루이 라벨의 '악의 현존과 고통'에 대한 긍정적인 해석 그리고 그 한계 - 『악과 고통들』을 중심으로 - (A positive interpretation of Louis Lavelle for the Presence of Evil and Pain, and its Limitations - Focusing on evil and suffering -)

  • 루이라벨의 사상에서는 현대철학에서 거의 다루지 않고 있는 '선과 악'에 대한 주제가 매우 중요한 개념으로 등장하고 있다. 그의 사유에서 악의 개념은 '악이란 존재의 부족'이라는 스콜라철학의 소극적인 규정에서 출발하고 있지만, 세계에 현존하는 악이란 '삶과 가치'를 파괴하는 의지적인 힘으로, 즉 도덕적인 차원에서 보다 적극적인 의미로 규정하고 있다. '악의 현존(la $pr{\acute{e}}sence$ du mal)'을 '세계의 스캔들(le scandale du monde)'로서 규정하는 그의 사유에는 모든 인간이 의지의 태도로서의 악의 가능성을 지니고 있으며, 세계와 인간사회에서의 악의 현존은 필연적인 것임을 말해주고 있다. 반면 악의 결과가 육체적 정신적인 '고통'으로서 나타나기에, 고통을 거부하고자 하는 인간의 '고통에 대한 태도'는 이러한 악을 극복하고자 하는 도덕적인 의지를 유발하게 한다. 악의 현존 앞에서 가지게 되는 '내적인 고통'인 '도덕적인 고뇌'는 그 자체 '도덕적인 존재'가 된다는 의식을 가지게 하고 여기서 자기만족을 가지게 되는 것이다. 비록 고통스럽지만 도덕적인 존재가 됨에 대한 자기만족과 행복감은 인간이 악의 현존에 맞서 이를 초극할 수 있는 유일한 계기이자 동기이다. 즉 라벨의 선과 악에 대한 사유는 '도덕 심리주의'에 기초해 있으며, 고통에 대한 거부와 도덕적 고뇌에 있어서의 '심리적 지평과 윤리적 지평의 일치'가 인간으로 하여금 끊임없이 악을 극복하고 선을 산출하도록 한다는 '윤리적 낙관론'을 가능하게 하고 있다. 이는 분명 스콜라 철학의 발전된 현대적인 적용이라고 할 수 있겠으나, 또한 '악을 유발하는 의지의 태도에 대한 구체적 양태의 분석', '사회 속에 현존하는 악의 구체적인 양태들' 그리고 이러한 악을 초극할 수 있는 '의지적인 노력의 다양한 양상' 등에 대한 분석은 이루어지고 있지는 않다. 이러한 구체성의 부족은 선과 악에 관한 라벨의 사유가 여전히 소극적인 면모를 보이고 있다고 할 수 있다.

미만성 거대 B세포 림프종으로 인한 척수더부신경병증 (Spinal Accessory Neuropathy Secondary to Diffuse Large B-Cell Lymphoma)

  • Spinal accessory neuropathy (SAN) is commonly caused by an iatrogenic procedure, and that caused by tumors is very rare. We present a case of a 49-year-old man suffering from weakness in the right trapezius and sternocleidomastoid muscle. An electrophysiology study confirmed proximal SAN. Fluorodeoxyglucose (FDG)-positron emission tomography (PET)/computed tomography (CT) revealed a diffuse large B-cell lymphoma compressing the right spinal accessory nerve. Ultrasonography showed definite atrophy on the trapezius and sternocleidomastoid muscles. In addition, post-chemotherapy FDG-PET/CT showed increased FDG uptake in the right upper trapezius, suggestive of denervation. This is the first report of SAN caused by direct compression by a diffuse large B-cell lymphoma, comprehensively assessed by an electrophysiology study, ultrasonography, and FDG-PET/CT.

단기간의 요부안정화 운동이 만성요통환자의 근력과 자세균형에 미치는 영향 (The Effect of Short-term Lumbar Stabilization Exercise for Lumbar Muscle Strength and Postural Balance on Chronic LBP)

  • PURPOSE: The purpose of this study was to investigate the effect of short-term lumbar stabilization exercise to patients suffering from chronic low back pain(CLBP) on the strength of their lumbar flexor and extensor and the postural balance of them and to suggest effective treatment duration for them. METHODS: Study subjects were 20 patients with CLBP who's been reported the pain for more than 12 weeks. For these subjects, muscle strength of both extensor and flexor of lumbar and the balancing ability were measured before and after the intervention. Lumbar stabilization exercise was composed of matt, ball and sling exercise. The intervention was applied for total 12 sessions for 4 weeks (3 times a week) and each session was for 40 minutes. RESULT: For muscle strength of both extensor and flexor of lumbar, there was significant differences after the application of lumbar stabilization exercise(p<.05). For balancing ability, stabilization shows significant differences in all of the positions except PC(eyes closed on pillows)(p<.05). Distribution of weight shows no significant differences but it was generally distributed within normal range after the exercise and Fall index shows significant differences between before and after the exercise(p<.05). CONCLUSION: These results could have positive effect on deciding different periods of therapeutic intervention.

파인다이닝 레스토랑 이용 행태에 따른 물리적 환경 중요도와 고객 만족에 관한 연구 (A Study on the Importance of Physical Environment and Customer Satisfaction by Fine Dining Restaurant Use Behavidor)

  • 본 연구에서는 국내 파인다이닝 레스토랑을 이용한 경험이 있는 소비자를 대상으로 이용 행태에 따른 물리적 환경의 중요도 차이와 고객 만족과의 관계를 파악하여 경기 불황으로 인해 어려움을 겪고 있는 파인다이닝 레스토랑 업계의 영업활성화를 위한 마케팅 전략 수립에 도움을 주고자 한다. 설문조사는 2009년 5월 1일부터 6월 30일까지 61일간 저녁 시간대에 실시하였으며, 서울 세종로 사거리에 위치한 파인다이닝 레스토랑 'Wood & Brick'과 'Benigni'를 방문한 서울지역 20세 이상 남녀 내국인 거주자를 대상으로 하였다. 자료의 처리는 SPSS 14.0 통계패키지를 사용하여 빈도분석, 신뢰도 검증, 요인분석, ANOVA, 다중회귀분석을 사용하였다. 그 결과 이용 행태에 따른 물리적 환경 중요도 차이는 월평균 이용 횟수와 1인당 이용 비용에 따라 차이가 나타났으며, 물리적 환경과 고객 만족과의 영향 관계에서는 청결성, 공간 배치, 매력성, 접근성이 고객 만족에 긍정적인 영향을 미치는 것으로 나타났다.

척추도수교정이 요통에 미치는 영향 (The Effect of spinal manipulation on the Lumbar Spine with LBP patients)

  • Purpose: Many people are suffering from Low back pain due to HIVD and muscular problems, lack of joint functions on lumbar spine. In this study we compared the change of the herniation index, Oswestry LBP disability index (OLDI), visual analog scale (VAS), lumbar flexion range of motion (ROM) between the pre-experiment and after 4 weeks treatment by maitland manual therapy. Method: we selected and managed both the 15 people. They are $46.80{\pm}15.46$ years old people with HIVD and Stenosis. We treated for the people with HIVD and Stenosis by manual therapy(maitland manipulation method) during 4 weeks. And then we compared with pre-experiment and after 12 weeks through measuring the herniation index change by using computor themograpy (CT), LBP OLDI, VAS, lumbar flexion ROM. Results: The changes in the herniation index, Oswestry lumbar Disability Index, VAS, lumbar flexion ROM between the pre-experiment and after 4 weeks treatment by maitland manual therapy, there was a statistically significant difference. Although there was a significant difference after 4 weeks in OLDI, VAS, lumbar flexion ROM. But disc herniation index was no significant difference. Conclusion: Manual therapy is very effective for Lumbago due to the HIVD and spinap stenosis patients. OLDI, VAS and lumbar flexion ROM were increased. But disc herniation index was no significant difference. We suggest the combination treatment between manual exercise and spinal traction.

요통환자의 체지방량이 폐기능 및 최대산소섭취량에 미치는 영향 (The Effects of Body Fat Rate on Pulmonary Function and Maximal Oxygen Uptake in Low Back Pain Patients)

  • My prior questionnaire has applied to 12 male and 29 female adults over 30-69 years old suffering from lumbago fer 6 months at least or having diagnosis of lumbago. I had the following conclusions from physical characteristics and cardiopulmonary capacity of lumbago Patients caused by obesity 1) The rate of lipids in the body was rotated to the maximum heart rate in the male group of lumbago patients (P<0.01). 2) The rate of lipids in the body was related to the maximum intake of oxygen(O2) in the male group of lumbago patients(P<0.01). 3) The maximum heart rae was related to the maximum intake of oxygen in the male group of lumbago patients(P<0.01). 4) The rate of lipids in the body was related to the maximum intake of oxygen in the female group of lumbago patients(P<0.05). 5) The rate of lipids in the body showed no relation to the maximum heart rate in the female group of lumbago patients(P>0.05). 6) The maximum heart rate was related to the maximum intake of oxygen in the female group of lumbago patients(P<0.01). 7) The cardiovascular system showed no relation te the function of the lungs in the male and female groups of lumbago patients(P>0.05) 8) The lung capacity per second of lumbago patients(FEV $1.0\%$) measured less than normal adults. 9) The maximum intake of oxygen(V02max) and heart rate was less than normal adults. Thus I summarized the conclusion so follows: the cardiovascular system of lumbago patients showed no relation to their pulmonary function, and the rate of lipids in the body Showed a correlation with the maximum intake of oxygen but the rate of lipids in the body showed no relation to their pulmonary function.

Effect of Neuro-Feedback Training and Transcutaneous Electrical Nerve Stimulation (TENS) in Stress, Quantitative Sensory Threshold, Pain on Tension Type Headache

  • Purpose: The objective of this study is to evaluate the effect of neuro-feedback training and transcutaneous electrical nerve stimulation (TENS) on stress, quantitative sensory threshold and pain in patients suffering from tension type headache. Methods: 22 participants who passed the preliminary evaluation were enrolled in the study and 11 participants were randomly assigned to each group. The control group (n=11) was subject to the TENS treatment of which was composed of a 20-minute session for 5 times a week during 4 weeks, and the experimental group (n=11) was subject to both neuro feedback training and TENS treatment for 10 minutes a day and 5 days a week during 4 weeks. The Perceived Stress Scale (PSS) was used to measure a level of stress and the quantitative sensory testing (QST) was used for the measurement of cold pain threshold (CPT) and heat pain threshold (HPT); A degree of pain was evaluated through the headache impact test-6 (HIT-6). Results: In comparision of all dependent variables between the control and subject groups, there were significant differences in stress, quantitative sensory threshold and pain after the treatment (p<0.05), and the experimental group showed significant differences in stress, CPT, HPT and pain (p<0.05) and the control group showed only a significant difference in HPT (p<0.05). Conclusion: Findings of this study demonstrate that the concomitant administration of the TENS treatment and neuro feedback training is effective on alleviation of stress, quantitative sensory threshold and pain in patients with tension type headache.

Effect of the Type and Positon of Scoliosis on the Static Balance of Adolescents

  • Purpose: This paper provides basic clinical data on the treatment of scoliosis patients by analyzing the effects of the type and position of scoliosis on the static balance using Tetrax on adolescents who have balance disorders as a consequence of structural changes, such as scoliosis. Methods: A total of 110 adolescents were divided into 6 groups according to the radiographs and 60 adolescents were sampled, 10 each for each group. The static balance was measured and analyzed on the existence of sight using Tetrax. Results: The changes followed by existence of sight in static balance group showed a significant difference statistically in Stability index in all groups (p<0.001). The changes in the static balance in each group in terms of the stability index were significant in all groups, where the experiments were performed under the eyes opened and eyes-closed condition (p<0.01)(p<0.001). The post-hoc comparison revealed the stability index to be the highest in the eyes-opened condition in group III, but not in group IV. In the eyes-closed condition, group III showed the highest stability index of all the groups. Conclusion: Adolescents with lumbar scoliosis had a lower static balance in single scoliosis with the eyes-closed condition or had little movement compensation. In addition, unlike the other normal scoliosis, back scoliosis has negative effects on the posture because it causes an increase in the disturbance of posture. Therefore, future studied will be needed to examine the imbalance of posture in people suffering from back pain.

정중신경가동기법이 수근관증후군 환자의 악력과 통증에 미치는 영향 (Effects of the Median Nerve Mobilization on Change of Grip Strength and Pain in Patients with Carpal Tunnel Syndrome)

  • Purpose:This study was to determine whether median nerve mobilization has any effect on the recovery of muscle power and the reduction of pain for the patients with carpal tunnel syndrome. Methods:20 patients suffering from carpal tunnel syndrome were randomly divided into two groups: 10 subjects treated both with electrical therapy and median nerve mobilization(experimental group) and the other 10 subjects treated only with electrical therapy(control group). The therapy was taken three times a week for 3 weeks. Results:The grip strength increased in both groups. However, the experimental group showed more significant treatment effect than the control group, and the significant changes appeared every week during the experiment period. In terms of pain, both groups showed significant effects. But the experimental group showed but more significant treatment effect than the control group, and the significant changes appeared every week during the experiment period. Conclusions:It was significantly more effective for the patients with carpal tunnel syndrome to apply median nerve mobilization during the electrical therapy in order to reduce pain and to increase grip strength, compared with the method only using electrical therapy. Further research should be conducted to generalize these results in terms of the clinical effectiveness of the nerve mobilization.

척추 안정화 운동과 등뼈가동술이 만성 허리통증환자의 척추 유연성에 미치는 영향 (Effects of Trunk Stability Exercises and Thoracic Manipulation on Spine Flexibility in Chronic Low Back Pain Patients)

  • PURPOSE: This study aimed to investigate the benefits of thoracic manipulation and the trunk stability exercises on spine trunk flexibility and the pain index of chronic low back pain patients. METHODS: The study included 44 patients suffering from chronic low back pain. The participants were divided into two groups: the first group was assigned for trunk stability exercises (TSE), and the second group was randomly assigned TSE and thoracic manipulation at the same time (TSE+TM). Both groups carried out each assigned treatment thrice a week for 8 weeks. The study outcome was based on assessment of spine trunk flexibility and the pain index. Spine trunk flexibility was measured by spine flexion and extension through a range of motion and thoracic cage circumference. The pain index was measured using a visual analog scale (VAS). RESULTS: Spine flexion and extension range of motion showed a significant difference within each group and between the groups before and after the treatment. The measurement of the thoracic cage circumference also showed a notable difference within each group and between the groups before and after the treatment. There was no change in the pain index. CONCLUSION: These results indicate that thoracic manipulation with the trunk stability exercises is an efficient treatment for improving the spine trunk flexibility and soothing pain for chronic low back pain patients.
