밸런스 패드와 모래에서의 균형운동이 뇌졸중 환자의 균형과 보행에 미치는 영향 (The effects of balance training on balance pad and sand on balance and gait ability in stroke patients)

  • PURPOSE: The purpose of this study was to determine the effects of balance training on balance pad and sand on balance and gait ability in stroke patients. METHODS: Sixty stroke patients were divided into a Balance Pad group(BPG, N = 20), a Sand group (SG, N = 20) and a Hard Ground group (HGG, N = 20) randomly. The subjects in the Hard Ground group stood in a comfortable position, faced a therapist, then threw a Swiss ball back and forth. They then performed balance training in which they raised and lowered their ankles while facing forward or moved objects from one table to another. The BPG performed same tasks in HGG, on an unstable surface using a balance pad. The SG performed same tasks on sand ground. All groups received training 30min per day, five times per week, for eight weeks. RESULTS: After intervention, all groups showed significant increases balance and gait components. And the BPG and the SG showed significant increase in weight distribution rate, Sway length and BBS compared with the HGG, but there was no significant difference in Cadence, Stride length among three groups. CONCLUSION: According to the results of this study, balance training on unstable surface using balance pad and sand was more effective in improving balance in stroke patients.

Improvement of dynamic responses of a pedestrian bridge by utilizing decorative wind chimes

  • A novel approach is presented to improve dynamic responses of a pedestrian bridge by utilizing decorative wind chimes. Through wind tunnel tests, it was verified that wind chimes can provide stabilization effects against flutter instability, especially at positive or negative wind angles of attack. At zero degrees of angle of attack, the wind chimes can change the flutter pattern from rapid divergence to gradual divergence. The decorative wind chimes can also provide damping effects to suppress the lateral sway motion of the bridge caused by pedestrian footfalls and wind forces. For this purpose, the swing frequency of the wind chimes should be about the same as the structural frequency, which can be achieved by adjusting the swing length of the wind chimes. The mass and the swing damping level are other two important and mutually interactive parameters in addition to the swing length. In general, 3% to 5% swing damping is necessary to achieve favorite results. In the study case, the equivalent damping level of the entire system can be increased from originally assumed 1% up to 5% by using optimized wind chimes.

The stability of semi-rigid skeletal structures accounting for shear deformations

  • The analysis and design of skeletal structures is greatly influenced by the behaviour of beam-to-column connections, where patented designs have led to a wide range of types with differing structural quantities. The behaviour of beam-to-column connections plays an important role in the analysis and design of framed structures. This paper presents an overview of the influence of connection behaviour on structural stability, in the in-plane (bending) mode of sway. A computer-based method is presented for geometrically nonlinear plane frames with semi-rigid connections accounting for shear deformations. The analytical procedure employs transcendental modified stability functions to model the effect of axial force on the stiffness of members. The member stiffness matrix were found. The critical load has been searched as a suitable load parameter for the loss of stability of the system. Several examples are presented to demonstrate the validity of the analysis procedure. The method is readily implemented on a computer using matrix structural analysis techniques and is applicable for the efficient nonlinear analysis of frameworks. Combined with a parametric column effective length study, connection and frame stiffness are used to propose a method for the analysis of semi-rigid frames where column effective lengths are greatly reduced and second order (deflection induced) bending moments in the column may be distributed via the connectors to the beams, leading to significant economies.

Balance Evaluation after Reconstruction of Medial Patellar Luxation in Small-Sized Dogs with Wii Balance Board

  • Wii® balance board (WBB, Nintendo, Japan) is a device that can measure and record the center of pressure path length (CPPL) and 95% confidence ellipse area (Area 95) in relation to body sway. For evaluating measure of improvement after reconstruction of medial patellar luxation (MPL) in small sized dogs, A total of 6 dogs with limping and lameness gait attributed to Grade II, III or IV MPL were evaluated. Dogs were measured for difference of extension and flexion range of motion in the stifle (dROM), muscle mass, lameness, willingness to bear weight on the affected limb while standing, and willingness to lift the contralateral limb scores, CPPL and Area 95 of WBB on pre-surgery, post-surgery 4, 8 weeks. CPPL was significantly different on pre-surgery compared with post-surgery 8 weeks (p < 0.05). Except for CPPL, measured variables were significantly different on pre-surgery compared with post-surgery 4 and post-surgery 8 weeks (p < 0.01).

Effects of Removable Ankle-Foot Orthosis in Chronic Patients With Hemiplegia During Gait Training: A Pilot Study

  • This study was conducted to investigate the effects of the removable ankle-foot orthosis (RAFO) which was developed to improve the gait of stroke patients. The subjects of this study were five stroke patients who agreed to participate in this study by signing a written consent form. To verify gait improvement after wearing the orthosis, a Timed Up and Go test and Functional Gait Assessment were performed, and spatiotemporal gait variables such as gait speed, cadence, stride length, double limb support, and the efficient gait test of body sway angle were performed. For every variable, the differences prior to and after wearing the RAFO were compared using the Wilcoxon signed-rank test. Every gait variable improved significantly after wearing the RAFO compared to prior to wearing it. The pilot study will enhance future efforts to evaluate orthotic function objectively during gait in stroke patients.

Effects of hallux valgus angle on one-legged stance and gait parameters in young adults: a preliminary study

  • Objective: Hallux valgus (HV) is a common musculoskeletal deformity that is accompanied with pain and continues to decrease one's quality of life and ability to perform daily life activities by affecting gait and static stability. Therefore, this study aimed to investigate the effect of the angle of HV (HVA) and to compare the one-legged stance and gait parameters in young adults with less HV and severe HV. Design: Cross-sectional study. Methods: Forty young adults were divided into two groups, where HVA ≥15° (n=20) was defined as HV, and HVA <15° (n=20) was defined as normal. For balance ability, the center of pressure (COP) path, velocity, length of axis of the COP path, deviation of the x-axis and y-axis, and percentage of foot pressure were measured, and gait, the foot rotation angle, step length, percentage of each phase of the gait cycle, time change from the heel to forefoot, and maximum pressure of the forefoot and midfoot were measured. Results: Significant differences were found in sway length and time change from heel to forefoot during walking between the normal and HV groups (p<0.05). Most parameters were not associated with the HVA, but parameters such as length of axis and time to change from heel to forefoot were significantly associated with the HVA (p<0.05). Conclusions: These results suggest that most one-legged stance and gait parameters were not significantly affected by the HVA in young adults; therefore, future studies are needed in order to address other dynamic parameters and other methods of gait analysis for detecting clinically meaningful conditions.

경피신경전기자극이 근피로에 의한 자세균형과 근수축력의 변화에 미치는 효과 (Effectiveness of Transcutaneous Electrical Nerve Stimulation(TENS) on the Changes of Postural Balance and Muscle Contraction following Muscle Fatigue)

  • 근피로는 자세균형과 근수축력의 악화를 유발하여 다양한 근골격계 손상을 초래한다. 본 연구는 하퇴삼두근에 근피로 유발에 의한 자세불균형과 근수축력 저하에 경피신경전기자극이 미치는 효과를 알아보고자 하였다. 2010년 3월부터 4개월간 전정계통과 시각계통에 문제가 없는 20명의 건강한 성인을 대상으로 반복운동 수행을 통하여 비복근에 근피로를 유발하였고, 유발 후 즉시 경피신경전기자극을 적용하였다. 근피로 및 경피신경전기자극에 의한 자세균형과 근수축력의 변화를 측정하기 위하여 자세동요 이동거리와 속도, 최대수의수축력을 사용하였다. 근피로 유발에 의하여 자세동요 이동거리 및 속도의 증가와 근수축력의 저하가 나타났다(p<.05). 근피로가 유발된 비복근에 경피신경 전기자극의 적용은 근피로에 의한 자세동요와 근수축력을 유의하게 개선하였다(p<.05). 이와 같은 연구결과를 통하여 족저굴곡근의 근피로는 자세균형과 근수축력에 관여되고, 경피신경전기자극은 근피로에 의한 자세불균형 및 근수축력 저하에 효과적임을 증명하였다. 경피신경전기자극은 일상생활에서 흔히 유발되는 근피로의 중재에 경피신경전기자극이 효과적인 중재 방법으로 사용될 수 있을 것으로 사료된다.

슬링을 이용한 능동 운동프로그램이 무릎 관절 전치환술 환자의 통증과 균형에 미치는 영향 (The Effects of Active Exercise Program using Sling on the Pain and Balance Following Total Knee Replacement)

  • 본 연구는 슬링을 이용한 능동 운동프로그램이 무릎 관절 전치환술 환자의 통증과 균형에 미치는 영향을 알아보고자 실시하였다. 대상자는 무릎 관절 전치환술을 받고 입원 중인 환자 20명이며, 수술 후에 CPM (Continuous passive motion) 만을 적용하는 집단 (CPM group; CG)과 CPM과 슬링을 이용한 능동 운동프로그램을 병행하는 집단 (CPM with active exercise program using sling group; CSG)으로 10명씩 무작위로 배치되었다. CG는 CPM을 주 5일, CSG는 CPM을 주 2일, 슬링을 이용한 능동 운동프로그램을 주 3일 실시하였고, 각 중재는 하루 40분씩 총 4주간 실시하였다. 통증은 VAS(Visual analog scale)을 이용하여 평가하였고, 균형은 BT4 (Balance training 4)를 이용하여 C90 area, Trace length, Sway average velocity를 눈을 뜬 상태와 감은 상태로 측정하였다. 그 결과, 두 집단 모두 통증에서 집단 내에서 유의하게 감소하였고, 집단 간의 변화량에서도 유의한 차이가 있었다. 균형에서 CG의 C90을 제외한 모든 변인이 중재 후 유의한 변화가 있었으며, 집단 간 변화량에서는 눈을 감은 상태의 C90과 Vel에서 유의한 차이가 있었다. 따라서 CPM과 슬링을 이용한 능동 운동프로그램을 함께 시행하는 것은 무릎 관절 전치환술 환자의 통증 감소와 균형 향상에 효과적인 중재라고 생각한다.

페달로 도구를 이용한 훈련이 만성 뇌졸중 환자의 균형에 미치는 영향: 예비 실험 (The Effects of Training Using Pedalo Equipment on Balance of Post-Stroke Patients: Pilot Study)

  • PURPOSE: The purpose of the present study was to investigate the effect of the training using Pedalo equipment on balance function in post-stroke patients. METHODS: The present study was case-series. Ten post-stroke patients participated in the study. Participants performed the training using Pedalo equipment. The training using four Pedalo equipment lasted 30 minutes, 3 times a week for 6 weeks. Force platform, Berg Balance Scale(BBS), and Timed Up and Go(TUG) test were used to assess balance ability before and after training. RESULTS: After training using Pedalo equipment, there were significantly improved on path length and sway velocity of post-stroke patients in the both of eye open and - close conditions comparing with baseline. Also, on the BBS and TUG, there were significant improvements after training. CONCLUSION: The results of this study showed that the training using Pedalo equipment may be effective on improving the balance ability in the post-stroke patients. Through this study, we were able to confirm the potential of training using Pedalo equipment as an intervention in the rehabilitation of post-stroke patients.

풍압력하에서 피예항중인 손상선박의 침로안정성에 관한 연구 (A Study on Course Stability of Towed Damaged-ship under Wind Pressure)

  • 본 논문은 손상선박의 안전대책에 관한 연구의 일환으로, 황천항행중인 선박이 충돌, 좌초 등 원인에 의해 손상을 받았을 때를 가상하고, 손상선박을 다른 안전한 장소로 예항하고자 할 때의 침로안정성 문제를 다루고 있다. 외력으로는 바람의 영향만을 고려할 때, 예선 피예선계의 침로안정성 평가를 위한 특성방정식을 도출하고, 피예선의 각 손상상태에 따른 침로안정성을 수치계산하였다. 그 결과 손상상태, 풍속, 풍향 및 예항삭의 길이 등이 침로안정성에 미치는 영향을 평가할 수 있었다.
