate the effect of intrinsic factor and extrinsic factor for growth of the mandibular condylar cartilage of 4 day-old rats in a serum-free medium for 1, 4, 7, 14 days. They were compared with normal growth in vivo and with growth of spheno-occipital synchondrosis in serum-free medium. The cellular kinetics of cartilages were evaluated by auto-radiography of tritiated thymidine. 1. Condylar cartilage was enlarged with rounded head on day 14 of experiment while in vivo the rounded-headed shape changed into functionally flattened appearance. 2. On day 14 of experiment, a severe reduction of the proliferative zone and a considerable increase of the hypertrophic zone were observed while in normal control group endochondrol bone formation and bone marrow were observed. 3. The proliferative activity in the proliferative zone of condylar cartilage detected by $^3H-thymidine$ incorporation was lower than that of normal control group and decreased more than that of spheno-occipital synchondrosis, but it continued during the 14 days of culture. 4. The continued maintenance of condylar cartilage and morphologic change were disturbed in this culture system, but cell division within the proliferative zone was continued and probably linked to intrinsic factor.

성인의 증세가 있는 부주상골 (The Symptomatic Accessory Navicular in Adult)

  • Purpose: to investigate the etiology and the results of surgical treatment of the symptomatic accessory navicular in adults. Materials and Methods: Between 1996 and 2000, 17 cases in 16 adult patients who were older than 20 years were diagnosed as painful accessory na vicular. 11 patients could recall a twisting injury of the ankle, and 8 of them were inversion sprain. 4 patients had tibialis posterior tendon lesions. 13 feet of 12 patients were treated by resection of accessory navicular, the synchondrosis, the medial portion of the navicular and reattachment of tibialis posterior tendon without transposition. 9 feet in 8 patients were followed for more than one year after surgery. In 4 patients with tibialis posterior tendon lesions, additional procedures were performed according to the state of the lesion. Results: All were type II accessory navicular bone which had synchondroses. There was gross motion of the synchondrosis in 'the operating field in all feet. Of the 9 feet which were followed for more than one year after surgery, results were excellent in five and good in four. Conclusion: The painful accessory navicular in adult might be closely associated with inversion ankle sprain, and also with the tibialis posterior tendon lesions. Satisfactory result could be obtained without transposition of the tibialis posterior tendon to the undersurface of the navicular and immediate postoperative weight bearing does not have harmful effect on the result.

완전 편측성 순구개열자의 두개저 성장과 상,하악골 위치의 상관 관계에 관한 연구 (The Relationship between the Growth of Cranial Base and the Position of Maxilla, Mandible in Complete Unilateral Cleft Lip and Palate Patients)

  • 본 연구는 연령 증가에 따른 구순구개열자의 두개저 성장 특징과 비상악복합체 및 하악골의 위치 변화와의 상관관계를 알아보고자 시행되었다. 교정치료 및 악정형적 치료의 경험이 없는 100명의 완전 편측성 구순구개열 남녀와 악골부조화가 없으며 교정치료, 악정형적 치료 및 악교정 수술의 경험이 없는 100명의 정상 남녀를 선정하고, 이들을 접형사골연골결합 성장기, 접형후두연골결합 성장기 그리고,연골결합 성장종료기의 세 연령집단으로 구분하였다. 이들의 측모두부규격방사선사진을 연령별로 비교하여 두개저와 비상악복합체 및 하악골 위치와의 관계를 비교하였고 이를 통해 완전 편측성 구순구개열자가 정상인에 비하여 다음과 같은 차이가 있음을 알 수 있었다. 1. 완전 편측성 구순구개열군에서 사춘기 시기까지 사대 길이 및 두께가 작게 나타났고, 사대 길이의 사춘기 시기 증가량 또한 정상군 보다 작게 나타났다. 두개저 전방 길이 및 사대의 연령에 따른 회전 양상은 정상군과 차이가 없었다. 2. 완전 편측성 구순구개열군에서 비상악복합체는 모든 연령에서 정상군 보다 후방 위치를 보였고, 특히 성장이 활발한 사춘기 시기까지 정상군과의 차이가 보다 크게 나타났다. 반면 수직적 위치는 정상군과 차이가 없었다. 이와 같은 비상악복합체의 전방 성장 결여는 두개저 형태와 깊은 상관관계를 나타내었다. 또한 상악골 자체의 후방성장을 나타내는 PtmS은 정상군과 달리 연령에 따른 증가량이 작아 상악결절부의 성장 결여를 나타내었다. 3. 완전 편측성 구순구개열군에서 하악골은 정상군과 차이가 적었으나, Articular angle이 정상군 보다 매우 작게 나타났고, 하악골 위치와 비상악복합체 및 두개저 계측 항목간에 높은 상관관계를 보였다.

Holstein 송아지의 두얼굴증(Diprosopia) 1례 (A case of diprosopia of Holstein calf)

  • A 20-day-old female Diprosopia in Holstein calf was observsd macroscopically and radiographically. Anterior head duplication(Diprosopia) was observed and all structures caudal to it were normal. She had two muzzles, three eyes, two ears and two tongues. The tongues were joined at their base just posterior and had one hyoid bone. Schistognathia and cleft lips were also observed in the lower jaws. She had an oral cavity, single epiglottis and a laryngeal cavity communicated with one trachea. The cerebral hemispheres were duplicated and fused together in the caudal region. The olfactory and optic nerves were duplicated. The incisive, nasal, pterygoid, parietal, ethmoid bones and vomer were duplicated, respectively. The skull of Diprosopia was shared by an occipital bone. The mandibules, palatines and sphenoid bones were duplicated incompletely. Three orbits and two fontanelles were observed. The medial lacrimal bones and maxillae were duplicated incompletely and fused with each other, respectively. Cleft palates were observed. Medial mandibular mass which was fused together and was duplicated incompletely at the part of its cranial, was shown synchondrosis with the left mandible, but not with the right.

I급과 III급 부정교합 어린이의 두개안면골 형태의 변화에 관한 연구 (A STUDY ON THE MORPHOLOGICAL CHANGES IN CHILDREN WITH CLASS I AND III MALOCCLUSIONS: CEPHALOMETRIC ANALYSIS)

  • I급과 III급 부정교합 어린이 두개안면골의 성장양상을 비교하고자, $7{\sim}11$세 146명의 치료전 측모두부계측분석에서 연령에 따른 두개저, 중안면, 하안면 구조물들의 차이를 평가하여 다음과 같은 결과를 얻었다. 1. III급은 I급에 비해 접사골연골결합의 성장이 더 늦은 시기까지 지속되었다. 2. 비상악복합체 전후방적 길이는 III급이 대체로 짧았고 높이는 유사하였다. 3. 하악골 길이와 하악체 길이는 III급이 모든 연령에서 다소 긴 경향이었으나 통계적 유의성은 없었으며, I급과 유사한 성장양상이었다. 4. 상하악골의 치아치조골 높이는 I급과 III급이 대체로 유사한 성장양상이었다. 5. I급은 전두개저 길이가 비상악복합체 및 하악골의 전후방 길이 증가에 뚜렷한 역할을 하였다. 6. III급의 전두개저 길이와 중두개저 길이는 연령이 증가하면서 하악골 길이와 상관성이 커졌다. 사춘기 이전의 시기에는 III급이 I급과 부분적으로 다른 골격적 특징을 지니고 있으나, 성장양상은 전반적으로 유사하게 나타났다. 따라서 III급의 골격성 부조화는 특징적인 골격적 차이가 나타나기 전 조기에 개선시키는 것이 바람직할 것으로 판단된다.
