This study was performed to investigate the characteristics of soft tissue profile of the class III malocclusion and to test the yardstick far differential diagnosis between surgical and orthodontic patients. Initial lateral cephalograms of orthodontic group(30 patients) that have acceptable occlusion and profile by orthodontic treatment alone and surgical group(30 patients) that have favorable occlusion and profile by combined surgical-orthodontic treatment were selected in Ajou university hospital. Powell and Burstone II analysis were made on the tracing. Descriptive, comparative, factor, cluster, and discriminant analysis were carried out with computer program. The results were as followings : 1. Patients who received surgery had a more concave profile and a longer lower facial height than patients who received orthodontic treatment alone. 2. Nasolabial angle, ratio of vertical height, and mentolabial sulcus were significantly different at the 5% level. And facial protuberance, upper lip protuberance, mentocervical angle, nasofrontal angle, nasomental angle, mandibular vertical height, angle between cervix and lower face, ratio of mandibular vertical height divided by cervical depth, ratio of vertical height between upper and lower lip, and maxillary protuberance were significantly different at the 1% level. 3. 8 factors were extracted and factor 2, 3, and 8 showed significant differences by factor analysis. 4. Orthodontic group (25) and surgical group (35) were classified by cluster analysis. 5. Discriminant function was D = 0.079Nasomental angle + 0.081Sn-Gn + 3.343Sn-Gn/C-Gn + 1.734Sn-St/St-Me' -26.460, and cutting score was 0, so we can discriminate that orthodontic group has the score above 0, and surgery group below 0. And 91.7% of original grouped cases were correctly classified.
Purpose : The purpose of this clinical study was to evaluate the effect of chelating and deproteinizing agent containing dental conditioning gel on alleviation of peri-implant mucosa inflammation. Methods: 36 patients with functionally loaded implants for at least 1 year and have clinical signs of peri-implant mucositis were recruited. At baseline, all implants received subgingival prophylaxis with ultrasonic scaler. In the test group, patients were provided a chelating and deproteinizing agent dental conditioning gel (Clinplant$^{(R)}$) and were given instructions to applicate it around the implants using an interdental brush for 2 weeks. Chlorhexidine and saline were provided to the positive control group and negative control group, respectively. The modified sulcus bleeding index (mSBI), modified plaque index (mPI), and probing pocket depth (PPD) were evaluated at baseline, 1 week, and 2 weeks. Results: In the Clinplant$^{(R)}$ and chlorhexidine group, mSBI (-0.81, -0.85 respectively; p<0.01), mPI (-0.46, -0.5 respectively; p<0.01), and PPD (-0.58, -0.48 respectively; p<0.01) at 2 weeks were significantly reduced from baseline. In the saline group, all the clinical parameters were reduced but there was no statistical significance. The saline may be attributed to the influence of prophylaxis at baseline. Conclusions: The present study demonstrated the beneficial clinical effects of chelating and deproteinizing agent containing dental conditioning gel to decrease peri-implant mucosa inflammation equivalent to chlorhexidine. This dental conditioning gel might be useful for alleviation of peri-implant mucosa inflammation.
Various alloplastic materials have been used on the periodontally diseased ossous defects. Hydroxyapatite, which is used the most common alloplastic material is a non-resorbable form of calcium phosphate and natural coral which is a biodegradable by carbonic anhydrase in osteoclast was introduced recently. The purpose of the present study was to evaluate the clinical effects of porous hydoxyapatite and natural coral on the human periodontal defects. Four males and three females who had adult periodontitis were selected for this study. The teeth that had similar bone loss radiographically and periodontal pocket deeper than 5mm were selected. Gingival recession, pocket depth, plaque index(Silness & Loe), sulcus bleeding index and tooth mobility (measured by Periotest$^{(r)}$) were examined before graft. Before insertion of alloplastic materials, the depth from CEJ to bone crest and from CEJ to base of the osseous defect was recorded. Porous particulate hydroxyapatite(Interpore 200$^{(r)}$, A group) was place on the defect and natural coral(Biocoral$^{(r)}$, B group) was placed on the defect of the opposing tooth. Six months post-surgically the same parameters were recorded by reentry procedures. A and B group showed 0.6mm of mean recession. Mean reduction of pocket depth were 5mm for A group and 4.9mm of B group. Reduced SBI and tooth mobility were recorded. Osseous defect fills of the original defects were 2.9mm for A and 3mm for B group. Percentage defect fills were 71% for A and 59% for B group. The difference of defect fill between pre- and post-insertion was statstically significant(p<0.05). But the difference between the two groups was not significant statistically(p<0.05). The clinical impression at 6 month re-entry and the numerical date indicate that natural coral as well as porous particulate hydoxyapatite has a definite potential as an alloplastic implant in the treatment of periodontal osseous defects.
The purpose of the present study was to evaluate and compare the periodontal conditions of distally extending cantilever bridges(CB) and removable patial dentures(RPD). 78 patients treated with removable partial dentures were divided into 2 groups; bilateral free-ending (type 1) and unilateral free-ending(type 2). 43 patients treated with distally extending cantilever bridges were divided into 4 groups according to position of missing molars and number of including abutment tooth (or teeth) (type 1-4). Prosthodontic status of the abutment teeth were assessed; overcontoured crown, overhanged margin, embrasure closure, and others. Their Periodonted status were assessed with Plaque Index Gingial Index(GI) Sulcus bleedy Index (SBI), Tooth mobility(TM) & Posher depth(PPD). 35.9% of RPD group and 38.1 of CB grouphad disconfort on mastication with prosthetics. 16.7% of RPD group and 6.1% of CB group showed on opposing sites. Overhanged margins were 58.3% for RPD group and 86.6% for GB group. 45.5% of RPD group and 72% of CB group had embrasure closure on the abutment teeth. 30.5% of GB group had oversized pontic. Gb group showed much higher clinical parameters than RPD groups, but statistically not significant. Type 2 of RPD group showed lower GI and SBI, but statistically not significant. Type 3 of CB group showed the highest GI and SBI and type 2 showed the highest tooth mobility, but there is no differences between the same type of groups. In conclusion, only minor differences in periodontal conditions were recorded in RPD and CB group.
Purpose: A common side effect of the scalp reduction is a creation of a 'slot' with the hair growing in the opposite directions away from the scar. Overcoming the unnatural appearance of the slot has been a vexing problem in the scalp reduction surgery. None of the conventional corrective surgical techniques provides a complete and satisfactory aesthetic result. The Frechet flap is a triple transposition flap used for the correction of the slot defect secondary to scalp reduction surgery, seldom needing further scar revision. The Frechet technique provides a solution to the problem of the central slot concealment that is unattainable by other means, such as; Z-plasty and mini-graft. Methods: Authors applied the Frechet technique to Asian patients who had undergone scalp reduction and operated on 4 patients from March, 2000 to January, 2001. Average follow-up period was 13 months. Patients with long scars passing through the temporoparietoccipital zone were excluded. All the undermining was performed in the subgaleal plane, reaching the upper auricular sulcus and stopping just above the nuchal ridge. Results: None of the patients experienced infection, hematoma, nor any permanent hair loss. Transient telogen effluvium at the distal end of flap 2 and 3 was noticeable in one case. Conclusion: In conclusion, the results are aesthetically satisfactory without any significant complications.
Purpose: The primary goal of palatoplasty is to enable normal speech with harmonious growth of face. Some children who had palatoplasty display typical findings of transverse maxillary deficiency requiring orthodontic widening of the maxilla. Levi (2009) described a cleft palate repair coupled with pedicled buccal fat pad flaps to cover bone exposed areas of the hard palate. Hence we report clinical experiences of cleft palate repair using pedicled buccal fat pad flap. Methods: Four Veau class II and a Veau class I cleft palate patients underwent palatoplasty with buccal fat pad flap by single surgeon from April 2009 to August 2009. Two patients received 2-flap palatoplasty and three patients 1-flap palatoplasty, respectively. After the cleft palate repair, sharp mosquito scissors was placed in the superior buccal sulcus just lateral to the maxillary tuberosity and inserted directly through the mucosa resulting in buccal fat pad extrusion. The elevated flap was moved to cover mucoperiosteal defect in hard palatal area. Results: Five patients underwent primary palatoplasty using buccal fat pad flap. Flap harvest and inset took on average 9 minutes per flap. Mucosal epithelization took 18 days on average. No patients had complications related to the buccal fat pad flap. Conclusion: Buccal fat pad pedicled flap has significant potential to function as an added vascularized tissue layer in cleft palate repair and we can expect better growth of maxilla with this method although longer duration of follow-up was unavailable.
Periauricular paresthesia may afflict patients for a significant amount of time after facelift surgery. When performing face and neck lift surgery, temple and posterior auricular flap dissection is undertaken directly over the auriculotemporal, great auricular, and lesser occipital nerve territory, leading to potential damage to the nerve. The auriculotemporal nerve remains under the thin outer superficial fascia just below the subfollicular level in the prehelical area. To prevent damage to the auriculotemporal nerve and to protect the temporal hair follicle, the dissection plane should be kept just above the thin fascia covering the auriculotemporal nerve. Around the McKinney point, the adipose tissue covering the deep fascia is apt to be elevated from the deep fascia due to its denser fascial relationship with the skin, which leaves the great auricular nerve open to exposure. In order to prevent damage to the posterior branches of the great auricular nerve, the skin flap at the posterior auricular sulcus should be elevated above the auricularis posterior muscle. Fixating the superficial muscular aponeurotic system flap deeper and higher to the tympano-parotid fascia is recommended in order to avoid compromising the lobular branch of the great auricular nerve. The lesser occipital nerve (C2, C3) travels superficially at a proximal and variable level that makes it vulnerable to compromise in the mastoid dissection. Leaving the adipose tissue at the level of the deep fascia puts the branches of the great auricular nerve and lesser occipital nerve at less risk, and has been confirmed not to compromise either tissue perfusion or hair follicles.

Modern orthodontics implies not only occlusal excellence, but also the positioning of teeth to produce optimal facial harmony for the individual patients. Several methods have been used in the study of facial height, width and depth were made from living subjects. These methods, however, complicate to control the subjects, therefore many investigators have used profile cephalometric technics. Practically, cephalometric technics were used m orthodontic treatment, maxillo-facial surgery and anthropometric studies. Author was studied to investigate the normal standards of soft tissue profile in Korean adolescences. The subjects consisted of 53 males and 54 females from 17 to 22 years of age and with normal occlusion and acceptable profile. Aluminum filter was designed to obtain both hard and soft tissue structures on a single film. Eight profile landmarks were plotted and drawn on the tracings of all cephalograms and eighteen depth, height and angles were measured from each landmarks of the cephalograms. The following conclusIOns were obtained from this studies; 1. Total facial convexity was 170. 75 in males and females samples and lower facial and: labiomandibular convexity were each of 141.44, 171.05. 2. Maxillary and mandibular sulcus angulations were 137.61, 129.52 and upper and lower lip inclinations were each of 123.26 and 49.56 in male and females. 3. Soft tissue depth of several points were as follows; Subnasale 18.74㎜ in males and 16.65㎜ in females Pogonion 13.40㎜ in males and 13.07㎜ in females upper lip 14.06㎜ in males and 11.91㎜ in females . lower lip 15.46㎜, 13.63㎜ in males and females 4. The protrusion of nose were 16.28㎜ in males and 15.56㎜ in females 5. The vertical length of upper and lower lips were 25.67㎜, 52.96㎜ and the lip posture was indicated 93.43 per cent (closed state) in centric occlusions.
The purpose of this study was to compare clinical results of guided tissue regeneration(GTR) using either a nonresorbable ePTFE membrane or a resorbable membrane made from a synthetic copolymer of glycolide and lactide(PLGA) in the treatment of human class Ⅱ furcation defects. The ePTEE membranes were applied to 16 patients with maxillary molar buccal class Ⅱ furcation defects as Group I, PLGA membranes were applied to 15 patients with maxillary molar buccal class Ⅱ furcation defects as Group Ⅱ, ePTFE membranes were applied to 20 patients with mandibular molar buccal class Ⅱ furcation defects as Group Ⅲ and PLGA membranes were applied to 20 patients with mandibular molar buccal class Ⅱ furcation defects as Group Ⅳ and bone graft materials(DFDBA) were applied in all groups. Probing depth, gingival recession, clinical attachment level, tooth mobility and sulcus bleeding index(SBI) were measured at baseline, 3, 6 and 12months postoperatively. In addition, membrane exposure levels were measured at surgery, 1, 2 and 6weeks postoperatively and postoperative complications were evaluated. The results were as follows: In all groups, there were statistically significant differences in probing depth reduction, gain of clinical attachment and mobility reduction at values of 3, 6 and 12months postoperatively compared to values of baseline, whereas no significant differences in SBI except Group I and gingival recession(p<0.05). Membrane exposure levels were increased at 1, 2 and 6weeks postopratively compared to value of baseline in Group I(p<0.05). There were no statistically significant differences between ePTFE and PLGA membrane in probing depth, clinical attachment level and SBI. There were minimal gingival recession and membrane exposure in Group Ⅳ and pain and swelling were the most common postoperative complications in Group Ⅱ, Ⅲ(p<0.05). In conclusion, this study showed that both nonresorbable membrane and resorbable membrane were effective similarly in the treatment of class Ⅱ furcation defects, without statistical differences in clinical measurements.
Problem : The several studies were reported to the effects of individual tooth tray with the parallel tooth, but not reported to the effect of individual tooth tray with severe undercut in impression making. Purpose : The purpose of this study is to examine the effect of individual tooth tray; 1) the accuracy of impression in terms of distance. 2) reproducibility of pocket. Material and methods : In this study, a metal model with 3 teeth including tooth prepared 30-degree undercut and gingival crevice of the depth 5mm, width 0.4mm was cast. Impressions of the metal model were taken in 5 times used 4 kinds of impression materials with individual tooth tray and another impression taking were made with conventional method without individual tooth, used same materials, procedure. The stone models were made. Distances between abutments were measured with a Micromeasurescoup-mm. Result : The following results were obtained ; 1. In the comparisonof inter-abutment distance between parallel tooth without individual tooth tray, 4elastomeric materials have not significant difference to the metal model(p>0.05). 2. In the comparisonof inter-abutment distance with undercut tooth without individual tooth tray, 4elastomeric materials have a significant difference to the metal model(p<0.05), especially polyether's dimensional change is large. 3. If individual tooth tray was used to take impression in abutment teeth with severe undercut, more stable results was achieved easily than conventional method. (p>0.05). 4. It is not true that the pressure of impression material with individual tooth tray increased to gingival sulcus. Conclusion : By the results, I knew the fact that individual tooth tray was useful impression method on the teeth with severe undercut for the dimensional stability. Also, I could not confirm the fact that individual tooth tray would increase the penetrating pressure into cervice.
