악하선관 내 거대 결석 1예 (One Case of a Large Wharton's Duct Stone)

  • Salivary gland calculi account for the most common disease of the salivary glands. We experienced a rare case of submandibular duct(Wharton's duct) calculi of unusual size and shape in a 40 year-old male patient, who presented with pain and swelling in the region of submandibular gland. The author describes the management of this patient and reviews the literature.

  • Salivary duct carcinoma is an uncommon aggressive malignant epithelial neoplasm with similarity to intraductal carcinoma of the breast. This neoplasm occurs most often in the parotid gland of middle-aged and older males. About 7% of reported tumors occured in the submandibular gland. The report of salivary duct carcinoma with calcification is rare. We report a case of salivary duct carcinoma with calcification in the submandibular gland. The patient was a 73-year-old male with a mass of the right submandibular gland for 1 year. On the fine needle aspiration cytology, the aspirate showed scant cellularity, small clusters of tumor cells, and scattered small calcifications. Nuclei of the tumor cells showed mild pleomorphism and round to oval in shape, and cytoplasm was abundant and finely granular. Nucleoli were indistinct and necrosis was not noted. There were no cribriform or papillary arrangements of tumor cells. Cytologlc findings of salivary duct carcinoma are variable depending on histologic findings, and calcifications could be an additional cytologic findings.

등압수액선조영술을 이용한 정상성인의 악하선에 관한 방사선학적 연구 (A ROENTGENOGRAPHIC STUDY OF NORMAL SUBMANDIBULAR GLANDS USING ISOBARIC SIALOGRAPHY)

  • The aim of this study was to establish the diagnostic criteria of normal submandibular glands in adults. Materials included 132 sialograms of selected person from 23 to 28 years of age. In subjects each variable was measured and evaluated statistically introducing 18 items respectively. Conclusions from this study were as follows. 1. The mean diameter of main duct was 1.96㎜ in distal end and 2.71㎜ in hilar end, so the diameter of distal end was smaller than that of hilar end in submandibular glands. 2. The mean length of main duct was 42.02㎜ in submandibular glands. 3. The mean angle of main duct was 35.67 degrees with inferior border of mandible and 103.05 degrees at posterior mylohyoid portion. 4. In configurations following shape of main duct, those revealed that 39.39% of cases of the curvilinear type, 16.67% of cases of the linear type, 22.73% of cases of the sigmiod type and 21.21% of cases of the angular type. And in configurations following course of main duct, those revealed that 83.33% of cases of the smooth type and 16.67% of cases of the tortuous type. 5. The mean caliber of submandibular glands was 45.59㎜ in superoinferior height, 43.56㎜ in anteroposterior width, 28.85㎜ in mediolateral width and 33.65㎜ in extent below mandibular angle. 6. Well-delineated acinar parenchyme was observed in 77.28% of cases and well-defined tertiary ducts in 50.06% of cases. 7. There were no differences between right and left submandibular glands in size and shape of main duct and parenchymal portion, but there were great variations in each individual.

타석에 대한 방사선학적 연구 (A RADIOLOGIC STUDY OF SALIVARY STONES)

  • The author examined fifty cases of sialolithiasis diagnosed in the Dept. of Oral Radiology in SNUH by conventional radiography and sialography, and analyzed patient's age, sex, location, radio density, numbers, shapes, and relation with ducts and parenchymas. 1. The results of this study were as follows: 1. The average age of patients was 38.6 years in submandibular sialolithiasis, and 39.2 years in parotid sialolithiasis. 2. There was slightly higher incidence in males (58.0%) than in females (42.0%). 3. Salivary stones were found to be much more in the submandibular gland and duct (82.0%) than in the parotid gland and duct (18.0%). 4. Of 62 submandibular salivary stones, 33 (53.2%) occurred in the main duct, 25 (40.3%) occurred in the hilum, and 4 (6.5%) occurred in the parenchyma. Of 18 parotid salivary stones, 9(50.0%) occurred in the main duct, 5(27.8%) occurred in the parenchyma, and 4(22.2%) occurred in the hilum. 5. Of the submandibular salivary stones, the number of radiopaque type was 45 (75.8%), and the number of radiolucent type was 17 (24.2%). Of the parotid salivary stones, the number of radiopaque type was 12 (66.7%), and the number of radiolucent type was 6(33.3%). 6. The single type was 30 cases (73.2%) in submandibular gland, 6 cases (66.7%) in parotid gland, and the multiple type was 11 cases (26.8%) in submandibular gland, and 3 cases (33.3%) in parotid gland. 7. Round shape was 35 cases (43.8%), ovoid shape was 22 cases (27.5%), irregular shape was 17 cases (21.3%), and cylindrical shape was 6 cases (7.5%).

악하선 분비관의 결찰 후 타액선 조직의 변화 (THE CHANGES OF SALIVARY GLAND AFTER THE LIGATION OF THE EXCRETORY DUCT IN SUBMANDIBULAR GLANDS)

  • Obstructive adenitis of the salivary gland following salivary stone or infectious disease of the gland and surrounding tissues is a common disease. It is often difficult to decide whether to perform total excision of the gland or to consider conservative treatment. The present study was designed to investigate histological, histochemical changes of submandibular gland after ligating the excretory duct for identifying the results of gland duct obstruction. A group of 40 rat of Sprague-Dawley weighing about $200{\sim}220gm$ were used in the present study. 30 rats had ligation of the main excretory ducts of submandibular glands just at the exit from the glands. For controls, 10 rats had a sham operation without duct ligation. They were inducted into euthanagia state by intracardial Ketamine injection in 1, 2, 3, 5, 7, 14, 21, 28, 35 and 42 days after the ligation. In each ligation period, 3 animals were used for ligation and one animal was for control. The submandibular glands were dissected out at sacrifice and stained with H&E, PAS, mucicarmine stain and histological examination were carried out under the light microscope. After examination and comparison of all specimens, the results were as follows: 1. In the features of H&E stain, acini disappeared by degrees after the ligation of the excretary duct and interstitial cells were displaced into fibrous connective tissue. Salivary gland had been atrophied with enlarging ducts and proliferating ductal cells. 2. Through total experimental period, a lot of vessels were observed and the atrophy of serous gland was severer than that of mucous gland. 3. The deep portion of submandibular glands showed severe degeneration rather than superficial portion of them after the ligation. 4. The changes which had enlarged ducts and proliferating ductal cells were observed in entire gland and more prominent in serous gland than mucous gland after the ligation. 5. Although PAS and mucicarmine reactions were decreased gradually after the ligation with the lapse of time, since 2 to 3 weeks they were strong positive reactions on entire gland, especially on duct-like structure. So, we can suggest that salivary gland will be atrophied but, survived acini will be redistributed around the ducts.

  • An accessory submandibular gland is a rare variation. As such, there is limited literature regarding the embryology, anatomy, variations, clinical imaging, and pathology of the accessory submandibular gland. In this article, we review the existing literature on the accessory submandibular gland from clinical and anatomical perspectives. The goal of this review is to provide comprehensive knowledge of this variation which can be useful for oral and maxillofacial/head and neck surgeons, radiologists, and anatomists. Within this review, the embryologic origin as well as the anatomy of the accessory submandibular gland is detailed. Several imaging modalities which can be used to visualize the accessory submandibular gland are outlined as well as its variations. Lastly, this review investigates several reported clinical considerations regarding the accessory submandibular gland including sialoliths, Wharton's duct obstruction, and pleomorphic adenoma.

구내 절개법에 의한 악하선 적출 (EXCISION OF THE SUBMANDIBULAR GLAND BY AN INTRAORAL APPROACH)

  • Surgical removal of the submandibular gland via intraoral approach was performed. The surgical procedure to be used for removal of submandibular gland had been performed via extraoral approach for many centuries. Disadvantages of extraoral technique are the esthetic distress due to an external scar, residual inflammation in Wharton's duct, and neurological complications. Indications of intraoral approach are unlimited in surgical cases of submandibular gland. Advantages of intraoral approach are esthetic satisfaction due to no remaining scars, preservation of adjacent anatomical structures and preservation of lower facial contour. This paper describes the surgical technique of the submandibular gland excision through an intraoral approach and variable incision lines tried. In 7 cases, excellent results were obtained after removal of the submandibular gland through an intraoral approach.

악하선 타석증의 치험례 (A CASE REPORT OF THE SIALOLITHIASIS ON THE SUBMANDIBULAR GLAND)

  • The Sialolithiasis is one of the most common disease that is found in the submandibular gland and they can be usually observed in the extra glandular area. As the lumen of Wharton's duct is larger and more expandable than that of Stensen's duct, the symptom of them is initially painless. As increasing their size, the sign and symptom are pain and sudden enlargement of gland. It can be observed with clinical exam and the scintigraphy. After removal of sialoliths, the majority of them can get the recovery of function. Our department performed the transoral sialolithotomy and the extraoral sialoadenectomy for three patients of sialolithiasis and all of them showed no complication after operation.

  • Sialolithiasis is the most common disease caused by calculi in a salivary gland and its duct. The pain and swelling of salivary gland is a chief complaint of patients presenting. Most salivary gland calculi occur in the submandibular gland, but can also occur in the parotid gland and the sublingual gland. One giant sialolith is rarely reported, while the several cases of one or multiple sialolith in the submandibular gland have reported in the literatures. In this case, we have removed the sialolith in which perforated mouth floor along Wharton's duct and report it.
