Sialolithiasis is the most frequent disease of the salivary glands, causing swelling and/or pain of the affected site. We report a 44-year-old woman who presented with severe pain in the lower left second molar region without swelling. Sialoliths on her left submandibular gland were confirmed by radiographic examinations. After robot-assisted sialoadenectomy, the pain did not recur but remained facial paralysis and unaesthetic scar.
The submandibular gland is one of the major salivary glands, salivary diseases frequently occuring site due to its anatomical weakness. This retrospective study evaluated data pertaining to history, sex, operation method, age distribution, diagnosis through the chart, operation record, radiographs, histologic finding of 51 patients operated on for the submandibular gland excision from 1986 to 1995 in our hospital so that we improve on the understand of the salivary gland diseases. The results were as follows ; 1. The chief complaints was mass, swelling mainly. 2. The ratio of affected site was 47.1 : 51 in left : right respectly, both sites was 1 case especially. 3. The ratio of men to women was 56.9 : 43.1 4. The operation performed all through the extraoral approach. 5. The sialolith presented at 24 cases. 6. The most patients had a two weeks duration period. 7. The age distribution was the third decade(29.4%), the second decade(27.5%) in order. 8. The symptoms accompanied the chief complaints was pain(37.3%), mass(17.6%), swelling(13.7%), trismus(13.7%) in order. 9. The diagnosis was sialadenitis with stone(S.W.S.) (45.1%), sialadenitis without stone(S.W.O.S.) (17.6%), pleomorphic adenoma(P.A.) (15.7%), abscess(Abs) (5.9%), tuberculosis(Tbc) (5.9%) in drder.
Background: Conventionally, indirect radiography has been used to diagnose salivary gland diseases. However, with the development of sialendoscopy, diagnosis and treatment of salivary gland diseases have become more effective. Herein, we report a case of sialolithotomy treated with sialendoscopy and compare it with the existing methods through a literature review. Case presentation: Two patients with a foreign body sensation under the tongue and dry mouth visited the Anam Hospital, Korea University. Radiographic examination revealed salivary stones inside the right Wharton duct, and the patients underwent sialolithotomy under local or general anaesthesia. The stones were totally removed, and there were no postoperative complications such as bleeding or pain. Conclusion: The development of sialendoscopy has enabled better definitive diagnosis of salivary gland diseases compared with the conventional methods; better treatment outcomes can be obtained when sialendoscopy is used in appropriate cases.
A 22-year old female visited CNUH due to palpable neck mass. Cytologic examination of a fine needle aspiration was performed and the result was Pap class II. Routine chest x-ray shows solitary pulmonary nodule. For rule-out malignancy, FNA at neck mass was repeated and pathologic finding was dysplasia. She was admitted to MI department for evaluation of solitary pulmonary nodule and percutaneous needle aspiration was done. Pathologic diagnosis was adenoid cystic carcinoma. Thereafter, the lesions were treated by excisional biopsy of submandibular gland mass with left supraomohyoid neck dissection and wedge resection of right lower lobe at ENT department and thoracic and cardiovascular surgery department, respectively. Final diagnosis was adenoid cystic carcinoma arising in submandibular gland with solitary lung metastasis. According to TMN staging system, surgical staging is stage IV of T2N0M1. Clinical follow-up to postoperative 13 months in this case showed that she is alive and well without evidence of recurrence.
A patient with actinomyces infection of the submandibular soft tissue was diagnosed by fine needle aspiration cytology (FNAC). A 38-year-old woman presented with a right submandibular mass which slowly grew in size over one month. Clinically and radiologically, the lesion was considered as tuberculous lymphadenitis or cellulitis. The polymerase chain reaction for tuberculosis was done by aspirated specimen but the result was negative. The smears of aspiration cytology showed characteristic colonies (sulfur granules) of actinomyces in inflammatory background. After antibiotic therapy for eight months, the patient has been well, showing no detectable mass. This patient was simply and rapidly diagnosed by FNAC and can avoid unnecessary surgical biopsy.
In currently, stress diseases are increased that present several sign and symptoms. Under stress condition, there are dry mouth, burning mouth syndrome, oral mucosa diseases and halitosis more frequently. Changing of salivary proportion is checked in almost patients with changing of function and structure in salivary gland. This study purpose are what effect stress does on salivary gland, and a-amylase on salivary gland. This study was resulted that 1. Under restraint stress, acinar cells are vacuolization and changing of intercellular spaces are separated, and peripheral tissues of duct are changed 2. Acinar cells were shrunk after 3 hours under restraint stress, intercellular space was separated after 6hours, peripheral tissues of duct started to change after 72 hours, and acinar cells and peripheral tissues of duct were all severely changed after 168hours. 3. In immunohistochemical study, amylase reaction was showed partially and irregularly after 3 hours, was getting little milder after 6 hours. And amylase reaction was gradually increased from the time of 12 hours after experiment up to the time of 48 hours after experiment. But after 168 hours, amylase appearance was diminished. According this result, emotional stress can change of salivary gland structure, and amylase secretion, the important digestive enzyme from salivary gland is changed and it is supposed to make digestive disorder and to make halitosis efficiency. So, we need to study about secretion of amylase.
Actinomycosis is a subacute or chronic suppurative infection caused by Actinomyces species, which are anaerobic Gram-positive bacteria that normally colonize the human mouth and digestive and urogenital tracts. Cervicofacial actinomycosis is the most frequent clinical form of actinomycosis, and is associated with odontogenic infection. Characterized by an abscess and mandibular involvement with or without fistula, but the cervicofacial form of actinomycosis is often misdiagnosed because the presentation is not specific and because it can mimic numerous infectious and non-infectious diseases, including malignant tumors. We report a rare case of actinomycosis infection with coexisting submandibular sialolithiasis. The patient presented with a $1{\times}1cm$ abscess-like lesion below the lower lip. Punch biopsy of the lesion revealed atypical squamous cell proliferation with infiltrative growth, suggestive of squamous cell carcinoma. The patient underwent wide excision of this lesion, where the lesion was found to be an abscess formation with multiple submandibular sialolithiases. The surgical specimen was found to contain Actinomyces without any evidence of a malignant process. We assumed that associated predisposing factors such as poor oral hygiene may have caused a dehydrated condition of the oral cavity, leading to coexistence of actinomycosis and sialolithiasis.
Purpose: To evaluate the usefulness of shear wave elastography in the diagnosis of oral and maxillofacial diseases. Materials and Methods: Ten patients with oral and maxillofacial diseases and 28 volunteers drawn from our student doctors were examined by shear wave elastography with a 14-MHz linear transducer using an Aplio 300 apparatus (Canon Medical Systems, Otawara, Japan). A statistical analysis of the shear elastic modulus(kPa) of healthy tissue (the sublingual gland, submandibular gland, anterior belly of the digastric muscle, and geniohyoid muscle) in the 28 volunteers was performed using 1-way repeated measures analysis of variance with the Tukey honest significant difference test. The maximum shear elastic modulus(kPa) of 8 patients with squamous cell carcinoma (SCC) and 2 patients with benign lesions was evaluated with the Mann-Whitney U test. The analysis used a 5% significance level. Results: The mean shear elastic modulus of the sublingual gland ($9.4{\pm}3.7kPa$) was lower than that of the geniohyoid muscle ($19.2{\pm}9.2kPa$, P=.000) and the anterior belly of the digastric muscle ($15.3{\pm}6.1kPa$, P=.004). The maximum shear elastic modulus of the SCCs($109.6{\pm}14.4kPa$) was higher than that of the benign lesions($46.4{\pm}26.8kPa$, P=.044). Conclusion: Our results demonstrated the usefulness of shear wave elastography in the diagnosis of oral and maxillofacial diseases. Shear wave elastography has the potential to be an effective technique for the objective and quantitative diagnosis of oral and maxillofacial diseases.
Foregut cystic developmental malformations are rare developmental anomalies. It is important to diagnose the diseases and manage them properly because these cysts may generate feeding or respiratory difficulties depending on the size and location of the lesions. A newborn was referred for a congenital cervical swelling to our clinic on the second day of his life. Neck SONO and MRI showed an about 6cm sized cystic mass at left submandibular area. Aspirations and sclerotherapies were done repeatedly due to recurred cystic mass. Under the suspicious of cystic hygroma, the mass and submandibular gland were excised. Histologically, it was a benign cyst including gastrointestinal and bronchogenic mucosa and pancreatic tissue. Foregut cyst was suggested for the final diagnosis and the patient was discharged at 9 days after the operation without a complication. He has visited our out-patient department. Although several image studies have been introduced to find out foregut cyst, it is difficult to go through differential diagnosis because of similarity of other benign tumor. Further studies for early diagnosis of cervical foregut cyst are needed for preventing possible related problems.
As the interest in oral health is increasing with quality of life. The most representative oral diseases include dental caries, periodontal disease, and malocclusion, as well as dry mouth and oral mucosa. Cinnamaldehyde have an antioxidant effect that has been studied not only to treat rheumatism and hypertension, but also to protect liver, and gastrointestinal tract, but there are few studies related to the control of oral diseases. The purpose of this study was the effect of enhanced orofacial pain when oral administration of cinnamaldehyde in the oral diseases. Cinnamaldehyde (5, 12.5, 25, and 50 mg/kg) orally administered at a dose of 1 mL, and the change in biological response was confirmed after a week. In addition, 5% formalin (30, 50 μL) was injected into TMJ and subcutaneous areas of the whiskers of rats to observe the change in the threshold of the improved orofacial pain model. As a result of the experiment, in the xerostomia model, drinking water was decreased in the cinnamaldehyde-administered group, feed intake and weight increased, and saliva was also increased compared to the naïve group. In particular, the most significant increase trend was observed at the concentrations of 25 and 50 mg/kg. In addition, it was confirmed that the pain behavioral response of the orofacial area improved by oral diseases decreased depending on the concentration of cinnamaldehyde. Based on these results, cinnamaldehyde effectively reduced symptoms related to xerostomia and showed improved pain relief in the orofacial areas.
