Background: Accurate mediastinal lymph node staging is vital for the optimal therapy and prognostication of patients with lung cancer. This study aimed to determine the preoperative risk factors for pN2 disease, as well as its incidence and long-term outcomes, in patients with clinical N0-1 non-small cell lung cancer. Methods: We retrospectively analyzed patients who were treated surgically for primary non-small cell lung cancer from November 2005 to December 2014. Patients staged as clinical N0-1 via chest computed tomography (CT) and positron emission tomography (PET)-CT were divided into two groups (pN0-1 and pN2) and compared. Results: In a univariate analysis, the significant preoperative risk factors for pN2 included a large tumor size (p=0.083), high maximum standard uptake value on PET (p<0.001), and central location of the tumor (p<0.001). In a multivariate analysis, central location of the tumor (p<0.001) remained a significant preoperative risk factor for pN2 status. The 5-year overall survival rates were 75% and 22.9% in the pN0-1 and pN2 groups, respectively, and 50% and 78.2% in the patients with centrally located and peripherally located tumors, respectively. In a Cox proportional hazard model, central location of the tumor increased the risk of death by 3.4-fold (p<0.001). Conclusion: More invasive procedures should be considered when preoperative risk factors are identified in order to improve the efficacy of diagnostic and therapeutic plans and, consequently, the patient's prognosis.

Currently, there are many problems due to the decline in school-age population. Moreover, Korea has the largest number of universities compared to the population, and the university enrollment rate is also the highest in the world. As a result, the minimum student retention rate required for the survival of each university is becoming increasingly important. The purpose of this study was to examine the effects of reducing the number of graduates of education and the social climate that prioritizes employment. And to determine what the basic direction is for students to manage the student retention rate, which can be maintained from admission to graduation, to determine the optimal input variables, Based on the input parameters, we will make associative analysis using apriori algorithm to collect training data that is most suitable for maintenance rate management and make base data for development of the most efficient Deep Learning module based on it. The accuracy of Deep Learning was 75%, which is a measure of graduation using decision trees. In decision tree, factors that determine whether to graduate are graduated from general high school and students who are female and high in residence in urban area have high probability of graduation. As a result, the Deep Learning module developed rather than the decision tree was identified as a model for evaluating the graduation of students more efficiently.
Currently, liver transplantation is the only available remedy for patients with end-stage liver disease. Conservation of transplanted liver graft is the most important issue as it directly related to patient survival. Carbonyl reductase 1 (CBR1) protects cells against oxidative stress and cell death by inactivating cellular membrane-derived lipid aldehydes. Ischemia-reperfusion (I/R) injury during living-donor liver transplantation is known to form reactive oxygen species. Thus, the objective of this study was to investigate whether CBR1 transcription might be increased during liver I/R injury and whether such increase might protect liver against I/R injury. Our results revealed that transcription factor Nrf2 could induce CBR1 transcription in liver of mice during I/R. Pre-treatment with sulforaphane, an activator of Nrf2, increased CBR1 expression, decreased liver enzymes such as aspartate aminotransferase and alanine transaminase, and reduced I/R-related pathological changes. Using oxygen-glucose deprivation and recovery model of human normal liver cell line, it was found that oxidative stress markers and lipid peroxidation products were significantly lowered in cells overexpressing CBR1. Conversely, CBR1 knockdown cells expressed elevated levels of oxidative stress proteins compared to the parental cell line. We also observed that Nrf2 and CBR1 were overexpressed during liver transplantation in clinical samples. These results suggest that CBR1 expression during liver I/R injury is regulated by transcription factor Nrf2. In addition, CBR1 can reduce free radicals and prevent lipid peroxidation. Taken together, CBR1 induction might be a therapeutic strategy for relieving liver I/R injury during liver transplantation.

Aircraft are affected by various vibrations during maneuvering. These vibrations may have a fatal effect on the survival of aircraft in some cases, so the safety of components applied to the aircraft should be proven against various vibrations through random vibration analysis. In this study, the structural robustness of an external fuel tank and pylon for military aircraft was evaluated under random vibration conditions using commercial software, MSC Random. In the random vibration analysis, a frequency response analysis was performed by imposing a unit load on the boundary condition point, and then excitation was performed with a PSD profile. In this process, the required mode data was extracted through a modal analysis method. In addition, the random vibration profile specified in the US Defense Environment Standard was applied as random vibration conditions, and the PSD profile given in units of G's was converted into units of gravitational acceleration. As a result of the numerical analysis, we evaluated the structural robustness of the external fuel tank and pylon by identifying the safety margins of beam elements, shell elements, and solid elements in a numerical model for random vibration in the x, y, and z directions.
The purpose of this paper is to reduce the reverse-causality and overestimate bias of analysis on how health affected middle-aged and elderly worker's early retirement. From the Korean Longitudinal Study of Ageing(KLoSA) panel data, I researched 1,049 people who were 45-52 years old in 2006. To eliminate the reverse-causality problem, I used the health data which is surveyed before retirement. To reduce bias, I controlled the health status when retirees worked. The main results are as follows. First, the worsened health still affects the hazard of early retirement, with reducing the endogeneity problem. Second, chronic illness is one of the strong predictors of early retirement to self-employed, and self-reported bad health is the main health predictor of wage workers. These results give two implications; first, the impact magnitude of the health indicator depends on employment type. Each employment type has different flexibility of working hours. It seems that the flexibility can reduce early retirement hazard with health problems. Self-employed, who has more flexibility of working hours can work until they have to stop working due to the serious health problem or doctor's advice. Second, to promote middle-aged and elderly workers to keep working, the long-term health policy which decreases chronic illness is needed.

They had commonly developed their visions about oriental peace on the basis of the theory of social evolution focusing on the survival of the fittest and the weak-to-the-wall kind of society that had been prevalent since 19th century. Furthermore, their logics were based on racism combined with the social evolution theory. Although the two men lived in the same era, their spirit of the times were quite different. An considered that the Japanese extortion of Korean sovereignty caused the corruption of peace of the East. On the other hand, Park believed that peaceful age came by way of the Greater East Asia Co-Prosperity constructed by the union of Korea and Japan. Especially Park put higher value on Japanese modern civilization than Korean sovereignty and also insisted Korea should take Japanese case as its model.

In this study Orostachys japonica A. Berger used is a medicinal herb that has long been used as a folk remedy for cancer treatment. In this study, the antioxidant efficacy of the ethyl acetate fraction of Orostachys japonica A. Berger was confirmed. The results of the Orostachys japonica A. Berge ethyl acetate fraction of antioxidant activity assays showed Antioxidant effect of Orostachys japonica A. Berger EtOAc fraction extract at 0.10 mg/mL was showed a DPPH radical scavenging rate of 78.54% and ABTS+ radical scavenging rate of 73.48%. Also, the toxicity result of Orostachys japonica A. Berger EtOAc fraction extracts using alternative experimental animal model zebrafish, confirmed a 100% the survival of the zebrafish embryo was shown that there was no coagulation and no hatching delay at all concentrations. also ROS generation induced by UV-B irradiation was confirmed that the fluorescence intensity decreased as a whole in all larvae treated with Orostachys japonica A. Berger EtOAc fraction extracts. In particular, it was confirmed that ROS generation was effectively suppressed by showing a 35.7% reduction rate compared to the positive control at a concentration of 3 ㎍/mL. These results were confirmed that Orostachys japonica A. Berger EtOAc fraction extracts has the possibility of application in the cosmetics field as a natural antioxidant.

Objective : The primary objective of this study was to identify predicting factors for local control (LC) of atypical meningioma, and we validated them with comparing the predicting factors for recurrence-free survival (RFS). We also examined the rate of LC after surgical resection with or without adjuvant treatment and RFS. Methods : Clinical and radiological records of patients with atypical meningiomas diagnosed at two institutes from January 2000 to December 2018 were reviewed retrospectively. Histopathological features were also reviewed using formalin-fixed paraffin embedded samples from pathological archives. Results : Of the 99 atypical meningiomas eligible for analysis, 36 (36.4%) recurred during the follow-up period (mean, 83.3 months; range, 12-232 months). The rate of 3-year LC and 5-year LC was 80.8% and 74.7%, respectively. The mean time-to-recurrence was 49.4 months (range, 12-150). The mean RFS was 149.3 months (95% confidence interval, 128.8-169.8 months) during the mean follow-up duration of 83.3 months (range, 12-232 months). Multivariate analysis using Cox proportional-hazard regression model showed that the extent of resection (hazard ratio [HR], 4.761; p=0.013), Ki67 index (HR, 8.541; p=0.004), mitotic index (HR, 3.275; p=0.044), and tumor size (HR, 3.228; p=0.041) were independently associated with LC. These factors were also statistically associated with RFS. In terms of radiotherapy after surgical resection, the recurrence was not prevented by immediate radiotherapy because of the strong effect of proliferative index on recurrence. Conclusion : The present study suggests that the extent of resection, proliferative index (according to Ki67 expression) and mitotic index, and tumor size are associated with recurrence of atypical meningiomas. However, our results should be further validated through prospective and randomized clinical trials to overcome the inborn bias of retrospective nature of the study design.
Hand-foot-and-mouth disease (HFMD) is a viral infectious disease that occurs in children under 5 years of age. Its main causes are coxsackievirus (CV) and enterovirus (EV). Since there are no efficient therapeutics for HFMD, vaccines are effective in preventing the disease. To develop broad coverage against CV and EV, the development of a bivalent vaccine form is needed. The Mongolian gerbil is an efficient and suitable animal model of EV71 C4a and CVA16 infection used to investigate vaccine efficacy following direct immunization. In this study, Mongolian gerbils were immunized with a bivalent inactivated EV71 C4a and inactivated CVA16 vaccine to test their effectiveness against viral infection. Bivalent vaccine immunization resulted in increased Ag-specific IgG antibody production; specifically, EV71 C4a-specific IgG was increased with medium and high doses and CVA16-specific IgG was increased with all doses of immunization. When gene expression of T cell-biased cytokines was analysed, Th1, Th2, and Th17 responses were found to be highly activated in the high-dose immunization group. Moreover, bivalent vaccine immunization mitigated paralytic signs and increased the survival rate following lethal viral challenges. When the viral RNA content was determined from various organs, all three doses of bivalent vaccine immunization were found to significantly decrease viral amplification. Upon histologic examination, EV71 C4a and CVA16 induced tissue damage to the heart and muscle. However, bivalent vaccine immunization alleviated this in a dose-dependent manner. These results suggest that the bivalent inactivated EV71 C4a/CVA16 vaccine could be a safe and effective candidate HFMD vaccine.

Background: Surgical closure of an atrial septal defect (ASD) is infrequently indicated during infancy. We evaluated the clinical characteristics and outcomes of patients who underwent surgical ASD closure during infancy. Methods: A single-center retrospective review was performed for 39 patients (19 males) who underwent surgical ASD closure during infancy between 1993 and 2020. The median body weight percentile at the time of operation was 9.3. Results: During a median follow-up of 60.9 months, 4 late deaths occurred due to chronic respiratory failure. A preoperative history of bronchopulmonary dysplasia (BPD) was the only risk factor for late mortality identified in Cox regression (hazard ratio, 3.54; 95% confidence interval [CI], 1.75-163.04; p=0.015). The 5-year survival rate was significantly lower in patients with preoperative history of BPD (97.0% vs. 50.0%, p<0.001) and preoperative ventilatory support (97.1% vs. 40.4%, p<0.001). There were significant postoperative increases in left ventricular end-diastolic (p=0.017), end-systolic (p=0.014), and stroke volume (p=0.013) indices. A generalized estimated equation model showed significantly better postoperative improvement in body weight percentiles in patients with lower weight percentiles at the time of operation (<10th percentile, p=0.01) and larger indexed ASD diameter (≥45 mm/m2, p=0.025). Conclusion: Patients with ASD necessitating surgical closure during infancy are extremely small preoperatively and remain small even after surgical closure. However, postoperative somatic growth was more prominent in smaller patients with larger defects, which may be attributable to an increase in postoperative cardiac output due to changes in ventricular septal configuration. The benefits of ASD closure in patients with BPD are undetermined.
