Effects of Increasing the Venous Return on the Heart Rate in the Water Turtle - Myogenic Regulatory Mechanisms in the so-called Bainbridge Reflex - (자라에 있어서 정맥환류량(靜脈還流量)의 증가(增加)가 심박(心博)에 미치는 영향(影響))

  • By the intravenous infusion of saline solution through the postcaval vein, the effects of increasing the venous return on the heart rate were studied in the water turtle (Amyda japonica). The following results were obtained: 1) Prior to saline infusion, when the initial heart rate was below $50{\sim}55/min$ the heart rate was increased by the infusion. When the initial rate was above this value no changes in heart rate were observed following the infusion. 2) When the heart rate was decreased by vagal stimulation, the infusion elicited a remarkable increase in the heart rate. 3) Increased heart rate caused by tile infusion was not affected by vagotomy or sympathectomy. 4) These results suggest that the increase in heart rate secondary to increased venous return is under the control of a myogenic regulatory mechanism, not a neural mechanism.

  • Excessive sweating, especially hyperhidrosis of palms, solies and axillae, is a distressing problem in the making of a person's social life. Most important in cases of localized hyperhidrosis is the emotional factor. Thoracoscopic sympathectomy is an effective method, but this may result in a postoperatively compensatory hyperhidrosis of facial anhidrosis in the treatment of localized hyperhidrosis. One case of excessive sweating of palms, solies and axillare was observed and treated with the emotional factor taken as primary. This patient was treated with Bokryungbosimtang-gamibang and improvement was seen.

  • Causalgia is an extremely incapacitating disease often associated with a major peripheral nerve injury, which is characterized by sustained diffuse burning pain, allodynia and hyperpathia. The condition follows traumatic nerve lesions, often combined with vasomotor and sudomotor disturbances and later trophic changes. While sympathectomy has been the classical treatment of causalgia, others nonsurgical therapies such as regional sympathetic block, IV regional sympathetic block, oral adrenolytic drugs, transcutaneous electrical nerve simulation, physical theraphy, cryotheraphy and psychotheraphy have been used. Causalgia is rare in children and early treatment is controversial because of the possibility of many different complications following aggressive treatment. This is a report of a 6-year-old girl with causalgia suffered after a right posterior tibial nerve injury following an intragluteal injection of an antipyretics. We successfully treated this syndrome with continuous epidural block using 0.5% lidocaine and no specific complication was encountered.

  • The first reported the neural blockade of ganglion impar for pain control of perineal pain in 1990 by Plancarte and his fellows. they used 6ml of 10 percent phenol. but the point of issues, same as other neurolytics, are that it is impossible to check and control its spreading, so it might be possible to destruct the coccygeal plexus and sacral nerve, and also it has only short action time. Because of these problems, it could be very dangerous to attempt this procedure especially not for relieving the pain on cancer terminal patient, but for the sympathectomy of ganglion impar on the other purpose. We used the RF generator which had the control ability to point out the destructive lesion accurately. inserted We made the small burr hole on the sacrum near the sacrococcygeal junction directly, through the hole, and performed thermocoagulation to the ganglion impar.

  • Postherpetic neuralgia (PHN) is defined as the persistence of pain after recovery from herpes zoster (HZ), when the rash has healed, usually after about 4 weeks. PHN is the most feared complication of herpes zoster and remains one of the most common and intractable chronic pain disorders. Recent evidence has shed some light on the possible mechanism of pain, and on the prophylactic and treatment approaches to PHN, but there is no secure therapy. This report is a case of a 70-year-old male with PHN, affecting the 8th to 10th thoracic dermatomes. Patient complains of allodynia and hyperalgesia on the affected skins. After sympathectomy, antidepressant, anticonvulsant, and capsaicin ointment application, much pain relief was achieved, but allodynia remained at the subcostal area about $7\times3 cm^2$ in size. We decided to remove the painful area. Skin excision was done under local anesthesia. After skin excision, the pain was decreased and patient did not complain of pain for 10 months.

  • Facial hyperhidrosis has a symptom of excessive sweating on the face with or without underlying disease. It can be surgically treated by video-assisted thoracic surgery(VATS). We encountered three cases of facial hyperhidrosis which we treated by VATS, which was performed by resection of the lower third of stellate ganglion and T2-T3 sympathetic ganglia with chains. Postoperative symptom was improved in all cases. There were no postoperative complications such as Horner's syndrome or postsympathectomy neuralgia.

  • Leriche syndrome is a chronic disorder mostly in male and is a specific symptom complex due to thrombotic obliteration of the aortic bifurcation as result of a atherosclerotic change. The symptoms include 1) extreme liability to fatigu of both lower limbs; 2) symmetric atrophy of both lower limbs; 3) pallor of the legs and feets; 4) inability to maintain stable erection. There are several methods for surgical management of this disorder; 1) thromboendarterectomy with or without sympathectomy; 2) aortoiliac bypass graft; 3) aortofemoral bypass graft. Here we report a case of Leriche syndrome which was successfully managed with arotobifemoral bypass graft with Dacron and reviewed literatures on it briefly.

  • Rectal tenesmus is a persistent, painful and ineffectual sensation of straining at stool or opening of the bowels. The pain is usually spasmodic in nature and most commonly encountered in patients with carcinoma of the rectum or other pelvic organs. In 1988, Bristowand Foster reported that patients with severe spasmodic painful tenesmus were relieved with chemical sympathectomy. In 1990, Plancarte introduced block of Ganglion impar. This technique is proposed as an alternative means of managing localized perineal pain of sympathetic origin. Ganglion impar block was performed on a 54-year-old female patient when analgesic or psychotropic drugs failed to control the symptoms of post-traumatic severe spasmodic painful tenesmus. Postoperatively, patient was free of tenesmoid pain for only 7 days. We then performed neurotomy by RF lesion generator which provided complete pain relief.

  • Management of chronic wound has been a great problem to many surgeons because the wound is usually associated with an underlying disease of the patient. Without accurate diagnosis and treatment of the disease, the wound can not be healed. Systemic vasculitis is a rare systemic disease which causes inflammation and obstruction of the vessels. This autoimmune disease involves multiple organs and may inflict skin wound spontaneously without traumas. It would improve or aggravate the wound in proportion to the activities of the disease. Our experience is a case of 28-year-old female who has had chronic ulcers on her right foot, especially on the great toe for 1 year. Although she had several operations of sympathectomy, debridement and artificial dermal graft, her wound was not improved. She has been diagnosed as systemic vasculitis during the evaluation for histopathology and cause of fever and pancytopenia. After medical treatments, she had the operation of adipofascial turnover flap coverage and skin graft, and the wound was improved without any complication or relapse. The diagnosis and treatment of the underlying disease should be ahead of the management of chronic wound.

  • 1. Objectives The Excessive sweating or Hyperhidrosis is a state of abnormal sweating on palm, sole and axillary region. But we do not know the exact cause and the healing method of that. It is known that the only treatment of Excessive sweating is surgical sympathectomy and a thoracic sympathetic gan-glion block with neurolytics. 2. Methods We have three case-reports of the patients who have the excessive sweating. 3. Results In this study the patients classified by Sasang constitutional medicine had a notable medical effects. And in the result, any symptom is not remained. 4. Conclusions So we report the healing processes and results of these patients in this study.
