Survival Analysis and Prognostic Factors for Neuroendocrine Tumors in Turkey

  • Background: This study aimed to determine the demographical distribution, survival and prognostic factors for neuroendocrine tumors monitored in our clinic. Materials and Methods: Data for 52 patients who were admitted to Cumhuriyet University Medical Faculty Training Research and Practice Hospital Oncology Center between 2006 and 2012 and were diagnosed and treated for neuroendocrine tumors were investigated. Results: Of the total, 30 (58%) were females and 22 (42%) were males. The localization of the disease was gastroenteropancreatic in 29 (56%) patients and other sites in 23 (44%). The most frequently involved organ in the gastroenteropancreatic system was the stomach (n=10, 19%) and the most frequently involved organ in other regions was the lungs (n=10, 19%). No correlation was found between immunohistochemical staining for proteins such as chromogranin A, synaptophysin, and NSE and the grade of the tumor. The patients were followed-up at a median of 24 months (1-90 months). The three-year overall survival rate was 71%: 100% in stage I, 88% in stage II, 80% in stage III, and 40% in stage IV. The three-year survival rate was 78% in tumors localized in the gastroenteropancreatic region, and 54% in tumors localized in other organs. In the univariate analysis, gender, age, performance status of the patients, grade, localization, surgical treatment, and neutrophil/lymphocyte ratio (${\leq}5$ versus >5) affected the prognosis of the patients. Conclusions: Most of the tumors were localized in the gastroenteropancreatic region, and the three-year survival rate in tumors localized in this region was better than the tumors localized in other sites. Surgical treatment was a positive independent prognostic factor, whereas Grade 3 and a neutrophil/lymphocyte ratio of >5 were negative independent prognostic factors.

Elevated Preoperative Platelet to Lymphocyte Ratio Associated with Decreased Survival of Women with Ovarian Clear Cell Carcinoma

  • This study was conducted to establish whether the preoperative platelet to lymphocyte ratio (PLR) is predictive of survival of women with ovarian clear cell carcinoma (OCCC). A PLR > 300 was deemed elevated. Progression-free survival (PFS) was estimated using the Kaplan-Meier method. Cox proportional hazard analysis was used to determine the independent effect of PLR. Thirty-six patients were reviewed. Elevated PLRs were more commonly noted in patients with an advanced vs an early stage of disease (88.9% vs 11.1%). Women with elevated PLR carried a higher rate of disease progression during primary therapy than that those in the normal PLR group (44.4 vs 22.2%). The median PFS for patients with elevated PLR was notably worse than that for patients with normal PLR (10 vs 34 months). Despite the impact of elevated PLR on PFS, it was found to be marginally significant when controlling for commonly applied prognostic markers. It, however, trended toward significance (HR=4.76; 95%CI, 0.95-23.8). In conclusion, an elevated PLR appears to be directly associated with adverse survival rather than being a surrogate for other indicators of a poor prognosis. PLR may be a useful biomarker for predicting survival of women with OCCC and merits further large-scale studies.

Cardiac diastolic dysfunction predicts poor prognosis in patients with decompensated liver cirrhosis

  • Background/Aims: Left ventricular diastolic dysfunction (LVDD) is an early manifestation of cardiac dysfunction in patients with liver cirrhosis (LC). However, the effect of LVDD on survival has not been clarified, especially in decompensated LC. Methods: We prospectively enrolled 70 patients with decompensated LC, including ascites or variceal bleeding, at Daejeon St. Mary's Hospital from April 2013 to April 2015. The cardiac function of these patients was evaluated using 2D echocardiography with tissue Doppler imaging. The diagnosis of LVDD was based on the American Society of Echocardiography guidelines. The primary endpoint was overall survival. Results: Forty-four patients (62.9%) had LVDD. During follow-up (22.3 months), 18 patients died (16 with LVDD and 2 without LVDD). The survival rate was significantly lower in patients with LVDD than in those without LVDD (31.1 months vs. 42.6 months, P=0.01). In a multivariate analysis, the Child-Pugh score and LVDD were independent predictors of survival. Moreover, patients with a ratio of early filling velocity to early diastolic mitral annular velocity (E/e') ${\geq}10$ (LVDD grade 2) had lower survival than patients with E/e' ratio < 10. Conclusions: The presence of LVDD is associated with poor survival in patients with decompensated LC. Therefore, it may be important to monitor and closely follow LVDD patients.

Survival of Stomach Cancer Cases in Khon Kaen, Thailand 2000-2012

  • Background: Stomach cancer is an aggressive malignancy that is difficult to detect at an early stage and therefore is characterized by poor survival rates. Over the last two decades, there has been no report of gastric cancer survival in Khon Kaen province, Thailand. The aim of this retrospective cohort study was to provide up-to-date information about the survival of gastric cancer patients in this province. Materials and Methods: Data from Khon Kaen population-based cancer registry, Faculty of Medicine, Khon Kaen University were newly obtained on 650 patients who were diagnosed with stomach cancer during the period 1 January, 2000 to 31 December, 2012. These were then followed up until death or the end of the study (31 December 2014). We calculated the observed survival with the actuarial life table method, and relative survival, defined as the ratio of observed survival in the group of the stomach cancer patients to the expected survival in the entire Thai population from the estimated generation life tables for Thailand of five-year birth cohorts from 1900 - 2000. Results: The 5 year observed and 5 year relative survival rates were 17.2 % (95% CI: 13.54-21.14) and 18.2 % (95% CI: 14.3-22.4), respectively. The highest 5 year relative survival rates were demonstrated among patients aged 45-65, with stage I or II lesions, with adenocarcinomas, with a body of stomach location, well differentiated and receiving surgery and/or chemotherapy. Conclusions: The observed and relative survival rates were close to each other. Our findings provide basic information beneficial to development of an effective treatment system and appropriately improved population-based cancer registration.

Disinfection Characteristic of Sewage Wastewater Treatment Using Solar Light/TiO2 Film System (태양광/광촉매를 이용한 오폐수 살균특성)

  • Currently, the application of $TiO_2$ photocatalyst has been focused on purification and treatment of wastewater. However, the use of conventional $TiO_2$ slurry photocatalyst results in disadvantage of stirring during the reaction and of separation after the reaction. And the usage of artificial UV lamp has made the cost of photocatalyst treatment system high. Consequently, we studied that solar light/$TiO_2$ film system was designed and developed in order to examine disinfection characteristics of sewage wastewater treatment. The optimum conditions for disinfection such as solar light intensity, characteristic of sewage wastewater, amounts of $TiO_2$ and comparison of solar ligth/$TiO_2$ systems with UV light/$TiO_2$ system was examined. The results are as follows: (1) photocatalytic disinfection process with solar light in the presence of $TiO_2$ film more effectively killed total coliform (TC) than solar light or $TiO_2$ film absorption only. (2) The survival ratio of TC and residual ratio of organic material (BOD, CODcr) decreased with remain resistant material. (3) The survival ratio of TC and residual ratio of organic material (BOD, CODcr) decreased with the increase of amounts of $TiO_2$. (4) TC survival ratio decreased linearly with increasing UV light intensity. (5) The disinfection effect of solar light/$TiO_2$ slurry system decreased more than UV light/$TiO_2$ film systems. (6) The disinfection reaction followed first-order kinetics. We suggest that solar light instead of using artificial UV light was conducted to investigate the applicability of alternative energy source in the disinfection of TC and the degradation of organic material.

Prognostic Significance of Preoperative Lymphocyte-Monocyte Ratio in Patients with Resectable Esophageal Squamous Cell Carcinoma

  • Background: The interaction between tumor cells and inflammatory cells has not been systematically investigated in esophageal squamous cell carcinoma (ESCC). The aim of the present study was to evaluate whether preoperative the lymphocyte-monocyte ratio (LMR), the neutrophil-lymphocyte ratio (NLR), and the platelet-lymphocyte ratio (PLR) could predict the prognosis of ESCC patients undergoing esophagectomy. Materials and Methods: Records from 218 patients with histologically diagnosed ESCC who underwent attempted curative surgery from January 2007 to December 2008 were retrospectively reviewed. Besides clinicopathological prognostic factors, we evaluated the prognostic value of the LMR, the NLR, and the PLR using Kaplan-Meier curves and Cox regression models. Results: The median follow-up was 38.6 months (range 3-71 months). The cut-off values of 2.57 for the LMR, 2.60 for the NLR and 244 for the PLR were chosen as optimal to discriminate between survival and death by applying receiver operating curve (ROC) analysis. Kaplan-Meier survival analysis of patients with low preoperative LMR demonstrated a significant worse prognosis for DFS (p=0.004) and OS (p=0.002) than those with high preoperative LMR. The high NLR cohort had lower DFS (p=0.004) and OS (p=0.011). Marginally reduced DFS (p=0.068) and lower OS (p=0.039) were found in the high PLR cohort. On multivariate analysis, only preoperative LMR was an independent prognostic factor for both DFS (p=0.009, HR=1.639, 95% CI 1.129-2.381) and OS (p=0.004, HR=1.759, 95% CI 1.201-2.576) in ESCC patients. Conclusions: Preoperative LMR better predicts cancer survival compared with the cellular components of systemic inflammation in patients with ESCC undergoing esophagectomy.

The survival rate, respiration and heavy metal accumulation of abalone (Haliotis discus hannai) rearing in the different copper alloy composition (동합금 조성에 따른 북방전복 (Haliotis discus hannai)의 생존, 호흡 및 중금속 축적률)

  • In order to investigate the effects of copper alloy on abalone physiology, we studied survival rate, respiration, excretion rate, and heavy metal accumulation in each organ of adults and spats. The survival rate of spats and adults showed 27-60% and 63-83% respectively, higher survival rate in adults. In particular, 100% of copper panel led to lowest survival rate and there was no sharp distinction according to copper alloy composition. The respiration rate and excretion rate of ammonia nitrogen was $1.81mgO_2/g$ D.W./h and 0.43 mg $NH_4-N/g$ D.W./h respectively at 100% of copper panel. In other words, there was a high significant difference at the level, but no significant difference at other test levels (P < 0.05). The atomic ratio (0: N) hit the lowest at the 100% of copper panel showing 3.79 and no significant differences were seen among other test groups with 6.57-7.18 of a very low range. This means that the species might have undergone nutritional stress. In case of copper accumulation, the 100% copper panel group showed the highest level in hepatopancreas and muscle showing 6.91 mg/kg and 1.60 mg/kg respectively but the rest of groups showed similar levels. Zinc accumulation raised at Cu-Zn alloy panel had high significance showing 18.50 mg/kg and 1.10 mg/kg in hepatopancreas and muscle respectively (P < 0.05). To sum up, a cage net made of 100% pure copper is expected to have a negative effect on abalone in light of survival rate, heavy metal accumulation, and atomic ratio (0: N). Moreover, given that the substratum used for the high adhesive species and nutritious stress that is represented through the atomic ratio need to be considered, the copper alloy net is thought not to be suitable for abalone aquaculture.

Cryoprotective Properties of Exopolysaccharide (P-21653) Produced by the Antarctic Bacterium, Pseudoalteromonas arctica KOPRI 21653

  • Twenty-five bacterial strains that secrete mucous materials were isolated from sediment obtained from King George Island, Antarctica. Seven of these strains proved capable of producing cryoprotective exopolysaccharides. The strain KOPRI 21653 was selected for the further study of an anti-ice-nucleating polysaccharide (ANP), which originated from a polar region. KOPRI 21653 was identified as Pseudoalteromonas arctica as the result of 16S rRNA analysis. The exopolysaccharide, P-21653, was purified completely from the KOPRI 21653 cell culture via column chromatography and protease treatment. The principal sugar components of P-21653 were determined to be galactose and glucose, at a ratio of 1:1.5, via GC-MS analysis. The cryoprotective activity of P-21653 was characterized via an E. coli viability test. In the presence of 0.1% (w/v) P-21653, the survival ratio of E. coli cells was as high as 82.6% over three repeated freeze-thaw cycles. The survival ratio decreased drastically to 71.5 and 48.1 %, respectively, in five and seven repeated cycle conditions; however, the survival ratios were greater over three (96.6-92.1%) to seven (100.5-91.6%) freeze-thaw cycles in the presence of 0.5 and 1.0% (w/v) P-21653. In addition, at much lower concentrations (0.1-1.0%), P-21653 resulted in survival ratios (83.1-98.4%) similar to those of two commercially available cryoprotectants ($V_{EG}$ plus X-1000, 92.9% and $V_{M3}$, 95.3%), which were utilized at the recommended concentrations (90%). The biochemical characteristics of exopolysaccharide P-21653 reflect that this compound may be developed as a useful cryoprotectant for use in medical applications and in the food industry.

Effects of microplastics and salinity on food waste processing by black soldier fly (Hermetia illucens) larvae

  • Background: The black soldier fly (Hermetia illucens) is gaining attention as an efficient decomposer of food waste. However, recalcitrant compounds such as plastics mixed into food waste may have negative effects on its growth and survival. Moreover, its efficiency of food waste degradation may also be affected by plastics. In addition, salt (NaCl) can also be present in high concentrations, which also reduces the efficiency of H. illucens-mediated food waste treatment. In this study, we assessed the growth of black soldier fly larvae (BSFL) reared on food waste containing polyethylene (PE) and polystyrene (PS) and NaCl. The weight of BSFL was measured every 2-4 days. Survival and substrate reduction rates and pupation ratio were determined at the end of the experiment. Results: The total larval weight of Hermetia illucens reared on food waste containing PS was greater than that of the control on days 20 and 24. However, the survival rate was lower in the group treated with 5% PS, as was substrate reduction in all PS-treated groups. The weight of BSFL reared on food waste containing PE was lower than that of the control on day 6. PE in food waste did not affect the survival rate, but the pupation ratio increased and substrate consumption decreased with increasing PE concentrations. Regardless of the plastic type, the addition of NaCl resulted in decreased larval weight and pupation ratio. Conclusions: Larval growth of black soldier fly was inhibited not by plastics but by substrate salinity. Additional safety assessments of larvae reared on food waste containing impurities are needed to enable wider application of BSFL in vermicomposting.

Effect of mulberry leaf picking in autumn on the non-budding and survival of grafting (추잠기의 적엽이 불발아 및 접목의 활착에 미치는 영향)

  • The purpose of this work is to investigate the effects of picking leaves in autumn on the non-budding and survival of grafting. The results are as follows. 1) The leaf picking in autumn causes an increase of the non-budding ratio in the following spring. When tue whole leaves picking is compared with the control, the non-budding ratio increases 46 percent. In case of tone leaves on a single stem are picked in various tray, the non-budding ratio of picked portion is higher than that of non-picked. 2) The leaf picking in autumn with winter bud being grafted on stock in the next spring, decrease the survival of grafting. In case some leaves on a single stem are picked in various way, the winter bud of picked part is always poor in survival. 3) In the event of leavf picking in autumn, the yield always decreases 7∼20 percent, especially this phenomenon becomes serious in the under half part of the stem(20%) and whole leaves picking(12%) treatments.
