Improvement of Parent and Family Satisfaction by Offering Informational Interventions throughout Surgery (수술 중 정보제공을 통한 환자 부모 및 가족의 만족도 증진)

  • Background : This study was designed to assess the effects of information on the satisfaction of parents and families whose children underwent elective surgery in the Pediatric Operating Room. Methods : We measured parent and family satisfaction with information given before and after offering informational interventions. The data were collected from 510 patients parents and families whose children had undergone elective surgery in the Pediatric Operating Room of Seoul National University Hospital in Seoul. South Korea. The data collection ran from May 3. 1999 to May 30. 1999 and from September 6, 1999 to September 30, 1999. The research instrument consisted of four categories (pre-operative period, intra-operative period, post-operative period, others) and 24 questionnaires. SPSS was used to analyze the data. Results : The major findings of this study are summarized as follows: 1. Before surgery, the satisfaction with the following was increased: the explanations of the necessary preparation for the surgery (6.0%), the time the patient would enter the operating room (20.6%), the operative procedure (2.0%), and the use of the waiting room (10.0%). 2. During the operation, the satisfaction with the situation board (15.1%), public announcements in the waiting room (8.4%), and the answering of the families questions (12.2%) was increased. however, the satisfaction with the surgeons explanations of the surgical outcomes decreased by 8.3%, even though the frequency of these explanations increased by 5%. 3. After surgery, the satisfaction with the following was increased: the explanations of patient's status of recovery(10.3%) the time the patient would be moved from the recovery room (17.6%), how the patient would be transferred to the ward (19.2%), and post-operative care (6.3%). Conclusion : Based upon the above findings, we concluded that pre-, intra-, and post-operative informational interventions were effective in increasing the satisfaction of the parents and families of children undergoing elective surgery.

  • The purpose of this paper is to schedule elective surgery patients using a stochastic programming approach and to illustrate how operating room utilization behaves when a decision-maker varies costs associated with utilization. Because of the uncertainty in surgery durations, the underage and overage costs that a decision-maker considers plays an important role in allocating surgery cases into available operating room capacity. We formulate the problem as a stochastic mixed integer programming and propose a sampling-based approximation method for a computational purpose. Newsvendor model is employed to explain the results from numerical experiments that are conducted with the actual data from a hospital. The results show that the operating room utilization is more sensitive when the unit overtime cost is relatively larger than the unit cost for underutilized time.

A clinical study of emergency room visits for oral and maxillofacial lacerations

  • Objectives: This study investigated patients with oral and maxillofacial lacerations who visited the emergency room over a three-year period in an effort to determine the optimal treatment for these injuries. Materials and Methods: This study examined 1,742 patients with oral and maxillofacial lacerations with 2,014 different laceration locations who visited the emergency room of Pusan National University Hospital (Busan, Korea) over three years, from January 2011 to December 2013. Patients were classified by sex, age, visit day, cause of injury, injury site, and the presence or absence of soft tissue and tooth injuries. Results: The male to female ratio was 2.50:1. Patients under 10 years old were seen most frequently. Most emergency room visits were on weekends. Among intra-oral lacerations, the lip area was the most vulnerable site; among extra-oral lacerations, the chin area was most frequently injured. The most frequent etiology was a slip down. Most lacerations occurred without bone fracture or tooth damage. Conclusion: Laceration may differ in large part as compared with the fracture. Therefore, it is necessary to continue collecting data on oral and maxillofacial lacerations to establish optimal emergency room diagnosis and treatment strategies.

Operation Room Fire: Caution for Using Electrocautery after Rinsing Operation Field at the End of the Surgery with Alcohol-Based Cleansing Solutions (수술방 화재: 수술 종료 시 알코올 함유 피부 소독액을 이용한 수술부위 세척 이후 전기소작기 사용 주의)

  • Fires in operating rooms rarely occur. However, this type of disaster can complicate almost any surgical procedure. Fuel, heat and oxygen are related with fire outbreak. When ignition sources such as alcohol-based surgical preparation solutions are present, the risk of an operating room fire increases, and burns are more severe in such conditions. Many manufacturers recommend waiting at least three minutes after application to allow complete drying for reduce fire risk. There are a few studies regarding flame burns in the operation room, although most of these studies are related to preoperative skin preparation. However, alcohol containing solutions can be used occasionally for cleansing of the operation field after the surgery, therefore, the surgical team should pay attention to surgical fires, even if they have completed the operation successfully. We present our case of a post-operative flame burn and introduce some precautions that will reduce the risk of alcohol burns.

  • Common carotid artery laceration is a life-threatening injury by causing hypovolemic shock. Nevertheless the initial management is very difficult until definitive surgery at operation room. Before neck exploration at operation room, arterial bleeding control by compressing the bleeding point is not always effective. We experienced one case with externally penetrating injuries in zone II neck, which was operated after clamping of common carotid artery in the emergency department. Here we report this case.

  • Inferior vena cava (IVC) injuries can have fatal outcomes and are associated with high mortality rates. Patients with IVC injuries require multiple procedures, including prehospital care, surgical techniques, and postoperative care. We present the case of a 67-year-old woman who stabbed herself in the abdomen with a knife, resulting in an infrarenal IVC injury. We shortened the transfer time by transporting the patient using a helicopter and decided to perform direct-to-operating room resuscitation by a trauma physician in the helicopter. The patient underwent laparotomy with IVC ligation for damage control during the first operation. The second- and third-look operations, including previous suture removal, IVC reconstruction, and IVC thrombectomy, were performed by a trauma surgeon specializing in cardiovascular diseases. The patient was discharged without major complications on the 19th postoperative day with rivaroxaban as an anticoagulant medication. Computed tomography angiography at the outpatient clinic showed that thrombi in the IVC and both iliac veins had been completely removed. Patients with IVC injuries can be effectively treated using a trauma system that includes fast transportation by helicopter, damage control for rapid hemostasis, and expert treatment of IVC injuries.

Effects of Noise Block on Anxiety and Vital Sign of Patients with Gynecologic Laparoscopic Surgery in Operating Room (수술실의 소음차단이 산부인과 복강경 수술 환자의 불안과 활력징후에 미치는 영향)

  • Purpose: This study was to examine the effects of noise block on anxiety and vital sign of gynecologic laparoscopic surgery. Methods: The data were collected from March to May 2011. Participants were sixty patients with gynecologic laparoscopic surgery, divided into 30 of experimental group and 30 of control group at C University hospital located in I city. The day before surgery, demographic data, trait-state anxiety and vital signs were measured at ward. After noise block, the data were measured using VAS anxiety and vital signs before anesthesia and in recovery room. And then state anxiety and vital signs were measured in ward after surgery. The data were analyzed by $x^2$-test, t-test, repeated measured ANOVA and Bonferroni comparison method using SPSS/WIN 19.0. Results: After conducting noise block program, the experimental group showed significant decrease in state anxiety and heart rate compared to those of the control group. But there were not significant differences in VAS anxiety, systolic pressure and diastolic pressure between two groups. Conclusion: This program can be regarded as an effective nursing intervention for the management of anxiety with gynecologic laparoscopic surgery.

Analysis of Patients with Facial Lacerations Repaired in the Emergency Room of a Provincial Hospital

  • Background Facial laceration is the most common injury encountered in the emergency room in the plastic surgery field, and optimal treatment is important. However, few authors have investigated this injury in all age groups or performed follow-up visit after repair. In the present study, the medical records of patients with lacerations in the facial area and underwent primary repair in an emergency room over a 2-year period were reviewed and analyzed. Methods Medical records of 3,234 patients with lacerations in facial area and underwent primary repair in an emergency room between March 2011 and February 2013 were reviewed and identified. Results All the 3,234 patients were evaluated, whose ratio of men to women was 2.65 to 1. The forehead was the most common region affected and a slip down was the most common mechanism of injury. In terms of monthly distribution, May had the highest percentage. 1,566 patients received follow-up managements, and 58 patients experienced complications. The average days of follow-up were 9.8. Conclusions Proportion of male adolescents was significantly higher than in the other groups. Facial lacerations exhibit a 'T-shaped' facial distribution centered about the forehead. Careful management is necessary if a laceration involves or is located in the oral cavity. We were unable to long term follow-up most patients. Thus, it is necessary to encourage patients and give them proper education for follow-up in enough period.

Factors Influencing on Fatigue in Operating Room Nurses (수술실 간호사의 피로 예측요인)

  • Purpose: The purpose of this study was to measure the fatigue of operating room nurses and examine factors contributing to their fatigue. Methods: The subjects of this study were 115 nurses who worked in operating rooms of three hospitals. Data were collected from September 15, 2009 to September 30, 2009. Fatigue and job stress were measured by using the Symptom's Fatigue Scale and the job stress measurement tools in operating room nurses. Results: The mean score of fatigue level was $3.10{\pm}0.61$. We analyzed fatigue according to characteristics of subjects and found that there were significant differences in marital status, number of children and sleeping hours in general characteristics. Career, daily participation hours in the surgery relating to job related characteristics. Significant positive correlations were found between job stress and fatigue(r=.233, p= .012). Three significant variables influencing fatigue of operation rooms nurses were job stress, daily participation hours in the surgery, and sleeping hours (Adj. $R^2$=0.284, F=4.773, p<.001). Conclusion: An integrative care program that takes job stress, daily participation hours in the surgery and sleeping hours into consideration is essential to reduce fatigue in operating room nurses.

Noise and Room Acoustic Conditions in a Tertiary Referral Hospital, Seoul National University Hospital

  • Background and Objectives: Noise levels and room acoustic parameters at a tertiary referral hospital, Seoul National University Hospital (SNUH) in Korea, are investigated. Materials and Methods: Through a questionnaire, acoustically problematic rooms are identified. Noise levels in emergency rooms (ERs) and intensive care units (ICUs) are measured over about three days. Acoustically critical and problematic rooms in the otolaryngology department are measured including examination rooms, operating rooms, nurse stations, receptions, and patient rooms. Results: The A-weighted equivalent noise level, LAeq, ranges from 54 to 56 dBA, which is at least 10 dB lower than the noise levels of 65 to 73 dBA measured in American ERs. In an ICU, the noise level for the first night was 66 dBA, which came down to 56 dBA for the next day. The noise levels during three different ear surgeries vary from 57 to 62 dBA, depending on the use of surgical drills and suctions. The noise levels in a patient room is found to be 47 dBA, while the nurse stations and the receptions have high noise levels up to 64 dBA. The reverberation times in an operation room, examination room, and single patient room are found to be below 0.6 s. Conclusions: At SNUH, the nurse stations and receptions were found to be quite noisy. The ERs were quieter than in the previous studies. The measured reverberation times seemed low enough but some other nurse stations and examination rooms were not satisfactory according to the questionnaire.
