고립성 폐결절의 예후에 관여하는 인자 (The Prognostic Factors of Solitary Pulmonary Nodule)

  • The solitary pulmonary nodule is considered as a round or ovoid lesion with sharp, circumscribed borders, surrounded by normal appearing lung parenchyme on all sides, and found on a simple chest X-ray without any particular symptoms or signs. There is a wide spectrum of pathologic conditions in the solitary pulmonary nodules prove to be malignant tumors, either primary or metastatic. Most Benign granulomas and other benign conditions can also be seen as solitary nodules. The resection of solitary malignant nodules results in a surprisingly high 5-year survival rate. On the contrary, most benign nodules do not need to be resected and a period of prolonged observation and nonsurgical management is usually indicated. Therefore, the best approach to the controversial management of solitary pulmonary nodules depends on finding factors affecting the probability of malignancy. In this article, clinical records and chest roentgenographies of 60 patients operated on over the past 8 years at the Department of Thoracic and Cardiovascular Surgery, Seoul National University Hospital were reviewed. There were 15 malignant nodules and 45 benign nodules and the prevalence of malignancy was 25%. The most common pathologic entity was tuberculoma [21 cases]. The mean age was 55.5*9.6 years in the malignant group, 45.8>12.5 years in the benign group and there was a significant statistical difference between the two groups [P < 0.05]. The malignant ratio in each age group increased with advancing age. The average smoking amount was 35.6*12.9 cigarettes per day in malignant smokers, 20.9* 12.0 cigarettes per day in benign smokers, and there was a significant statistical difference between the two groups [p< 0.05]. The malignant ratio also increased with the increasing smoking amount. Comparing the appearance of the nodule on chest films, 6 calcifications and 7 cavitations were found only in benign nodules, not in malignant nodules. Therefore, calcification and cavitation can be considered as preferential findings for benignity. Previous cancer history was also a significant factor deciding the prognosis of the nodule [p< 0.05]. The average diameter on chest X-ray was 3.07*0.82 cm in malignant nodules, 3.25*1.04 cm in benign nodules and there was no significant statistical difference between the two groups [p< 0.05]. The author used Bayes theorem to develop a simple method for combining individual clinical or radiological factors of patients with solitary nodules into an overall estimate of the probability that the nodule is malignant. In conclusion, patient age, smoking amount, appearance of nodule on chest film such as calcification and cavitation, and previous cancer history were found to be strongly associated with malignancy, but size of nodule was not associated with malignancy. Since these prognostic factors have been found retrospectively, prospective controlled studies are needed to determine whether these factors have really prognostic significance.

고령의 국소 진행된 식도암환자에서 동시 항암화학방사선치료 (Concurrent Chemoradiotherapy in Elderly Patients with Locally Advanced Esophageal Carcinoma)

  • 목 적: 국소 진행된 식도암으로 동시 항암화학방사선치료를 시행한 고령의 환자들을 대상으로 동시 항암화학방사선치료에 대한 효과를 알아보고자 하였다. 대상 및 방법: 2001년 1월부터 2007년 7월까지 병리학적 편평상피세포암으로 확인 된 65세 이상의 식도암 환자 중동시 항암화학방사선치료를 받은 28명을 대상으로 후향적 분석을 하였다. 환자의 병기는 IIa 8명(28.8%), IIb 10명 (35.7%), III 10명(35.7%)이었다. 방사선치료는 6 MV 또는 10 MV X-선으로 45~63 Gy (중앙값: 59.4 Gy)를 분할 조사하였다. 항암화학요법은 방사선치료 시작과 동시에 Cisplatin 75 mg/$m^2$을 제1일에 정주하였고, 5-FU는 1,000mg/$m^2$을 제1일에서 제4일까지 4일간 지속적 정주하여 방사선치료 동안은 3주 간격으로 2회 시행하였고, 방사선치료 후 2회의 항암화학요법을 추가 시행하였다. 결 과: 추적관찰기간은 3~72개월(중앙값: 19개월)이었다. 동시 항암화학방사선치료 후 치료 반응은 완전관해가39.3% (11명), 부분반응은 50.0% (14명), 무반응이 10.7% (3명)로 치료 반응률은 89.3% (25명)이었다. 전체 환자의 1, 2, 3년 생존율은 각각 55.9%, 34.6%, 24.2%이었고 중앙 생존기간은 15개월이었다. 완전관해, 부분반응, 무반응의 종양 반응을 보인 환자들의 2년 생존율은 각각 46.2%, 33.%, 0%이었다. 생존율과 관련된 예후인자로서 병기와 방사선치료 후 종양 반응이 유의하였다. 치료에 의한 합병증으로 사망한 환자는 없었다. 결 론: 고령의 국소 진행된 식도암환자에 대한 동시 항암화학방사선치료는 심각한 부작용 없이 비교적 효과적이었다.

구충증(鉤蟲症)에 관(關)한 연구(硏究) 제1편(第1篇) 구충(鉤蟲)의 감염(感染) 및 구충성빈혈(鉤蟲性貧血)에 관(關)한 고찰(考奈) (Studies on Ancylostomiasis I. An Experimental Study on Hookworm Infection and Anemia)

  • In view of its prevalence in the Far East area, a more detailed knowledge on the hookworm infection is one of the very important medical problems. The present study was aimed to; determine the infectivity of the artificially hatched ancylostoma duodenale larvae in man after its oral administration, evaluate the clinical symptomatology of such infection, determine the date of first appearance of the ova in the stool, calculate the blood loss per worm per day, assess the relation-ships between the ova count, infectivity(worm load), blood loss and severity of anemia. An erythrokinetic study was also done to analyse the characteristics of hookworm anemia by means of $^{59}Fe\;and\;^{51}Cr$. Materials and Methods Ten healthy male volunteers(doctors, medical students and laboratory technicians) with the ages ranging from 21 to 40 years were selected as the experimental materials. They had no history of hookworm infection for preceding several years, and care was taken not to be exposed to reinfection. A baseline study including a through physical examinations and laboratory investigations such as complete blood counts, stool examination and estimation of the serum iron levels was done, and a vermifuge, bephenium hydroxynaphoate, was given 10 days prior to the main experiment. The ancylostoma duodenale filariform larvae were obtained in the following manner; The pure ancylostoma duodenale ova were obtained from the hookworm anemia patients and a modified filter paper method was adopted to harvest larger number of infective larvae, which were washed several times with saline. The actively moving mature larvae were put into the gelatine capsules, 150 in each, and were given to the volunteers in the fasting state with 300ml. of water. The volunteers were previously treated with intramuscular injection of 15mg. of chlorpromazine in order to prevent the eventual nausea and vomiting after the larvae intake. The clinical symptoms and signs mainly of the respiratory and gastrointestinal tracts, appearance of the ova and occult blood in the stool etc. were checked every day for the first 20 days and then twice weekly until the end of the experiment, which usually lasted for about 3 months. Roentgenological survey of the lungs was also done. The hematological changes such as the red blood cell, white blood cell and eosinophil cell counts, hemoglobin content and serum iron levels were studied. The appearance of the ova in the stool was examined by the formalin ether method and the ova were counted in triplicate on two successive days using the Stoll's dilution method. The ferrokinetic data were calculated by the modified Huff's method and the apparent half survival time of the red blood cells by the modified Gray's method. The isotopes were simultaneously tagged and injected intravenously, and then the stool and blood samples were collected as was described by Roche et al., namely, three separate 4-day stool samples with the blood sample drawing before each 4-day stool collection. The radio-activities of the stools ashfied and the blood were separately measured by the pulse-height analyser. The daily blood loss was calculated with the following formula; daily blood loss in $ml.=\frac{cpm/g\;stool{\times}weight\;in\;g\;of\;4-day\;stool}{cpm/ml\;blood{\times}4}$ The average of these three 4-day periods was given as the daily blood loss in each patient. The blood loss per day per worm was calculated by simply dividing the daily blood loss by the number of the hookworm recovered after the vermifuge given twice a week at the termination of the experiment. The iron loss in mg. through the gastrointestinal tract was estimated with the daily iron loss in $mg=\frac{g\;Hgb/100ml{\times}ml\;daily\;blood\;loss{\times}3.40}{100}$ 3.40=mg of iron per g Hgb following formula; Results 1. The respiratory symptoms such as cough and sputum were noted in almost all cases within a week after the infection, which lasted about 2 weeks. The roentgenological findings of the chest were essentially normal. A moderate degree of febril reaction appeared within 2 weeks with a duration of 3 or 4 days. 2. The gastrointestinal symptoms such as nausea, epigastric fullness, abdominal pain and loose bowel appeared in all cases immediately after the larvae intake. 3. The reduction of the red blood cell count was not remarkable, however, the hemoglobin content and especially the serum iron level showed the steady decreases until the end of the experiment. 4. The white blood cells and eosinophil cells, on the contrary, showed increases in parallel and reached peaks in 20 to 30 days after the infection. A small secondary rise was noted in 2 months. 5. The ova first appeared in the stool in 40. 1 days after the infection, ranging from 29 to 51 days, during which the occult blood reaction of the stool became also positive in almost cases. 6. The number of ova recovered per day was 164, 320 on the average, ranging from 89,500 to 253,800. The number of the worm evacuated by vermifuge was in rough correlation with the number of ova recovered. 7. The infectivity of ancylostoma duodenale was 14% on the average, ranging from 7.3 to 20.0%, which is relatively lower than those reported by other workers. 8. The mean fecal blood loss was 5.78ml. per day, with a range of from 2.6 to 11.7ml., and the mean blood loss per worm per day was 0.30ml., with a range of from 0.13 to 0.73ml., which is in rough coincidence with those reported by other authors. There appeared to exist, however, no correlation between the blood loss and the number of ova recovered. 9. The mean fecal iron loss was 2.02mg. per day, with a range of from 1.20 to 3.89mg., which is less than those appeared in the literature. 10. The mean plasma iron disappearance rate was 0.80hr., with a range of from 0.62 to 0.95hr., namely, a slight accerelation. 11. The hookworm anemia appeared to be iron deficiency in origin caused by continuous intestinal blood loss.

비누수 해가림 재배가 인삼의 생육 및 품질에 미치는 영향 (Effects of Shading material of Rain Shelter on Growth and Quality in Panax Ginseng C. A. Meyer)

  • 본 연구는 비누수 해가림 재배가 인삼 생육 및 품질에 미치는 영향을 구명하기 위하여 인삼의 차광재료를 비누수 해가림과 차광망을 이용하여 실험하였던 바, 얻어진 결과를 요약하면 다음과 같다. 1. 차광재료에 따른 맑은 날의 광량은 비누수해가림이 차광망에 비하여 낮은 경향이었고, 온도는 비누수해가림이 차광망에 비하여 $3{\sim}5^{\circ}C$ 낮게 나타났다. 2. 차광재료에 따른 3, 4, 5년근 인삼의 생존본수는 비누수해가림에 비하여 차광망이 낮게 나타났다. 3. 차광재료에 따른 인삼의 경태는 비누수해가림에서 증가하였고, 생근중도 비누수해가림이 차광망에 비하여 3년근에서는 5.0 g, 4년근에서는 10 g, 5년근에서는 8 g 높았다. 4. 차광재료에 따른 3, 4, 5년근 모두 수삼의 수량은 비누수해가림이 차광망보다 훨씬 높게 나타났다. 5. 차광재료별 3, 4, 5년근 수삼의 크기는 비누수해가림이 차광망에 비하여 대편이 많이 분포하였으며, 진세노사이드 함량도 비누수해가림이 차광망에 비해서 높았다

단삼(Salvia Miltiorrhiza) 추출물의 암세포 증식 억제 효과에 관한 연구 (Inhibitory Effect of Salvia miltiorrhiza Extract on Growth of Some Cancer Cells)

  • 본 연구에서는 단 삼(Salvia miltiorrhiza)의 추출물이 인체 결장암세포인 HT-29, 간암세포인 HepG2 및 직장 암세포인 HRT-18의 증식에 미치는 영향을 in vitro에서 확인하였다. 암세포의 배양액에서 단삼의 수용성 추출물과 에탄올 추출물의 암세포 증식억제효과는 농도에 비례하여 에탄올 추출물에서 더 효과적이었다. 이를 토대로 단삼의 에탄올 추출물의 급성독성, 수명연장 및 고형암형 성억제를 살펴보기 위해 in vivo 실험을 하였다. 급성독성실험결과 대조군의 15일 째의 평균체중은 32.3 g 이었고 실험군은 31.6 g으로 정상적인 상태를 유지하였으며, 흰쥐의 육종암세포인 sarcoma-180를 접종한 mouse의 수명연장실험결과 대조군에 비해 61%의 수 명연장 효과가 있음을 관찰하였다. 고형암억제 실험 결과에서도 대조군에 비해 에찬올 추출 물 처리군이 35%의 고형암 형성억제능을 나타내었다. 따라서 단삼의 에탄올 추출물 중에는 in vitro와 in vivo 실험을 통해 항암활성을 갖는 성분이 존재하며, 이성분은 유효한 항암제 로 개발될 수 있으리라 사료된다.

High-frequency shoot regeneration from leaf explants through organogenesis in bitter melon (Momordica charantia L.)

  • An efficient protocol for in vitro organogenesis was achieved from callus-derived immature and mature leaf explants of Momordica charantia, a very important vegetable and medicinal plant. Calluses were induced from immature leaf explants excised from in vitro (15-day-old seedlings) mature leaf explants of vivo plants (45 days old). The explants were grown on Murashige and Skoog (MS) medium with Gamborg (B5) vitamins containing 30 g $1^{-1}$ sucrose, 2.2 g $1^{-1}$ Gelrite, and 7.7 lM naphthalene acetic acid (NAA) with 2.2 ${\mu}M$ thidiazuron (TDZ). Regeneration of adventitious shoots from callus (30-40 shoots per explant) was achieved on MS medium containing 5.5 ${\mu}M$ TDZ, 2.2 ${\mu}M$ NAA, and 3.3 ${\mu}M$ silver nitrate ($AgNO_3$). The shoots (1.0 cm length) were excised from callus and elongated in MS medium fortified with 3.5 ${\mu}M$ gibberellic acid ($GA_3$). The elongated shoots were rooted in MS medium supplemented with 4.0 ${\mu}M$ indole 3-butyric acid (IBA). Rooted plants were acclimatized in the greenhouse and subsequently established in soil with a survival rate of 90%. This protocol yielded an average of 40 plants per leaf explant with a culture period of 98 days.

일측 폐이식 실험견에서 이식폐의 기능 평가연구 (An Assessment of Allograft Function in Canine Single Lung Transplantation)

  • We experienced 7 cases of left single lung transplantation in 14 mongrel dogs and analyzed graft lung function by hemodynamics, blood gas analysis, chest X-ray, biopsy and perfusion lung scan. We performed right pulmonary artery cuff[PA cuff for analysis of graft lung function in 3 cases. The donor lungs were flushed with modified Euro-Collins solution[n=3 or low potassium dextran glucose solution[n=4 and preserved for 4 to 5 hours[n=4 or 24 hours[n=3 at 10o C and implanted to the dogs with similar weight . Assessment of left graft lung was done by occluding the right pulmonary artery for 10 minutes using PA cuff. Assessment for graft lung function was done immediately after an operation and after 3 days, 7days and 3 weeks postoperatively. Four dogs survived for 3days, 7days[2 cases and 3 weeks respectively. Other three dogs expired within 3 hours of reperfusion. Immediate perfusion scans of left lung in four survived dogs after reperfusion were 42.1%, 36% , 11% and 5.9% respectively, and another dog with 4.8% perfusion to left lung was dead due to left atrial thrombi after 3 hours reperfusion. In one case among three acute rejections follow-up perfusion scan was done on 3rd and 11th postoperative day and the result decreased from 36% perfusion immediate postoperatively to 21% and 15% respectively. Three expired dogs postoperatively couldn`t tolerate occlusion of right pulmonary artery with above 40 mmHg of mean pulmonary artery pressure. On the other hand, three survival dogs postoperatively tolerated occlusion of right pulmonay artery with less than 30 mmHg of mean pulmonary artery pressure. and one dog couldn`t tolerate same procedure immediate postopertively but in 2 hours reperfusion later tolerated with 29 mmHg of mean pulmonary artery pressure.In conclusion we couldn`t compare the effect of two flushing solutions but low potassium dextran glucose solution showed relatively safe preservation effect in cases with preservation of more than 20 hours. Also canine left single lung transplantation model with PA cuff indicated useful method for the assessment of graft lung function with effect of lung preservation.

Institutional Experience of Interstitial Brachytherapy for Head and Neck Cancer with a Comparison of High- and Low Dose Rate Practice

  • Aims: To describe our institutional experience with high dose rate (HDR) interstitial brachytherapy (IBT) compared with previously reported results on the low dose rate (LDR) practice for head and neck cancer. Materials and Methods: Eighty-four patients with oral cavity (n=70) or oropharyngeal cancer (n=14) were treated with 192Ir HDR-IBT. Seventy-eight patients had stage I or II tumour. The patients treated with IBT alone (n=42) received 39-42 Gy/10-14 fractions (median=40 Gy/10 fractions). With respect to the combination therapy group (n=42), prescription dose comprised of 12-18 Gy/3-6 fractions (median=15 Gy/5 fractions) for IBT and 40-50 Gy/20-25 fractions (median=50 Gy/25 fractions) for external radiotherapy. Brachytherapy was given as 2 fractions per day 6 hours apart with 4 Gy per fraction for monotherapy and 3 Gy per fraction for combination therapy. Results: Four patients were not evaluable in the analysis of outcome. The primary site relapse rates were 23.8% (10/42) and 68.4% (26/38) in patients treated with IBT alone and combination therapy, respectively (p<0.001). Salvage surgery was performed in 19 patients. The 5-year local control rate was estimated at 62% and the disease-free survival (DFS) rate at 52% for all patients. Local control with respect to T1 and T2 tumours was 84% and 42%, respectively. Conclusions: Our present series on HDR-IBT and the previous report on LDR-IBT for head and neck cancer demonstrated similar DFS rates at 5 years (52%). The rate of regional failure in node-negative patients was <20% in both of our series. HDR-IBT offers similar results to LDR-IBT for head and neck cancer.

복숭아 Solid Pack 적정(適定) 살균조건(殺菌條件) 구명(究明)을 위(爲)한 선발(選拔) 효모(酵母)의 열저항성(熱抵抗性)에 관(關)한 연구(硏究) (Studies on Thermal Resistance of Selected Yeast Strain for Pasteurization of Solid Packed Peach)

  • 복숭아 solid pack을 변패시키는 내열성 효모의 열저항성 및 최적살균조건을 구명하기 위하여 원료복숭아의 초기 효모오염도를 조사하고 61균주를 분리보관하였다. 이중 내열성이 가장 강한 효모 No.15 균주에 대한 환경요인별 내열성 및 최적살균조건 시험결과는 아래와 같다. 1. 펩톤수와 복숭아쥬스에서 열처리하였을 때 대수기의 생장이 활발한 세포가 90시간 진탕배양후 10일간 유지한 것보다 내열성이 약했다. 2. 열매체로 펩톤수를 사용하여 열처리한 것이 복숭아쥬스에서 열처리한 것보다 내열성이 강했다. 3. 열처리후 회복배지에 있어서 YM 한천보다 복숭아쥬스 한천에서 회복시킨 것이 약했다. 4. 열매체나 회복배지에서의 pH에 따른 내열성은 pH 4.0에서가 pH 3.5에서 보다 내열성이 강했다 5. 일정한 온도로 가열할 때 생잔곡선은 초기의 열에 민감한 부분과 다음의 열에 안정한 부분의 broken curve를 나타내어 D 값을 구할 수 없었다. 6. 복숭아쥬스(pH 3.5)에서 열처리하여 복숭아쥬스 한천(pH 3.5)에 회복시켰을 때의 TDT 곡선에서 직선을 얻었으며 z값은 $4.8^{\circ}C$이었다. 7. 선발된 내열성 효모의 균학적 특성을 검토한 결과 Torulopsis candida로 추정되었으며 복숭아 solid pack에서의 변패성도 확인하였다. 8. 실제 4 oz 복숭아 solid pack에서의 inoculated pack test 결과 최적살균치 P.U. 70/5는 168(Z값$5^{\circ}C$, 기준온도 $70^{\circ}C$, 살균시간 18분, 관내 중심 온도 $78.5^{\circ}C$)이었다.

TTFA의 마우스 공장 소낭선에 대한 방사선 방호작용 (Protective Effects of Trithioformaldehyde against Radiation Damage of Mouse Jejunal Crypt Cells)

  • Patt등이 1949년 처음으로 cysteamine과 히utathione등이 정상조직의 방사선 내성을 증가시킨다는 사실을 규명한 이후로 많은 방사선 방어제가 연구되어 왔으나, 그들의 독성때문에 임상적적용이 매우 제한되었다. 본 실험에서는 3개의 sulfur기를 지닌 환상구조의 Trithioformaldehyde(TTFA)를 합성하여 방사선 조사 후 생존율에 대한 영향과 마우스 공장 소낭선 세포에 대한 방호 효과를 관찰하였다. TTFA 2.0 g/kg을 방사선 조사 15분 전에 복강내로 투여하고 800 rad를 조사한 후 30일간 관찰했을 때 78%의 생존율을 보였다. 또, 공장 소낭선에 대한 실험에서 TTFA는 1.0g/kg과 2.0g/kg에서 공장 소낭선에 대한 protection factor가 각각 1.17와 1.23 임을 나타냈다.
